RESUMEN

Hypertriglyceridemia (HTG) is a common dyslipidemia associated with an increased risk of cardiovascular disease and pancreatitis. It is well stablished that the severe cases of disease often present with an underlying genetic cause. In this study, we determined the frequency and variation spectrum of genes involved in the triglyceride metabolism in a series of Brazilian patients with severe HTG. A total of 212 patients with very high HTG, defined with fasting triglycerides (TG) ≥ 880 mg/ dL, that underwent a multi-gene panel testing were included in this research. Germline deleterious variants (i.e. Pathogenic/Likely Pathogenic (P/LP) variants) were identified in 28 out of 212 patients, reflecting an overall diagnostic yield of 13% in our cohort. Variants of unknown significance (VUS) were identified in 87 patients, and represent 80% of detected variants in this dataset. We confirm the LPL as the most frequently mutated gene in patients with severe HTG, and we had only one suspected case of familial chylomicronemia syndrome, caused by a homozygous variant in LMF1, in our cohort. Notably, we report 16 distinct and novel variants (P/LP and VUS), each of them representing a single case, not previously reported in any public databases or other studies. Our data expand our knowledge of genetic variation spectrum in patients with severe HTG in the Brazilian population, often underrepresented in public genomic databases, being also a valuable clinical resource for genetic counseling and healthcare programs in the country.

Biomass resulting from food production represents valuable material to recover different biomolecules. In our study, we used apple pomace to obtain pectin, which is traditionally extracted using mineral acids. Our hypothesis consisted of carrying out extractions with organic acids, assisted by ultrasound, by varying processing parameters including time, temperature, and type of acid. The analytical determinations of galacturonic acid content, methoxylation and esterification degree, ζ-potential and extraction yield were used as pectin quality indicators. Pectins extracted using treatment conditions with better performance were assessed biologically in vitro for their potential to be used in biomedical applications. Overall, the extracted pectin presented a galacturonic acid content, methoxylation and esterification degree ranged from 19.7 to 67%, 26.8-41.4% and 58-65.2% respectively, and were negatively charged (-24.1 to -13.2 mV). It was found that factors of time and temperature greatly influenced the response variables excepting the esterification degree, while the acid type influenced the ζ-potential, methoxylation and esterification degrees. Additionally, it was seen that the longer extraction time (50 min) and higher temperature (50 °C) exhibited the better extraction yield (∼10.9%). Finally, the selected pectin showed high cytocompatibility up to 500 µg/mL of concentration when seeded with Neonatal Normal Human Dermal Fibroblasts.

Physically-induced depolymerisation procedures are often preferred for obtaining alginate and chitosan oligosaccharides as they either do not use or make minimal use of additional chemicals; therefore, separation of the final products is facile. In this work, solutions of three types of alginate with different mannuronic and guluronic acid residues ratio (M/G ratio) and molecular weights (Mw) and one type of chitosan were non-thermally processed by applying high hydrostatic pressures (HHP) up to 500 MPa (20 min) or pulsed electric fields (PEF) up to 25 kV cm-1 (4000 µm) in the absence or presence of 3 % hydrogen peroxide (H2O2). The impact on the physicochemical properties of alginate and chitosan was investigated by rheology, GPC, XRD, FTIR, and 1H NMR. In the rheological investigations, the apparent viscosities of all samples decreased with increasing shear rate, indicating a non-Newtonian shear-thinning behaviour. GPC results reported Mw reductions that ranged between 8 and 96 % for all treatments. NMR results revealed that HHP and PEF treatment predominantly reduced the M/G ratio of alginate and the degree of deacetylation (DDA) of chitosan, whilst H2O2 promoted an increase in the M/G ratio in alginate and DDA of chitosan. Overall, the present investigation has demonstrated the feasibility of HHP and PEF for rapidly producing alginate and chitosan oligosaccharides.

