To explore the influence of the 5E rehabilitation nursing model integrated with mindfulness training on mitigating psychological distress and adjusting coping styles in patients with acute myocardial infarction (AMI) after percutaneous coronary intervention (PCI). The clinical data of 94 patients with AMI who underwent PCI from August 2020 to January 2022 were retrospectively analyzed. Patients were divided into 2 groups based on different nursing modes. Among them, 47 cases received routine interventions were categorized into the control group, and 47 cases received the 5E rehabilitation nursing model integrated with mindfulness training on the basis of routine interventions were categorized into the study group. After 3 months of intervention, both groups exhibited a significant reduction in Self-Rating Anxiety Scale and Depression Scale scores compared to the pre-intervention period, with the study group demonstrating lower scores than the control group (P < .05). Herth Hope Index scores significantly improved in both groups after intervention, with the study group exhibiting higher scores than the control group (P < .05). After 3 months of interventions, Self-Care Agency scores significantly elevated in both groups, with the study group demonstrating higher scores than the control group (P < .05). WHO Quality of Life-BREF scores significantly improved in both groups, with the study group demonstrating higher scores than the control group (P < .05). The study group exhibited higher compliance and satisfaction levels and lower complication rate compared to the control group (P < .05). Integrating the 5E rehabilitation nursing model with mindfulness training effectively alleviates psychological distress, ameliorates quality of life, and improves satisfaction in AMI patients after PCI.

BACKGROUND AND SIGNIFICANCE: Nurses are disproportionately affected by mental and physical health problems that are a result of exposure to traumatic events in the workplace. PURPOSE: The purpose of this study was to use intervention mapping strategies to construct interventions to address traumatic stress in nurses through trauma-informed professional development (TIPD) in a rehabilitation hospital. DESIGN: This study used a community-based participatory research design with an intervention mapping approach. Logic models were created through input from theory, evidence from the literature, and feedback from 12 focus groups with leaders, nursing professional development practitioners, and direct-care nurses. FINDINGS: Major themes in the logic models included safety, empowerment, peer support, and awareness. These findings were used to create specific TIPD interventions to be used with nurses in a rehabilitation hospital.

RESUMEN

ABSTRACT: The Rehabilitation 2030: Call to Action initiative highlighted the global unmet need for rehabilitation services and the importance for enhancing health systems in low- and middle-income countries to deliver these services. Dedicated interprofessional volunteers, such as Association of Rehabilitation Nurses (ARN) nurse leaders, have been integral in working with the technical groups to develop the World Health Organization Rehabilitation Competency Framework and the Package of Interventions for Rehabilitation. These resources are available for countries to "adopt and adapt." The recent launch of the World Rehabilitation Alliance is important to note as they support the Rehabilitation 2030's mission through advocacy activities and offers an opportunity for further ARN involvement.

BACKGROUND: Providing adequate care for the person with a fragility fracture is essential to prevent recurrences. A key strategy involves training by improving nursing care in the fields of osteoporosis and fragility fractures. However, in Portugal, there is no report on the level of knowledge of nurses, nor experimental studies on how to improve it. OBJECTIVE: The study aimed to assess the knowledge of Rehabilitation Nurses in Portugal on osteoporosis and fragility fractures. Additionally, it sought to evaluate the impact of a specific educational programme on nurses' knowledge. METHODS: In Phase I, a cross-sectional study involved 452 participants, utilizing a 26-question knowledge test. In Phase II, a quasi-experimental study included 42 nurses from 28 hospitals, subjected to a 30-h hybrid educational programme. The program comprised 9 online (2 h 30 min each) and 2 live sessions, covering assessment, pharmacological and non-pharmacological treatment, monitoring, project planning, consultations, and outcome indicators measurement. A before-and-after programme knowledge test was administered. RESULTS: Phase I revealed an average knowledge score of 69.6%. In Phase II, there was a significant improvement with programme (70.4% vs. 85.8%, p < 0.01). Specialized nurses performed better than non-specialized nurses (80% vs. 75%, p = 0.011), and those from orthopaedic services showed the greatest improvement (92% vs. 83%, p = 0.014). CONCLUSIONS: Rehabilitation Nurses in Portugal have room to improve their knowledge of osteoporosis and fragility fractures. The hybrid educational programme proved effective in improving nurses' knowledge, especially among specialist and orthopaedic service nurses. We hope that this knowledge can be translated into continuous improvement in healthcare provision.

