This research assesses the capability of texture analysis (TA) derived from high-resolution (HR) T2-weighted magnetic resonance imaging to identify primary sequelae following 1-5 hours of controlled cortical impact mild or severe traumatic brain injury (TBI) to the left frontal cortex (focal impact) and secondary (diffuse) sequelae in the right frontal cortex, bilateral corpus callosum, and hippocampus in rats. The TA technique comprised first-order (histogram-based) and second-order statistics (including gray-level co-occurrence matrix, gray-level run length matrix, and neighborhood gray-level difference matrix). Edema in the left frontal impact region developed within 1 hour and continued throughout the 5-hour assessments. The TA features from HR images confirmed the focal injury. There was no significant difference among radiomics features between the left and right corpus callosum or hippocampus from 1 to 5 hours following a mild or severe impact. The adjacent corpus callosum region and the distal hippocampus region (s), showed no diffuse injury 1-5 hours after mild or severe TBI. These results suggest that combining HR images with TA may enhance detection of early primary and secondary sequelae following TBI.

BACKGROUND: Medical image analysis has evolved to facilitate the development of methods for high-throughput extraction of quantitative features that can potentially contribute to the diagnostic and treatment paradigm of cancer. There is a need for further improvement in the accuracy of predictive markers of response to neo-adjuvant chemotherapy (NAC). The aim of this study was to develop a radiomic classifier to enhance current approaches to predicting the response to NAC breast cancer. METHODS: Data on patients treated for breast cancer with NAC prior to surgery who had a pre-NAC dynamic contrast enhanced breast MRI were included. Response to NAC was assessed using the Miller-Payne system on the excised tumor. Tumor segmentation was carried out manually under the supervision of a consultant breast radiologist. Features were selected using least absolute shrinkage selection operator regression. A support vector machine learning model was used to classify response to NAC. RESULTS: 74 patients were included. Patients were classified as having a poor response to NAC (reduction in cellularity < 90%, n = 44) and an excellent response (> 90% reduction in cellularity, n = 30). 4 radiomics features (discretized kurtosis, NGDLM contrast, GLZLM_SZE and GLZLM_ZP) were identified as pertinent predictors of response to NAC. A SVM model using these features stratified patients into poor and excellent response groups producing an AUC of 0.75. Addition of estrogen receptor status improved the accuracy of the model with an AUC of 0.811. CONCLUSION: This study identified a radiomic classifier incorporating 4 radiomics features to augment subtype based classification of response to NAC in breast cancer.

PURPOSE: To assess the feasibility of delivering microwave ablation for targeted treatment of aldosterone producing adenomas using image-based computational models. METHODS: We curated an anonymized dataset of diagnostic 11C-metomidate PET/CT images of 14 patients with aldosterone producing adenomas (APA). A semi-automated approach was developed to segment the APA, adrenal gland, and adjacent organs within 2 cm of the APA boundary. The segmented volumes were used to implement patient-specific 3D electromagnetic-bioheat transfer models of microwave ablation with a 2.45 GHz directional microwave ablation applicator. Ablation profiles were quantitatively assessed based on the extent of the APA target encompassed by an ablative thermal dose, while limiting thermal damage to the adjacent normal adrenal tissue and sensitive critical structures. RESULTS: Across the 14 patients, adrenal tumor volumes ranged between 393 mm3 and 2,395 mm3. On average, 70% of the adrenal tumor volumes received an ablative thermal dose of 240CEM43, while limiting thermal damage to non-target structures, and thermally sparing 83.5-96.4% of normal adrenal gland. Average ablation duration was 293 s (range: 60-600 s). Simulations indicated coverage of the APA with an ablative dose was limited when the axis of the ablation applicator was not well aligned with the major axis of the targeted APA. CONCLUSIONS: Image-based computational models demonstrate the potential for delivering microwave ablation to APA targets within the adrenal gland, while limiting thermal damage to surrounding non-target structures.

Fibrous capsule (FC) formation, secondary to the foreign body response (FBR), impedes molecular transport and is detrimental to the long-term efficacy of implantable drug delivery devices, especially when tunable, temporal control is necessary. We report the development of an implantable mechanotherapeutic drug delivery platform to mitigate and overcome this host immune response using two distinct, yet synergistic soft robotic strategies. Firstly, daily intermittent actuation (cycling at 1 Hz for 5 minutes every 12 hours) preserves long-term, rapid delivery of a model drug (insulin) over 8 weeks of implantation, by mediating local immunomodulation of the cellular FBR and inducing multiphasic temporal FC changes. Secondly, actuation-mediated rapid release of therapy can enhance mass transport and therapeutic effect with tunable, temporal control. In a step towards clinical translation, we utilise a minimally invasive percutaneous approach to implant a scaled-up device in a human cadaveric model. Our soft actuatable platform has potential clinical utility for a variety of indications where transport is affected by fibrosis, such as the management of type 1 diabetes.

