RESUMEN

The present study has demonstrated that the children suffering from chronic post-traumatic knee-joint synovitis after arthroscopy develop an aseptic inflammatory process in the joints associated with the disturbances of local microcirculation, manifestations of sclerosis and fibrosis in the articular soft tissues. An algorithm for the treatment of synovitis in the postoperative period with the use of galvanic muds has been developed. The estimation of the clinical efficacy of this physiotherapeutic method was performed based on the results of routine medical examination, dynamic ultrasound investigation, and dynamic computed termography.

RESUMEN

Acceleration of scientific and technological progress calls for further improvement in the system of regulation of neurologic research in the USSR. Recently the role of the Scientific Council, Task commissions and leading institutions in shaping research policy, in active planning and coordination of research work, and in its methodological and organizational regulation has increased significantly. The most important task facing neurologists today is increase in the efficacy of investigations and the introduction of the obtained results into health practice.

On the basis of the data of the Cerebral Stoke Register some problems of the medical aid delivery to patients with cerebral stroke in the acute phase are analyzed. It was found that in 30% of all calls the first aid was given by the local doctor; in all other calls the aid was given by emergency and urgent aid teams. The necessity of improving the diagnosis of the stroke type is emphasized. 61.5% of the patients were hospitalized: the indications for the hospitalization were determined with consideration for the clinical picture gravity and the stroke type. Within the first 3 days of the stroke a comatose state was registered in 18.6%; a soporous one, in 11.6%; a stunning, in 19.8%; and clear consciousness, in 50% of the patients. The mortality in the acute period of the stroke was higher among patients who were left for the treatment at home, than among those who were hospitalized. The relationship between the mortality and the patients' age was analyzed incomparable groups. It is stressed that a local doctor has to do a lot od diagnostic, therapeutic, and organizational work. In this connection it is suggested to specialize local doctors at large hospitals or chairs of medical schools (by giving them lectures on the principal problems of diagnosing cerebral stroke, organizing and giving the first medical aid). To raise the level of the cerebral stroke, diagnosis and the quality of the medical aid to be given to patients at home, the work of the local therapist and neuropathologist should be better coordinated.

RESUMEN

A study of 788 cases of spontaneous subarachnoidal hemorrhages, which were observed in some neurological clinics during the past 5 years, permitted an analysis of the statistical data concerning the distribution and outcomes of this form of strokes depending upon the age of the patient and etiology. Among the observed population, individuals of a young and middle age prevailed (78% of the patients were younger than 60 years). The role of separate etiological factors is different in different age groups in individuals under 40, subarachnoidal hemorrhages are mainly produced by a rupture of cerebral vascular aneurysms; in the age group of 40--59 years by arterial hypertension, while in the elderly by atherosclerosis in combination with arterial hypertension. The outcomes of subarachnoidal hemorrhages in general are more favourable in the older age groups. However, in hemorrhages of an aneurysmal etiology the lethality amounts to 21%, and is especially high in repeated strokes. In the group of patients with hemorrhages of an aneurysmal etiology the highest lethality was recorded in a localization of the aneurysm in the anterior communicating artery and anterior cerebral artery.

RESUMEN

A clinical analysis of focal epileptic attacks in 18 patients with ischemic disturbances of blood circulation in the zone of mixed blood supply by the frontal and median cerebral arteries was carried out. The epileptic attacks observed were found to be due to irritation of the frontal adversive and pre-moter fields. Focal clonic convulsions in the foot due to irritation of the brain paracentral lobule were also observed. The versatility of the clinical forms of the epileptic attacks was determined mainly by the involvement of particular cytoarchitectonic fields of the frontal lobe of the brain (8th, 6th, 4th) belonging to the zone of mixed blood supply by the frontal and median cerebral arteries and provided with blood by different cortical branches of those arteries. Clinico-angiographic and clinico-morphological comparisons showed that in most patients with focal epileptic attacks there was combined involvement of the internal carotid arteries.

RESUMEN

The authors present a clinico-anatomical correlation of 3 cases with an epileptical status developing in the acute period of a hemorrhage stroke of hypertensive disease. The morphological examination, beside new hemorrhages in the hemisphere, revealed preceding focal and diffuse changes in the brain matter, due to a severe development of the hypertensive disease, brain traumas and disorders of brain circulation. It was established that the epileptical status appearing for the first time during the acute period of brain hemorrhages, has a complex mechanism. One of the conditions of its development is preceding focal and diffuse brain changes. Hemorrhages with oedema is a precipitating factor for a convulsive readiness of the brain.
