RESUMEN

The migration phenomenon is increasingly common worldwide. It is essential for radiologists to be aware of the endemic diseases of the migrant's country as well as the characteristics of the journey to be able to understand and interpret radiological findings when admitted to our centre. This article aims to use imaging from our centre to describe the most common pathologies that migrant patients present with after long journeys by boat.

BACKGROUND: In the past few years, the Canary Islands received immigrants from sub-Saharan countries that arrive to the coast after long boat trips in extreme adverse conditions. The named «trench foot¼ is a previously described infectious entity developed after feet wounds that get infected by being in contact with water, urine and excrements in these small and crowded boats. We describe a new clinical entity, not published yet, that consists in massive edema in the extremities associating necrosis of the skin and subcutaneous tissue that characteristically respects the muscle and with a non-infectious etiology. MATERIAL AND METHODS: A database including 86 patients arrived by boat («patera¼) from sub-Saharan countries from September 2020 to January 2022 was made and 39 qualitative and quantitative items were analyzed. The Research Unit performed an observational prospective statistical analysis. RESULTS: A total of 16 patients developed the entity described as necrotizing cellulitis. Its physiopathology completely differs from the one described in the trench foot, since all the cultures resulted sterile. CONCLUSIONS: We postulate an inflammatory theory due to the osmotic effect from ingesting sea water and/or the aggressive fluid reposition when they arrive to the emergency room with severe dehydration and hypernatremia. Early surgical evacuation of the edema with escharotomies incisions alleviate the symptoms and prevents progression of the disorder.

The Martian crustal dichotomy (MCD) between the southern highlands and the northern lowlands is the planet's most ancient crustal structure, but its origins and evolution remain enigmatic. Understanding of the MCD comes largely from present-day and shallow crustal constraints. Lacking ancient and deeper constraints, hypotheses for the origin of the MCD range from an early giant impact, partial melting from sustained mantle convection, or some combination. We investigate with seismological modeling the best-preserved case of the "antipodal effect"-energy from an impact that concentrates and induces uplift and fracturing promoting volcanism at its antipode-the Hellas crater and the Alba Patera volcano on Mars. The volcano is latitudinally offset âˆ¼2° (∼119 km) from the expected antipode, and we explore whether the MCD can explain this deflection. Variations across the MCD in topography, thickness, and composition have only minor effects. Simulations capable of sufficiently decelerating southern surface waves require the presence of 2%-5% more partial melt in the southern highlands. As the age of impact ca. 4 billion years ago post-dates the formation of the MCD, our partial melting results thus imply that, with or without an early giant impact, the MCD was modified by mantle convection in order to supply enough heat for crustal melts for several hundreds of millions of years after Mars formation.

INTRODUCTION: Shewanella spp. can cause severe skin and soft-tissue infections, gastrointestinal infections, otitis and bacteraemia, generally upon contact with seawater or consumption of raw seafood. Recently, a new condition termed 'patera foot' characterized by acute skin and soft-tissue infection has been described in irregular immigrants arriving to the Canary Islands, Spain, in rudimentary boats. Most infections are caused by a single species, Shewanella algae. The improvement of the diagnostic capabilities in clinical microbiology laboratories has resulted in a growing number of cases being reported worldwide, most of them coming from warm regions. CASE PRESENTATION: In this work, we reviewed the medical records of all the patients with Shewanella infections in the two university hospitals of Gran Canaria (the Canary Islands, Spain) during the period 2000-2016, resulting in the identification of 31 cases. We also conducted a literature review of Shewanella infections reported worldwide in recent years. CONCLUSION: This case series suggests that Shewanella infections are nosocomially acquired more frequently than previously thought. In addition, the unexpectedly high proportion of multidrug-resistant isolates raises concerns.
