AIM: Hypernatraemia typically reflects dehydration, yet in rare instances may be caused by salt poisoning. Identifying these rare cases is a difficult challenge. Making the diagnosis of salt poisoning can have severe consequences, such as the removal of the child from its home or even prison sentences for the implicated carer. It is therefore imperative to get the diagnosis right. Guidelines for the assessment of hypernatraemia emphasise the importance of the fractional excretion of sodium to distinguish between dehydration and salt poisoning, but no generally accepted cut-off value exists. Opinions about the diagnosis of salt poisoning in some cases consequently may differ. Here, we aim to highlight the challenges and stimulate discussion on how to improve the tools for the assessment of hypernatraemia. METHODS: Report of a case of unexplained hypernatraemia in which the treating paediatrician raised the suspicion of salt poisoning. RESULTS: Two consulted experts made opposing judgements about the aetiology of the observed hypernatraemia. CONCLUSION: Clear diagnostic criteria for the diagnosis of salt poisoning are lacking and more data are needed for their establishment. Without this, victims may experience further harm and carers are at risk of devastating, yet potentially erroneous accusations.

Salt poisoning is a rare cause of severe hypernatremia in children resulting from the ingestion of toxic amounts of sodium chloride, either from accidental or intentional administration of salted solutions. We present the case of a newborn admitted to a pediatric emergency department for lethargy and reduced oral intake; his laboratory evaluation showed severe hypernatremia ([Na+] of 174 mmol/L). The infant developed convulsive status epilepticus during treatment. Neuroimaging showed a tetraventricular hemorrhage, a large right-sided parenchymal hemorrhage with midline shift, and several left hemorrhagic foci. Etiologic evaluation for hypernatremia did not reveal a renal or extrarenal source of water loss nor an intercurrent illness to explain the reduced oral intake. A careful review of how the parents prepared the infant formula revealed an error in dosing the ratio of powder/water, resulting in hyperosmolar infant formula. The infant was diagnosed with salt poisoning as the major cause of hypernatremia. After careful correction of hypernatremia and the use of antiseizure medication, the patient improved and was discharged. The parents were given a careful review of instructions for infant formula preparation. Due to its rarity, a high index of suspicion is mandatory for a correct diagnosis of salt poisoning. Timely and adequate treatment is needed due to the high risk of intracerebral bleeding, seizures, and irreversible neurologic injury. Children, particularly newborns and infants, depend upon adults to ingest water and, thus, have more difficulty in maintaining electrolyte balance. Therefore, it is of utmost importance that parents are educated about childcare, particularly on the importance of careful infant formula preparation.

Brazil is the fourth largest exporter of pork worldwide, with its production concentrated in the Southern region of the country. To reduce the economic impacts and increase herd health, an efficient diagnosis of diseases is necessary. The frequency, seasonal, and annual distribution of diseases that affecting the swine nervous system in Southern Brazil were determined through a retrospective study conducted from 2008 to 2018. A total of 231 pigs were evaluated, and distributed in the following phases: suckling piglets, nursery, growing, finishing, and adults. Piglets in the nursery phase were the most affected with 58.01%. Infectious diseases accounted for 87.01% (201/231) of lesions in the nervous system. Bacterial meningitis was observed in 58.87% of cases, presenting an endemic distribution, and a propensity to occur in the months of autumn/winter. The highest number of cases of porcine circovirus-2 were observed in 2008 and 2009, with a subsequent decline. Porcine circovirus, salt poisoning, and brain abscesses exhibited no association between the seasonal occurrence and annual distribution. Salt poisoning was observed more frequently in the last years of this study. Both, non-suppurative encephalomyelitis and selenium toxicosis exhibited an association between their occurrence and seasonal distribution in, the months of winter, and summer, respectively.(AU)


Brasil é o quarto maior exportador de carne suína do mundo, com sua produção concentrada na região Sul do país. Para reduzir os impactos econômicos e aumentar a saúde do rebanho, é necessário o diagnóstico eficiente das doenças. A frequência, distribuição sazonal e anual das doenças que afetam o sistema nervoso de suínos no Sul do Brasil foram determinadas através de um estudo retrospectivo no período de 2008 a 2018. Foram avaliados 231 suínos, distribuídos nas seguintes fases: maternidade, creche, crescimento, terminação e adultos. Leitões da fase de creche foram os mais afetados com 58,01%. As doenças de origem infecciosa foram responsáveis por 87,01% (201/231) das causas de lesões no sistema nervoso. Meningite bacteriana foi observada em 58,87% dos casos, apresentando distribuição endêmica, e uma tendência a ocorrer nos meses de outono/inverno. O maior número de casos de circovírus suíno foram observados em 2008 e 2009, com declínio posterior. Circovírus suíno, intoxicação por sal e os abscessos encefálicos não apresentaram associação entre a ocorrência sazonal e distribuição anual. A intoxicação por sal foi observada com maior frequência nos últimos anos deste estudo. A encefalomielite não supurativa e a intoxicação por selênio exibiram associação entre a sua ocorrência e distribuição sazonal, nos meses de inverno e verão, respectivamente.(AU)

BACKGROUND: Hypernatremia due to salt poisoning is clinically rare and standard care procedures have not been established. We report a case of salt poisoning due to massive intake of seasoning soy sauce. CASE PRESENTATION: A 40-year-old woman presented to the emergency department with seizures and remarkable hypernatremia with a serum sodium concentration of 183 mEq/L. The initial brain computed tomography scan showed brain shrinkage, which could occur during the acute phase of hypernatremia. We reduced her serum sodium level rapidly, rather than at the recommended slow rate. On day 3, the patient's brain computed tomography scan showed widespread low-density areas and edema. The patient died 8 days after admission. CONCLUSION: After reviewing instances of resuscitation following salt intoxication, aggressive rapid correction of serum sodium concentration should only be considered in acute phases of hypernatremia within a few hours from ingestion, and 2-3 h could be one of the criteria.

