RESUMEN

STUDY QUESTION: What are the potential risk factors for poor oocyte recuperation rate (ORR) and oocyte immaturity after GnRH agonist (GnRHa) ovulation triggering? SUMMARY ANSWER: Lower ovarian reserve and LH levels after GnRHa triggering are risk factors of poor ORR. Higher BMI and anti-Müllerian hormone (AMH) levels are risk factors of poor oocyte maturation rate (OMR). WHAT IS KNOWN ALREADY: The use of GnRHa to trigger ovulation is increasing. However, some patients may have a suboptimal response after GnRHa triggering. This suboptimal response can refer to any negative endpoint, such as suboptimal oocyte recovery, oocyte immaturity, or empty follicle syndrome. For some authors, a suboptimal response to GnRHa triggering refers to a suboptimal LH and/or progesterone level following triggering. Several studies have investigated a combination of demographic, clinical, and endocrine characteristics at different stages of the treatment process that may affect the efficacy of the GnRHa trigger and thus be involved in a poor endocrine response or efficiency but no consensus exists. STUDY DESIGN, SIZE, DURATION: Bicentric retrospective cohort study between 2015 and 2021 (N = 1747). PARTICIPANTS/MATERIALS, SETTING, METHODS: All patients aged 18-43 years who underwent controlled ovarian hyperstimulation and ovulation triggering by GnRHa alone (triptorelin 0.2 mg) for ICSI or oocyte cryopreservation were included. The ORR was defined as the ratio of the total number of retrieved oocytes to the number of follicles >12 mm on the day of triggering. The OMR was defined as the ratio of the number of mature oocytes to the number of retrieved oocytes. A logistic regression model with a backward selection method was used for the analysis of risk factors. Odds ratios (OR) are displayed with their two-sided 95% confidence interval. MAIN RESULTS AND THE ROLE OF CHANCE: In the multivariate analysis, initial antral follicular count and LH level 12-h post-triggering were negatively associated with poor ORR (i.e. below the 10th percentile) (OR: 0.61 [95% CI: 0.42-0.88]; P = 0.008 and OR: 0.86 [95% CI: 0.76-0.97]; P = 0.02, respectively). A nonlinear relationship was found between LH level 12-h post-triggering and poor ORR, but no LH threshold was found. A total of 25.3% of patients suffered from oocyte immaturity (i.e. OMR < 75%). In the multivariate analysis, BMI and AMH levels were negatively associated with an OMR < 75% (OR: 4.34 [95% CI: 1.96-9.6]; P < 0.001 and OR: 1.22 [95% CI: 1.03-1.12]; P = 0.015, respectively). Antigonadotrophic pretreatment decreased the risk of OMR < 75% compared to no pretreatment (OR: 0.72 [95% CI: 0.57-0.91]; P = 0.02). LIMITATIONS, REASONS FOR CAUTION: Our study is limited by its retrospective design and by the exclusion of patients who had hCG retriggers. However, this occurred in only six cycles. We were also not able to collect information on the duration of pretreatment and the duration of wash out period. WIDER IMPLICATIONS OF THE FINDINGS: In clinical practice, to avoid poor ORR, GnRHa trigger alone should not be considered in patients with higher BMI and/or low ovarian reserve, balanced by the risk of ovarian hyperstimulation syndrome. In the case of a low 12-h post-triggering LH level, practicians must be aware of the risk of poor ORR, and hCG retriggering could be considered. STUDY FUNDING/COMPETING INTEREST(S): None. BACKGROUND: Testicular sperm aspiration (TESA) is widely used in the diagnosis and management of nonobstructive azoospermia. However, its ability for predicting microdissection testicular sperm extraction in nonobstructive azoospermia (NOA) patients with AZFc deletion remains uncertain. To investigate whether TESA affected the sperm retrieval rate (SRR) in NOA patients with AZFc deletion, a retrospective analysis of the clinical data of NOA patients with AZFc deletion who underwent microdissection testicular sperm extraction (micro-TESE) was conducted. The effects of age, testicular volume, follicle-stimulating hormone (FSH) levels, luteinizing hormone (LH) levels, testosterone (T) levels and TESA on the SRR were analyzed in this group of patients. RESULTS: A total of 181 individuals had their sperm successfully collected and underwent micro-TESE, with an SRR of 67.4%. The patients were separated into two groups based on their micro-TESE results (sperm acquisition and nonsperm acquisition), with no significant variations in age, testicular volume, FSH levels, LH levels, or T levels between the two groups. There was no significant difference in the SRR between any of the groups into which patients were classified based on reproductive hormone reference value ranges. Binary logistic regression was used to explore the absence of significant effects of age, testicular volume, FSH levels, LH levels, and T levels on sperm acquisition in patients undergoing micro-TESE. In the preoperative testicular diagnostic biopsy group, the sperm acquisition and nonsperm acquisition groups had SRRs of 90.1% and 65.1%, respectively. More significantly, there was no significant difference in the SRR between the negative preoperative testicular diagnostic biopsy group and the nonpreoperative testicular diagnostic biopsy group (65.1 vs. 63.8%, p = 0.855). CONCLUSION: There is a high probability of successful sperm acquisition in the testis of men undergoing micro-TESE. In this group of patients, age, testicular volume, FSH levels, LH levels, and T levels may have little bearing on the micro-TESE outcome. In patients whose preoperative TESA revealed the absence of sperm, the probability of obtaining sperm by micro-TESE remained high (65.1%); negative TESA results appeared to not influence the SRR (63.8%) in patients undergoing micro-TESE.


