RESUMEN

OBJECTIVE: The objective of this study was to investigate the longitudinal changes in cranial growth following fronto-orbital advancement (FOA) surgery in patients with unilateral and bilateral coronal craniosynostosis. METHODS: This retrospective review analyzed head circumference (HC) and CT data during preoperative (T0), immediate postoperative (T1), and final follow-up (T2) visits in 40 patients (23 female, 17 male) who underwent FOA using either the open approach or distraction osteogenesis (DO) between 1987 and 2018. The mean follow-up period was 90.62 months. The z-scores of HC, CT-based intracranial volume, anteroposterior diameter (APD), biparietal diameter (BPD), and cranial height (CH) were calculated using sex- and age-specific standards. Logistic regression analysis was performed. RESULTS: While the z-scores of HC, intracranial volume, and BPD remained within the normal range, the z-scores of APD fluctuated between -2 and -1, and the z-scores of CH were > 2, indicating a substantial elevation compared with norms from T0 to T2. Age at surgery significantly influenced the z-scores of HC, BPD, and CH at T2 (all p < 0.05). Delayed surgical timing was correlated with increased BPD and CH z-scores from T1 to T2 (p = 0.007 and 0.019, respectively). The DO for FOA resulted in elevated HC z-scores at T2 and increased APD from T0 to T1, followed by a significant APD relapse from T1 to T2. CONCLUSIONS: These findings suggest that delayed surgical timing may support better cranial growth, as indicated by increased HC at long-term follow-up. However, delayed timing is also associated with worsening abnormally elevated CH. Despite the immediate APD expansion and long-term HC increase with DO, potential relapse warrants caution. While intentional overcorrection of APD is recommended, careful consideration of surgical timing and planning is essential.

Molecular oxygen (O2) concentration is measured by employing nanosecond laser-induced plasmas (ns-LIP) over a broad temperature spectrum ranging from 300 K to 1000 K, in the presence of an additional oxygen-containing molecule, CO2. Typically, emission spectra emanating from ns-LIP are devoid of molecular information, as the ns-LIP causes the dissociation of molecular species within the plasma. However, atomic oxygen absorption lines that momentarily appear at 777 nm in the broadband emission from the early-stage plasma are determined to be highly sensitive to the O2 mole fraction but negligibly affected by the CO2 mole fraction. The atomic O absorbing the plasma emission originates from the O2 adjacent to the plasma: robust UV radiation from the early-stage plasma selectively dissociates adjacent O2, exhibiting a relatively low photodissociation threshold, thus generating the specific meta-stable oxygen capable of absorbing photons at 777 nm. A theoretical model is introduced, explicating the formation of the meta-stable O atom from adjacent O2. To sustain the UV radiation from the plasma under high-temperature and low-density ambient conditions, a preceding breakdown is triggered by a split laser pulse (532 nm). This breakdown acts as a precursor, seeding electrons to intensify the inverse-Bremsstrahlung photon absorption of the subsequent laser pulse (1064 nm). Techniques such as proper orthogonal decomposition (POD) and support vector regression (SVR) are employed to precisely evaluate the O2 mole fraction (<1% uncertainty), by analyzing the short-lived (<10 ns) O2-indicator depicted in the early-stage plasma.

Gas composition in randomly distributed and fast-moving bubbles was optically measured aided by laser-induced breakdown spectroscopy (LIBS). Laser pulses were focused at a point in a stream of bubbles to induce plasmas for the LIBS measurements. The distance between the laser focal point and liquid-gas interface, or 'depth,' plays a major role in determining the plasma emission spectrum in two-phase fluids. However, the 'depth' effect has not been investigated in previous studies. Therefore, we evaluated the 'depth' effect in a calibration experiment near a still and flat liquid-gas interface using proper orthogonal decomposition, and a support vector regression model was trained to exclude the influence of the interfacing liquid and extract gas composition information from the spectra. The gaseous molecular oxygen mole fraction in the bubbles was accurately measured under realistic two-phase fluid conditions.

BACKGROUND: Elastic ear cartilage is a good source of tissue for support or augmentation in plastic and reconstructive surgery. However, the amount of ear cartilage is limited and excessive use of cartilage can cause deformation of the auricular framework. This animal study investigated the potential of periosteal chondrogenesis in an ear cartilage defect model. METHODS: Twelve New Zealand white rabbits were used in the present study. Four ear cartilage defects were created in both ears of each rabbit, between the central artery and marginal veins. The defects were covered with perichondrium (group 1), periosteum taken from the calvarium (group 2), or periosteum taken from the tibia (group 3). No coverage was performed in a control group (group 4). All animals were sacrificed 6 weeks later, and the ratio of neo-cartilage to defect size was measured. RESULTS: Significant chondrogenesis occurred only in group 1 (cartilage regeneration ratio: mean± standard deviation, 0.97± 0.60), whereas the cartilage regeneration ratio was substantially lower in group 2 (0.10± 0.11), group 3 (0.08± 0.09), and group 4 (0.08± 0.14) (p= 0.004). Instead of chondrogenesis, osteogenesis was observed in the periosteal graft groups. No statistically significant differences were found in the amount of osteogenesis or chondrogenesis between groups 2 and 3. Group 4 showed fibrous tissue accumulation in the defect area. CONCLUSION: Periosteal grafts showed weak chondrogenic potential in an ear cartilage defect model of rabbits; instead, they exhibited osteogenesis, irrespective of their embryological origin.

