ABSTRACT Objective: to describe scientific evidence on good practices for peripheral arterial catheter management. Method: this is an integrative review, carried out through a search in the Latin American and Caribbean Literature in Health Sciences, Virtual Health Nursing Library, National Library of Medicine, Cochrane Library, Cumulative Index to Nursing & Allied Health, Excerpta Medica dataBASE, SciVerse Scopus TopCited and Web of Science databases in March 2021. Articles in Portuguese, English and Spanish, without time limits in the search, were included. Results: forty-nine articles were found. At insertion, measures involved hand hygiene, skin preparation, no-touch technique, aseptic technique and protective barrier, sterile components and transducers, peripheral arterial catheter insertion, insertion attempts, ultrasound and comfort measure use. During maintenance, issues regarding insertion site, invasive blood pressure circuit, connectors, dressing and stabilization were identified, and, during removal, aspects such as local and systemic complications after peripheral arterial catheter removal. Conclusion: the study provides crucial information for the effective management of peripheral arterial catheters, contributing to the reduction of complications and improvement of clinical results. By updating their practices, healthcare professionals can ensure greater safety and well-being for patients, always seeking to provide excellent care.


RESUMEN Objetivo: describir la evidencia científica sobre buenas prácticas para el manejo de catéteres arteriales periféricos. Método: revisión integradora, realizada a través de una búsqueda en las bases de datos Literatura Latinoamericana y del Caribe en Ciencias de la Salud, Biblioteca Virtual en Enfermería en Salud, National Library of Medicine, Cochrane Library, Cumulative Index to Nursing & Allied Health, Excerpta Medica dataBASE, SciVerse Scopus TopCited y Web of Science en marzo de 2021. Se incluyeron artículos en portugués, inglés y español, sin límite de tiempo en la búsqueda. Resultados: se encontraron 49 artículos. En la inserción, las medidas incluyeron higiene de manos, preparación de la piel, técnica de no contacto, técnica aséptica y barrera protectora, componentes y transductores estériles, Inserción de catéter arterial periférico, intentos de inserción, uso de ultrasonido y medidas de comodidad. Durante el mantenimiento se identificaron problemas relacionados con el sitio de inserción, circuito de presión arterial invasiva, conectores, vendaje y estabilización y, durante el retiro, aspectos como complicaciones locales y sistémicas, luego del retiro del catéter arterial periférico. Conclusión: el estudio proporciona información crucial para el manejo eficaz de los catéteres arteriales periféricos, contribuyendo a la reducción de complicaciones y mejora de los resultados clínicos. Al actualizar sus prácticas, los profesionales de la salud pueden garantizar una mayor seguridad y bienestar a los pacientes, buscando siempre brindar una excelente atención.

Foram incluídos artigos em português, inglês e espanhol, sem delimitação de tempo na busca. Resultados: foram encontrados 49 artigos. Na inserção, as medidas envolveram higienização das mãos, preparo da pele, técnica no touch, técnica asséptica e barreira de proteção, componentes estéreis e transdutores, inserção do cateter arterial periférico, tentativas de inserção, uso do ultrassom e medidas de conforto. Na manutenção, questões sobre o sítio de inserção, circuito da pressão arterial invasiva, conectores, curativo e estabilização foram identificadas e, na retirada, aspectos como complicações locais e sistêmicas, após retirada do cateter arterial periférico. Conclusão: o estudo fornece informações cruciais para o eficaz manejo do cateter arterial periférico, contribuindo para a redução de complicações e aprimoramento dos resultados clínicos. Ao atualizar suas práticas, os profissionais de saúde podem assegurar maior segurança e bem-estar aos pacientes, buscando sempre oferecer um atendimento de excelência.

Introdução: O Pronto Socorro é destinado a prestar assistência cujos agravos à saúde necessitam de atendimento imediato. Uma estratégia para atender as demandas desse serviço é a formação de equipes interdisciplinares. Objetivos: Analisar a atuação fisioterapêutica nos pacientes com emergências neurológicas internados, identificar os recursos fisioterapêuticos utilizados e associar com o desfecho clínico do paciente. Métodos: Estudo retrospectivo observacional descritivo. A coleta de dados foi realizada por meio de análises dos prontuários fisioterapêuticos. Resultados: O diagnóstico clínico mais prevalente foi de Acidente Vascular Encefálico isquêmico. As principais intervenções realizadas foram mobilização passiva e alongamentos. A comparação de proporções entre mobilização precoce e desfecho foi significante, assim como entre desfecho e extubação. Conclusões: A atuação do fisioterapeuta no Pronto Socorro é ampla, sendo realizada por meio de diversos recursos, principalmente mobilização precoce e manejo ventilatório, levando ao favorecimento do desfecho clínico do paciente. Contudo, mais pesquisas nessa área são necessárias.


