RESUMEN

The U.S. Ending the HIV Epidemic initiative was launched in 2019 with the goal of reducing new HIV infections by 90% by 2030. In this retrospective chart review in 2019, we determined the incidence of HIV testing and PrEP counseling among patients without known HIV who were diagnosed with gonorrhea or chlamydia in a safety-net health care system. Our results showed that 58% of positive gonorrhea and chlamydia tests had a follow-up HIV test within 90 days and 19% of individuals testing positive for gonorrhea received PrEP education within 180 days. Initial care in an emergency department and female gender were associated with a lower likelihood of follow-up HIV testing and PrEP counseling. There are missed opportunities to offer HIV testing and PrEP education to those at risk of HIV acquisition.

We present two cases of Babesia-induced splenic injury at a single institution. In the late summer, two patients presented with left-sided abdominal pain radiating to the shoulder. They were both found to have hemolytic anemia, thrombocytopenia, and acute splenic infarction on imaging. Blood smears showed intracellular ring forms consistent with Babesia spp. and low parasitemia (<1%). Diagnosis was confirmed by PCR for Babesia microti. Both patients improved with azithromycin and atovaquone, without blood products or surgical intervention. Several weeks following treatment, repeat blood smears revealed no parasites. Splenic infarct and hemorrhage have been previously reported as rare complications of babesiosis. However, given the steady rise in Babesia microti cases in the USA, even these rare complications will become more prevalent. We review both the diagnosis and management of Babesia-induced splenic complications, which can be challenging in patients with low-level parasitemia. Clinicians should consider babesiosis as a cause of atraumatic splenic injury.

RESUMEN

Hepatitis C virus (HCV) is the most common blood-borne virus in the U.S., and its incidence continues to rise. With approval of direct-acting antiviral medications, treatment for Chronic Hepatitis C (CHC) has become highly efficacious with a minimal side effect profile. Primary care physicians are well-positioned to address this increased demand, yet most do not feel comfortable treating CHC. In this case report, we describe implementation of a multidisciplinary team-based approach for treating CHC at multiple primary care sites across a large safety net health system. We focus on the evolving nature of implementation of our model through iterative Plan-Do-Study-Act (PDSA) cycles, highlighting the importance of developing an interdependent, multidisciplinary team, providing training, and ongoing support of Primary Care Hepatitis C Specialists, responding to the evolving nature of CHC treatment and policies, and ensuring high quality treatment. This process allowed us to continually grow and adapt our approach to make it feasible and successful. We share our "lessons" learned for others looking to bring CHC treatment, and potentially other specialty-based treatment, into the primary care setting.