BACKGROUND: Successful reconstruction of the feet and ankles remains challenging due to limited quantities of soft tissue and laxity. The free lateral arm flap (LAF) is an alternative to conventional flaps and has been widely used due to advancements in its flap characteristics. This study is aimed at utilizing the advantages of this flap to validate its increased applications for foot and ankle defects. METHODS: Twenty patients with various LAF types between May 2011 and May 2020 were enrolled. Clinical data was retrospectively collected, and defect sites were classified according to the subunit principle. We utilized various LAF types, such as LAFs with sensate, extended, osteomyocutaneous, or myocutaneous flaps, as necessary. A two-point discrimination test was performed, and results were statistically compared between flaps. RESULTS: Among the diverse etiologies of skin defects, chronic inflammation was the most common cause of defects. Various LAF types, including LAFs with fasciocutaneous, extended fasciocutaneous, musculocutaneous, and osteomyocutaneous flaps, were used. The versatility of free LAF helped successfully cover various defects in all cases. Results of the two-point discrimination test were statistically significant between groups. CONCLUSIONS: Free LAF is a unique soft tissue free flap that is more versatile than other flaps, allowing flaps to be continuously modified and applied to various foot and ankle defects under different clinical conditions.

To learn through feedback, feedback should be reliable. However, if feedback is blurred by irrelevant social information, learning in a volatile environment, which requires fast learning and adaptation, might be disturbed. In this study, we investigated how feedback with social noise interferes with learning in a volatile environment by designing a probabilistic associative learning task in which the association probability changes dynamically, and the outcome was randomly blurred by an emotional face with incongruent valence. Learning in this situation was modelled by HGF-S such that emotionally incongruent feedback induces perceptual uncertainty called social noise. The Bayesian model comparison showed that the HGF-S model explains the subjects' behaviour well, and the simulation showed that social noise interrupts both learning the association probability and the volatility. Furthermore, the learning interruption influenced the subsequent decision. Finally, we found that the individual difference in how the same emotionally incongruent feedback induces social noise in varying degrees was related to the differences in event-related desynchronization induced by happy and sad faces in the right anterior insula, which encodes the degree of emotional feeling. These results advance our understanding of how feedback with emotional interference affects learning.

Previous studies have reported an association between verbal abuse in early childhood and structural and functional alterations in the young adult brain, supporting the existence of critical periods in human brain development. In addition, exposure to verbal abuse in early childhood is strongly correlated with lifetime psychiatric illness. Resilience is defined as the ability to avoid the negative psychological, biological, and social consequences of stress that impair psychological and physical homeostasis and is used to cope with these psychiatric diseases. We attempted to explain the mediatable present function of resilience and its associations with several psychiatric disorders, with verbal abuse exposure in early childhood and with the present value of the readily measurable and conceptually connected generative Bayesian model parameter. Thirty-six subjects performed a cross-modal associative learning task requiring them to learn the predictive strength of auditory cues and predict a subsequent visual stimulus. The probability of the association changed across each trial block. Subjects' responses were modeled as a hierarchical Bayesian belief-updating process using a hierarchical Gaussian filter (HGF) with three levels, a Sutton K1 model, and a Rescorla-Wagner model. Subjects completed the Korean version of the Verbal Abuse Questionnaire (VAQ) for segmented periods (aged 0 to 6, 7 to 12, and 13 to 18 years), and their positive self-appraisal was estimated using the Resilience Appraisal Scale (RAS). Random-effects Bayesian model selection identified HGF as the best model. Of the VAQ values for specific periods, only preschool VAQ scores were negatively correlated with RAS scores. The tonic volatility parameter, ω2, of HGF showed a negative relationship with RAS emotion coping scores. The linear regression model explained 18.3% of the variance of emotion coping appraisal with ω2 and preschool VAQ scores. Based on the results obtained from young adults, decrease in emotion coping appraisal can be explained by an increase in the number of experiences of verbal abuse in early childhood and the increased tendency to update beliefs about the cue-outcome associative probability in a volatile environment.
