RESUMEN

In this paper, the authors explore the depiction of perversion and the associated interplay of life and death drives in Roman Polanski's 1992 film Bitter Moon. To begin with, a theoretical discussion is presented regarding perverse organizations of mastery and sadomasochism. Perversion is viewed as an expression of the death drive under erotic disguise, in which the destructive fingerprint of the death drive is revealed at every stage, having as its ultimate purpose the destruction of the other. Based on these theoretical insights a dialogue is developed with Polanski's film, which brings to life the theory of sadomasochistic relations through the multidimensional aesthetic medium of cinema. It is shown how Polanski's cinematic oeuvre conveys the essence of the difficult and complex experience of perverse relations, where the life and death drives and their transformations are manifested. The portrayal of the sadomasochistic relations in this film contributes to the experiential knowledge with which the authors promote insight that would potentially enrich the clinical work with patients with perverse organizations.

PURPOSE: To examine the rate of adherence of Israeli dentists who treat children and their rate of adherence, in 2010-2011 and in 2016, to changes made to the 2008 American Heart Association (AHA) guidelines for endocarditis prevention. METHODS: A questionnaire on adherence to AHA guidelines was handed out in two consecutive continuing education courses in 2010-2011 and again in 2016. Data were analyzed using student's t test, chi-square, and analysis of variance. RESULTS: Response rates were 62 percent in 2010 -2011 and 73 percent in 2016. Residents and pediatric dentists adhered more to the guidelines than general dentists (P=.036 and P=0.034 in 2011 and 2016, respectively). Knowledge of guidelines for dental procedures that required antibiotic prophylaxis was better than for medical conditions (52.8 percent and 79.6 percent, respectively, in 2011; 64 percent and 80 percent, respectively, in 2016). Lowest adherence was scored for prophylaxis in moderate-risk patients (32.8 percent in 2010 to 2011, 26 percent in 2016; P=.151), which was a pivotal change in the updated guidelines. Participants chose to consult with either a pediatrician or a cardiologist in 77.4 percent and 62.2 percent of the listed medical conditions in 2010 to 2011 and in 2016, respectively (P=.055). CONCLUSIONS: Approximately a third of dentists who treat children in Israel did not follow the AHA guidelines' fundamental changes. Most dentists tended to consult physicians prior to prophylaxis administration for moderate- and high-risk groups.

RESUMEN

The authors discuss the relation between perverse psychic formations and the ability to develop a mind of one's own. The authors characterize the formation of perversity in terms of failure to develop the key capacities of thinking, mourning, and loving. These failures result in the abolishment of thinking and the repudiation of separateness and lead to the creation of different kinds of twisted coalitions, which shape the transference-counter-transference matrix. Persons with perverse psychic organizations have difficulties developing their own minds due to their refusal to acknowledge human limitations and their inability to accept the fundamental differences of human existence: self-other, child-adult, male-female. These characterizations are illustrated by clinical material, including a detailed analytic session, demonstrating the perverse aggresivization and sexualization of the analytic relationship. The role of the analyst is to detoxify the violence and destruction and to translate the language of perversion into the language of love.

BACKGROUND/AIM: Self-injurious behavior (SIB) is a serious and chronic condition frequently seen in special needs populations, affecting 10% to 17% of individuals diagnosed with intellectual and/or developmental disabilities. A 2.5-year-old infant with SIB, whose presenting symptoms were severe tongue and lip lacerations accompanied by much hemorrhage, is presented here to illustrate the problem and to show how this may be prevented. MATERIALS AND METHODS: An appliance is described which effectively limits the damage caused by SIB and permits rapid healing of existing injuries. RESULTS: The method provides for a stable, retentive, and comfortable device on the infant's undererupted and largely nonretentive crowns of the deciduous teeth as well as for all permanent teeth in children, adolescents, and adults. CONCLUSIONS: The appliance has been successfully employed for the past 10 years in patients with SIB who have attended for treatment in the Special Needs Clinic in our Department.

RESUMEN

This article weaves together two threads: the intricacies of the analysis of a difficult-to-reach yet extraordinary patient and the literary works of Jorge Luis Borges, which played a significant role in the analysis as a source of inspiration, enriching the analyst's reverie and opening up new psychic spaces. The authors demonstrate the analyst's recourse to several of Borges's stories in order to enrich his own inner world and to better understand the analysand. Some of these stories are briefly presented through the analyst's dialogue with them, and there is a discussion of their function in facilitating the process of working through issues of time, memory, mortality, and identity, contributing to the enhancement of the patient's ability to come face to face with the unwanted, split-off parts of his self and of reality.