RéSUMé: CONTEXTE: L'aspiration testiculaire de spermatozoïdes (TESA) est largement utilisée dans le diagnostic et la prise en charge de l'azoospermie non obstructive. Cependant, sa capacité à prédire la présence de spermatozoïdes testiculaires lors de l'extraction par microdissection chez les patients atteints d'azoospermie non obstructive (NOA) et porteurs de la délétion AZFc reste incertaine. Pour déterminer si la TESA affectait le taux de récupération de spermatozoïdes (SRR) chez les patients atteints d'ANO avec délétion AZFc, nous avons mené une analyse rétrospective des données cliniques des patients atteints de NOA et d'une délétion AZFc ayant subi une extraction testiculaire de spermatozoïdes (micro-TESE) par microdissection. Les effets de l'âge, du volume testiculaire, des taux d'hormone folliculostimulante (FSH), d'hormone lutéinisante (LH), de testostérone (T) et de TESA sur le SRR ont été analysés chez ces patients. RéSULTATS: Au total, 181 personnes ont eu leur spermatozoïdes collectés avec succès par micro-TESE, avec un SRR de 67,4%. Les patients ont été répartis en 2 groupes en fonction de leurs résultats à la micro-TESE (obtention de spermatozoïdes et non obtention de spermatozoïdes), sans variations significatives de l'âge, du volume testiculaire, des taux de FSH, LH ou de T entre les 2 groupes. Aucune différence significative du SRR n'a été retrouvée entre les groupes dans lesquels les patients ont été classés en fonction des plages de valeurs de référence des hormones reproductives. La régression logistique binaire a été utilisée pour explorer l'absence d'effets significatifs de l'âge, du volume testiculaire, des taux de FSH, de LH et de T sur la récupération de spermatozoïdes chez les patients bénéficiant d'une micro-TESE. Dans le groupe de biopsie diagnostique testiculaire préopératoire, les groupes avec récupération de spermatozoïdes et sans récupération de spermatozoïdes avaient respectivement des SRR de 90,1% et 65,1%. Plus important encore, il n'y avait pas de différence significative du SRR entre le groupe de biopsie diagnostique testiculaire préopératoire négatif et le groupe sans biopsie diagnostique testiculaire préopératoire (65,1 vs 63,8%, p = 0,855). CONCLUSIONS: Il existe une forte probabilité de récupération réussie de spermatozoïdes testiculaires chez les hommes bénéficiant d'une micro-TESE. Dans ce groupe de patients, l'âge, le volume testiculaire, les taux de FSH, de LH et de T ont peu d'incidence sur le résultat de la micro-TESE. Chez les patients dont la TESA préopératoire a révélé l'absence de spermatozoïdes, la probabilité d'obtenir des spermatozoïdes par micro-TESE est restée élevée (65,1%); les résultats négatifs d'une TESA ne semblaient pas influencer le SRR (63,8%) chez les patients bénéficiant d'une micro-TESE.