INTRODUCTION: Whether political, scientific and medical development in a country is associated with better clinical results according to gender in patients with COVID-19 has not yet been clearly elucidated. OBJECTIVE: To determine the trends of COVID-19-related in-hospital mortality in women and men from March 2020 to February 2022. METHODS: Clinical data of all patients with COVID-19 cared for at 21 Spanish hospitals were used, both of those who were discharged and of those who died during hospitalization. The association between hospital length of stay and mortality was analyzed with logistic regression models. RESULTS: Out of 7,974 patients that were included, 3,234 were women; 928 patients died. A significant decreasing trend in mortality was identified. When the analysis was carried out by gender, no significant mortality trend was found in women (OR = 0.96 [0.90-1.03], p = 0.239), while in men there was a significant decreasing trend identified (OR = 0.87 [0.82-0.92], p < 0.001). CONCLUSION: Health policies, together with clinical and preventive interventions, may explain these results. Response to treatment and behavioral differences may explain why mortality does not decrease for women.

Amikacin is the antibiotic of choice for the treatment of Gram-negative infections, namely, those in neutropenic oncology patients. No populational pharmacokinetic studies are currently available reporting amikacin pharmacokinetics in neutropenic oncology patients despite their specific pathophysiological features and treatments. A large-scale retrospective study was herein conducted to specifically investigate the effects that tumor diseases have on the pharmacokinetic parameters of amikacin and identify whether chemotherapy, the lag time between administration of chemotherapy and amikacin, age and renal function contribute to amikacin pharmacokinetics in neutropenic cancer patients. A total of 1180 pharmacokinetic analysis from 629 neutropenic patients were enrolled. The daily dose administered to oncology patients was higher than that administered to non-oncology patients (p < 0.0001). No statistical differences were found in amikacin concentrations, probably because drug clearance was increased in cancer patients (p < 0.0001). Chemotherapy influenced amikacin pharmacokinetics and drug clearance decreased as the lag time enhanced. The elderly group revealed no statistical differences between the doses administered to both the oncology groups, suggesting that the impact of ageing is stronger than chemotherapy. Our research suggests that cancer patients require higher initial doses of amikacin, as well as when chemotherapy is received less than 30 days before amikacin treatment has started.

Physical activity and exercise contribute to the accumulation of cognitive reserve, which is instrumental for preserving cognitive health in old age. In a large sample of 701 older adults (mean age = 70.36 years), we investigated whether the relationship between having experienced a fall in the past and lower performance in cognitive functioning was mediated via less physical activity engagement as a cognitive reserve contributor. General cognition was assessed using the mini-mental state examination (MMSE), long-term memory using a word-pair delayed recall test and working memory using a backward digit-span test. In face-to-face interviews, individuals reported information on falls during the past 12 months and their habitual physical activity engagement. Our analyses demonstrated that the relationship between having experienced a fall in the past and lower performance in the cognitive functioning measures was partly mediated (by 16.3% for general cognition, 30.6% for long-term memory, and 33.1% for working memory, respectively) via less physical activity engagement. In conclusion, we suggest as a core bio-psychological mechanism that experiencing a fall at an older age is a critical life event that hinders sufficient physical activity engagement and thereby impedes cognitive reserve build-up, resulting in lower cognitive functioning outcomes.

A síndrome da pele escaldada estafilocócica é um distúrbio bolhoso induzido por toxinas esfoliativas associado à infecção por Staphylococcus aureus, cujo exato mecanismo ainda é incerto manifestando-se com a formação de lesões bolhosas difusas pelo corpo que, usualmente, afeta crianças menores de 5 anos. O caso relatado apresenta uma criança de 4 meses de idade que manifestou lesões na pele de início recente, com hiperemia e bolhas no tronco, membros superiores e face, as quais evoluíram com descamação, além de edema peripalpebral, secreção ocular bilateral e crostas em região perioral. O prognóstico é favorável e o tratamento consiste em antibioticoterapia sistêmica ou oral, além de terapia de suporte na vigência de alterações hidroeletrolíticas e da termorregulação. Como o paciente apresentou melhora das lesões e encontrava-se em bom estado geral, o esquema foi trocado por Sulfametoxazol + Trimetoprima após o antibiograma evidenciar sensibilidade, por mais 7 dias a nível ambulatorial. O paciente apresentou boa resposta ao tratamento e evoluiu sem cicatrizes.


