RESUMEN

OBJECTIVE: Aim: To analyze the impact of medical reform on the motivational component of improving the quality of medical care in health care facilities in Sumy based on the results of a medical and sociological study of doctors. PATIENTS AND METHODS: Materials and Methods: The study involved 154 doctors working in inpatient and outpatient departments of health care facilities in Sumy. Sumy in June-August 2023. The study used a systematic approach, bibliosemantic, comparative and statistical analysis, and logical generalization. The data were processed and statistically analyzed using Google Forms and Microsoft Excel 2010 for Windows. RESULTS: Results: The study showed that 49 respondents (31.8%) are satisfied with the material and technical support at the workplace and working conditions. Almost all doctors (138 people (89.6%)) said that the actual amount of their salary does not correspond (partially or fully) to the workload at the workplace. Only 4.5% of them said that they receive extra payments for the quality of healthcare services, 21.4% of them said that they receive extra payments periodically, and 74.1% said that they do not receive extra payments at all. Despite the high level of workload and dissatisfaction with salaries, the majority of respondents (109 people (70.2%)) would not agree to change their profession to another one, even if the salary was higher. According to doctors, the most important incentives for improving the quality of healthcare services are: moral satisfaction from work and well-coordinated teamwork (76.6% of answers), financial incentives (74.7% of answers), opportunities for professional and career growth (48.7% of answers), respect from patients and society (46.8% of answers), and management recognition (13.6% of answers). CONCLUSION: Conclusions: The study has shown that today, in the context of health care system reform, there is practically no effective motivational component to improve the quality of health care in health care facilities in Sumy: 40.9% of people gave negative answers, 42.2% of people indicated only its partial existence. Regardless of the length of service, for all respondents, one of the most important motivational incentives is not only material but also moral factors and public recognition.

OBJECTIVE: The aim: To study the results of a medical and sociological research as to the attitude of rural population in amalgamated hromadas (AH), i.e. united territorial communities, of Sumy region to the results of primary care reform. PATIENTS AND METHODS: Materials and methods: The form of research - a survey with closed questionnaire. The study was conducted on a specially designed three-tier quota sample, which was calculated allowing for the territorial and socio-demographic indicators. A total of 320 respondents residing in the specified rural communities (hromadas) were surveyed with quota sampling. The theoretical sampling error makes 3%. The questionnaires were processed by the laboratory staff with the help of the "OСA" program. RESULTS: Results: Among the surveyed in Nyzhniosyrovatska AH (amalgamated hromada), 23.5% of the population emphasized that their authorities neglected medical problems, while in Bezdrytska AH this indicator was 6.7%, as evidenced by the difference in the industry financing from own assets (274 thousand less than in Bezdrytska AH). The survey revealed that in the communities where the population knows their family doctors better and trusts them, the satisfaction with the quality of medical care and the attitude to prophylaxis (prevention) is much higher. CONCLUSION: Conclusions: A significant part of the population demonstrates a low level of awareness of the course of the reforms, their goals and objectives. The specified trends may indicate a lack of communication or distortion of information on the implementation of medical reform. It is an information support that is one of the key instruments for effective introduction of medical reform. At the local level, this function is assigned to a family doctor who becomes a determining factor not only for providing medical services to patients, but also for communication, keeping the population informed and prophylaxis.

RESUMEN

OBJECTIVE: The aim: The aim of the study is to analyze financial indicators and assess the effectiveness of financing secondary medical care enterprises during the transition period in the context of remuneration to doctors. PATIENTS AND METHODS: Materials and methods: To perform the analysis, the financial characteristics of 26 medical facilities have been collected and processed by means of involving the bibliosemantic method, data extract and content analysis. CONCLUSION: Conclusions: Presents a comparative analysis of labour payment expenses to doctors of cardiology, obstetric and gynecological and surgical departments of medical facilities. The predicted labour payment expenses for the number of services provided have been calculated. Besides, the article makes it clear that the allocation of funds according to bed-day loads and the number of medical services rendered is unsustainable and does not correspond to the real load on the actual position.

OBJECTIVE: Introduction: The urgency of the topic is due to the fact that in recent years in Ukraine a part of youth who use narcotic drugs and psychoactive substances is growing. Today there are more than a quarter of billion drug addicts in the world. This number includes those who have tried drugs at least once. Among them - about 27 million drug addicts that are in need of treatment. The aim: to analyze the knowledge of the students of higher educational institutions of Poltava about various issues related to drug addiction. PATIENTS AND METHODS: Materials and method: 600 questionnaires of students who study at the universities (Poltava). The following methods were used: historical - analytical and bibliosemantic, medico-statistical, sociological, system approach and system analysis. RESULTS: Results: Student youth responded to various questions that were presented in the questionnaire, about various issues related to drug addiction. 26 % of respondents indicated that they know places where drugs can be purchased. To the question «Do you know what drugs are?¼ оnly 7 % answered «No¼. 2, 5 % of those surveyed met with drug users on the street. 13 ± 1, 51 % (р<0, 05) of the respondents admitted that they were irritated when they were criticized for using narcotic drugs and only 9 ± 1, 25 % (р<0, 05) felt guilty about abuse. CONCLUSION: Сonclusions: Students should be aware of the problem of the use of psychoactive substances, be aware of the legislative framework for the prevention of consumption, turnover and any manipulations with psychoactive substances.