Introduction: The Emergency Unit is assigned to provide assistance to health problems that require immediate care. One strategy to meet the demands of this service is the creation of interdisciplinary teams. Objectives: To analyze the physiotherapeutic work in hospitalized patients with neurological emergencies, to identify the physiotherapeutic resources used and to associate them with the patient's clinical outcome. Methods: Retrospective, observational and descriptive study. The information was collected through analysis of physical therapy records. Results: The prevailing clinical diagnosis was ischemic stroke. The main interventions performed were passive mobilization and stretching. The comparison between the proportions of early mobilization and outcome was significant, as well as between clinical outcome and extubation. Conclusions: The work of the physical therapist in the Emergency Unit is vast, and it is carried out through several resources, mainly early mobilization and ventilatory management, aiding the patient's clinical outcome. However, more studies are necessary in this field.

Introdução: As úlceras de difícil cicatrização caracterizam-se como uma patologia que afeta cerca de 20 milhões de pessoas pelo mundo. A terapia fotodinâmica TFD é um método que atua nas fases da cicatrização, bioestimulando o tecido e promovendo a morte dos microorganismos Objetivo: O objetivo deste estudo foi avaliar a TFD como técnica de ação bactericida na cicatrização das úlceras de usuários de um serviço público de saúde acometidos por úlceras venosas UV. Métodos: Para avaliar a presença de bactérias nas úlceras, foi utilizado um swab stuart. Foi aplicado o medicamento à base de curcumina na úlcera e a mesma foi imediatamente ocluída com papel alumínio durante 20 minutos. Resultados: Durante todo período de coleta houve crescimento de bactérias nas úlceras. Os participantes obtiveram redução da área das úlceras, avaliadas pela quantificação do software Image J Conclusão: A TFD foi capaz de acelerar o tempo de cicatrização de úlceras venosas, ao efeito bactericida, a técnica carece ainda de mais estudos.


Introduction: Ulcers that are difficult to heal are characterized as a pathology that affects about 20 million people around the world. PDT photodynamic therapy is a method that acts in the healing phases, biostimulating the tissue and promoting the death of microorganisms affected by UV. Methods: To assess the presence of bacteria in the ulcers, a stuart swab was used. the medicine based on curcumin was applied to the ulcer and it was immediately occluded with aluminum foil for 20 minutes. Results: During the entire collection period, there was growth of bacteria in the ulcers. The participants obtained a reduction in the area of the ulcers, evaluated by the quantification of the Image J software. Conclusion: PDT was able to accelerate the healing time of venous ulcers, due to its bactericidal effect, the technique still needs further studies.

BACKGROUND: Previous trials have demonstrated the effects of fluvoxamine alone and inhaled budesonide alone for prevention of disease progression among outpatients with COVID-19. OBJECTIVE: To determine whether the combination of fluvoxamine and inhaled budesonide would increase treatment effects in a highly vaccinated population. DESIGN: Randomized, placebo-controlled, adaptive platform trial. (ClinicalTrials.gov: NCT04727424). SETTING: 12 clinical sites in Brazil. PARTICIPANTS: Symptomatic adults with confirmed SARS-CoV-2 infection and a known risk factor for progression to severe disease. INTERVENTION: Patients were randomly assigned to either fluvoxamine (100 mg twice daily for 10 days) plus inhaled budesonide (800 mcg twice daily for 10 days) or matching placebos. MEASUREMENTS: The primary outcome was a composite of emergency setting retention for COVID-19 for more than 6 hours, hospitalization, and/or suspected complications due to clinical progression of COVID-19 within 28 days of randomization. Secondary outcomes included health care attendance (defined as hospitalization for any cause or emergency department visit lasting >6 hours), time to hospitalization, mortality, patient-reported outcomes, and adverse drug reactions. RESULTS: Randomization occurred from 15 January to 6 July 2022. A total of 738 participants were allocated to oral fluvoxamine plus inhaled budesonide, and 738 received placebo. The proportion of patients observed in an emergency setting for COVID-19 for more than 6 hours or hospitalized due to COVID-19 was lower in the treatment group than the placebo group (1.8% [95% credible interval {CrI}, 1.1% to 3.0%] vs. 3.7% [95% CrI, 2.5% to 5.3%]; relative risk, 0.50 [95% CrI, 0.25 to 0.92]), with a probability of superiority of 98.7%. No relative effects were found between groups for any of the secondary outcomes. More adverse events occurred in the intervention group than the placebo group, but no important differences between the groups were detected. LIMITATION: Low event rate overall, consistent with contemporary trials in vaccinated populations. CONCLUSION: Treatment with oral fluvoxamine plus inhaled budesonide among high-risk outpatients with early COVID-19 reduced the incidence of severe disease requiring advanced care. Mangabeira (Hancornia speciosa) is a Brazilian tree and a socioeconomic key due to the commercialization of its food products and tree parts to treat health conditions empirically. This review gathers the main chemical, and microbiological characteristics of the mangabeira tree parts (leaves, fruits, tree bark, latex, and seeds), emphasizing its applicability in food science and focusing on its bioapplicability in health conditions. Leaves, fruits, and tree bark can be used to develop functional foods, and phytochemical products; the tree latex have great potential in the bioengineering material field; and the seeds in sustainable energy production. Leaves and fruits were the main samples bioapplied in health conditions in vitro (oxidative stress and chemopreventive effect) and in vivo (gastrointestinal and cardiovascular health, anti-inflammatory, and antidiabetic effect), whereas tree bark and latex also exhibited health effects and seeds showed low cytotoxicity. All parts of the mangabeira tree can be explored by extractivist families and industries from a sustainable point of view.