Staphylococcal scalded skin syndrome is a bullous disorder induced by exfoliative toxins associated with Staphylococcus aureus infection, its exact mechanism of which is still uncertain, manifesting itself with the formation of diffuse bullous lesions throughout the body, that usually affects children under 5 years. The case reported presents a 4-month-old child who presented skin lesions of recent onset, with hyperemia and blisters on the trunk, upper limbs and face, which evolved with scaling, in addition to peripalpebral edema, bilateral eye secretion and crusts in the perioral region. The prognosis is favorable and the treatment consists of systemic or oral antibiotic therapy, in addition to supportive therapy in the presence of hydroelectrolytic changes and thermoregulation. As the patient had improved lesions and was in good general condition, the scheme was replaced by Sulfamethoxazole + Trimethoprim, after the antibiogram showed sensitivity, for another 7 days on an outpatient basis. The patient presented good response to treatment and evolved without scars.

This study investigated the impact of heterogeneity of crosslinking on a range of physical and mechanical properties of calcium alginate networks formed via external gelation with 0.25-2% sodium alginate and 2.5 and 5% CaCl2. Crosslinking in films with 1-2% alginate was highly heterogeneous, as indicated by their lower calcium content (35-7 mg Ca·g alginate-1) and apparent solubility (5-6%). Overall, films with 1-2% alginate showed higher resistance (tensile strength = 51-147 MPa) but lower elasticity (Elastic Modulus = 2136-10,079 MPa) than other samples more homogeneous in nature (0.5% alginate, Elastic Modulus = 1918 MPa). Beads with 0.5% alginate prevented the degradation of ß-carotene 1.5 times more efficiently than 1% beads (5% CaCl2) at any of the storage temperatures studied. Therefore, it was postulated that calcium alginate networks crosslinked to a greater extent and in a more homogeneous manner showed better mechanical performance and barrier properties for encapsulation applications.

Quality of life (QoL) is a matter of concern in both healthy and diseased individuals. Lifestyle factors such as physical activity and sleep have a direct impact on QoL. In this context, interactions between activity time expenditure and QoL might be different in comorbid and non comorbid patients. Besides, the quantification and evaluation of time expenditure is ordinarily measured as the absolute time devoted to each activity. The objective of this study is the evaluation of the influence and interactions of activity-relative time expenditure and co-morbidity in Physical QoL.The study involved 302 consecutive patients, from an Internal Medicine ambulatory evaluation. Validated questionnaires were used to collect demographic variables and time expenditure variables. QoL was gathered with de survey short form-36questionnaire. Comorbidity was compiled with de Charlson Comorbidity Index. SPSS v20.0 was used for statistical analysis.As hypothesized, healthy subjects had higher Physical QoL score than comorbid subjects (P < .05). Physical activity and sleep relative time expenditure were statistically significant and associated to a better QoL in comorbid patients (P < .05). Interestingly, sleep was found to have statistically significant interaction with a score of ≥2 in the Charlson Comorbidity Index. Age, gender, comorbidity, physical activity relative time expenditure, and the interaction between relative time dedicated to sleep and comorbidity were found statistically significant in a multivariate model on Physical QoL prediction.Activity-relative time expenditure could be an adequate measure of daily activity pattern in the evaluation of QoL. Relative time spent in physical activity and sleep might be positively associated to Physical QoL. Sleep and comorbidity could have a statistically significant interaction in the prediction of Physical QoL.

Flash glucose monitoring system (FGMS) is an improved subset of continuous glucose monitoring with a recognized effectiveness on glycemic control, though validation in patients with Liver Cirrhosis (LC) is lacking. To evaluate the accuracy of FGMS in patients with Type 2 Diabetes Mellitus (DM) and LC, a prospective, case-control study was performed in 61 ambulatory patients with LC and DM (LC group, n = 31) or DM (Control group, n = 30). During 14 days, patients performed 4 assessments per day of self-monitoring of blood glucose (SMBG, reference value) followed by FGMS scanning. There were 2567 paired SMBG and FGMS values used in the accuracy analysis, with an overall mean absolute relative difference (MARD) of 12.68% in the LC group and 10.55% in the control group (p < 0,001). In patients with LC, the percentage of readings within Consensus Consensus Error Grid analysis Zone A and A + B were 80.36% and 99,26%, respectively. Sensor clinical accuracy was not affected by factors such as body mass index, age, gender, Child-Pugh score or edematoascitic decompensation. This is the first study to approach FGMS clinical accuracy in LC, revealing a potential usability of this system to monitor glycemic control in this population.

