RESUMEN

AIM: To elucidate whether and how tacrolimus affects the cumulative survival of patients after living related kidney donor transplantation. SUBJECTS AND METHODS: The clinical materials of 246 related kidney transplant recipients, including 108 patients in whom tacrolimus (Prograf and Advagraf) Astellas Pharma US, Inc) was included in the immunodepression protocol (Group 1) and 138 patients who did not receive the agent (Group 2), were analyzed. Comparative analysis used the following tests: the Kaplan Meier test estimating the cumulative survival of recipients and transplants; the Cox test assessing the cumulative risk; and the log-rank test. Allorenal graft losses and mortality rates were also calculated. RESULTS: Mathematical analysis of the above indicators demonstrated that the allograft and survival rates were far higher (p < 0.05) and the cumulative risk was much less in Group 1 (p < 0.02). Graft losses and the recipients' deaths were higher in Group 2. CONCLUSION: The analysis suggests that the incorporation of tacrolimus into the immunodepression therapy protocol positively impacts the results of lung related kidney donor transplantation.

RESUMEN

A test-system based on enzyme-linked immunosorbent assay (ELISA) for quantitative determination of cyclosporin A (CSA) in human whole blood has been developed. The detection limit of the method was 25 ng/ml, the linearity of the method in the concentration range 60-1400 ng/ml varied from 94 to 105%, the variation coefficient did not exceed 8%. The novel method exhibited good correlation with radio-immunoassay and polarization fluoroimmunoassay methods; the linear regression coefficients were 0.965 and 0.983, respectively. The developed test system is stable for at least 9 months when stored at 4 degrees C and can be used in clinical practice.

RESUMEN

Hyperbaric oxygenation (HBO) and pyridinolcarbamat were mainly applied in the treatment of elderly patients with peptic ulcer attended by coronary heart disease and atherosclerosis. To evaluate the condition of intracellular regulators after the treatment, a study was made of the time-course of changes in the content of cyclic nucleotides in blood plasma and gastric mucosa. The use of HBO in the treatment of peptic ulcer raised the efficacy of the multimodality therapy and accelerated the epithelization of erosive and ulcerative defects. The use of pyridinolcarbamat turned to produce a beneficial therapeutic effect and to be protective with respect to the gastric mucosa. Thus it was found desirable to apply the drug to the treatment of ulcers mainly occurring in the stomach. The changes in the content of cyclic nucleotides in blood plasma and gastric mucosa may play an important role in the realization of positive metabolic changes in gastric mucosa, induced by pyridinolcarbamat and HBO in peptic ulcer patients.

Experiments made on 127 white random-bred male rats weighing 200 +/- 10 g with transitory coronary insufficiency (TCI) with varying duration of myocardial ischemia (MI) have revealed consistent changes in the heart cAMP and cGMP. During MI, there was a biphasic variation in the concentration of cyclic nucleotides: an initial appreciable increase in the concentration was replaced by its lowering. At the same time the time course of cGMP content was more mobile in nature as compared to cAMP Reperfusion made at an early period (within the first 40 min) did not normally bring about the normalization of heart content of cyclic nucleotides whose concentration time course depended on the duration of the preceding MI. The pattern of changes in the concentration of cyclic nucleotides in the heart in TCI correlated to a significant degree with the previously described time course of the activity of the sympath- and cholinergic mechanisms by which heart work, contractile function and rhythm are controlled during TCI.

RESUMEN

Two hours after the ligation of the descending branch of the left coronary artery in rabbits the contractile function of the heart decreased, concentration of phosphocreatine, ATP, glycogen dropped, and the content of lactic acid in the ischaemic and intact areas of the left ventricle elevated. Two-hour ischaemia of the myocardium was accompanied by increase of the cAMP and decrease of cGMP levels in the ischaemic and intact areas. Hyperbaric oxygenation (HBO, 2 at. abs, 1 h) started 30-40 min after coronary occlusion led to partial restoration of the cardiac contractility function, improvement of the energy and carbohydrate metabolism in the ischaemic and intact areas of the left ventricle and prevented cardiac fibrillation. It is surmised that favourable action of HBO on the functional state of the heart in its acute focal ischaemia is related not only to the antihypoxic effect but also to the rearrangement of the system of neurohormonal control of the heart.