PURPOSE: Reduced activity because of pain is a major health issue associated with total knee arthroplasty. This study evaluated the effectiveness of a nurse-led app-based home exercise program for patients who underwent total knee arthroplasty. METHODS: A nonequivalent control group of pretest-posttest design was used. Data from 45 patients were collected. The control group received individualized face-to-face exercise education, whereas the experimental group received training on the use of app-based home exercise education, including exercise videos and app push notifications to encourage exercise after discharge. Pain, range of motion, exercise self-efficacy, and quality of life were measured at baseline and 3 and 12 weeks after surgery. Nursing care satisfaction was evaluated 12 weeks after surgery. RESULTS: There were significant overall reductions in pain intensity ( p = .001), improvements in limited range of motion ( p < .001), and increases in exercise self-efficacy ( p = .034) and quality of life ( p = .033) in the experimental group ( n = 22) compared with those in the control group ( n = 23). Nursing care satisfaction was significantly higher in the experimental group than in the control group ( p < .001). CONCLUSIONS: Rehabilitation nurses can offer app-based home exercise education with push notifications to alleviate pain, enhance range of motion, improve exercise self-efficacy, and increase nursing care satisfaction for patients who have had a knee arthroplasty.

RESUMEN

The nursing context in pediatric rehabilitation is that of caring for children with disabilities and complex developmental differences and health conditions in an ever-changing and demanding environment. Rehabilitation nurses aim to continuously advance nursing leadership, practice, education and research to meet service needs. Strengths-Based Nursing and Healthcare (SBNH) is a philosophy and value-driven approach that aligns with and enables the advancement of strengths-based rehabilitation nursing and family-centred care. This paper describes the leadership approach undertaken to implement SBNH in a Canadian pediatric rehabilitation hospital context over a 10-year period. We will share what we did and what we learned.

ABSTRACT: Rehabilitation nurses possess knowledge and skills that are ideally suited to a variety of roles. This article informs rehabilitation nurses about opportunities to work in private case management for medical-legal cases. A brief overview of the process of litigation gives nurses the context in which case management interventions are needed. Case examples illustrate the services that nurses provide to attorneys and their clients that help obtain needed care and aid in progressing litigation to resolution.