Acute non-cardiogenic pulmonary edema is a rare clinical entity which sometimes complicates the perioperative period. Even though one of the main causes is the acute obstruction of the airway, which may occur during a laryngospasm event, its etiology is varied and must be known for an adequate clinical management. We report the case of an 18 year old patient who developed postoperative acute non-cardiogenic pulmonary edema, probably of mixed etiology.


El edema agudo de pulmón (EAP) no cardiogénico es una entidad poco habitual que complica en ocasiones el período perioperatorio. Aunque una de las principales causas es la obstrucción aguda de la vía aérea que puede ocurrir durante un episodio de laringoespasmo, su etiología es variada y debe ser conocida para un adecuado manejo clínico. Presentamos el caso de un paciente de 18 años que desarrolló un EAP no cardiogénico postoperatorio, de etiología probablemente mixta.

The objectives of this study were: (1) to investigate the age-related differences in cognitive function (CF), nutritional status (MNA), physical activity (AF), quality of life (QoL), depression, social sat-isfaction (SS) and socioeconomic status (SES), and (2) to explore the relationships between CF and the previous variables. This cross sectional study included 268 men and 433 women (aged 71.4 ± 7.0 years). CF was determined with the Cognitive Telephone Screening Instrument (COGTEL) and the Mini-Mental State Examination (MMSE). Correlates were as follows: Mini Nutritional Assessment (MNA), PA (Baecke questionnaire modified for older adults), Quality of life (QoL SF-12), Geriatrics Depression Scale (GDS), Satisfaction and Social Support Scale, and Socioeconomic status (SES). All instruments were applied in a face to face interview. An independent t-test identi-fied significantly higher scores in young-old adults (≤ 69 years) for CF (p < 0.001), PA (p= 0.046) and SES (p= 0.007), compared to old-old adults (≥ 70 years). The results of multiple linear regression analysis indicated that the most significant CF correlates were SES (ß = 0.45;p< 0.001), age (ß = -0.12;p< 0.001), SS (ß = 0.12;p= 0.001), GDS (ß = -0.11; p= 0.003) and QoL (ß = 0.08;p= 0.017). The overall regression model explained 36% of the total variance in the COGTEL. The oldest and the more depressed adults obtained lower scores for FC. The present study suggests that, between the correlates studied, SES was the strongest predictor in the explanation of CF in older adults


Os objetivos deste estudo foram: (1) investigar as diferenças associadas à idade na função cognitiva (FC), status nutricional (MNA), atividade física (AF), qualidade de vida (QV ), depressão, satisfação social (SS) e estatuto socioeconômico (ESSE), e (2) explorar as relações entre a FC e as varáveis anteriores. Este estudo, de natureza transversal, incluiu 268 homens e 433 mulheres (71,4 ± 7,0 anos de idade). A FC foi determinada a partir do Cognitive Telephone Screening Instrument (COGTEL) e do Mini-Mental State Examination (MMSE). Os preditores analisados incluíram: o Mini Nutritional Assessment (MNA), a AF (questionário de Baecke modificado para idosos), a QV (SF-12), a Escala de Depressão em Geriatria (GDS), a Escala de Satisfação com o Suporte Social e o ESSE (Estatuto Socioeconômico). Todos os instrumentos foram aplicados sob forma de entrevista. Um t-teste de medidas independentes identificou scores significativamente mais elevados nos adultos-idosos mais jovens (60 - 69 anos de idade) na FC (p < 0,001), AF (p = 0,046) e ESSE (p = 0,007), comparativamente aos mais idosos (70 - 91 anos de idade). Os resultados da análise de regressão linear múltipla indicaram que os preditores mais significativos da FC foram: ESSE (ß = 0,45; p < 0,001), idade (ß = -0,12; p < 0,001), SS (ß = 0,12; p = 0,001), GDS (ß = -0,11; p = 0,003) e QV (ß = 0,08; p = 0,017). O modelo de regressão testado, explicou 36% da variância total do COGTEL. As pessoas mais idosas e deprimidas obtiveram scores mais baixos na FC. Este estudo sugere que entre os preditores estudados, o ESSE é a variável mais forte na explicação da FC nos adultos idosos

