RESUMEN

A new model of cirrhosis of the liver was created in experiment on 30 rats. Over 3 weeks, experimental animals in addition to the standard diet daily intragastrically received corn oil in combination with intragastric administration of acetaminophen in a dose of 500 mg/day. High-fat load in combination with acetaminophen over 3 weeks led to the development of focal biliary cirrhosis.

RESUMEN

The study examined implication of tributyrin, a metabolic precursor in small intestinal microflora, in stimulation of electrical activity of duodenum and jejunum. Enteral administration of tributyrin enhanced electrical activity of both examined structures in small intestine with elimination of the rest periods. The stimulatory effect was manifested in a prolongation of the periods of irregular activity. The up-regulating effects of tributyrin on electrical activity in upper segments of small intestine are mediated via the cholecystokinin receptors being also associated with activation of cholinergic and blockade of nitrinergic signal transduction pathways.

RESUMEN

Studying the mechanisms of the small intestine motor function disorders during endotoxemia and searching ways to mitigate them remain relevant. The article discusses the role of inflammatory mediators, in particular nitric oxide as a key factor in the generation of inflammatory response and brake the main neurotransmitter in the gut in the pathogenesis of the small intestine motor disorders during endotoxemia. Also discusses anti-inflammatory cholinergic path, which is realized with the participation of the autonomic nervous system. Possible mechanisms by which tributyrinte as a component of nutritional support creates a multiplier effect in arresting the inflammatory response and normalization of intestinal motility are suggested.

RESUMEN

The effect of enteral administration of tributyrin on electrical activity in the upper segments of the small intestine was examined in rats on the model of postoperative ileus. This postoperative state is characterized with pronounced and long-term disturbances in generation of migrating myoelectric complex of the small intestine. The enteral administration of tributyrin in the early postoperative period aimed to suppress the non-adrenergic non-cholinergic influences and activation of the cholinergic anti-inflammatory pathways is an effective procedure to normalize the migrating myoelectric complex and therefore the coordinated propulsive peristalsis in the small intestine.

RESUMEN

The review include actual facts, demonstrating high probability of glutamatergic neurotransmitter system role in the regulation of the gastrointestinal tract motor activity. These facts suggest significant role of the glutamatergic neurotransmitter system dysfunction in forming motor activity disorders of the digestive tract, including in patients in critical condition. The analysis is based on results of multiple experimental and clinical researches of glutamic acid and other components of the glutamatergic neurotransmitter system in central nervous system and autonomic nervous system (with the accent on the enteral nervous system) in normal conditions and with functioning changes of the glutamatergic neurotransmitter system in case of inflammation, hupoxia, stress and in critical condition.

The work focuses on the comparison of influence of domperidone and metoclopramide on rehabilitation of motor-evacuation function of stomach and small intestine in 82 patients with post-surgical flatulent distention. For group 1 (n=50) - metoclopramide was administered intravenously from the 1st postoperative day, 10 mg 3 times a day. For group 2 (n=32) - domperidone was used from the 1st postoperative day (motilium suspension), 20 ml 4 times a day. The results of gastrointestinal tract capacity rehabilitation were evaluated using the data of peripheral electrogastroenterography, enteral balance, a complex of radial diagnostics methods. Application of domperidone in patients with post-surgical flatulent distention resulted in quicker gastrointestinal tract motor resolution.

By using some psychotropic agents as an example, investigations of the oxidative activity of electrochemically synthesized sodium persulfate solutions were continued. The derivatives of phenothiazines, xanthene, and dibenzazepines were shown to be oxidized by synthesized sodium persulfate solution to low-toxic products. Oxidation products were ascertained to coincide with the known products of their biotransformation in the body.

The effects of neostigmine and calcium pantothenate on electrical activity of the stomach and small intestine were studied in chronic experiments on rats after laparotomy with implantation of a probe into the jejunum and electrodes into different portions of the gastrointestinal tract. At the early terms after surgery, stimulation of endogenous acetylcholine release intensified electrical activity of the stomach, duodenum, and jejunum. Treatment with neostigmine and calcium pantothenate did not accelerate the recovery of the migrating myoelectrical complex, but promoted the recovery of the general intensity of action potential generation in the stomach and small intestine.

We studied the effect of drugs with various mechanisms of action on propulsive activity of the small intestine in healthy rats. Blockade of the major inhibitory influences realized via nonadrenergic noncholinergic inhibitory effector neurons was not followed by stimulation of intestinal transit. Propulsive activity of the small intestine increased upon treatment with drugs, whose effects are realized via acetylcholine or acetylcholine and serotonin.

