BACKGROUND: Dental offices are among the highest risk for transmission of the COVID-19, having the potential to transmit the virus via routine dental procedures. This cross-sectional study assessed the preparedness and perception of infection control measures against the COVID-19 pandemic by dentists in Saudi Arabia. MATERIALS AND METHODS: This online survey addressed the impact and perception of the COVID-19 pandemic on dental practice in Saudi Arabia. The questionnaire comprised 26 closed-ended questions. Descriptive statistics included frequency distributions with percentages. In addition, the significance between the different demographic variables and questions about dentists' perception of the COVID-19 pandemic was tested using the Chi-square test. RESULTS: COVID-19 management in dental clinics varied in terms of adherence to the Ministry of Health (MOH) guidelines. Dental clinics' screening questionnaire for patients showed good adherence (67%), while the lowest agreement was detected with the question on the existence of an airborne infection in the isolation room (15%). Almost two-thirds of the respondents agreed that the dental reception area adopted the proper COVID-19 preventive measures. Greatest accord was observed in their answers on questions about dentists' perception of the COVID-19 pandemic, ranging from 64%-89%. In addition, there were statistically significant differences in questions about the perception of dentists towards the COVID-19 pandemic by different demographic variables such as age and years of work experience (p < 0.05). CONCLUSION: The response of most dentists regarding the preparedness and perception of infection control measures against the COVID-19 pandemic was positive. Dental clinics need to adhere more to the MOH recommendations in preparedness of their facilities or by educating their dentists and staff.

RESUMEN

Permanente Dental Associates includes 17 offices in the Pacific Northwest. Among the distinguishing characteristics of this model are a predominantly HMO structure and integration of care in a general medical program. Staff dentists are on salary and are largely relieved of the business details of practice. Ultimate control of the system is vested in a group of shareholders--the dentists who practice chairside. One of the shareholder-practitioners discusses his perspective on this system.

RESUMEN

The nonprofit dental delivery model is appropriate for the needs of specific patient populations. The Indian Health Service is an example of how care can be provided where traditional fee-for-service and indemnity mechanisms may be insufficient. Separating care from management in this context gives dentists greater power over individual treatment decisions, increased choice of patient-relevant care options, and control over development of the practice model and its evolution. The needs of various populations groups and the funding or profit model inevitably influence the composition of the dental team and assignment of dental duties.

OBJECTIVE: The purpose of this study was to present the impact of demography and market dynamics on the organizational and technological development of dental practices in Bulgaria during the period of socio-economic transition from 1990 to 2010. MATERIALS AND METHODS: Relevant data were gathered from the official registers of the Bulgarian Dental Association, National Health Insurance Fund, National Health Information Institute and research studies, published during the period 1990-2010. They were analysed and are discussed in this paper. RESULTS: During the period there was a decrease in population of Bulgaria, from 8,149,468 in 2000 to 7,563,000 in 2010 and a movement from rural areas to cities. There was an aging population with an increase in oral health care needs at a time of decrease in the size of the national labour force and economic growth. Paradoxically, over the last 20 years, the number of dentists in Bulgaria increased by more than 50% from 1997 to 2008. The average age of dentists changed from 39.7 years in 1997 to 41.7 years in 2008. The number of group dental practices increased and the number of individual practices and group practices for specialized dental care and dental centres decreased. The utilization of dental services is currently "about half" that of the mean for all European Union Member States, which is over 80% per year. CONCLUSION: The growing number of elderly people who have retained their teeth suggests an increase in treatment needs. However, the decline in their purchasing power limits the demand for oral health care services. In order to meet this and other challenges Bulgarian dentists have started changing their traditional solo-practice to group-practice.