(1) Background: Multiparametric MRI (mp-MRI) is used to manage patients with PCa. Tumor identification via irregular sampling or biopsy is problematic and does not allow the comprehensive detection of the phenotypic and genetic alterations in a tumor. A non-invasive technique to clinically assess tumor heterogeneity is also in demand. We aimed to identify tumor heterogeneity from multiparametric magnetic resonance images using texture analysis (TA). (2) Methods: Eighteen patients with prostate cancer underwent mp-MRI scans before prostatectomy. A single radiologist matched the histopathology report to single axial slices that best depicted tumor and non-tumor regions to generate regions of interest (ROIs). First-order statistics based on the histogram analysis, including skewness, kurtosis, and entropy, were used to quantify tumor heterogeneity. We compared non-tumor regions with significant tumors, employing the two-tailed Mann-Whitney U test. Analysis of the area under the receiver operating characteristic curve (ROC-AUC) was used to determine diagnostic accuracy. (3) Results: ADC skewness for a 6 × 6 px filter was significantly lower with an ROC-AUC of 0.82 (p = 0.001). The skewness of the ADC for a 9 × 9 px filter had the second-highest result, with an ROC-AUC of 0.66; however, this was not statistically significant (p = 0.08). Furthermore, there were no substantial distinctions between pixel filter size groups from the histogram analysis, including entropy and kurtosis. (4) Conclusions: For all filter sizes, there was poor performance in terms of entropy and kurtosis histogram analyses for cancer diagnosis. Significant prostate cancer may be distinguished using a textural feature derived from ADC skewness with a 6 × 6 px filter size.

RATIONALE AND OBJECTIVE: This study aimed to assess the quality of Internet information about common radiological investigations. MATERIALS AND METHODS: Four search engines (Google, Bing, Yahoo, and Duckduckgo) were searched using the terms "X-ray," "cat scan," "MRI," "ultrasound," and "pet scan." The first 10 webpage results returned for each search term were recorded, and their quality and readability were analyzed by two independent reviewers (DJB and LCY), with discrepancies resolved by consensus. Analysis of information quality was conducted using validated instruments for the assessment of health-care information (DISCERN score is a multi-domain tool for assessment of health-care information quality by health-care professionals and laypeople (max 80 points)) and readability (Flesch-Kincaid and SMOG or Simple Measure of Gobbledygook scores). The search result pages were further classified into categories as follows: commercial, academic (educational/institutional), and news/magazine. Several organizations offer website accreditation for health-care information, and accreditation is recognized by the presence of a hallmark or logo on the website. The presence of any valid accreditation marks on each website was recorded. Mean scores between groups were compared for significance using the Student t test. RESULTS: A total of 200 webpages returned (108 unique website addresses). The average DISCERN score was <50 points for all modalities and search engines. No significant difference was seen in readability between modalities or between search engines. Websites carrying validated accreditation marks were associated with higher average DISCERN scores: X-ray (39.36 vs 25.35), computed tomography (45.45 vs 31.33), and ultrasound (40.91 vs 27.62) (P < .01). Academic/government institutions produced material with higher DISCERN scores: X-ray (40.06 vs 22.23), magnetic resonance imaging (44.69 vs 29), ultrasound (46 vs 31.91), and positron emission tomography (45.93 vs 38.31) (P < .01). Commercial websites produced material with lower mean DISCERN scores: X-ray (17.25 vs 31.69), magnetic resonance imaging (20.8 vs 40.1), ultrasound (24.11 vs 42.35), and positron emission tomography (24.5 vs 44.45) (P < .01). CONCLUSIONS: Although readability is adequate, the overall quality of radiology-related health-care information on the Internet is poor. High-quality online resources should be identified so that patients may avoid the use of poor-quality information derived from general search engine queries.

This case report describes a young man presenting with iliac fossa pain secondary to iliac vein thrombus due to inferior vena cava aplasia. No other identifiable risk factor or cause for deep vein thrombosis was demonstrated.

Falls on the outstretched hand, with resultant pain in the carpal region, account for a significant number of referrals to emergency rooms worldwide. Not only do they represent a significant proportion of the radiological workload arising from emergency rooms, interpretation of the images acquired is often difficult due to the complex anatomy of this region, compounded by an inability to obtain adequate views due to patient discomfort. Often, despite apparently normal radiological examinations, patient discomfort persists, prompting a need for further imaging. It is vital that the radiologist be entirely familiar with the bony and ligamentous anatomy of this body region, as well as possess an understanding of the frequent mechanisms of injury. Using a variety of imaging techniques, we illustrate a spectrum of carpal injuries, common and otherwise, explaining the mechanism and typical appearances of each.