O presente trabalho relata um surto de intoxicação por sal em ovinos no Brasil, em uma propriedade no estado do Pará. De um total de 545 ovinos, oito animais adoeceram (1,46%) e quatro destes morreram (50%). A avaliação das instalações e do manejo indicaram como fatores predisponentes a ingestão excessiva de mistura mineral e a restrição hídrica. Os principais sinais clínicos foram decúbito, diminuição ou ausência da sensibilidade cutânea, ausência dos reflexos de ameaça, palpebral e auricular, midríase, nistagmo, opistótono, espasticidade de membros, sonolência e estupor. Havia ainda, timpanismo, diarreia, taquipneia, taquicardia, desidratação e poliúria. A evolução do quadro clínico nos animais que morreram variou de duas horas e meia a 48 horas. As médias das concentrações séricas de sódio e de potássio de 31 ovinos do mesmo lote afetado pela intoxicação, em amostras colhidas durante o surto, revelaram hipernatremia (190mEq/l) e hipercalemia (8,2mEq/l). À necropsia, observou-se em um animal, achatamento das circunvoluções cerebrais. Microscopicamente, neste animal, evidenciou-se vacuolização moderada do neurópilo, particularmente nas lâminas intermediárias do córtex cerebral, com aumento dos espaços perineural e perivascular. Nessas áreas foram observados ainda, acentuada tumefação e edema dos astrócitos e necrose neuronal aguda. A dosagem de sódio no encéfalo de um ovino, revelou-se elevada com valor de 3.513ppm. O diagnóstico foi realizado com base na epidemiologia, nos sinais clínicos, nas lesões macro e microscópicas e nas dosagens de sódio no soro e no encéfalo dos ovinos.(AU)


This paper reports an outbreak of salt poisoning in sheep on a farm in the state of Pará, northern Brazil. Eight (1.46%) animals were affected from a total of 545 sheep and four (50%) of them died. The evaluation of the facilities and the handling indicated as predisposing factors excessive intake of the mineral supplement and water restriction. The main clinical signs were recumbency, decrease or absence of cutaneous sensibility, absence of auricular, palpebral and menace reflex, mydriasis, nystagmus, opisthotonus, spasticity of limbs, somnolence and stupor. Beside these signs bloat, diarrhea, tachypnea, tachycardia, dehydration and polyuria were observed. The course of the disease in animals that died ranged from two hours and a half to 48 hours. The medium of serum concentration of sodium and potassium of 31 sheep from the same group affected on samples collected during the outbreak revealed hypernatremia (190mEq/l) and hyperkalemia (8.2mEq/l). At necropsy, there was flattening of the cerebral gyri in one sheep; in this animal was found vacuolization of the neuropil in the cerebral cortex with moderate intensity in the intermediary and mild on the superficial layers, with increased perineural and perivascular spaces. In these areas there were markedly swollen nucleus of astrocytes with acute neuronal necrosis. The dosage of sodium in the brain of a sheep revealed a high value of 3.513ppm. The diagnosis was made based on the epidemiology, clinical signs, macro and microscopic lesions and in the dosages of sodium in serum and brain.(AU)

Descreve-se a ocorrência de surto de intoxicação por sal em suínos de duas propriedades, onde 70 porcos morreram. Soro de leite proveniente da salga de queijo era servido no cocho como única fonte hídrica aos animais. Não havia bebedouros nas instalações. Um dia após o fornecimento do soro, os porcos começaram a adoecer e apresentaram salivação excessiva, movimentos de pedalagem, opistótono e tremores cíclicos. Na necropsia, dois suínos apresentaram achatamento das circunvoluções do córtex telencefálico e um outro, edema no córtex telencefálico. Na histopatologia, observou-se necrose neuronal laminar difusa acentuada no córtex telencefálico, astrócitos de Alzheimer tipo II (AIIA), infiltrado multifocal perivascular de eosinófilos e linfócitos e edema perivascular. Outros achados incluíram marcação imuno-histoquímica fraca ou ausente para proteína glial fibrilar ácida (GFAP), mas intensa marcação positiva no citoplasma dos AIIA para S-100. As concentrações de sódio sérico e do líquor mensuradas nos porcos estudados foram de 140 e 156mmol/L, respectivamente.


A total of 70 pigs were affected and died due to salt poisoning in two farms in southern Brazil. The only source of drinking water available to the pigs was whey from cheese salting process. One day after receiving the whey, animals started getting sick and showed excessive salivation, opisthotonus, paddling, and cyclic tremors. At necropsy of three pigs, two of them presented flattening of gyri and the other one presented cortical telencephalon edema. Microscopically, severe diffuse neuronal necrosis in telencephalic laminar cortex, Alzheimer type II astrocytes (AIIA), eosinophilic and lymphocytic perivascular cuffing apart of perivascular edema were observed. Weak or absent anti-glial fibrillary acidic protein (GFAP) immunolabeling was associated with positive immunostaining for S-100 in AIIA cytoplasm. Concentration of sodium in serum and liquor samples from dead pigs resulted 140 and 156mmol/L, respectively.