PURPOSE: Early intervention in smoldering multiple myeloma (SMM) requires optimal risk stratification to avoid under- and overtreatment. We hypothesized that replacing bone marrow (BM) plasma cells (PC) for circulating tumor cells (CTC), and adding immune biomarkers in peripheral blood (PB) for the identification of patients at risk of progression due to lost immune surveillance, could improve the International Myeloma Working Group 20/2/20 model. EXPERIMENTAL DESIGN: We report the outcomes of 150 patients with SMM enrolled in the iMMunocell study, in which serial assessment of tumor and immune cells in PB was performed every 6 months for a period of 3 years since enrollment. RESULTS: Patients with >0.015% versus ≤0.015% CTCs at baseline had a median time-to-progression of 17 months versus not reached (HR, 4.9; P < 0.001). Presence of >20% BM PCs had no prognostic value in a multivariate analysis that included serum free light-chain ratio >20, >2 g/dL M-protein, and >0.015% CTCs. The 20/2/20 and 20/2/0.015 models yielded similar risk stratification (C-index of 0.76 and 0.78). The combination of the 20/2/0.015 model with an immune risk score based on the percentages of SLAN+ and SLAN- nonclassical monocytes, CD69+HLADR+ cytotoxic NK cells, and CD4+CXCR3+ stem central memory T cells, allowed patient' stratification into low, intermediate-low, intermediate-high, and high-risk disease with 0%, 20%, 39%, and 73% rates of progression at 2 years. CONCLUSIONS: This study showed that CTCs outperform BM PCs for assessing tumor burden. Additional analysis in larger series are needed to define a consensus cutoff of CTCs for minimally invasive stratification of SMM.

O presente estudo é do tipo quase experimental e teve como objetivo verificar os efeitos da intervenção com o Método Pilates (MP) na presença e intensidade dos sintomas musculoesqueléticos, na força de preensão palmar e na flexibilidade toracolombar de trabalhadores com lesões por esforços repetitivos/distúrbios osteomusculares relacionados ao trabalho (LER/DORT). Participaram trabalhadores encaminhados para atendimento fisioterapêutico a um serviço municipal de nível secundário de atenção à saúde. Foi disponibilizada aos pesquisadores uma lista com 605 usuários com diagnóstico de afecções musculoesqueléticas (sem referência se as afecções tinham ou não relação com o trabalho). Realizou-se contato telefônico para identificar quais eram trabalhadores, convidando-os a participar da pesquisa. Os 244 trabalhadores que aceitaram foram agendados; e, destes, 166 compareceram, sendo avaliados os aspectos sociodemográficos, ocupacionais, história clínica detalhada, sintomas musculoesqueléticos e exame físico, permitindo o estabelecimento do nexo causal (diagnóstico de LER/DORT) para 142 desses trabalhadores. Destes, 17 apresentaram disponibilidade para participar da intervenção nos dias e horários disponibilizados para a aplicação do MP. O protocolo foi constituído por 12 exercícios, por seis semanas, com duração de 60 minutos. Os resultados do MP foram significantes (p<0,05) na redução do número de trabalhadores com sintomas nas regiões dos ombros e punhos/mãos, redução significante da intensidade dos sintomas em oito das nove regiões corporais avaliadas e aumento significante da força da mão não dominante. O MP proporcionou efeitos positivos na redução dos sintomas das LER/DORT.


The aim of the present quasi-experimental study was to verify the effects of intervention with the Pilates Method (PM) on the presence and intensity of musculoskeletal symptoms, handgrip strength and thoracolumbar flexibility of workers with Repetitive Strain Injuries/Musculoskeletal Disorders Work Related (LER/DORT). Workers referred for physical therapy care to a municipal service at the secondary level of health care participated. A list of 605 users with a diagnosis of musculoskeletal disorders was made available to the researchers (without reference to whether the disorders were related to work or not). Telephone contact was made to identify which workers were, inviting them to participate in the research. The 244 workers who accepted were scheduled and, of these, 166 attended, and the sociodemographic, occupational, detailed medical history, musculoskeletal symptoms and physical examination were evaluated, allowing the establishment of the causal link (RSI/WMSD diagnosis) for 142 of these workers. Of these, 17 were available to participate in the intervention on the days and times available for the application of the MP. The protocol consisted of 12 exercises, for six weeks, lasting 60 minutes. The MP results were significant for a reduction in the number of workers with symptoms for the shoulder and wrist/hand regions, a significant reduction in the intensity of symptoms for eight of the nine body regions evaluated, and a significant increase in the strength of the non-dominant hand. MP provided positive effects in reducing RSI/WMSD symptoms.