In the present study, we examined the development of mental fatigue during the kinesthetic motor imagery (MI) of isometric force contractions performed with the dominant upper limb. Participants (n = 24) underwent four blocks of 20 MI trials of isometric contractions at 20% of the maximal voluntary contraction threshold (20% MVCMI) and 20 MI trials of maximal isometric contractions (100% MVCMI). Mental fatigue was assessed after each block using a visual analogue scale (VAS). We assessed maximal isometric force before, during and after MI sessions. We also assessed MI ability from self-report ratings and skin conductance recordings. Results showed a logarithmic pattern of increase in mental fatigue over the course of MI, which was superior during 100% MVCMI. Unexpectedly, maximal force improved during 100% MVCMI between the 1st and 2nd evaluations but remained unchanged during 20% MVCMI. MI ease and vividness improved during 100% MVCMI, with a positive association between phasic skin conductance and VAS mental fatigue scores. Conversely, subjective measures revealed decreased MI ability during 20% MVCMI. Mental fatigue did not hamper the priming effects of MI on maximal force performance, nor MI's ability for tasks involving high physical demands. By contrast, mental fatigue impaired MI vividness and elicited boredom effects in the case of motor tasks with low physical demands.

The adsorption characteristics of titanium dioxide nanoparticles (nano-TiO2) for the removal of Pb(II) from irrigation water were investigated in this work. To accomplish this, several adsorption factors, such as contact time and pH, were tested to assess adsorption efficiencies and mechanisms. Before and after the adsorption experiments, commercial nano-TiO2 was studied using X-ray diffraction (XRD), scanning and transmission electron microscopy (SEM and TEM), energy dispersive spectroscopy (EDS), and X-ray photoelectron spectroscopy (XPS). The outcomes showed that anatase nano-TiO2 was remarkably efficient in cleaning Pb(II) from water, with a removal efficiency of more than 99% after only one hour of contact time at a pH of 6.5. Adsorption isotherms and kinetic adsorption data matched the Langmuir and Sips models quite well, showing that the adsorption process occurred at homogenous sites on the surface of nano-TiO2 by forming a Pb(II) adsorbate monolayer. The XRD and TEM analysis of nano-TiO2 following the adsorption procedure revealed a non-affected single phase (anatase) with crystallite sizes of 9.9 nm and particle sizes of 22.46 nm, respectively. According to the XPS data and analyzed adsorption data, Pb ions accumulated on the surface of nano-TiO2 through a three-step mechanism involving ion exchange and hydrogen bonding mechanisms. Overall, the findings indicate that nano-TiO2 has the potential to be used as an effective and long-lasting mesoporous adsorbent in the treatment and cleaning of Pb(II) from water bodies.

At the stroke consultation desk, a stroke specialist is in charge, and each professional who is familiar with stroke, such as certified nurses, medical social workers, physical therapists, occupational therapists, speech therapists, pharmacists, registered dietitians, and clinical psychologists (certified public psychologists), collaborate as counselors for stroke care and support. Those team provide information and support to their families on medical care, welfare, nursing care, etc., and share information with collaborating medical institutions.

Traumatic brain injury (TBI) remains one of the leading causes of death and disability worldwide. To better understand its impact on various outcome domains, this study pursues the following: (1) longitudinal outcome assessments at three, six, and twelve months post-injury; (2) an evaluation of sociodemographic, premorbid, and injury-related factors, and functional recovery contributing to worsening or improving outcomes after TBI. Using patient-reported outcome measures, recuperation trends after TBI were identified by applying Multivariate Latent Class Mixed Models (MLCMM). Instruments were grouped into TBI-specific and generic health-related quality of life (HRQoL; QOLIBRI-OS, SF-12v2), and psychological and post-concussion symptoms (GAD-7, PHQ-9, PCL-5, RPQ). Multinomial logistic regressions were carried out to identify contributing factors. For both outcome sets, the four-class solution provided the best match between goodness of fit indices and meaningful clinical interpretability. Both models revealed similar trajectory classes: stable good health status (HRQoL: n = 1944; symptoms: n = 1963), persistent health impairments (HRQoL: n = 442; symptoms: n = 179), improving health status (HRQoL: n = 83; symptoms: n = 243), and deteriorating health status (HRQoL: n = 86; symptoms: n = 170). Compared to individuals with stable good health status, the other groups were more likely to have a lower functional recovery status at three months after TBI (i.e., the GOSE), psychological problems, and a lower educational attainment. Outcome trajectories after TBI show clearly distinguishable patterns which are reproducible across different measures. Individuals characterized by persistent health impairments and deterioration require special attention and long-term clinical monitoring and therapy.