Objetivo: refletir sobre a importância do trabalho das equipes de reabilitação, orientando e habilitando os cadeirantes para o desempenho seguro das transferências diárias, fundamentais na realização das atividades cotidianas e inclusão social. Métodos: trata-se de estudo teórico-reflexivo fundamentado na teoria do déficit de autocuidado, com a utilização integrada dos diagnósticos de enfermagem, da classificação internacional das práticas de enfermagem e do instrumento de avaliação das transferências, adequados às necessidades de pessoas com lesão medular, atendidas nos ambientes institucionais de cuidados, visando o preparo para o desempenho das atividades cotidianas. Resultados: cabe aos enfermeiros assumir liderança nas equipes de reabilitação física, norteando suas intervenções no treinamento dessas pessoas e seus cuidadores para o desempenho e ajuda segura nas transferências para cuidar de si. Conclusão: considerando a lesão medular entre os maiores problemas da saúde coletiva que afeta a humanidade contemporânea, tanto pelos comprometimentos na qualidade de vida das pessoas, quanto no aumento das despesas hospitalares e reabilitação requeridos, a adoção de estratégias de cuidados preventivos de complicações musculoesqueléticas é sempre bem-vinda. Essas pessoas, quando não orientadas, executam movimentos repetitivos para se deslocar em transferências de uma superfície para outra, correndo elevados riscos de contrair lesões nas articulações, pele e mucosas. Objective: to reflect on the importance of the work of rehabilitation teams, guiding and enabling wheelchair users to safely perform daily transfers, essential for carrying out daily activities and social inclusion. Methods: this is a theoretical-reflective study based on the theory of self-care deficit, with the integrated use of nursing diagnoses, the international classification of nursing practices and the transfer assessment instrument, adapted to the needs of people with spinal cord injury, attended in institutional care environments, aiming to prepare for the performance of daily activities. Results: it is up to nurses to assume leadership in physical rehabilitation teams, guiding their interventions in the training of these people and their caregivers for performance and safe help in transfers to take care of themselves. Conclusion: considering spinal cord injury among the biggest collective health problems that affect contemporary humanity, both because of the compromises in people's quality of life, as well as the increase in hospital and rehabilitation expenses required, the adoption of preventive care strategies for musculoskeletal complications is always welcome. These people, when not guided, perform repetitive movements to move in transfers from one surface to another, running high risks of contracting injuries to the joints, skin and mucous membranes. Métodos: se trata de un estudio teórico-reflexivo basado en la teoría del déficit de autocuidado, con el uso integrado de los diagnósticos de enfermería, la clasificación internacional de prácticas de enfermería y el instrumento de evaluación de la transferencia, adaptado a las necesidades de las personas con lesión medular. asistidos en ambientes de atención institucional, con el objetivo de preparar para el desempeño de las actividades diárias. Resultados: corresponde a los enfermeros asumir el liderazgo en los equipos de rehabilitación física, orientando sus intervenciones en la formación de esas personas y sus cuidadores para el desempeño y ayuda segura en las transferencias para cuidarse. O compromisso da deglutição pós-extubação (CDPE) acarreta um conjunto de complicações que impactam negativamente a reabilitação destes doentes. No entanto, a evidência de intervenções para a abordagem terapêutica ao CDPE é limitada. Por conseguinte, identificar as intervenções relevantes neste contexto é imprescindível para a construção de programas de reabilitação baseados em evidência. Objetivo: Mapear as intervenções direcionadas à pessoa com CDPE. Métodos: Trata-se de uma scoping review, orientada pela metodologia proposta pelo Joanna Briggs Institute. Dois revisores pesquisaram a MEDLINE (via PubMed), a Biblioteca Cochrane, a Scielo, a Science Direct e a CINAHL em outubro de 2023. Foi efetuada uma pesquisa adicional de literatura cinzenta. A extração de dados foi guiada pelos critérios de inclusão: adultos submetidos a ventilação mecânica invasiva por mais de 48 horas e diagnóstico de compromisso da deglutição (população), CDPE (conceito) e unidades de cuidados intensivos (contexto). Os resultados são apresentados de acordo com o tipo de intervenção: estratégias compensatórias e reabilitação. Resultados: Identificados 1891 registos, acrescidos de 59 registos encontrados na literatura cinzenta. Após triagem e seleção, foram incluídos 14 artigos para análise. Os exercícios de fortalecimento muscular e os ajustamentos posturais são as estratégias de reabilitação e compensação mais frequentemente referenciadas. Conclusão: Estes resultados poderão contribuir para enformar o processo de tomada de decisão dos enfermeiros especialistas em enfermagem de reabilitação na construção de intervenções dirigidas à pessoa com CDPE. A relevância dada ao CDPE é recente, assim como a investigação desenvolvida neste domínio, o que foi corroborado por estes resultados.


Post-extubation swallowing impairment (PESI) entails several complications that negatively impact the rehabilitation of intensive care patients. However, the evidence on interventions for the therapeutic approach to PESI is limited. Therefore, identifying relevant interventions in this context is essential for developing evidence-based rehabilitation programs. Aim: To map interventions for patients with post-extubation swallowing impairment. Methods: This is a scoping review guided by the methodology proposed by the Joanna Briggs Institute. Two reviewers searched MEDLINE (via PubMed), the Cochrane Library, Scielo, Science Direct, and CINAHL in October 2023. An additional grey literature search was conducted. Data extraction was guided by the inclusion criteria: adults undergoing invasive mechanical ventilation for more than 48 hours (population), PESI (concept), and intensive care units (context). The results are presented according to the type of intervention: compensatory strategies and rehabilitation. Results: 1891 records were identified, and 59 were found in the gray literature. After screening and selection, 14 articles were included for analysis. Muscle strengthening exercises and postural adjustments were the most frequently mentioned rehabilitation and compensation strategies. Conclusion: These results may contribute to informing the decision-making process of rehabilitation nurses when designing interventions for patients with PESI. The relevance given to PESI is recent, as is the research carried out in this field, which was corroborated by these results.

Context: In China, the dearth of adept rehabilitation nurses has escalated into a pressing concern. Conventional nursing education has stymied nurses' autonomous scholarly pursuits. The case-teaching methodology can augment students' competencies and zeal, culminating in the enrichment of educational excellence. Objective: The study intended to elucidate the principles, taxonomy, and enactment of the case-teaching methodology and examine its salutary impacts on nursing practitioners and educators. Design: The research team performed a narrative review by searching BMJ Best Practice, OVID EBM, NGC et al databases. The search used the keywords 'Rehabilitation nursing', 'nursing', 'case teaching method'. Setting: The study took place at the Rehabilitation Medicine Center at West China Hospital of Sichuan University in Chengdu, China. Results: The current study scrutinizes the assimilation of the case-teaching methodology within the realm of nursing, dissects emerging paradigms in the standardized training of rehabilitation nursing staff, and furnishes precedents for the evolution of training frameworks in the field of rehabilitative care. Conclusions: The case-teaching methodology not only serves as an instructional tool but also embodies a fundamental transformation in the modernization of nursing education, embodying the aspiration for excellence, the desire for continuous improvement, and a dedication to the highest standards of patient care.