Normal cell counterparts of solid and myeloid tumors accumulate mutations years before disease onset; whether this occurs in B lymphocytes before lymphoma remains uncertain. We sequenced multiple stages of the B lineage in elderly individuals and patients with lymphoplasmacytic lymphoma, a singular disease for studying lymphomagenesis because of the high prevalence of mutated MYD88. We observed similar accumulation of random mutations in B lineages from both cohorts and unexpectedly found MYD88L265P in normal precursor and mature B lymphocytes from patients with lymphoma. We uncovered genetic and transcriptional pathways driving malignant transformation and leveraged these to model lymphoplasmacytic lymphoma in mice, based on mutated MYD88 in B cell precursors and BCL2 overexpression. Thus, MYD88L265P is a preneoplastic event, which challenges the current understanding of lymphomagenesis and may have implications for early detection of B cell lymphomas.

Objetivo: Avaliar as relações entre espiritualidade e comportamentos saudáveis em idosos atendidos na atenção primária à saúde. Métodos: Estudo transversal realizado com amostra de 201 idosos atendidos em três Unidades Matriciais de Saúde (UMS) selecionadas aleatoriamente, em Uberaba, Minas Gerais, Brasil. Avaliou-se a espiritualidade pela Escala de Espiritualidade com cinco itens, cuja pontuação varia de 5 (menor espiritualidade) a 20 (maior espiritualidade). Os comportamentos de saúde incluíram tabagismo, consumo de bebidas alcoólicas, água, frutas e vegetais, satisfação com o sono, relações sociais e prática de atividade física. Os indicadores sociodemográficos englobaram sexo (masculino/feminino), faixa etária (60-74/75+) e escolaridade (até 4 anos/5 anos ou mais). Realizaram-se análises descritivas, teste t Student e regressão linear múltipla, adotando-se o intervalo de confiança de 95%. Resultados: Encontrou-se a média do escore na Escala de Espiritualidade de 16,02±2,71. Observaram se as pontuações maiores entre os idosos que não fumavam (p=0,014); que consumiam frutas (p=0,003), vegetais (p=0,002) e água (p=0,017) em quantidades adequadas; entre aqueles classificados como fisicamente ativos (p<0,001); e entre aqueles que estavam satisfeitos com o sono (p=0,032) e com as relações sociais (p<0,001). Prática de atividade física (ß:0,225; IC=0,507-2,131) e satisfação com as relações sociais (ß:0,190; IC=0,360-2,351) apresentaram-se relacionadas à espiritualidade mais elevada, mesmo após controle por indicadores sociodemográficos. Conclusão: A espiritualidade relaciona-se com comportamentos saudáveis nos idosos investigados atendidos na atenção primária à saúde.


Objective: To evaluate the relationship between spirituality and healthy behaviors in older adults assisted in primary health care.Methods: A cross-sectional study was carried out with a sample of 201 older adults in three randomly selected matrix health units (UMS) in Uberaba-MG. Spirituality was assessed by the Spirituality Scale with five items, whose final score ranges from 5 (less spiritual) to 20 (higher spirituality). Health behaviors included smoking, consumption of alcoholic beverages, water, fruits and vegetables, satisfaction with sleep and social relationships, and physical activity. The sociodemographic indicators were sex (male/female), age group (60-74/75+), and schooling (up to 4/5 years or more). Descriptive analyses, Student's t-test, and multiple linear regression were performed, adopting a 95% confidence interval. Results: The mean score on the Spirituality Scale was 16.02±2.71. Higher scores were observed among participants who did not smoke (p=0.014); who consumed fruits (p=0.003), vegetables (p=0.002), and water (p=0.017) in adequate amounts; among those classified as physically active (p<0.001); who were satisfied with sleep (p=0.032) and with social relationships (p<0.001). Physical activity practice (ß:0.225; CI=0.507-2.131) and satisfaction with social relationships (ß:0.190; CI=0.360-2.351) were related to higher spirituality even after controlling for sociodemographic indicators. Conclusion: Spirituality is related to healthy behaviors in the investigated older adults assisted in primary health care.

Práctica de actividad física (ß:0,225; IC=0,507-2,131) y satisfacción con las relaciones sociales (ß:0,190; IC=0,360-2,351) se presentaron relacionadas con la espiritualidad más elevada, incluso después de control por indicadores sociodemográficos. Conclusión: La espiritualidad está relacionada con comportamientos saludables en los ancianos investigados atendidos en la atención básica de salud.

