RESUMEN

The occurrence of unprobable adverse events during laparoscopic surgery has increased over the years. Among them, pituitary apoplexy has been reported only twice. The increase in the abdominal pressure might play a role in the pituitary apoplexy, as well as hemodynamic instability, anticoagulant drugs and air-embolism due to insufflation of CO2 during pneumoperitoneum. We report a case of pituitary apoplexy during laparoscopic resection of sigmoid colon.

RESUMEN

Golgi cells have a central position in the cerebellar cortical network and are indirectly connected to Purkinje cells, which are important for the acquisition of learned responses in classical conditioning. In order to clarify the role of Golgi cells in classical conditioning, we made extracellular Golgi cell recordings during different stages of conditioning, using four different conditional stimuli. Our results show that forelimb and superior colliculus stimulation, but not mossy fiber stimulation, evokes a short latency increase in Golgi cell firing. These results suggest that Golgi cells are involved in modulating input to the cerebellar cortex. There were however no differences in Golgi cell activity between naïve and trained animals, which suggests that Golgi cells are not intimately involved in the plastic changes that occur during classical conditioning. The absence of long latency effects of the conditional stimulus also questions whether Golgi cells contribute to the generation of a temporal code in the granule cells.

RESUMEN

When adsorbed on the strongly anisotropic Pt(110) surface Br forms a sequence of (n × 1) structures. In the present study we investigate the (4 × 1) structure by scanning tunneling microscopy, quantitative low-energy electron diffraction and density-functional calculations. We show that the optimal structural model contains essentially the same adsorption sites as the (3 × 1) structure, but with a different preference. The positions of the substrate atom are consistent with a frozen surface phonon of fourfold periodicity, suggesting that the phase diagram can be understood on the basis of a tunable charge density wave (Swamy et al 2001 Phys. Rev. B 86 1299). The structure could also be explained by assuming short-range interactions only, but evidence is presented that adsorbate-adsorbate interactions mediated by quasi-one-dimensional surface resonances play a major role in both cases.

Contrast injection cerebral angiography has been considered for several decades the "gold-standard" technique for diagnosis and operative planning of carotid disease. More recently, however, an increasing number of institutions are using duplex ultrasound as the single independent preoperative test. The objective of this investigation was to evaluate the impact of the utilization of duplex ultrasonography as the only preoperative test on the outcome of the procedure. Between 1993 and 1996, the authors performed 1,149 carotid procedures. Duplex ultrasound as the only preoperative test was employed with increasing frequency in a total of 728 cases. In 1995 and 1996, a cerebral arteriogram was performed only if duplex ultrasound was technically inadequate or questionable or showed an atypical pattern of disease. During the 4 years analyzed in this study, the number of the procedures increased from 165 in 1993 to 412 in 1996. The thirty-day mortality rate was 0.43%, and neurologic morbidity was 1.65%. According to the year in which the procedure was performed, the mortality/morbidity rates were 1.2/2.4 in 1993, 0.52/2.08 in 1994, 0.26/1.57 in 1995, and 0.24/1.21 in 1996. Indication to perform an arteriogram became very selective in 1995. Regardless of these changes in the diagnostic work-up, some degree of reduction in both 30-day mortality and neurologic morbidity was recorded. Considering a cost of 724 European Currency Units (ECU) per arteriogram, 527,072 ECU were saved in this period. In the last 4 years, duplex ultrasound has replaced arteriography as the first-choice technique for preoperative assessment of carotid disease at the authors' institution. There was definitely no detrimental effect on the clinical results that, on the contrary, improved during the same period. This policy has allowed a significant reduction in the cost of the procedure and has most likely prevented several arteriography-related complications. The authors recommend this policy to all institutions in which accurate duplex ultrasound is available.

One thousand and fourteen women who were delivered in the Obst. Gynec. Clinic "L. Mangiagalli" were investigated to evaluate the real incidence of varicose disease, its correlation with some factors and the development of complication. The observation period was comprised between the 1st and the 4th day of puerperium in which women were investigated by obstetricians through a detailed questionnaire and underwent an angiological visit. The mean age of the puerperae was 29.6 years (range 15-47 yrs); 587 women were primiparas (57.9%). The prevalence of varicose disease's different aspects resulted 57.9%, while the incidence related to pregnancy was 26.1%. The prevalence and the incidence resulted to be respectively 26.8% and 14.4% for varicosities, 5.6% and 3.1% for saphenous varicosis. Complications were observed in 10 patients (0.99%). 45.3% of patients reported family positivity for varicose disease; this group has shown a statistically significant difference (p less than 0.01) for the onset of different aspects of varicose syndrome.

RESUMEN

A wavelength-modulation spectrometer with a spectral range of 2000-8000 A has been constructed. The system measures simultaneously the derivative spectrum and the conventional spectrum. The derivative spectrum has a sensitivity of DeltaR/R ~ 10(-5) and is completely free of background trouble. With proper changes in the optical components, the spectral range of the spectrometer can be extended further into the infrared and into the uv.
