RESUMEN

Giant cell tumour is a commonly occurring benign bone tumour in the Indian population. The common sites of involvement in descending order of frequency are distal femur, proximal tibia, distal radius and proximal humerus. The less commonly occurring sites are distal humerus, pelvis and proximal femur. We present six cases of giant cell tumour involving the distal humerus in rural India. After obtaining a tissue diagnosis by Trucut biopsy and classifying using Enneking's classification, we proceeded to perform wide resection followed by endoprosthetic reconstruction using custom mega prosthesis. We present here six patients (M: F: 2: 4) who were managed by us between 2008-2014. They presented to us with pain around the elbow and restriction in range of movements. They were each noted radiographically to have a lytic lesion involving the distal humerus with the likely diagnosis of giant cell tumour. Closed biopsy was done in all of them to obtain a definitive diagnosis. All patients underwent wide resection and reconstruction using distal humerus custom prosthesis. All patients were followed up at 6, 12, 18 and 24 weeks and thereafter six monthly until the last review. They were assessed using the DASH scoring system. All patients were well with no evidence of recurrence with good to fair functional outcome. We conclude that careful pre-operative planning with meticulous soft tissue dissection and good implant metallurgy and design, these tumours can be treated with good long term functional results.

Facial reconstruction procedures are immensely challenging and are done for a multitude of reasons. The purpose of this report is to provide nationally representative estimates of different types of facial reconstructive procedures and to examine prevalence and predictors of a wide range of complications associated with these procedures in the USA. The Nationwide Inpatient Sample, the largest inpatient dataset for the USA, was used. Data for the years 2004-2010 related to facial reconstruction procedures were identified through ICD-9-CM procedure codes. Associated complications were identified using secondary diagnosis field codes. Multivariable logistic regression models were used to examine the association between patient/hospital-level factors and the occurrence of complications. A total 26,374 facial reconstruction procedures were performed. About 20% of all patients who had facial reconstruction procedures developed a complication. Frequently occurring complications included postoperative pneumonia (4.9% of hospitalizations), hemorrhage (3.9%), other infections (3.6%), non-healing wounds (3.5%), and iatrogenically induced complications (3.2%). Significant factors found to be consistently associated with different types of complications included age, co-morbid burden, sex, and type of admission. The reported results are generalizable within limitations and can be used by health care providers to tailor quality improvement initiatives to minimize or better treat complications in the high-risk cohorts.

The number of total knee arthroplasties (TKA) performed is around two million annually worldwide and this number is expected to increase fivefold by 2025. The most common indication is osteoarthritis of the knee. Blood loss is significant during the post-operative period and blood transfusion when necessary has its own drawbacks. The use of intravenous tranexamic acid has significantly reduced blood loss. We analysed 35 patients who underwent staged bilateral TKA between August 2013 and February 2016 and had administered intra-articular tranexamic acid for one knee and intravenous tranexamic acid for the other knee. The results were analysed based on post-operative blood loss, change in haemoglobin (Hb) level and haematocrit (PCV) and the need for blood transfusion. The average postoperative blood loss was 129.57 ml and 277.71 ml for intra articular group and intravenous group respectively. A control group (no drug or placebo group) with age matched patients (n= 21) was chosen from medical records. The average blood loss in the control group was 493.81 ml. The fall in Hb level and PCV was 0.72 gm/dl and 2.62 % (Intra-articular Group), 1.36 gm/dl and 4.34 % (Intravenous Group) and 2.62 gm/dl and 5.52 % (Control). The number of transfusions were two (Intra-articular Group), five (Intravenous Group) and nine (Control). We conclude that when compared with intravenous route, intra-articular administration has significantly reduced blood loss, Hb level and PCV fall and the rate of blood transfusion.

OBJECTIVE: The purpose of this study is to evaluate the ultimate compressive strength of 50% and 25% Portland cement mixed with Polymer-reinforced zinc oxide eugenol and zinc oxide eugenol cement after 1 hour, 24 hours, and 7 days. MATERIALS AND METHODS: One hundred and eighty samples were selected. The samples were made cylindrical of size 6 × 8 mm and were divided into six groups as follows with each group consisting of 10 samples. Group 1: Polymer-reinforced zinc oxide eugenol with 50% Portland cement (PMZNPC 50%) Group 2: Polymer-reinforced zinc oxide eugenol with 25% Portland cement (PMZNPC 25%) Group 3: Polymer-reinforced zinc oxide eugenol with 0% Portland cement (PMZNPC 0%) Group 4: Zinc oxide eugenol with 50% Portland cement (ZNPC 50%) Group 5: Zinc oxide eugenol with 25% Portland cement (ZNPC 25%) Group 6: Zinc oxide eugenol with 0% Portland cement (ZNPC 0%) These samples were further subdivided based on time interval and were tested at 1 hour, 24 hours and at 7 th day. After each period of time all the specimens were tested by vertical CVR loaded frame with capacity of 5 tones/0473-10kan National Physical laboratory, New Delhi and the results were statistically analyzed using ANOVA and Scheffe test. RESULTS: Polymer-reinforced cement with 50% Portland cement, Zinc oxide with 50% Portland cement, Polymer-reinforced cement with 25% Portland cement and Zinc oxide with 25% Portland cement exhibited higher compressive strength when compared to Zinc oxide with 0% Portland cement and Polymer-reinforced cement with 0% Portland cement, at different periods of time. The difference between these two groups were statistically significant (P < 0.05) and it is suggested that mixture of 50% and 25% Portland cement in Zinc oxide eugenol and Polymer-modified zinc oxide cement can be used as core build up material and permanent filling material. CONCLUSION: It is concluded that 50% and 25% Portland cement in zinc oxide eugenol and polymer-modified zinc oxide eugenol results in higher compressive strength and hence can be used as permanent filling material and core built-up material.