BACKGROUND: Recent evidence indicates a potential therapeutic role of fluvoxamine for COVID-19. In the TOGETHER trial for acutely symptomatic patients with COVID-19, we aimed to assess the efficacy of fluvoxamine versus placebo in preventing hospitalisation defined as either retention in a COVID-19 emergency setting or transfer to a tertiary hospital due to COVID-19. METHODS: This placebo-controlled, randomised, adaptive platform trial done among high-risk symptomatic Brazilian adults confirmed positive for SARS-CoV-2 included eligible patients from 11 clinical sites in Brazil with a known risk factor for progression to severe disease. Patients were randomly assigned (1:1) to either fluvoxamine (100 mg twice daily for 10 days) or placebo (or other treatment groups not reported here). The trial team, site staff, and patients were masked to treatment allocation. Our primary outcome was a composite endpoint of hospitalisation defined as either retention in a COVID-19 emergency setting or transfer to tertiary hospital due to COVID-19 up to 28 days post-random assignment on the basis of intention to treat. Modified intention to treat explored patients receiving at least 24 h of treatment before a primary outcome event and per-protocol analysis explored patients with a high level adherence (>80%). We used a Bayesian analytic framework to establish the effects along with probability of success of intervention compared with placebo. The trial is registered at ClinicalTrials.gov (NCT04727424) and is ongoing. FINDINGS: The study team screened 9803 potential participants for this trial. The trial was initiated on June 2, 2020, with the current protocol reporting randomisation to fluvoxamine from Jan 20 to Aug 5, 2021, when the trial arms were stopped for superiority. 741 patients were allocated to fluvoxamine and 756 to placebo. The average age of participants was 50 years (range 18-102 years); 58% were female. The proportion of patients observed in a COVID-19 emergency setting for more than 6 h or transferred to a teritary hospital due to COVID-19 was lower for the fluvoxamine group compared with placebo (79 [11%] of 741 vs 119 [16%] of 756); relative risk [RR] 0·68; 95% Bayesian credible interval [95% BCI]: 0·52-0·88), with a probability of superiority of 99·8% surpassing the prespecified superiority threshold of 97·6% (risk difference 5·0%). Of the composite primary outcome events, 87% were hospitalisations. Findings for the primary outcome were similar for the modified intention-to-treat analysis (RR 0·69, 95% BCI 0·53-0·90) and larger in the per-protocol analysis (RR 0·34, 95% BCI, 0·21-0·54). There were 17 deaths in the fluvoxamine group and 25 deaths in the placebo group in the primary intention-to-treat analysis (odds ratio [OR] 0·68, 95% CI: 0·36-1·27). There was one death in the fluvoxamine group and 12 in the placebo group for the per-protocol population (OR 0·09; 95% CI 0·01-0·47). We found no significant differences in number of treatment emergent adverse events among patients in the fluvoxamine and placebo groups. INTERPRETATION: Treatment with fluvoxamine (100 mg twice daily for 10 days) among high-risk outpatients with early diagnosed COVID-19 reduced the need for hospitalisation defined as retention in a COVID-19 emergency setting or transfer to a tertiary hospital. Os efeitos do sedentarismo nos sistemas musculoesquelético e neuromuscular podem influenciar negativamente o desempenho dos idosos em atividades com dupla-tarefa, o que aumenta o risco de quedas, declínio funcional e morte. O objetivo foi investigar as relações entre sedentarismo e mobilidade com e sem dupla tarefa em idosos usuários da atenção básica em saúde. Trata-se de estudo transversal, com 139 idosos que frequentam três unidades matriciais de saúde aleatoriamente selecionadas, em Uberaba, Minas Gerais. O sedentarismo foi avaliado pelo International Physical Activity Questionnaire (IPAQ), sendo classificados como sedentários os idosos inativos ou insuficientemente ativos. O estado cognitivo foi avaliado pela Prova Cognitiva de Leganés. A mobilidade com e sem dupla tarefa, bem como, a força muscular foram avaliadas por testes de desempenho físico. As características sociodemográficas e o número de medica- mentos foram avaliados por autorrelato. Os modelos multivariados foram testados por regressão linear múltipla em blocos, com intervalo de confiança de 95%. A maioria era mulheres (81,3%); idade média 66,96 (8,24) anos; 17,3% eram sedentários. Pior desempenho na mobilidade com dupla tarefa motora (p=0,040) e com dupla tarefa cognitiva (p=0,040) foi observado entre os idosos sedentários. O sedentarismo foi preditor de baixa mobilidade com dupla-tarefa motora e cognitiva controlado por variáveis sociodemográficas, entretanto, tais efeitos não se mantiveram após inclusão de variáveis de saúde. Não foram observadas relações entre sedentarismo e mobilidade sem dupla tarefa. Conclui-se que o sedentarismo influencia a mobilidade com dupla tarefa motora e cognitiva em idosos, porém outras condições de saúde podem interferir nessas relações.(AU)