Assisted reproductive technology (ART) is increasingly influencing the fertility trends of high-income countries characterized by a pattern of delayed childbearing. However, research on the impact of ART on completed fertility is limited and the extent to which delayed births are realized later in life through ART is not well understood. This study uses data from Australian fertility clinics and national birth registries to project the contribution of ART for cohorts of women that have not yet completed their reproductive life and estimate the role played by ART in the fertility 'recuperation' process. Assuming that the increasing trends in ART success rates and treatment rates continue, the projection shows that the contribution of ART-conceived births to completed fertility will increase from 2.1% among women born in 1968 to 5.7% among women born in 1986. ART is projected to substantially affect the extent to which childbearing delay will be compensated at older ages, suggesting that its availability may become an important factor in helping women to achieve their reproductive plans later in life. Pharmaceuticals are able to evade conventional wastewater treatments and therefore, are recurrently found in the environment with proven potential to cause harm to human and wildlife. Adsorption onto activated carbon (AC) is a promising complement. However, AC production from non-renewable resources and its difficult after-use recuperation are prohibitive. Hence, a waste-based magnetic activated carbon (MAC) was produced from paper mill sludge, via an ex-situ synthesis, for the adsorptive removal of carbamazepine (CBZ), sulfamethoxazole (SMX) and ibuprofen (IBU) from ultrapure water and wastewater. The MAC was obtained through the promotion of electrostatic interactions between magnetic and activated carbon particles in a water suspension at controlled pH between the points of zero charge of both surfaces. The optimized condition (MACX3) presented remarkable properties regarding specific surface area (SBET=795 m2 g-1) and saturation magnetization (MS=19 emu g-1). Kinetic and equilibrium adsorption studies were performed under batch conditions. Adsorption equilibrium was reached in up to 30 min for all pharmaceuticals in both matrices, proving the low dependence on the adsorbate and the broad applicability of MACX3 in pharmaceutical adsorption. Regarding equilibrium experiments, high Langmuir maximum adsorption capacities (qm) were achieved in ultrapure water (up to 711 ± 40 µmol g-1). Equilibrium studies in wastewater revealed a decay in qm when compared to ultrapure water: 28% for CBZ (468 ± 20 µmol g-1 (111 ± 5 mg g-1)), 78% for SMX (145 ± 10 µmol g-1 (37 ± 3 mg g-1)) and 62% for IBU (273 ± 8 µmol g-1 (56 ± 2 mg g-1)), attributed to the wastewater pH, which dictates the speciation of the pharmaceuticals and controls electrostatic interactions between pharmaceuticals and MAC, and to competition effects by organic matter. It was demonstrated the promising applicability of a waste-based ex-situ MAC, rapidly retrievable from water, as an alternative tertiary wastewater treatment for pharmaceuticals removal.

