The present study reports a rare case of a renal clear cell carcinoma with thyroid and parotid metastases. A 56-year-old female, with a painless, right preauricular mass present for 6 months was referred to Renji Hospital (Shanghai, China). Physical examination revealed a mass of 3×3 cm, which was smooth, firm, immobile and non-tender. There was no accompanying facial weakness. Parotid ultrasonography revealed a hypoechoic mass within the right parotid gland, which was potentially a parotid mixed tumor. In July 2011, the patient underwent a superficial parotidectomy with preservation of the facial nerve. Pathology confirmed as right parotid clear cell carcinoma (metastasis). The patient's relevant medical history included a right radical nephrectomy for renal clear cell carcinoma (clinical stage III) in 2004. Additionally, in 2009, the patient underwent a resection of thyroid metastatic renal cell carcinoma. To the best of our knowledge, no similar case has previously been reported in English-language literature. The present study discusses a case report, and investigates the clinical features and treatment strategy.

OBJECTIVES/HYPOTHESIS: Abnormal interaction of epithelial cells with laminin component of basement membrane may account for altered biological behavior of cells, influencing proliferation, adhesion, and motility. In the current study, we investigated the role of 67-kDa laminin receptor (67LR), a high affinity receptor for laminin, in aggressiveness of laryngeal squamous cell carcinoma. METHODS: Thirty paraffin-embedded specimens and 20 fresh tissues of patients with laryngeal squamous cell carcinoma were analyzed using immunohistologic and reverse-transcriptase polymerase chain reaction techniques, respectively. Expression of 67LR on the surface of AMC-HN-8 cells was examined by flow cytometry. The effect of 67LR monoclonal antibody (MLuC5) on the adhesive and invasive abilities of AMC-HN-8 cells was determined by adherence and invasion inhibition assay in vitro. RESULTS: Both at the mRNA and protein level, laryngeal carcinoma cells expressed higher level of 67LR than normal epithelial cells (P < .01). The expression of 67LR correlated inversely with differentiation extent of tumor (P < .05). 67LR level was significantly increased in patients with lymph node metastases than those without lymph node involvement (P < .05). Flow cytometry showed 80.9 +/- 0.9% of AMC-HN-8 cells expressed 67LR. After 60 minutes and 120 minutes of incubation, MluC5 induced 57.1 +/- 3.6% and 63.2 +/- 2.8% inhibition of adhesion, respectively. The invasive ability of AMC-HN-8 cells to matrigel was reduced by MLuC5. CONCLUSIONS: Laryngeal carcinoma cells over-expressing 67LR have a stronger aggressive potential, which might make 67LR a promising target for the treatment of metastatic tumor.

OBJECTIVE: To discuss outcome of thyroid tumor patients treated with surgery. METHODS: Total number of patients was 2228. These patients of thyroid tumors from 1992-2004 (2072 cases of benign thyroid diseases and 156 cases of thyroid carcinoma) were recruited. The clinical and follow-up datum were retrospective analyzed. RESULTS: (1) Benign thyroid tumors with near-total thyroidectomy including 1761 thyroid adenoma, 207 nodular goiter and 104 Hashimoto thyroiditis, the incidence of recurrent laryngeal nerve paralysis was 0.2%, 55 cases (2.6%) received secondary surgery. All the patients have no hypocalcemia or hemorrhage after operation. (2) Eighty-one cases of papillary carcinoma of the thyroid ( > 1 cm) and 60 cases of microcarcinoma. Unilateral thyroidectomy, contralateral near-total thyroidectomy and ipsilateral modified neck dissection were performed in unilateral papillary carcinoma of thyroid. Among the 9 cases of follicular carcinoma of thyroid, 7 were performed of near-total thyroidectomy without neck dissection, others were the same as papillary carcinoma. Bilateral total thyroidectomy and bilateral modified neck dissection were performed in 2 cases of the medullary thyroid cancer and 1 case of the undifferentiated thyroid cancer. By direct method the 5-year survival was 95.5% (64/67), and by Kaplan-Meier method, it was 98.0%. The treatment of microcarcinoma are multiple. There is no relapse or metastases in 60 cases of papillary thyroid microcarcinoma. The 5-year survival was 100.0%, 1 cases occurred recurrent laryngeal nerve paralysis in thyroid cancer. No hypocalcemia or hemorrhage. Eight case relapsed in 156 cases of thyroid carcinoma,3 cases died. CONCLUSION: The correct surgical management for the patients with thyroid tumor should benefit for the prognosis and reduce the complications and the recurrence of the operation.

OBJECTIVE: To investigate the role of 67 000 laminin receptor (LN-R) in the processes of invasion and metastasis of laryngeal squamous cell carcinoma. METHODS: Expression of 67 000 LN-R mRNA in 20 cases with laryngeal squamous cell carcinomas and corresponding normal tissues was determined by RT-PCR, and the relationship between its expression level and patients' clinicopathological features was analyzed. Expression of 67 000 LN-R on the surface of AMC-HN-8 laryngeal carcinoma cells was examined by flow cytometry. The effect of 67 000 LN-R monoclonal antibody (MLuC5) on the adhesive and invasive abilities was observed by adhesion test and Boyden chamber invasiveness test in vitro. RESULTS: The expression level of 67 000 LN-R mRNA in human laryngeal squamous cell carcinoma was significantly higher than that in normal tissues (P < 0.05). The expression level of 67 000 LN-R mRNA in laryngeal squamous cell carcinomas with cervical lymph node metastases was higher than in those without cervical lymph node metastases (P < 0.05). There was a significant negative correlation between the expression level of 67 000 LN-R mRNA and the degree of tumor differentiation, the level being higher in poorly differentiated tumors (P < 0.05). Flow cytometry showed that (80.9 +/- 0.9)% of AMC-HN-8 cells expressed 67 000 LN-R. MLuC5 inhibited the adhesion of AMC-HN-8 cells on LN, and after treated with MLuC5 for 60 and 120 minutes, the adhesion inhibition rate was 57.1% and 63.2%, respectively. The invasive ability to artificial basement membrane was reduced by MLuC5. CONCLUSION: Laryngeal carcinoma overexpressing 67 000 LN-R has stronger invasiveness, and 67 000 LN-R monoclonal antibody may contribute to prevent metastasis of laryngeal carcinoma.