The effects of sedentarism on the musculoskeletal and neuromuscular systems may negatively influence performance of older adults in dual-task activities, which increases risk of falls, functional decline, and death. The objective was to investigate the relationships between sedentarism and mobility with and without double tasks among older adult usersof primary health care. This is a cross-sectional study with 139 older adults from three health care units randomly selected in the city of Uberaba, Minas Gerais. Sedentarism was identified by the International Physical Activity Questionnaire (IPAQ). Inactive or insuf- ficiently active elderly individuals were classified as sedentary. Cognitive status was assessed by the Leganés Cognitive Test. Mobility with and without double tasks, as well as, muscle strength were assessed by physical performance tests. Sociodemographic characteristics and number of medications were assessed by self-report. Multiple linear regressions were performed in blocks, with 95% confidence interval. Most participants were women (81.3%); whose average age was 66.96 (8.24) years; 17.3% were sedentary. Poorer performance in mobility with double motor task (p = 0.040) and double cognitive task (p = 0.040) was observed among sedentary participants. In multivariate models, physical inactivity influenced the motor and cognitive double-tasks performance controlled by sociodemographic variables; however, such effects were not remained after inclusion of health variables. No relationships were observed between sedentarism and mobility without double tasks. In conclusion, sedentarism influences mobility with dual motor and cognitive tasks among older adults, but other health conditions may play a role in these relationships.(AU)

Resumo Objetivo Verificar o cumprimento às Precauções-Padrão por profissionais de enfermagem e fatores associados. Método Estudo descritivo, transversal, com 522 profissionais de enfermagem, realizado em dois hospitais brasileiros, entre janeiro de 2017 a março de 2018. Os dados foram coletados por meio de um formulário contendo variáveis demográficas e profissionais e a Compliance with Standard Precautions Scale (versão Português-Brasil); posteriormente, analisados por estatísticas descritivas e exploratórias e um modelo de regressão linear múltiplo. Resultados O escore médio global 12,9 (DP=2,5). Técnicos de enfermagem tiveram escores médios estatisticamente significativos maiores (p <0,01) do que enfermeiros. Houve diferença significativa quanto à categoria profissional (p <0,01) e escolaridade (p <0,01), e, marginalmente significativa em relação à idade (p = 0,06). Não houve diferenças quanto à experiência profissional (p = 077), participação em treinamentos (p = 0,79), tipo de hospital (p = 0,13), respectivamente. A escolaridade não contribuiu para um maior cumprimento às medidas (p <0,01), assim como o ensino superior (p ≤ 0,01). Conclusão e Implicações para a prática O aumento na escolaridade e experiência profissional não contribuiu para maior cumprimento às Precações. Por contemplar aspectos da prática de enfermagem, estratégias de prevenção de exposição ocupacional podem ser revistas e aperfeiçoadas.

Resultados Puntaje medio global de 12,9 (DS=2,5). Los auxiliares de enfermería obtuvieron puntajes promedio mayores, estadísticamente significantes (p<0,01) respecto de los enfermeros. Existió diferencia significante respecto de la categoría profesional (p<0,01) y la escolarización (p<0,01); y marginalmente significante en relación a la edad (p=0,06). No hubo diferencias relativas a la experiencia profesional (p=0,77), participación en capacitaciones (p=0,79) y tipos de hospital (p=0,13). La escolarización no contribuyó a una mayor observancia de las medidas (p<0,01), al igual que los estudios superiores (p≤0,01). Conclusión e Implicaciones para la práctica Mayores grados de escolarización y experiencia profesional no contribuyeron a la observancia de las Precauciones. En razón de contemplar aspectos de la práctica de enfermería, las estrategias de prevención y exposición profesional merecen ser revisadas y perfeccionadas.


Abstract Objective To verify compliance with the Standard Precautions by nursing professionals and associated factors. Method A descriptive, cross-sectional study was carried out with 522 nursing professionals, in two Brazilian hospitals, between January 2017 and March 2018. Conclusion and implications for practice Increased education and professional experience did not contribute to greater compliance with the Standards Precautions. By considering aspects of nursing practice, occupational exposure prevention strategies can be reviewed and improved.

The acute improvement of performance after photobiomodulation therapy (PBMT) has been reported in different types of exercise. However, the effect on high-intensity and intermittent exercises that are relevant for team sports is unknown. Thus, we evaluated the effect of prior acute application of PBMT on high-intensity and intermittent exercise performance, muscle oxygenation, and physiological/perceptual indicators in amateur female futsal players. Thirteen players (24.1 ± 3.7 years) performed a testing battery (countermovement jump (CMJ), Illinois agility and YoYo intermittent recovery test level 1 (YYIR1)) preceded by 15 min of PBMT (1 min 30 s each muscular point; five muscular points in each lower limbs) or 15 min of placebo (SHAM), in a counterbalanced randomized cross-over design (one-week in-between PBMT/SHAM). All test performance did not differ (p > 0.05) between PBMT and SHAM, as well as blood lactate, rating of perceived exertion, heart rate, and muscle oxygenation (via near infrared spectroscopy) responses. The acute application of PBMT prior to a physical testing battery does not influence high-intensity and intermittent exercises performance, neither physiological nor perceptual responses in amateur female futsal players.