OBJECTIVES: Soluble toll-like receptor-2 (sTLR-2) and cytokines in saliva were assessed as clinical markers for chronic periodontitis in a longitudinal study. MATERIALS AND METHODS: Unstimulated whole saliva was collected from 20 periodontally healthy individuals and 20 patients with chronic periodontitis at diagnosis and at 1 and 6 weeks following scaling and root planing (SRP). Biomarkers including the cytokines (IL-4, IL-6, IL-10, and IL-17), sTLR-2, and sCD14 in saliva were measured by enzyme-linked immunosorbent assay. Mann-Whitney U-test and Student's t-test were used to determine the significance between healthy and chronic periodontitis groups and that between pre- and post-SRP samples, respectively. RESULTS: Salivary sTLR-2, IL-17, and IL-10 levels were significantly lower and those of sCD14, IL-6, and IL-4 were significantly higher in patients with chronic periodontitis as compared with healthy controls. Furthermore, sTLR-2 and IL-4 in saliva reached levels comparable to those of healthy individuals at 6-week re-evaluation visit, implicating a correlation of the two markers with the disease process. CONCLUSIONS: Our data suggest that salivary sTLR-2 is a potential prognostic or maintenance marker for chronic periodontitis. The observed variability of salivary cytokines is consistent with the role of these cytokines in the progression of chronic periodontitis.

BACKGROUND AND OBJECTIVE: Chronic periodontitis is initiated by sequential colonization with a broad array of bacteria and is perpetuated by an immune-inflammatory response to the changing biofilm. Host recognition of microbes is largely mediated by toll-like receptors (TLRs), which interact with conserved pathogen-associated molecular patterns. Based on ligand recognition, TLR-2 and TLR-4 interact with most periodontal pathogens. Extracrevicular bacterial reservoirs, such as the oral epithelial cells, contribute to the persistence of periodontitis. Human saliva is a rich source of oral epithelial cells that express functional TLRs. In this study we investigated the role of salivary epithelial cell (SEC) TLR-2 and TLR-4 in patients with generalized chronic periodontitis. MATERIAL AND METHODS: Unstimulated whole saliva (UWS) was collected from patients with generalized chronic periodontitis and from healthy individuals after obtaining informed consent. Epithelial cells isolated from each UWS sample were assessed for TLR-2, TLR-4, peptidoglycan recognition protein (PGRP)-3 and PGRP-4 by quantitative real-time PCR. In addition, the SECs were stimulated in vitro with microbial products for up to 24 h. The culture supernatant was assessed for cytokines by ELISA. RESULTS: Stimulation with TLR-2- or TLR-4-specific ligands induced cytokine secretion with differential kinetics and up-regulated TLR2 and TLR4 mRNAs, respectively, in cultures of SECs from patients with periodontitis. In addition, the SECs from patients with periodontitis exhibited reduced PGRP3 and PGRP4 mRNAs, the TLR-responsive genes with antibacterial properties. CONCLUSION: SECs derived from the UWS of patients with chronic periodontitis are phenotypically distinct and could represent potential resources for assessing the epithelial responses to periodontal pathogens in the course of disease progression and persistence.

This study aimed to investigate possible means by which hepatitis C virus (HCV) might be transmitted between drug injecting individuals without the sharing of needles and syringes. A questionnaire-based survey of 143 (out of 287) attendees was conducted at an Infectious Diseases Unit-based HCV clinic. Those patients (all of whom were positive for antibodies to HCV) who asked about risk activities and those that admitted to a history of recreational drug injecting were questioned in detail about their past and current drug preparation practices. Ten per cent denied any history of needle and/or syringe sharing and had no other apparent source of their HCV infection, but instead admitted to having shared drug preparation eqiupment. The existence among drug injectors of such practices with the potential to transmit blood borne viruses is important as it may explain how HCV, which is capable of being spread via very small quantities of blood, can be passed between drug injecting individuals who might otherwise never come into contact with another drug injector's blood. Clinical and public health messages regarding the prevention of the spread of HCV may need to be revised and strengthened.

RESUMEN

According to the "International Passenger Survey," published in 1996 by the Office of Trading Standards, 534,000 British people traveled to the Caribbean area (personal communication, ABTA, 1998). The Dominican Republic, the eastern end of a large Caribbean island, has become in recent years one of the most popular destinations for UK holidaymakers as well as for travelers from many other countries. Cyclospora cayatensis has been firmly identified as a cause of gastroenteritis among international travelers,1 including human immunodeficiency virus (HIV)-positive individuals,2 but it has not been previously reported in the literature in British individuals returning from this increasingly popular vacation destination.