ABSTRACT: BACKGROUND: Most critical thinking assessment tools are resource intensive and require significant time and money to administer. Moreover, these tools are not tailored to evaluate critical thinking skills among inpatient rehabilitation facility (IRF) nurses. This pilot study explores the efficacy of using short videos to evaluate critical thinking for nurses working in an IRF. METHODS: We developed and filmed 3 clinical scenarios representative of common IRF events that require critical thinking on behalf of the nurse. Thirty-one IRF nurses participated in the study and independently scored their own critical thinking skills using a visual analog scale. Using the same scale, nurse managers and assistant managers who worked closely with the nurses also rated the critical thinking ability of each nurse. The nurse then viewed and responded in narrative form to each of the 3 videos. A scoring rubric was used to independently evaluate the critical thinking skills for each nurse based on the nurses' responses. RESULTS: Nurses rated their own critical thinking skills higher than mangers rated them (m = 85.23 vs 62.89). There was high interrater reliability for scoring video 1k (0.65), video 2k (0.90), and video 3k (0.84). CONCLUSION: The results demonstrate efficacy for further study of low-cost alternatives to evaluate critical thinking among neuroscience nurses providing IRF care.

Introdução: A osteoartrite do joelho (OAJ) é a mais prevalente e a sua incidência continua a aumentar em paralelo com o envelhecimento da população. Nas pessoas com OAJ avançada, o tratamento cirúrgico através da prótese total do joelho (PTJ) é a intervenção mais recomendada, mas com implicações na mobilidade, condicionando o retorno ao exercício físico (EF). Atualmente, há uma dispersão de programas direcionados a esta população, sendo que os enfermeiros especialistas em enfermagem de reabilitação (EEER) apresentam competências e conhecimento científico para prescrever EF direcionado a populações com vulnerabilidade acrescida. Desta forma, este estudo pretende desenvolver recomendações de EF para pessoas com PTJ. Objetivo: Desenvolver recomendações de promoção de EF para pessoas com PTJ em contexto hospitalar e comunitário. Metodologia: O estudo adotou um desenho metodológico baseado no modelo de intervenções complexas do Medical Research Council Framework. A Etapa I consistiu numa revisão da literatura para sintetizar o conhecimento sobre estratégias de promoção do EF para pessoas com PTJ. A Etapa II focou na elaboração de recomendações com base na evidência científica e na discussão por peritos num grupo focal. A validação das recomendações ocorreu na Etapa III através de um Painel de Delphi. Resultados: Há convergência significativa entre a literatura científica e a opinião dos especialistas em relação à estruturação dos programas de EF em fases distintas. Sublinha-se a importância de exercícios de fortalecimento muscular, exercícios funcionais e exercícios aeróbios como caminhadas e exercícios em meio aquático. A progressão gradual da intensidade, assegura a segurança e eficácia dos exercícios, sendo a escala de Borg uma ferramenta essencial para a sua monitorização. A frequência dos exercícios deve ser ajustada de acordo com a fase de recuperação e o tipo de exercício. A preferência por instrumentos de avaliação acessíveis e clinicamente relevantes foi evidenciada. Conclusão. O tratamento cirúrgico através da colocação da PTJ é a intervenção mais recomendada, mas que acarreta compromissos na mobilidade, condicionando a prática de EF. Considera-se que um programa de EF é sobretudo multicomponente, com aplicação multidisciplinar, devendo incluir fortalecimento muscular, funcionalidade e treino aeróbio. Deve adaptar-se e monitorizar o programa, de forma a promover a sua eficácia e o retorno à atividade na comunidade.