Since August 2009, members of the Primary Care Dentistry Research Forum (www.dentistryresearch.org) have taken part in an online vote to identify questions in day-to-day practice that they felt most needed to be answered with conclusive research. The question that receives the most votes each month forms the subject of a critical appraisal of the relevant literature. Each month a new round of voting takes place to decide which further questions will be reviewed. Dental practitioners and dental care professionals are encouraged to take part in the voting and submit their own questions to be included in the vote by joining the website. The paper below details a summary of the findings of the twelfth critical appraisal. In order to address the question, searches of the literature were conducted to identify any articles discussing or evaluating the use of plastic in dental practice, or interventions that might reduce the use of plastics in dental practice. There were no studies identified that directly answered the question of this review. In a narrative review, environmental audit was suggested as a strategy that could help dental practitioners make more informed decisions in their practice; but there were not any studies that evaluated this. Further studies are need to demonstrate what proportion of dental waste includes plastics and whether specific interventions such as environmental audit can decrease the amount of plastic use and waste along with improving other environmental outcomes.

INTRODUCTION: Western Australia (WA), Australia's largest state, consists of approximately one-third of the country's landmass and has a population of approximately 2 million people. However, over 85% of this population live in a handful of urban centres with the majority (1.6 million) living in the state capital, Perth. Healthcare provision in this complex environment is difficult and a clear understanding of the distribution of the population and health service suppliers is critical to understanding and addressing the problem. The vast majority (80% or more) of oral health care in WA is provided by private practitioners on a fee-for-service basis. Although previous studies have examined the macro-level distribution of dentists, this study utilized a high acuity examination to examine the distribution of private dental practices in WA, especially in rural and remote areas. METHODS: Dental practice locations were collected from open access sources and geo-coded. Population data were obtained from the Australian Bureau of Statistics and divided by census districts. The Index of Relative Socio-Economic Disadvantage (IRSD) was aggregated to census district level. Population and socio-economic data were then geo-coded using ArcGIS v9 (ESRI; Redlands, CA, USA). With Perth primary post office used as a central datum point, a sequence of maps at differing magnification was overlayed with a grid of latitude and longitude lines, or graticules. RESULTS: Of the 602 dental practices mapped, 75% were within 32 km of the Perth General Post Office, and 95% were within 256 km. In metropolitan Perth, fewer dental practices were located in areas that have residents of lower socioeconomic status and the density of distribution of these practices was greatest in the inner city region. CONCLUSION: This study highlights the complex nature of providing care to irregularly distributed populations in economic environments that are driven by factors not purely related to disease burden.

To assess amalgam use and waste management protocols practised by Pakistani dentists, a cross-sectional study was made of 239 dentists in Islamabad and Rawalpindi, recruited by convenience and cluster sampling. Amalgam was the most frequently used restorative material, with the choice dictated by patients' financial constraints. While 90.4% of dentists perceived amalgam as a health risk, only 46.4% considered it an environmental hazard. The majority disposed of amalgam waste in the trash, down the sink or as hospital waste. Very few (5.9%) had an amalgam separator installed in their dental office. Amalgam waste management protocols and mercury recycling should be introduced in Pakistan.

OBJECTIVES: The authors conducted a study to describe the general dentists, practices, patients and patient care patterns of the dental practice-based research network (PBRN) Northwest Practice-based REsearch Collaborative in Evidence-based DENTistry (PRECEDENT). METHODS: Northwest PRECEDENT is a dental PBRN of general and pediatric dentists and orthodontists from five U.S. states in the Northwest: Idaho, Montana, Oregon, Utah and Washington. The authors collected data from general dentists in Northwest PRECEDENT (n = 101) regarding the diagnosis and treatment of oral diseases in a survey with a systematic random sample of patients (N = 1,943) visiting their practices. They also obtained demographic data from the general dentists and their patients. RESULTS: The authors found that 50 percent of the general dentists were 51 to 60 years of age, 14 percent were female and 76 percent were non-Hispanic white. More than one-half (55 percent) of the dentists had practiced dentistry for more than 20 years, 83 percent had private solo practices and 32 percent practiced in rural community settings. The majority (71 percent) of patients visiting the dental practices was in the age range of 18 to 64 years, 55 percent were female and 84 percent were non-Hispanic white. In terms of reasons for seeking dental care, 52 percent of patients overall visited the dentist for oral examinations, checkups, prophylaxis or caries-preventive treatment. In the preceding year, 85 percent of the patients had received prophylaxis, 49 percent restorative treatments, 34 percent caries-preventive treatments and 10 percent endodontic treatments. CONCLUSIONS: Northwest PRECEDENT general dentists are dispersed geographically and are racially and ethnically diverse, owing in part to efforts by network administrators and coordinators to enroll minority dentists and those who practice in rural areas. Estimates of characteristics of dentists and patients in Northwest PRECEDENT will be valuable in planning future studies of oral diseases and treatments.