Plants play a cardinal role in removing various pollutants through the synergistic interaction with filling materials and microbes of constructed wetlands (CWs). However, the information regarding the selection of plant species to remove pharmaceutically active compounds (PhACs) is not adequate. The present study attempted to select an appropriate plant species for CWs, considering their characteristics and physiological response to PhACs. In this regard, batch hydroponics studies were carried out to assess the removal, fate, and antioxidative response of carbamazepine (CBZ) in four wetland plant species (Canna indica, Colocasia esculenta, Phragmites australis, and Chrysopogon zizanioides). The specific uptake potential of CBZ (in terms of plant dry biomass) was found to be in the order: C. indica (14.48 mg/g) >P. australis (11.71 mg/g) >C. esculenta (8.67 mg/g) >C. zizanioides (6.04 mg/g). The results showed that exposure to CBZ (0-30 days) caused an accumulation of reactive oxygen species (ROS) in the plant tissues, causing a decline in chlorophyll content, root activity, and increased oxidative stress. However, the selected plants could recover from the oxidative damages to a certain extent in the recuperation phase (31-60 days). C. indica exhibited relatively lesser ROS accumulation and oxidative damage during the experimental phase than other selected plants. The study also showed that plant biomass, transpiration rate, chlorophyll content, root exudates, and root activity influenced the removal of CBZ by various plants (r - 0.76 to 0.98, P < 0.05). The mass balance analysis indicated that a significant proportion of CBZ (49.2 to 72.7 %) underwent metabolism in the plant tissues. Apart from higher removal, lesser accumulation, and lower oxidation stress, multi-criteria decision analysis showed that C. indica is a potential plant species for the removal of CBZ.

To solve the problem of the design of the old people's recuperation space, the virtual information interaction platform is used to study the public art application in the design of the old people's recuperation space. Firstly, the principles of interactive design are expounded, and secondly, the existing institutions for the old people are investigated. Under the premise of optimizing the functions of the facilities, the concepts of humanistic care, emotional care and humanization in public art are integrated into the design of the old people's rehabilitation space, to solve the long-term negative impression of the old people's repression and indifference to the old people's care institutions. The construction of the scene allows the old people to experience some operations with the help of the virtual information interaction platform. In the modern elderly rehabilitation space, the attention and application of public art design will inevitably bring spiritual and material help to the old people in their later years, and create a happy, peaceful, and comfortable elderly life for them. The survey results manifest that 65.3% of urban old people and 71.8% of rural old people feel that they cannot keep up with the pace of development. Through the analysis and discussion of the physiological and psychological characteristics of the old people, the whole survey denotes that the physiological functions of the old people are declining, which seriously affects their normal life. Therefore, the design of the rehabilitation space for the old people should not only meet the basic needs of life, but also analyze the space design from the perspective of humanization and emotion. An ecological, natural, and human settlement environment has been established. The recuperation space is designed for the needs of different old people, which helps the old people to eliminate loneliness, enhance their value of the old people, and make life full of joy and meaning for the old people.

Unaccustomed exercise causes muscle damage resulting in loss of muscle function, which may be attributable to exercise-induced increases in skeletal muscle reactive oxygen species. This study examined the effect of mitochondria-targeted antioxidant supplementation on recovery of muscle function following exercise. Thirty-two untrained men received MitoQ (20 mg/day) or a placebo for 14 days before performing  300 maximal eccentric contractions of the knee extensor muscles of 1 leg. Muscle function was assessed using isokinetic dynamometry before, immediately after, and 24, 48, 72, and 168 hours after exercise. Muscle soreness was assessed using a visual analogue scale 24, 48, 72, and 168 hours after exercise. Blood samples were collected before, immediately after, and 2, 24, 48, 72, and 168 hours after exercise and urine samples were collected before and during the 48 hours after exercise. The reduction in maximal voluntary isometric contraction force and peak concentric torque following exercise was unaffected by MitoQ while recovery of peak eccentric torque was delayed in the MitoQ group. Exercise-induced increases in urine F2-isoprostanes were unaffected by MitoQ. MitoQ augmented exercise-induced increases in plasma creatine kinase levels, while plasma IL-6 was similar between groups. Muscle soreness was not affected by MitoQ. These results indicate that MitoQ does not attenuate post-exercise muscle soreness and may delay recovery of muscle function following eccentric exercise. Trial registration number: ACTRN12620001089921. Novelty: Post-exercise recovery of maximal voluntary isometric contraction force and peak concentric torque were unaffected by MitoQ. MitoQ delayed post-exercise recovery of peak eccentric torque. Post-exercise muscle soreness was unaffected by MitoQ.