OBJECTIVES: (1) To study the relation of physical activity (PA) to high-density lipoprotein cholesterol (HDL-C) and (2) to investigate if the strength of these associations holds after adjustments for sex, age, and other key correlates. METHODS: This study included 550 older adults from Amazonas. HDL-C was derived from fasting blood samples. PA at sport and leisure, smoking, alcohol consumption, and socioeconomic status (SES) were interviewed. Waist circumference (WACI) was assessed. RESULTS: HDL-C was positively related to PA sport, PA leisure, and SES (0.22≤r≤0.34; p≤0.001) and negatively related to smoking and WACI (r≤-0.10; p<0.05). Controlling for sex and age did not affect these relationships. Hierarchical multiple regression analyses showed that the relation of HDL-C to PA sport and leisure remained significant when controlling for all other investigated correlates (0.14≤ß≤0.24; p≤0.001). DISCUSSION: In order to prevent low HDL-C in older adults, promoting PA seems to be an important additional component besides common recommendations concerning weight reduction.

AIMS: The present study set out to investigate the relation of the high-density lipoprotein cholesterol (HDL-C) level to cognitive performance and its interplay with key markers of cognitive reserve in a large sample of older adults. METHODS: We assessed tests of working memory, immediate and delayed cued recall in 701 older adults from Amazonas, Brazil. The HDL-C level was derived from fasting blood samples. In addition, we interviewed individuals on their education, past occupation, and cognitive leisure activity. RESULTS: A critically low HDL-C level (<40 mg/dL) was significantly related to lower performance in working memory, immediate and delayed cued recall. Moderation analyses suggested that the relations of the HDL-C level to working memory and delayed cued recall were negligible in individuals with longer education, a higher cognitive level of the job, and greater engagement in cognitive leisure activity. CONCLUSION: Cognitive reserve accumulated during the life course may reduce the detrimental influences of a critically low HDL-C level on cognitive functioning in old age.

BACKGROUND: Very little is known about the association between infection with Toxoplasma gondii (T. gondii) and diabetes mellitus. We perform an age- and gender-matched case-control study to determine the association of T. gondii infection and diabetes mellitus. METHODS: Cases included 156 patients with diabetes mellitus and 156 controls without diabetes mellitus who attended in two public clinics in Durango City, Mexico. Sera of cases and controls were tested for the presence of anti-Toxoplasma IgG and IgM antibodies using commercially available enzyme-linked fluorescence assays (ELFA). RESULTS: Anti-T. gondii IgG antibodies were found in 10 (6.4%) of the 156 cases and in five (3.2%) of the 156 controls (odds ratio (OR): 2.06; 95% confidence interval (CI): 0.69 - 6.19; P = 0.18). The frequency of high (> 150 IU/mL) anti-T. gondii IgG levels in seropositive cases (1/10: 10.0%) was comparable to the one (1/5: 20%) in seropositive controls (OR: 0.44; 95% CI: 0.02 - 9.03; P = 1.00). None of the 10 cases and five controls with seropositivity to anti-T. gondii IgG antibodies were positive for anti-T. gondii IgM antibodies. Stratification by gender showed similar frequencies of T. gondii infection in female cases (7/107: 6.5%) and female controls (4/107: 3.7%) (OR: 1.80; 95% CI: 0.51 - 6.34; P = 0.53), and in male cases (3/49: 6.1%) and male controls (1/49: 2.0%) (OR: 3.13; 95% CI: 0.31 - 31.19; P = 0.61). CONCLUSIONS: We conclude that there is not serological evidence of an association between T. gondii infection and diabetes mellitus in the studied subjects in Durango City, Mexico. Further studies to elucidate the role of T. gondii in diabetes should be conducted.