Non-alcoholic fatty liver disease (NAFLD) is a metabolic disorder caused by excess consumption of hypercaloric foods. Guavira (Campomanesia sp.) pulp has broad technological applicability, yet the peel and seeds are considered industrial residue. The objective of this unprecedented study was to evaluate the effects of the flour from guavira's industrial residue (GF) consumption in rats fed with hypercaloric diet (HD). During 65 days, 50 rats were separated into a control group: 1%, 2%, 4% and 8% HD with GF complementation in the diet. The GF chemical composition, phenolic compounds, antioxidant capacity, serum biochemical parameters (glucose, cholesterol, HDL, non-HDL, triglycerides, AST, ALT, and oral glucose tolerance test), fat liver content, and hepatic histomorphology had been characterized. GF is mainly composed of fibres, with phenolic content of 7,391.09 mg AGE/100 g GF and relevant antioxidant capacity (IC50 2.22 and ORAC 155.68 µmol/TE g-1 ). Serum biochemical analysis did not differ statistically (except ALT reduction, p < .05). Fat liver content was smaller on HD2%GF (p < .0001). The control group and 1% GF showed greater diffuse microvesicular steatosis compared to the other groups when using hepatic morphological analysis (p < .05). GF consumption attenuated NAFLD caused by a hypercaloric diet, and this effect may be related to the fibre content and bioactive compounds in GF.

We performed a systematic review to identify and summarize the effects of distinctive aerobic exercise (AE) protocols on chronic allergic lung inflammation reported in asthma animal models. We identified 255 preclinical studies of asthma and AE protocols by comprehensive searches in PubMed, ScienceDirect and LILACS for peer-reviewed journals, using the search components "exercise," "lung inflammation," and "asthma," without restrictions on publication date. Twenty-two studies were selected that met all eligibility criteria. Most studies used an ovalbumin experimental model and performed experiments with BALB/c mice. Most studies performed treadmill AE, using protocols of 30 to 60 minutes, 3 to 5 times/week, for 4 to 8 weeks. The results showed that the effects of AE on lungs were protocol dependent, but generaly, there was a decrease in inflammatory cell influx and Th2 cytokines, as well as an increase in anti-inflammatory defenses. However, the immunoregulation of globulins and nitric oxide levels in asthma seemed to be associated with the onset of AE. The results suggest that AE plays an important immunoregulatory role in animal models of asthma, with effects mainly mediated by increased anti-inflammatory defenses and reduced Th2 response. In parallel, prophylactic and therapeutic AE also played important roles in decreasing bronchial responsiveness and attenuating lung remodeling in animal models of asthma, secondary outcomes in this systematic review. Introdução: Os benefícios da atividade física na população geral estão bem estabelecidos, gerando melhora da qualidade de vida e diminuição significativa do risco de desenvolver doenças cardiovasculares e metabólicas. A perda da massa muscular com redução da força muscular e do desempenho físico é um problema relevante em indivíduos com câncer. O exercício físico com fim terapêutico constitui um valioso instrumento da reabilitação para estes pacientes. Objetivo: Descrever o impacto do treinamento físico combinado em dois pacientes com linfoma. Casuística e Métodos: Descreve-se o caso de dois pacientes com linfoma, sendo um do sexo masculino, com 30 anos de idade, com linfoma tipo Hodgkin e outro do sexo feminino, com 29 anos de idade com linfoma tipo não Hodgkin. Em ambos os pacientes foram avaliados o hemograma, as variáveis respiratórias, a capacidade física funcional, força de preensão palmar, funcionalidade e teste de repetições máximas antes e após treinamento físico combinado. Resultados: O treinamento físico combinado demonstrou melhora nas variáveis respiratórias, na capacidade física funcional, na força muscular global e na funcionalidade dos pacientes com linfoma. Conclusão: É possível concluir que o treinamento físico combinado pode trazer benefícios nos aspectos físicos e funcionais dos pacientes em tratamento oncológico. Sugere-se que o treinamento físico combinado seja incluído na reabilitação destes pacientes.


Introduction: The benefits of physical activity in the general population are well established, leading to improved quality of life and a significant reduction in the risk of developing cardiovascular and metabolic diseases. Loss of muscle mass with reduced muscle strength and physical performance is a relevant problem in individuals with cancer. Physical exercise for therapeutic purposes is a valuable rehabilitation tool for these patients. Objective: To describe the impact of combined physical training in two patients with lymphoma. Patients and Methods: We describe the case of two patients with lymphoma, a 30-year-old male patient with hodgkin's lymphoma and a 29-year-old female with non-hodgkin's lymphoma. The blood count, respiratory variables, functional physical capacity, palmar grip strength, functionality and maximal repetition test in the pre and post combined physical training were evaluated in both patients. Results: Combined physical training evidenced improvement in respiratory variables, functional physical capacity, overall muscle strength and functionality of patients with lymphoma. Conclusion: It is possible to conclude that the combined physical training can bring benefits in the physical and functional aspects of the patients in cancer treatment. It is suggested that combined physical training be included in the rehabilitation of the patient with cancer.