In order to set up a cost-efficient biorefinery in a Tunisian dairy industry, the production unit effluents are recovered. The main objective is to develop an optimum method for the production of bioethanol from whey. An energy analysis as well as environmental and economic analyses are performed for a bioethanol production plant. Four production scenarios are examined in order to determine the most provident as well as the less polluting ones. The process and cost models were developed using SuperPro Designer software which a simulation program that is able to estimate both process and economic parameters. This software uses energy and mass balances. The model can be used to assess the efficiency, the resources consumption, the profitability and the environmental impact of each scenario. The results demonstrate that the third scenario, in which a reverse osmosis procedure is added to concentrate the whey, a continuous stoichiometric reaction procedure is integrated to model the biotransformation in the fermenter and where streams are added in order to recycle the biomass, produces the highest amount of bioethanol with 1.65 MT/year but the second one (where no streams were added) is the most profitable one with revenues as high as 570 000 $/year. The corresponding cost of ethanol production is 0.271 US $ ethanol per liter. The net present value (NPV) and the return on investment (ROI) of each scenario are positive. Such result indicates that all these investments could be undertaken in order to find an eco-friendly issue for the dairy industry effluents. Cheese whey could serve as an alternative raw material for producing ethanol.

Maximal voluntary contraction force (MVC), potentiated twitch force (Qpot), and voluntary activation (%VA) recover to baseline within 90 s following extreme-intensity exercise. However, methodological limitations mask important recovery kinetics. We hypothesized reductions in MVC, Qpot, and %VA at task failure following extreme-intensity exercise would be less than following severe-intensity exercise, and Qpot and MVC following extreme-intensity exercise would show significant recovery within 120 s but remain depressed following severe-intensity exercise. Twelve subjects (6 men) completed 2 severe-intensity (40, 50% MVC) and 2 extreme-intensity (70, 80% MVC) isometric knee-extension exercise bouts to task failure (Tlim). Neuromuscular function was measured at baseline, Tlim, and through 150 s of recovery. Each intensity significantly reduced MVC and Qpot compared with baseline. MVC was greater at Tlim (p < 0.01) and at 150 s of recovery (p = 0.004) following exercise at 80% MVC compared with severe-intensity exercise. Partial recovery of MVC and Qpot were detected within 150 s following Tlim for each exercise intensity; Qpot recovered to baseline values within 150 s of recovery following exercise at 80% MVC. No differences in %VA were detected pre- to post-exercise or across recovery for any intensity. Although further analysis showed sex-specific differences in MVC and Qpot, future studies should closely examine sex-dependent responses to extreme-intensity exercise. It is clear, however, that these data reinforce that mechanisms limiting exercise tolerance during extreme-intensity exercise recover quickly. Novelty: Severe- and extreme-intensity exercise cause independent responses in fatigue accumulation and the subsequent recovery time courses. Recovery of MVC and Qpot occurs much faster following extreme-intensity exercise in both men and women.

The objective of this study was to validate an automated self-administered 24-hour dietary recall web application (R24W) against recovery biomarkers for sodium, potassium and protein intakes and to identify individual characteristics associated with misreporting in a sample of 61 men and 69 women aged 20-65 years from Québec City, Canada. Each participant completed 3 dietary recalls using the R24W, provided two 24-hour urinary samples and completed questionnaires to document psychosocial factors. Mean reported intakes were 2.2%, 2.1% and 5.0% lower than the urinary reference values, respectively, for sodium, potassium and proteins (significant difference for proteins only (p = 0.04)). Deattenuated correlations between the self-reported intake and biomarkers were significant for sodium (r = 0.48), potassium (r = 0.56) and proteins (r = 0.68). Cross-classification showed that 39.7% (sodium), 42.9% (potassium) and 42.1% (proteins) of participants were ranked into the same quartile with both methods and only 4.8% (sodium), 3.2% (potassium) and 0.8% (proteins) were ranked in opposite quartiles. Lower body esteem related to appearance was associated with sodium underreporting in women (r = 0.33, p = 0.006). No other individual factor was found to be associated with misreporting. These results suggest that the R24W has a good validity for the assessment of sodium, potassium and protein intakes in a sample of French-speaking adults. Novelty: The validity of an automated self-administered 24-hour dietary recall web application named the R24W was tested using urinary biomarkers. According to 7 criteria, the R24W was found to have a good validity to assess self-reported intakes of sodium, potassium and proteins.

