RESUMEN

Ameloblastic fibro-odontoma (AFO) is a rare, slow-growing neoplastic lesion classified as a benign, epithelial odontogenic mesenchymal tumor. This tumor exhibits histological features characteristic of both ameloblastic fibromas and complex odontomas. The clinical manifestation of AFO is typically characterized by the asymptomatic enlargement of the jawbones. Radiographically, it presents as a distinct radiolucent region, indicating the presence of radiopaque substances with varying degrees of irregularities in size and morphology. Standard therapeutic intervention involves enucleation. Despite its benign nature, AFO can cause significant morbidity if left untreated. Therefore, prompt diagnosis and appropriate management are essential to ensure optimal patient outcomes. The present study describes the case (clinical presentation and management) of an 18-year-old male patient with an AFO lesion located in the posterior mandible. This particular case was treated with conservative measures involving surgical enucleation along with the extraction of the impacted tooth and the curettage of residual bone.

In this paper, an encryption and trust evaluation model is proposed on the basis of a blockchain in which the identities of the Aggregator Nodes (ANs) and Sensor Nodes (SNs) are stored. The authentication of ANs and SNs is performed in public and private blockchains, respectively. However, inauthentic nodes utilize the network's resources and perform malicious activities. Moreover, the SNs have limited energy, transmission range and computational capabilities, and are attacked by malicious nodes. Afterwards, the malicious nodes transmit wrong information of the route and increase the number of retransmissions due to which the SNs' energy is rapidly consumed. The lifespan of the wireless sensor network is reduced due to the rapid energy dissipation of the SNs. Furthermore, the throughput increases and packet loss increase with the presence of malicious nodes in the network. The trust values of SNs are computed to eradicate the malicious nodes from the network. Secure routing in the network is performed considering residual energy and trust values of the SNs. Moreover, the Rivest-Shamir-Adleman (RSA), a cryptosystem that provides asymmetric keys, is used for securing data transmission. The simulation results show the effectiveness of the proposed model in terms of high packet delivery ratio.

The rapid rise of data value, such as social media and mobile applications, results in large volumes of data, which is what the term "big data" refers to. The increased rate of data growth makes handling big data very challenging. Despite a Bloom filter (BF) technique having previously been proposed as a space-and-time efficient probabilistic method, this proposal has not yet been evaluated in terms of big data. This study, thus, evaluates the BF technique by conducting an experimental study with a large amount of data. The results revealed that BF overcomes the efficiency not present in the space-and-time of indexing and examining big data. Moreover, to address the increase of false-positive rate in using BF with big data, a novel false-positive rate reduction approach is proposed in this paper. The initial experimental results of evaluating this method are very promising. The novel approach helped to reduce the false-positive rate by more than 70%.

Biologic agents are increasingly used for many autoimmune and inflammatory conditions, as they are both steroid sparing and can potentially induce and maintain remission. Notably tumor necrosis factor (TNF) alpha antagonists are particularly useful in inflammatory bowel disease (IBD) such as Crohn's disease (CD) and ulcerative colitis (UC). Infliximab is a chimeric monoclonal antibody that targets TNF alpha (cytokine involved in modulation of inflammatory responses) and neutralizes its effects. As infliximab is a generic TNF alpha inhibitor, it can cause non-specific immune mediated side effects in addition to its intended therapeutic effect on the target organ (i.e., the gut in IBD). We present a case of a gentleman developing a rare dermatological side effect of an acneiform reaction, after the use of infliximab for his CD. Monitoring anti-TNF alpha antibodies may help identify patients at a higher risk of developing adverse reactions. In addition, gut specific biologic agents (vedolizumab) may be the next preferable step in individuals with IBD who demonstrate reactions and/or intolerance to non-specific TNF alpha inhibitors.

Extrapulmonary sarcoidosis accounts for only two percent of the total sarcoid cases. Sarcoidosis affecting the musculoskeletal system alone is even rarer. Diagnosis is based on suggestive history, clinical features, imaging followed by confirmation of non-caseating granulomas on a biopsy. Steroids form the first line of treatment for patients where musculoskeletal system is involved. We report the case of a 26-year-old gentleman who presented with right foot pain and unintentional weight loss. A magnetic resonance imaging (MRI) scan of feet confirmed bilateral stress fractures of both feet. After a battery of investigations and resultant myriad constellation of investigation findings, the diagnosis of sarcoidosis was confirmed on a tissue biopsy obtained via bronchoscopy. This case report discusses the complex journey from presentation to diagnosis and subsequent treatment while also exploring important differentials that need to be ruled out in such scenarios.

Real time crowd analysis represents an active area of research within the computer vision community in general and scene analysis in particular. Over the last 10 years, various methods for crowd management in real time scenario have received immense attention due to large scale applications in people counting, public events management, disaster management, safety monitoring an so on. Although many sophisticated algorithms have been developed to address the task; crowd management in real time conditions is still a challenging problem being completely solved, particularly in wild and unconstrained conditions. In the proposed paper, we present a detailed review of crowd analysis and management, focusing on state-of-the-art methods for both controlled and unconstrained conditions. The paper illustrates both the advantages and disadvantages of state-of-the-art methods. The methods presented comprise the seminal research works on crowd management, and monitoring and then culminating state-of-the-art methods of the newly introduced deep learning methods. Comparison of the previous methods is presented, with a detailed discussion of the direction for future research work. We believe this review article will contribute to various application domains and will also augment the knowledge of the crowd analysis within the research community.

PURPOSE OF REVIEW: Moderate hypertriglyceridemia is exceedingly common in diabetes, and there is growing evidence that it contributes to residual cardiovascular risk in statin-optimized patients. Major fibrate trials yielded inconclusive results regarding the cardiovascular benefit of lowering triglycerides, although there was a signal for improvement among patients with high triglycerides and low high-density lipoprotein (HDL)-the "diabetic dyslipidemia" phenotype. Until recently, no trials have examined a priori the impact of triglyceride lowering in patients with diabetic dyslipidemia, who are likely among the highest cardiovascular-risk patients. RECENT FINDINGS: In the recent REDUCE IT trial, omega-3 fatty acid icosapent ethyl demonstrated efficacy in lowering cardiovascular events in patients with high triglycerides, low HDL, and statin-optimized low-density lipoprotein (LDL). The ongoing PROMINENT trial is examining the impact of pemafibrate in a similar patient population. Emerging evidence suggests that lowering triglycerides may reduce residual cardiovascular risk, especially in high-risk patients with diabetic dyslipidemia.

RESUMEN

Frank-Ter Haar Syndrome (FTHS) is a rare hereditary inherited disorder with many abnormalities. The main clinical features are brachycephaly, wide fontanels, prominent forehead, hypertelorism, prominent eyes, macrocornea with or without glaucoma, full cheeks, small chin, congenital heart defects, kyphoscoliosis, skeletal dysplasia, developmental delay, coccygeal skin folds and flexion deformity of the fingers. This case report describes Frank-Ter Haar syndrome in a 4 months old girl suffering from club foot, dysmorphism, prominent coccyx with skin fold, atrial septal defect, patent ductus asteriosus and megalocornea.
