RESUMEN

BACKGROUND: The anterolateral (juxtacondylar) approach with limited mastoidectomy is a suitable option to expose the postero-inferior part of the jugular foramen (JF). It is particularly indicated for tumors extending in the neck beyond the jugular foramen, especially in those cases necessitating both neck control as well as control of the mastoid segment of facial nerve. METHOD: We describe here the steps to safely perform an anterolateral approach with mastoidectomy along with a brief description of its indications and limits. CONCLUSION: This approach represents a valid option to reach the JF. Its knowledge can improve the process of optimal approach selection when dealing with complex pathology involving the JF.

RESUMEN

BACKGROUND: The optimal management of petroclival meningiomas (PCMs) continues to be debated along with several controversies that persist. METHODS: A task force was created by the EANS skull base section along with its members and other renowned experts in the field to generate recommendations for the management of these tumors. To achieve this, the task force reviewed in detail the literature in this field and had formal discussions within the group. RESULTS: The constituted task force dealt with the existing definitions and classifications, pre-operative radiological investigations, management of small and asymptomatic PCMs, radiosurgery, optimal surgical strategies, multimodal treatment, decision-making, and patient's counselling. CONCLUSION: This article represents the consensually derived opinion of the task force with respect to the management of PCMs.

RESUMEN

BACKGROUND: The anterolateral approach (ALA) enables to access the craniovertebral junction (CVJ), lower and middle clivus, jugular foramen, and cervical spine from a lateral perspective. It is particularly indicated when dealing with extradural bone tumors. Other rare indications are represented by spondylotic myeloradiculopathy and vascular diseases. METHOD: We describe here the steps to safely perform an anterolateral approach along with a brief description of its indications and limits. CONCLUSION: ALA represents a valid option to treat cervical spine and CVJ bone tumors such as chordomas. Its knowledge can improve the process of approach selection when dealing with such complex cases.

RESUMEN

Nickel deficiency leads to reduced iron content in organs and to reduced hemoglobin levels and haematocrit counts. We carried out a clinical trial of oral hyposensitization therapy with low doses of nickel on a group of 20 patients affected by systemic allergy to this sensitizer element and with a chronic relapsing low hemoglobin level and haematocrit count. We obtained interesting results on maintaining these values in treated patients.

RESUMEN

Lichen amyloidosus is a cutaneous dermatosis clinically characterized by an hyperkeratotic brownish-grey papular eruption located on trunk and extremities, associated to severe pruritus. Histologically it is characterized by amyloid deposits in the papillary derma. Amyloid is an amorphous substance, probably derived by apoptotic keratinocites and other protein such as Apolipoprotein E. The exact pathogenesis of this deposition is not yet understood but some factors can act as favourable agents, such as pruritus. In fact this dermatosis is often associated to intensely pruritic conditions. Its treatment is very difficult but is important, because patients refer a bad quality of life. In literature have been described a lot of therapeutic options, often unefficacious. We describe herein the case of a 39-year old patient affected by Lichen amyloidosis, treated with acitretin a good improvement of cutaneous lesions and a complete resolution of pruritus after only three weeks of treatment. At actual follow-up of eight months the results are maintained.