BACKGROUND: Teleworking became a necessary practice as an alternative to office work during the long COVID-19 era. OBJECTIVES: To recognize the telework associated benefits and health problems and to assess the relationship between some teleworkers' characteristics, their health problems with job satisfaction and recuperation for promoting telework practices. METHODS: A cross-sectional online survey was conducted among employees working remotely in different work sectors using a questionnaire assessing job satisfaction and recuperation. RESULTS: Telework benefits included saving time and money (89.3%), minimizing the COVID-19 spread (86.9%), and balancing of work and life (63.4%). Telework associated health problems included musculoskeletal (78.2%), work-related stress (65.9%), and visual problems (47%). Telework job satisfaction was significantly higher among married male workers, working less than 40hs/week, and with previous remote working experience before COVID-19. On the other hand, married female teleworkers working more than 40hs/week and without previous remote working experience before COVID-19 had significantly more perceived fatigue and less recuperation. All teleworkers with reported health problems showed significantly more perceived fatigue and less recuperation. CONCLUSIONS: More than half of the participants recommended continuing teleworking post-COVID-19 due to its benefits on their working and social life. Telework would enables the employers to adapt and satisfy the teleworkers' expectations to maintain their work and productivity.

This commentary tackles the subtle at-the-edge problem of passing locally by a mesoscopic matter-aggregating system from a classical stochastic to a quantum stochastic description. A d-dimensional entropy-productive aggregation of the matter is taken as the starting point. Then, a dimensional reduction towards a one-dimensional quantum-wire type matter-aggregation system is proposed, resulting in postponing surface-tension conditions for the effectively d = 1-dimensional quantum-wire type or nanorod-like cluster/polycrystal, which is qualitatively consistent with a physical-metallurgical (high-temperature) Louat's grain growth model. A certain recuperative interplay based on maneuvering between subtle temperature rises applied to the system under study while maintaining its quantum character (the so-called Nelson's quantum-stochastic procedure) within the limits of a vanishing Planck's constant, involved in the diffusivity measure of the aggregation, is discussed. Certain applications towards the formation of d = 1-dimensional semiconductors and other nanostructures (possibly using soft materials or (bio)polymeric materials such as nanofibers) are envisioned. As a special example, one may propose a nanotechnological process which is termed the Van der Waals heteroepitaxy. The process itself contains the main quantum vs. classical crossover due to the involvement of weak repulsion (quantum) vs. attraction (treated classically) interactions, which are represented by a Lennard-Jones-type potential.

INTRODUCTION: Urolift implant placement may be preferred to conventional endoscopic surgery for patients who wish to preserve their sexuality or for those who prefer a rapid post-operative recovery. The absence of general anaesthesia is an important element that reinforces the minimally invasive aspect of the procedure and improves the speed of recovery. The aim of this work was to report our preliminary experience of Urolift treatment under local anaesthesia. MATERIALS AND METHODS: A retrospective analysis was conducted including all patients treated with Urolift between 2017 and 2021 in our centre. Local anaesthesia was based on the instillation of 2 Xylocaine gels at 4°C into the urethra 15minutes before the procedure. The primary endpoint was the successful completion of the procedure without interruption due to pain or the need for any other form of anaesthesia or analgesia. RESULTS: Twenty-seven patients were included with a median age of 65 years and a prostate volume of 46mL. The International Prostate Symptom Score (IPSS) was 23. The first 3 patients were operated on under general anaesthesia. Local anaesthesia was introduced from the fourth patient onwards. There was no recourse to other modalities of analgesia or anaesthesia or interruption of the procedure. The operating time was 10minutes and pain was assessed at 1 on a visual analogue scale. At 3 months, the IPSS score was 9 (P=0.001). CONCLUSION: This preliminary experience confirms the feasibility of placing the Urolift implant under local anaesthesia without any failure of the proposed management. The improvement in IPSS score was consistent with previously published clinical trials. LEVEL OF EVIDENCE: 3.