AIM: To explore the application effect of competency-based education (CBE) combined with multi-disciplinary team (MDT) teaching mode in respiratory rehabilitation nursing teaching. BACKGROUND: Respiratory rehabilitation, as an important non drug treatment for chronic respiratory disease, started late in China, with low implementation rate in hospitals and insufficient knowledge of nursing staff. Therefore, it is urgent to cultivate high professional level nurses with strong professional ability. DESIGN: A randomized controlled trial. METHODS: We selected nurses from the respiratory and critical care department at a Grade III, Level A hospital in Beijing, who participated in the respiratory rehabilitation training program between March 2020 and August 2022, as our study participants. Participants were grouped using a random number method. Nurses who participated in the study from March 2020 to May 2021 were set as the control group, and the nurses who participated in the study from June 2021 to August 2022 were set as the test group. The control group used traditional teaching methods, and the test group used the CBE combined MDT teaching model. At the end of the test, the theory, operating skills, satisfaction and core competence were assessed. RESULTS: After training, the theoretical examination score of the test group (93.71 ± 1.94) was higher than that of the control group (92.37 ± 1.92), the operational examination score of the test group (93.11 ± 2.12) was higher than that of the control group (91.61 ± 1.93), the overall teaching satisfaction of the test group (4.45 ± 0.50) was higher than that of the control group (4.13 ± 0.57), and the total score of the core competence of the test group (148.73 ± 7.08) was higher than that of the control group (141.02 ± 6.41). The difference between the two groups was statistically significant (P<0.05). CONCLUSION: CBE combined with MDT teaching mode has a good effect in Pulmonary and Critical Care Medicine (PCCM) respiratory rehabilitation nursing teaching.

RESUMEN

ABSTRACT: Candida auris is a highly transmissible yeast that is capable of causing invasive and fatal infections, particularly among persons with underlying medical conditions. Its incidence is rising, especially among patients cared for in post-acute care facilities. Individuals colonized with the yeast may be cared for in inpatient rehabilitation settings, without heightened risk for invasive infection and/or transmission to others, as long as appropriate infection control measures are followed. This article reviews key information for rehabilitation nurses caring for persons with C. auris , including risk factors for infection, the need for contact precautions, appropriate disinfection practices for therapy and diagnostic equipment, and critical components of safe transitions in the care of these patients.

This study aims to evaluate the effect of an accelerated rehabilitation nursing programme on the incidence of surgical site wound infections in patients undergoing laparoscopic cholecystectomy. Relevant studies regarding the use of an accelerated rehabilitation nursing programme in laparoscopic cholecystectomy were retrieved from databases, including PubMed, Web of Science, the Cochrane Library, EMBASE, CNKI and Wanfang Database. The search was conducted from the inception of each database until June 2023. Two independent researchers performed the literature screening, data collection and quality assessment of the included studies. Odds ratio (OR) and 95% confidence interval (CI) were used as the measures of effect. Statistical analysis was conducted using Stata 17.0, and a sensitivity analysis and publication bias evaluation were performed. A total of 21 studies involving 2480 patients (1179 in the intervention group and 1301 in the control group) were included. The meta-analysis revealed that the incidence of surgical site wound infections in the intervention group was significantly lower than in the control group (1.18% vs. 5.99%, OR: 0.322, 95% CI: 0.168-0.556, p < 0.001). Current evidence suggests that implementing accelerated rehabilitation nursing programmes for patients undergoing laparoscopic cholecystectomy has a clinically significant effect, leading to a substantial reduction in the incidence of surgical site wound infections. However, owing to the low quality of some of the included studies, further high-quality, multicentre, large-sample randomised controlled trials are required to validate the conclusions of this study.

We sought to investigate the effects of fast-track rehabilitation nursing on pressure ulcers, length of hospital stay and postoperative complications in patients with inter-trochanteric fractures (ITF). The PubMed, Web of Science, Embase, Cochrane Library, China National Knowledge Infrastructure and WanFang databases were searched for randomised controlled trials (RCTs) published from inception to September 2023 on the application of fast-track rehabilitative nursing to ITF. Two investigators independently screened the literature and performed data extraction and quality assessments using the Cochrane Risk of Bias Assessment Tool. The meta-analysis was performed using RevMan 5.4. Overall, 22 RCTs involving 1904 patients were included. Meta-analysis revealed that after fast-track rehabilitation nursing intervention in patients with ITF, the occurrence of pressure ulcers (odds ratio [OR]: 0.29; 95% confidence interval [CI]: 0.18-0.47; p < 0.001) and postoperative complications (OR: 0.19; 95% CI: 0.14-0.26; p < 0.001) were significantly reduced and hospital stay was significantly shorter (standardised mean difference: -3.10; 95% CI: -3.82 to -2.38; p < 0.001). Nursing care for patients with ITF based on the concept of fast-track rehabilitation is conducive to reducing the occurrence of pressure ulcers, lowering the occurrence of complications, shortening the length of hospitalisation and promoting postoperative rehabilitation.