Mangafodipir, a manganese-containing hepatobiliary contrast agent, is excreted in bile. We review the principles and practice of a contrast-enhanced MRC technique using mangafodipir and compare it with standard T2-weighted magnetic resonance cholangiography (MRC) sequences. Potential applications include the evaluation of leaks and strictures; the assessment of drainage in normal, surgically by-passed, stented and obstructed biliary systems; the diagnosis of cholecystitis; and the evaluation of normal and variant biliary anatomy.

BACKGROUND AND OBJECTIVES: The anterior approach to the sciatic nerve block may be associated with a high failure rate because the nerve lies posterior to the lesser trochanter of the femur at the level of needle insertion. However, previous work using cadavers demonstrated that internal rotation of the leg renders the nerve more accessible to the anterior approach. METHODS: Ten volunteers consented to undergo magnetic resonance imaging. Markers were placed on the surface where a needle would have been inserted for an anterior approach to the sciatic nerve. Three scans were then performed: the first with both legs in the neutral position, the second with maximal bilateral internal rotation at the hip, and the third with maximal bilateral external rotation at the hip. RESULTS: Examination of the scans by a consultant radiologist showed that, as the thigh is rotated, the number of scans showing an unobstructed needle passage from the skin marker to the sciatic nerve rate increased from 5% in external rotation to 85% in internal rotation. The number of times the needle path passed through femoral neurovascular bundle also fell from 55% in external rotation to 15% in internal rotation. CONCLUSIONS: The results confirm that, as the thigh is moved from an externally to an internally rotated position, the sciatic nerve becomes more accessible by the anterior approach at the level of the lesser trochanter, and the risk of femoral artery or nerve puncture is reduced but not eliminated.

PURPOSE: To evaluate differences in the magnitude and time course of renal cortical contrast uptake in patients with minimal, moderate, and severe renal artery stenosis (RAS) using contrast-enhanced magnetic resonance renography (CE-MRR). MATERIALS AND METHODS: CE-MRR was performed on 56 patients with renovascular disease using a three-dimensional volume interpolated breath-hold examination (VIBE) perfusion sequence. After administration of 2 mL of contrast, nine sequential axial VIBE datasets were acquired: at baseline, 7, 14, 21, 45, 60, 120, 180, and 240 seconds. Aortic peak signal enhancement and cortical peak signal enhancement through the mid portion of each kidney was recorded, along with the time delay between each peak. Each renal artery was subsequently examined using three-dimensional contrast-enhanced MR angiography, and graded as being minimally (0%-30%), moderately (31%-70%), or severely (71%-100%) stenotic. RESULTS: When the data were subdivided by RAS category, the cortical to aortic peak enhancement ratio (CAPR) reduced with increasing RAS. Further, the cortical to aortic time delay (CATD) increased with increasing RAS. These measurements were statistically significant between patients with minimal and moderate RAS compared to severe RAS CONCLUSION: CE-MRR can assist in the differentiation of patients with minimal or moderate RAS from those with severe RAS.

PURPOSE: To determine the effect of age, sex, and presence of carotid atheromatous disease on the presence of aortic spiral blood flow pattern using two-dimensional flow quantitative magnetic resonance imaging (MRI). MATERIALS AND METHODS: Sixty subjects (37 women, 23 men) were examined. Prospective phase contrast flow quantitative MRI (1.5 T, Siemens Symphony) sequences in the plane of the aortic arch, and three-dimensional contrast-enhanced MR angiography of the vessels from the aortic arch to the circle of Willis, were performed. Flow quantitative analysis, using circular regions of interest, in the root, apex, and descending aortic arch to determine the presence of a spiral blood flow pattern was undertaken. The results were correlated with the subjects age, sex, and presence of significant carotid arterial disease. RESULTS: A spiral blood flow pattern was seen during diastole in 43 of 50 (86%), 42 of 48 (88%), and in 26 of 28 (93%) subjects in the root, apex, and descending aortic arch, respectively. Spiral flow was seen during systole in 14 of 35 (40%), 20 of 47 (42%), and 11 of 31 (35%) subjects in the root, apex, and descending aortic arch, respectively. There was no clear effect of age or sex on the presence of spiral flow. Carotid disease was associated with a significant reduction in the prevalence of systolic spiral flow from 51%-19% subjects (P < 0.05). CONCLUSION: Spiral flow pattern can be seen in the arch of the aorta in clinical practice using flow quantitative MRI. The prevalence of spiral flow pattern does not appear affected by subject age or sex. Carotid atheromatous disease is associated with a reduction in prevalence of systolic spiral flow pattern in the aortic arch.