Patients with colorectal cancer are at risk of malnutrition before surgery. Multimodal prehabilitation (nutrition, exercise, stress reduction) readies patients physically and mentally for their operation. However, it is unclear whether extent of malnutrition influences prehabilitation outcomes. We conducted a pooled analysis from five 4-week multimodal prehabilitation trials in colorectal cancer surgery (prehabilitation: n = 195; control: n = 71). Each patient's nutritional status was evaluated at baseline using the Patient-Generated Subjective Global Assessment (PG-SGA; higher score, greater need for treatment of malnutrition). Functional walking capacity was measured with the 6-minute walk test distance (6MWD) at baseline and before surgery. A multivariable mixed effects logistic regression model evaluated the potential modifying effect of PG-SGA on a clinically meaningful change of ≥19 m in 6MWD before surgery. Multimodal prehabilitation increased the odds by 3.4 times that colorectal cancer patients improved their 6MWD before surgery as compared with control (95% confidence interval (CI): 1.6 to 7.3; P = 0.001, n = 220). Nutritional status significantly modified this outcome (P = 0.007): Neither those patients with PG-SGA ≥9 (adjusted odds ratio: 1.3; 95% CI: 0.23 to 7.2, P = 0.771, n = 39) nor PG-SGA <4 (adjusted odds ratio: 1.3; 95% CI: 0.5 to 3.8, P = 0.574, n = 87) improved in 6MWD with prehabilitation. In conclusion, baseline nutritional status modifies prehabilitation effectiveness before colorectal cancer surgery. Patients with a PG-SGA score 4-8 appear to benefit most (physically) from 4 weeks of multimodal prehabilitation. Novelty: Nutritional status is an effect modifier of prehabilitation physical function outcomes. Patients with a PG-SGA score 4-8 benefited physically from 4 weeks of multimodal prehabilitation.

Acute kidney injury is a common complication in intensive care unit. Its incidence is variable according to the studies. It is considered to occur in more than 50 % of patients. Acute kidney injury is responsible for an increase in morbidity (length of hospitalization, renal replacement therapy) but also for excess mortality. The commonly accepted definition of acute kidney injury comes from the collaborative workgroup named Kidney Disease: Improving Global Outcomes (KDIGO). It made it possible to standardize practices and raise awareness among practitioners about monitoring plasma creatinine and also diuresis. Acute kidney injury in intensive care unit is a systemic disease including circulatory, endothelial, epithelial and cellular function involvement and an acute kidney injury is not accompanied by ad integrum repair. After prolonged injury, inadequate repair begins with a fibrotic process. Several mechanisms are involved (cell cycle arrest, epithelial-mesenchymal transition, mitochondrial dysfunction) and result in improper repair. A continuum exists between acute kidney disease and chronic kidney disease, characterized by different renal recovery phenotypes. Thus, preventive measures to prevent the occurrence of kidney damage play a major role in management. The nephrologist must be involved at every stage, from the prevention of the first acute kidney injury (upon arrival in intensive care unit) to long-term follow-up and the care of a chronic kidney disease.

Neuroimaging studies have displayed aberrant brain activities in individual sensory- and emotional-linked regions in postherpetic neuralgia (PHN) patients. However, multi-dimensional dysfunction in chronic pain may rely on the interplay between networks. Little is known about the changes in the functional architecture of resting state networks (RSNs) in PHN. In this cross-sectional study, we recruited 31 PHN patients, 33 RHZ patients and 34 HCs; all participants underwent resting-state functional magnetic resonance imaging scans. We investigated the differences of within- and cross-network connectivities between different outcomes of HZ patients [including PHN and recuperation from herpes zoster (RHZ)] and healthy controls (HCs) so as to extract a characteristic network pattern of PHN. The abnormal network connectivities were then correlated with clinical variables in respective groups. PHN and RHZ patients could be similarly characterized by abnormal within-default mode network (DMN), DMN-salience network (SN) and SN-basal ganglia network (BGN) connectivity relative to HCs. Of note, compared with RHZ patients, PHN patients could be characterized by abnormal DMN-BGN and within-BGN connectivity. Furthermore, the within-DMN connectivity was associated with pain-induced emotional scores among PHN patients. Our study presented that network-level imbalance could account for the pain-related dysfunctions in different outcomes of herpes zoster patients. These insights are potentially useful for understanding neuromechanism of PHN and providing central therapeutic targets for PHN.
