RESUMEN

OBJECTIVE: To evaluate the outcome and effects of single high-dose radiation therapy with the aid of liquid fiducial markers in dogs following resection of soft tissue sarcomas (STSs). ANIMALS: 36 client-owned dogs. METHODS: Dogs with a histologic diagnosis of a grade II or III STS that underwent liquid fiducial guided single fraction, 20-Gy stereotactic radiation therapy following surgical excision of an STS between May 2017 and March 2019 were prospectively enrolled in this study. Data collected from the medical records included patient signalment, tumor-related information, treatment details, and outcome. Kaplan-Meier survival analysis was performed for overall survival time (OST) and disease-free interval (DFI). The median OST and DFI were not reached, so restricted mean OST and DFI were also calculated. RESULTS: 36 dogs were included in the study. All dogs underwent radiation therapy a mean of 36.1 days (range, 20 to 59 days) after surgery. Acute and delayed radiation toxicity effects occurred in 80.5% and 36.1% of dogs, respectively, all of which affected the skin. Tumor recurrence was noted in 24.3% of dogs with a median time to recurrence of 272 days (range, 14 to 843 days). The restricted mean OST was 1,556 days (range, 1,383 to 1,728 days) and restricted mean DFI was 1,330 days (range, 1,101 to 1,559 days). CLINICAL RELEVANCE: The results of this study showed that administering a single 20-Gy fraction of radiation in combination with a liquid fiducial marker to treat marginally or incompletely resected STS in the absence of gross disease resulted in similar OST and DFI compared to other previously reported radiation protocols.

The objective of this study was to evaluate a novel liquid fiducial marker, BioXmark, to improve identification of the superficial component of oral tumors in dogs with computed tomography imaging. Liquid fiducial marker was injected in 6 patients at the visible and palpable extent of each tumor. Gross tumor volumes with and without BioXmark were compared in terms of volume and conformity using a Paddick conformity index, Dice similarity coefficient, and gross tumor volumes mismatch analysis. All patients showed an increase in gross tumor volumes defined by BioXmark compared with the conventionally identified post-contrast gross tumor volumes contours. Volumetric conformity and gross tumor volumes mismatch analysis of the superficial component of gross tumor volumes resulted in a median conformity index of 0.61 and median Dice similarity coefficient of 0.76. The superficial gross tumor volumes showed a median increase of 47% when BioXmark was used. This study demonstrated a potential utility to combining liquid fiducial markers to post-contrast computed tomography images for improved oral tumor localization and gross tumor volumes contouring for radiation therapy planning.


Potentiel du marqueur de repère liquide BioXmark à améliorer l'identification d'éléments superficiels de tumeurs orales canines pour la planification de radiothérapie assistée par ordinateur. L'objectif de la présente étude était d'évaluer un nouveau marqueur de repère liquide, BioXmark, à améliorer l'identification des éléments superficiels des tumeurs orales canines par tomodensitométrie. Le marqueur de repère liquide fut injecté à six patients à la limite visible et palpable de chaque tumeur. Les volumes bruts des tumeurs avec et sans BioXmark furent comparés en termes de volume et de conformité en utilisant l'index de conformité de Paddick, le coefficient de similarité de Dice, et une analyse de disparité des volumes bruts des tumeurs. Tous les patients montrèrent une augmentation des volumes bruts des tumeurs déterminés par BioXmark comparativement aux volumes bruts des tumeurs déterminés par la méthode conventionnelle d'identification des contours post-contrastes. La conformité volumétrique et l'analyse de disparité des volumes bruts des tumeurs du composant superficiel des volumes bruts des tumeurs a résulté en un index de conformité médian de 0,61 et un coefficient de similarité de Dice médian de 0,76. Les volumes bruts superficiels des tumeurs montraient une augmentation médiane de 47 % lorsque le BioXmark était utilisé. In ultrasound/computed tomography (CT) fusion images, ultrasound allows visualization of the target in real time. CT provides a navigation for ultrasound scanning and improves the overview in areas of limited visualization with ultrasound. This study was performed to investigate the feasibility of ultrasound/CT fusion based on an electromagnetic tracking technique using external fiducial markers for canine ocular and periocular regions. In 7 Beagle dogs, contrast-enhanced CT images of the head were obtained with placing external fiducial markers over the frontal region and both sides of the forepaws of the dog. Ultrasonography was performed under a magnetic field by installing a position sensor in the linear probe, without changing the dog's position. The positions of the external fiducial markers were adjusted and matched, based on the CT images. The execution time of co-registration and the distance between the regions of interest and the co-registration points, the frontal bone, cornea, retina, and optic nerve, were estimated. Approximately 60% of external fiducial markers were properly recognized in all dogs. After adjustment, all external fiducial markers were precisely matched. The co-registration execution time was less than 1 min. The distances between the regions of interest and co-registration points were less than 3 mm in all dogs. The electromagnetic tracking technique using external fiducial markers was a simple and applicable method for fusion imaging of a canine head using real-time ultrasonography and CT. This technique can be useful for interventional procedures of retrobulbar and periorbital lesions.

OBJECTIVE: To construct and optimize a fiducial marker suitable for both CT and MRI. SAMPLE: Fiducial markers containing serial dilutions of iopamidol mixed with water. PROCEDURES: IV tubing sets were infused with serial dilutions (0% to 100%; increments of 10%) of iopamidol. Tubing ends were sealed; additional seals were added to create an equilateral triangle. A reference point was created by placing a crimp in 1 side. Markers were fixed to a gelatin soft tissue-attenuating phantom and evaluated by use of CT and MRI. For CT, simple linear regression analysis was used to assess the relationship between the percentage of marker contrast medium and quantitative variables, including marker attenuation, attenuation changes in the phantom, and beam-hardening artifact length. A subjective grading scheme for artifact creation on CT images and marker visibility on MRI images was used. Measurements were obtained by investigators who were unaware of the contents of each marker. RESULTS: Percentage of contrast medium in each marker was strongly correlated with marker attenuation (r(2) = 0.96), artifact length (r(2) = 0.765), and mean attenuation changes within the phantom (r(2) = 0.826) for CT. Subjective CT scores indicated that concentrations of contrast medium > 50% resulted in excessive artifacts. Markers with concentrations of iopamidol > 50% had poor subjective MRI visibility scores. No artifacts were seen on MRI. CONCLUSIONS AND CLINICAL RELEVANCE: A marker containing a 10% solution of iodinated contrast medium mixed with water provided ideal contrast for both CT and MRI.
