RESUMEN

BACKGROUND: Lateral wall fractures represent crucial risk factors for postoperative internal fixation failure in intertrochanteric femoral fractures. However, no consensus exists on the type of lateral wall fracture requiring interventional management. This study aimed to investigate the effect of residual lateral wall volume on the postoperative stability of intertrochanteric femur fractures with associated lateral wall fractures, providing valuable reference for the clinical management of the lateral wall. METHODS: Eleven bone defect models of intertrochanteric femur fractures with varying residual lateral wall volumes were constructed using finite element analysis. These models were fixed with proximal femoral nail antirotation (PFNA). Simulations of von Mises stress and displacement distribution of the PFNA and femur during normal walking were conducted. Statistical analysis was performed to assess the correlation between volume and the maximum von Mises stresses and displacements of the PFNA and femur. RESULTS: In all 11 models, the maximum von Mises stress and displacement of the helical blade, intramedullary nail, and femur occurred at the same locations. As residual lateral wall volume increased, the maximum von Mises stress and displacement of the helical blade, intramedullary nail, and maximum femoral displacement gradually decreased. However, the overall trend of the maximum femoral von Mises stress gradually decreased. At 70% retention of the residual lateral wall volume, there was a more pronounced change in the value of the maximum stress change of the helical blade and the intramedullary nail. Statistical analysis, including the Shapiro-Wilk test and Pearson correlation analysis, demonstrated a significant negative correlation between volume and the maximum von Mises stress and displacement of the helical blade, intramedullary nail, and femur. Linear regression analysis further confirmed this significant negative correlation. CONCLUSION: Finite element analysis of the residual lateral wall revealed a significant correlation between volume and the postoperative stability of intertrochanteric femur fractures. A volume of 70% may serve as the threshold for stabilizing the residual lateral wall. Volume emerges as a novel index for evaluating the strength of the residual lateral walls.

The flowering period is one of the important indexes of wheat breeding. The early or late flowering affects the final yield and character stability of wheat. In order to solve the problem that it is difficult to accurately and quickly detect the flowering period of a large number of wheat breeding materials, a determination method of flowering period for field wheat based on the improved You Only Look Once (YOLO) v5s model was proposed. Firstly, a feature fusion (FF) method combing RGB images and corresponding comprehensive color features was proposed to highlight more texture features and reduce the distortion caused by light on the extracted feature images. Second, the YOLOv5s model was selected as a base version of the improved model and the convolutional block attention model (CBAM) was adopted into the feature fusion layer of YOLOV5s model. Florets and spikelets were given greater weight along the channel and spatial dimensions to further refine their effective feature information. At the same time, an integrated Transformer small-target detection head (TSDH) was added to solve the high miss rate of small targets in wheat population images. The accurate and rapid detection of florets and spikelets was realized, and the flowering period was determined according to the proportion of florets and spikelets. The experimental results showed that the average computing time of the proposed method was 11.5ms, and the average recognition accuracy of florets and spikelets was 88.9% and 96.8%, respectively. The average difference between the estimated flowering rate and the actual flowering rate was within 5%, and the determination accuracy of the flowering period reached 100%, which met the basic requirements of the flowering period determination of wheat population in the field.

BACKGROUND: We aimed to evaluate the effect of different capnoperitoneum pressures on stress responses in pediatric laparoscopic inguinal hernia repair. METHODS: In this prospective randomized controlled study, 68 children with indirect inguinal hernia who underwent high ligation of hernia sac were randomly divided into 3 groups: high-pressure group (12 mm Hg, HP group, n = 26); low-pressure group (8 mm Hg, LP group, n = 20); open operation group (OP group, n = 22). Heart rate (HR), blood pressure, and end-tidal CO2 (PetCO2) were recorded, as well as the levels of adrenocorticotropic hormone (ACTH) and cortisol (COR) were measured by ELISAs before operation, during operation, and after operation, respectively. RESULTS: After establishing capnoperitoneum, HR, blood pressure, and PetCO2 were significantly increased in the HP group compared with the OP and LP groups (P < 0.05). Comparing the intraoperatively measured ACTH and COR concentrations of the HP group to the LP group, we noted higher values in the first (P < 0.05). There was no significant difference in the postoperative concentrations of ACTH and COR among the HP, LP, and OP groups. CONCLUSIONS: Laparoscopic surgery under LP capnoperitoneum or open operation may reduce stress responses and are superior to HP capnoperitoneum.