The prevalence of gastrointestinal disorders, such as constipation, has been increasing. Genetic factors and lifestyle are some of the etiologies of this issue, affecting the health of the population. Natural products have properties that contribute to health maintenance and health promotion, including reduction of the inflammatory process. Hancornia speciosa, popularly known as mangaba, is an abundant and native fruit in the Brazilian Cerrado, commercialized for culinary purposes and used because of its pharmacological properties. The objective of this study was to evaluate if the supplementation of different concentrations of mangaba pulp can improve intestinal motility and bowel health in Wistar rats. Forty male rats were divided into five groups. The experiment lasted 14 days and the groups were tested with water, industrialized laxative jelly made from tamarind as medication, or mangaba at 5, 10, and 15 mL/kg of body weight. Food intake, weight gain, ion balance, intestinal motility, and histopathological analysis of the small intestine, large intestine, and liver were evaluated. Supplementation of mangaba pulp at its highest concentration (15 mL/kg body weight) caused a 15% increase in the distance traveled by the charcoal meal, and a decrease in serum magnesium levels and white cells in both the small and large intestines. The results suggest that mangaba pulp presents laxative, anti-inflammatory properties and that its consumption is beneficial and should be encouraged.

BACKGROUND: Despite bone marrow (BM) immunophenotyping by flow cytometry has progressively been recognized as an important tool for the diagnosis of myelodysplastic syndromes (MDS), the sparse knowledge about normal erythroid maturation and the lack of markers for erythroid characterization is a major shortcoming. METHODS: Here, we analyzed the expression of CD43 and CD49d, two markers included in the diagnostic panel for B-cell chronic lymphoproliferative disorders (B-CLPD), in the CD34+ compartment of normal BM and along the normal and dysplastic erythroid maturation. For this, 13 normal BM aspirates and 18 BM aspirates from MDS patients were studied by flow cytometry. RESULTS: Normal BM presented a higher expression of CD43 and CD49d among CD34+ erythroid precursors, compared to CD34+ cells committed to the remaining hematopoietic cell lineages. CD43 expression progressively decreased along the normal erythroid maturation, whereas CD49d levels increased from Stage I to Stage II, were maintained in Stages II and III, and then decreased until the last stage of maturation. In MDS, the expression of CD43 and CD49d followed a similar pattern, but with decreased expression levels for both markers, observed in all erythroid maturation stages (P < 0.05). CONCLUSIONS: Our results point to the usefulness of CD43 and CD49d, two markers commonly present in B-CLPD diagnosis panels, in the identification of dysplastic phenotypic features in the erythroid lineage. This allows a feasible and inexpensive way to identify patients who would benefit from a more extensive study to evaluate the presence of MDS, during the processing of suspected B-CLPD samples. © 2019 International Clinical Cytometry Society.

Patients older than 75 years old with multiple myeloma (MM) have shorter survival and are usually treated differently from what features in clinical trials. In this study, the authors characterized the Portuguese population of MM patients above 75 years old, treated between 2009 and 2016. We compared the outcomes obtained with bortezomib-based protocols (BBP), thalidomide-based protocols (TBP), and chemotherapy (CT) using univariate and multivariate controlling for age, performance status, International Staging System score, renal impairment, and number of comorbidities. We retrieved data from 386 patients, treated in 12 hospitals. Three hundred thirty-one cases were analyzed: 119 patients treated with BBP, 65 with TBP, 147 with CT. Median age was 79 years; CT-treated patients were older, had a worse performance status, and have more comorbidities. The median follow-up was 25 months. The 2-year OS was 58% and the median OS was 29.5 months. Patients treated with BBP had more frequently very good partial response (VGPR) or better response, and the subgroup of more fit patients had a significantly longer progression-free survival (PFS) and OS. The most frequently grade 3-4 toxicities were hematologic, infectious, and neurologic and were significantly lower in TBP and CT groups vs BBP. The most common second line was CT, followed by lenalidomide. Patients treated with lenalidomide had a higher probability of VGPR or better and a superior 1-year PFS. Despite the limitations of a retrospective study, our cohort represents the reality of older patients with MM in a western country. The hazard of death or progression was higher for old, fit patients treated, in first line, with CT and with TBP compared with that of BBP.

RESUMEN

In this report, we report a case study on a 50-year-old male patient with different chronic wounds, such as venous ulcer and arterial ulcer in both legs. These wounds have persisted for more than 10 years, despite the different treatments applied, including, different dressings, hyperbaric camera, as well as the used of several medicines of oral and topical application. The case is addressed with the aim to evaluate if treatment that uses combined techniques such as low level laser therapy (LLLT), photodynamic therapy and cellulose membrane is able to improve healing and reduction of time of treatment in those types of chronic wounds. The results show the clinical protocol is effective for the healing of arterial and venous ulcers and can be considered as a promising possibility.