RESUMEN

The effect of NO on electrical activity of the stomach and small intestine was studied in chronic experiments on rats during the early postoperation period. Generation of migrating myoelectric complex in the stomach and small intestine was impaired after abdominal surgery. Administration of NO synthesis blocker suppressed the inhibitory influences, which potentiated generation of the myoelectric complex in the small intestine. Our results suggest that NO is involved in the regulation of the migrating myoelectric complex in the small intestine during postoperation motor disturbances in the gastrointestinal tract.

RESUMEN

Results of the treatment of 90 patients with generalized peritonitis and syndrome of intestinal insufficiency were analyzed. In the study group (45 patients) enteral administration of 1% pectin solution and glutamin solution (15-30 g/day) were included in combined therapy. Clinical and laboratory control, radiation monitoring and bacteriological studies carried out for evaluation of efficacy of therapy established. A decrease of endogenous intoxication and time of repair of functional activity of the gastrointestinal tract, immunomodulation, normalization of microbiocenosis of the small intestine. This therapy diminishes the number of complications and lethality.

Baseline nutritional impairments, patient's malnutrition, and inadequate correction of metabolic disorders considerably reduce the effects of therapeutic measures, increase the risk of septic and infectious complications, negatively influence the hospital length of stay, increase mortality rates. Concomitant and profound disorders of metabolic homeostasis system in various diseases and critical conditions dictate a multi-component approach to the correction of metabolic disorders requiring specially chosen nutrient compositions and the routes of their administration. The development of currently available nutritional formulae was based on the theory of balanced nutrition reflecting the physiological requirements in nutrients and calories of a healthy person, and also taking into account specific features of pathogenesis, clinical course, disease stage, the nature and extent of metabolic disorders, functional condition of the gastrointestinal tract, the effects of specific nutrients on the intensity of metabolic processes. Nutrition adapted to the patient's disease and the digestive tract functional condition provided by diets containing specific nutrients (arginine, glutamine, omega-3 fatty acids) has a number of advantages over standard nutritional formulae, and poses a directed therapeutic effect. The nutritional formulae "Nutrien" are represented by the standard ("Nutrien Standard"), and semi-elemental ("Nutrien Elemental") formulae, as well as by the formulae of directed effect ("Nutrien Hepa", "Nutrien Nephro", "Nutrien Pulmo", "Nutrien Diabet", "Nutrien Immun").

The efficiency of nutritive therapy was analyzed in cases of 37 patients with gastrointestinal pathologies. Group 1 comprised 12 patients with fistulas of different etiologies and localizations; and group 2 comprised 15 patients with esophageal pathologies, including 7 children with esophageal atresia and 8 children with post-burn cicatricial stenosis of the esophagus. A method of nutrition-status correction by means of both enteral and parenteral feeding is suggested on the basis of examination findings comprising both clinical and laboratory-and-instrumental data. Preparations for parenteral feeding, i.e. 10-20% fatty emulsions, 10% amino acids solutions and 15-20% glucose solutions, were made use of. Enteral diets: semi-element oligopeptide solutions, like Nutrilon pepti TSC, Alphare. Balanced mixtures: sour-milk Nan, AL 110, Nutrizon, Nutridrink. Practical recommendations were defined, on the basis of study results, as to the therapeutic feeding schemes during the in-hospital treatment stages.

This study was aimed to investigate the electrophysiological and morphological characteristics resulting from the structural and functional transformation of gastric tissue transplanted to the small intestine. Twelve adult mongrel dogs were studied up to 3 years. Gastric transplants preserved its main microstructure and minimal compensatory-adaptive processes developed in the mucosa and muscle layers of the graft. A significant influence on the electrical activity of the small intestine was observed, with a 10% reduction of the slow wave frequency (SWF) in the proximal and distal jejunum adjacent to the graft after meals. The SWF of the gastric graft itself, however, corresponded to the frequency of the native stomach, did not depend and was not associated with adjacent intestinal areas. In summary, the stomach graft transplanted to the small intestine keeps the properties of gastric tissue, there are functional adaptations to conditions of digestion in the small intestine and the graft has minimal effects on intestinal motility.

Effect of mesodiencephalic modulation on small intestinal mucosa was studied in rats subjected to massive blood loss. Massive blood loss induced structural changes in intestinal epitheliocytes typical of tissue hypoxia: intraintestinal edema, mitochondrial alterations, and microcirculation disturbances. A single session of mesodiencephalic stimulation carried out 60 min after massive blood loss produced a pronounced adaptive effect, restored disturbed structure of the intestinal villi, and induced compensatory hypertrophy of the mitochondria. This suggests that mesodiencephalic modulation promotes normalization of intracellular energy metabolism, improves the absorption capacity of bordered enterocytes, and restores the disturbed intestinal barrier.