PURPOSE: The purpose of this article is to examine data and results from the 2008 Survey of Prosthodontists. Survey results are used to examine current trends and characteristics of prosthodontists in private practice. MATERIALS AND METHODS: Characteristics of prosthodontists and conditions of private practice are based on surveys conducted in 2002, 2005, and 2008 sponsored by the American College of Prosthodontists. Survey results are used to estimate several characteristics including age, gender, number of patient visits, hours in the practice, employment of staff, referral sources, and financial conditions (gross receipts, expenses of the practice, and net income of prosthodontists). RESULTS: The average age of a private-practicing prosthodontist reached 51 years in 2007; 12.3 is the number of years in the current practice; and most prosthodontists (71%) are solo private practitioners. The average amount of time per week by prosthodontists in the practice averaged 36.1 hours, and prosthodontists treated an average of 44.1 patient visits per week. The largest source of patient referrals is the patient themselves. The largest percentage of a prosthodontist's treatment time is spent rendering procedures in fixed prosthodontics, but this percentage has declined since 2001. In 2007, the average gross billings of a practicing prosthodontist reached $805,675; average total practice expenses were $518,255; the mean net earnings of practitioners were $268,930. CONCLUSION: In 2007, prosthodontists in private practice paid out about $1.4 billion in practice expenses to provide $2.2 billion dollars in prosthodontic care. Based on survey results from 2007 and the previous 6 years, specialization in prosthodontic care continues to be an economically attractive and productive healthcare profession in the United States.

OBJECTIVES: Little is known about the delegation of care within skill mix in dentistry, therefore this study aimed to explore dentists' and dental care professionals' (DCPs') perceptions of factors that might influence the referral of paediatric patients to dental hygienists and therapists for fissure sealants. METHOD: Qualitative semi-structured interviews were conducted with 10 dentists and 10 DCPs. Qualitative content analysis of the interview transcripts was used to identify themes in the data. RESULTS: A predominant view was that there were no characteristics that systematically influenced the referral of patients to DCPs to place fissure sealants. However, idiosyncratic factors were said to occasionally play a role. Structural factors included use of resources, payment and contracting systems and practice characteristics. Individual patient-related factors were parents' and patients' attitudes and patient characteristics. Dentist-related factors included dentists' preferences, perceptions of DCPs, their perceived role of DCPs and providing a service to patients. CONCLUSION: This study has identified a variety of factors that may influence a dental practitioner's decision to refer child patients to DCPs for fissure sealant placement. However, these factors do not appear to be systematic.

INTRODUCTION: In dentistry, as in surgery, there is a risk of cross-infection for both patients and staff The aim of this research was to evaluate procedures and behaviors enacted by dental staff which might engender a risk for themselves and their patients. METHODS: A questionnaire was administered to 106 dental workers in Genoa (Italy), both public and private. Whenever personal interviews were conducted, the facilities involved were also inspected and the activities of the dental staff were observed directly. RESULTS: This research highlighted some critical points in both structural and organizational features and in the management of infective hazards in the sample considered. In some cases, inadequacies were noted with regard to the prevention of cross-infections, such as the lack of disinfection of work surfaces and the handles of chair-set accessories. DISCUSSION AND CONCLUSIONS: The particular nature of dental work, in which aerosols of blood and saliva may be produced by rotating instruments, engenders a risk of infection. Application of the various preventive measures available can significantly reduce microbial contamination and the risk of occupational infection and cross-infections. Furthermore, improvement in the structural and organizational features of dental surgeries and the continuing education of health-care workers is indispensable to the control and prevention of infectious diseases.