Objetivo: Describir la percepción del cuidado humanizado de enfermería por parte del paciente con enfermedad oncológica hospitalizado en una IPS de IV nivel. Metodología: Estudio de tipo descriptivo, de corte transversal y enfoque cuantitativo, la población de estudio fue de 80 pacientes hospitalizados con enfermedad oncológica en la fundación cardio infantil-instituto de cardiología en un periodo de marzo a mayo , y a quienes se les aplico el instrumento PCHE tercera versión, previo cumplimiento de criterios de inclusión, teniendo en cuenta el orden de llegada a hospitalización de los pacientes diagnosticados con enfermedad oncológica a la institución, se asignó numeración continua y sucesiva en una base de datos de Excel, hasta completar el periodo de recolección. Los datos se tabularon y graficaron en Excel, fueron analizados en forma precisa aplicando medición estadística para la interpretación de resultados, guardando el rigor metodológico y las características éticas correspondientes. Resultados: En la categoría Priorizar el sujeto de cuidado (la persona) 63 pacientes encuestados (87.5%) SIEMPRE percibieron los estímulos, sentimientos y pensamientos por parte la enfermera que les permitieron identificar la presencia de un cuidado humano, continuo y de calidad, el 9.16 VII % Casi siempre lo percibió y el 1,25 % Algunas veces lo percibió; en la categoría Apertura a la comunicación para proporcionar educación en salud a la persona 31 pacientes encuestados (39,7%) SIEMPRE percibieron un cuidado efectivo, satisfactorio, mediante una relación armónica y de ayuda que facilita el intercambio de información suficiente y respetuosa para la toma de decisiones respecto al estado de salud, descriptores de esta categoría, el 54.56% Casi siempre y el 5.8 % Algunas veces percibió esta apertura a la comunicación; en la categoría Cualidades del hacer de enfermería 38 pacientes encuestados (28.57%) SIEMPRE percibieron un cuidado compresivo hacia los sentimientos de la persona, brindando una atención de calidad individualizada, dedicada, alerta y dispuesta que mejora las condiciones de salud de los pacientes, el 72,85 % casi siempre lo percibió y el 0.7% algunas veces. CONCLUSIONES: Tras la aplicación del instrumento percepción del cuidado humanizado de enfermería (PCHE) 3 versión, en una IPS de IV nivel se puede concluir que el cuidado de enfermería brindado en la institución es un cuidado Humanizado, que tiene en cuenta las necesidad del paciente como individuo, es un cuidado integral, respetuoso, honesto, basado en conocimiento científico y practico, que apunta siempre a dar un cuidado de calidad, el cual es percibido satisfactoriamente por los pacientes en sus diferentes etapas de manejo y cuidado de su enfermedad, cumpliendo así con el fin primordial de la disciplina. El cuidado humanizado de enfermería y las categorías Priorizar el sujeto de cuidado (la persona), Apertura a la comunicación para proporcionar educación en salud a la persona y Cualidades del hacer de enfermería según la definición de los autores del instrumento PCHE 3 versión concretan un cuidado holístico en la aplicación del VIII cuidado de enfermería diario y el complemento con las definiciones hace que el instrumento sea sólido para describir el cuidado humanizado de enfermería

RESUMEN

This study aimed to assess the environmental and social vulnerability and identify critical highway stretches for accidents involving transportation of hazardous products on the BR-101 highway between the cities of Osório and Torres in Rio Grande do Sul State, Brazil. The study's approach consisted of a multiple-criteria analysis combining highway safety analysis and environmental and social vulnerability analysis in the occurrence of accidents with hazardous products, plus cartographic analysis techniques. Thirty-eight kilometers of the highway showed high vulnerability, of which 8 kilometers with critical vulnerability, associated with bridges over rivers, water uptake points, a tunnel, environmental preservation areas, and an urban area. These stretches should be prioritized when developing action plans for accident mitigation and development of public policies for this highway. This proved to be an unprecedented approach when compared to existing studies and a potentially useful tool for decision-making in emergency operations.
