RESUMEN

BACKGROUND: This study is a cost-effectiveness study of two implementation strategies designed to train therapists in college and university counseling centers to deliver interpersonal psychotherapy. Costs of implementing a train-the-trainer (TTT) strategy versus an expert consultation strategy were estimated, and their relative effects upon therapist outcomes were calculated and compared. METHODS: Twenty four counseling centers were recruited across the United States. These centers were randomized to either a TTT (experimental) condition, in which an in-house therapist trained other center therapists, or an expert consultation condition, in which center therapists participated in a workshop and received 12 months of ongoing supervision. The main outcome was therapist fidelity (adherence and competence) to interpersonal psychotherapy, assessed via audio recordings of therapy sessions, and analyzed using linear mixed models. Costs of each condition were quantified using time-driven activity-based costing methods, and involved a costing survey administered to center directors, follow up interviews and validation checks, and comparison of time tracking logs of trainers in the expert condition. Mean costs to produce one therapist were obtained for each condition. The costs to produce equivalent improvements in therapist-level outcomes were then compared between the two conditions. RESULTS: Mean cost incurred by counseling centers to train one therapist using the TTT strategy was $3,407 (median = $3,077); mean cost to produce one trained therapist in the control condition was $2,055 (median = $1,932). Therapists in the TTT condition, on average, demonstrated a 0.043 higher adherence score compared to therapists in the control condition; however, this difference was not statistically significant. For the competence outcome, effect size for therapists in the TTT condition was in the large range (1.16; 95% CI: 0.85-1.46; p < .001), and therapists in this condition, on average, demonstrated a 0.073 higher competence score compared to those in the expert consultation condition (95% CI, 0.008-0.14; p = .03). Counseling centers that used the TTT model incurred $353 less in training costs to produce equivalent improvements in therapist competence. CONCLUSIONS: Despite its higher short run costs, the TTT implementation strategy produces greater increases in therapist competence when compared to expert consultation. Expanding resources to support this platform for service delivery can be an effective way to enhance the mental health care of young people seeking care in college and university counseling centers. RESUMEN

This study is the first to report the results of a scan of the official webpages of all public and private colleges and universities in the state of Oregon, including but not limited to student health services webpages and student handbooks, to identify available services, policies, and educational information on HPV vaccination. The purpose is to describe vaccination services (with a focus on the HPV vaccine), prevention efforts, and institutional policies on vaccination on college and university websites in Oregon. An online scan was conducted from September to November 2020 to collect and describe data from college/university websites across Oregon. Colleges/universities were obtained from the National Center for Education Statistics website. The final sample included 48 colleges/universities. Twenty colleges/universities have a health center with 11 providing the HPV vaccine on campus: most are large public universities and non-religious private colleges. A few religious private universities and one community college offer the vaccine on campus. More than half (n = 26) of Oregon colleges/universities had a webpage specifically designated for immunization information, but only 11 mentioned the HPV vaccine on their immunization webpage. Twelve colleges/universities recommend, and none require the vaccine. Information on HPV vaccination services and education was limited on college/university websites, especially on community college and religious private university websites. Most colleges/universities with a student health center recommend and offer the HPV vaccine on campus. College/university websites may be a low-cost and effective medium to increase HPV vaccination awareness, encourage immunizations, and prevent HPV-associated cancers.

RESUMEN

College health centers are an important source of health care for large numbers of young adults throughout the country. With 16.9 million students enrolled in 2023 in post-secondary institutions in the United States, the potential impact that health centers at these institutions have is significant. Late adolescence and early adulthood is a particularly vulnerable stage of both cognitive and psychosocial development, and college health centers that provide comprehensive medical and mental-health care play an important role in providing comprehensive care for this age group. As we move into a post-pandemic era, colleges and universities across the country continue to navigate all the changes that have been brought about by the COVID-19 pandemic. It is perhaps now more important than ever to review and assess the clinical care college health centers provide, who they provide this care to, and identify opportunities for improvement and where there may be need for change.

RESUMEN

In this study, we pilot tested Therapeutic Assessment (TA) in a university counseling center using a replicated single-case design to generate hypotheses on the effectiveness and applicability for this setting and population. We aimed to see whether TA could be an effective brief intervention to address students' presenting mental health concerns. Further, we explored whether different types of presenting concerns were associated with differential symptomatic improvement during the intervention. An independent clinician interviewed participants before the baseline period to develop individualized rating scales pertaining to their presenting concerns. Eight consecutive students accessing the counseling center enrolled in the study and rated their presenting problems across baseline, intervention, and follow-up periods. The intervention involved five TA sessions. The results suggested that TA is associated with statistically significant reductions in clients' symptoms in the context of a university counseling center. Idiographic trajectory analysis of participant data who experienced significant and insignificant change was used to test whether changes were associated with the onset of TA. The findings suggest TA might be more effective for certain presenting concerns than for others. The implications for the implementation of TA in university counseling centers is discussed.

RESUMEN

College students are undergoing a mental health crisis and existing clinical resources at universities may be inadequate to meet their needs. One solution to this problem could be the use of self-guided, online programs for mental health that can be easily distributed to students. A persistent issue, however, is the transition from program development to implementation of a self-guided program at campus-wide scale. We describe, in a self-narrative format, the steps taken to translate an online program we developed based in acceptance and commitment therapy, called Acceptance and Commitment Therapy Guide, from a research context to a campus-wide service at our university. We also present demographic, previous treatment, and referral data of over 1,500 student sign-ups to describe the reach of our program, how it was adopted into existing university services and systems, and how we maintained the program long term. Findings from these implementation efforts are discussed in the context of suggestions for using self-guided programs for students to fill gaps in mental health service provision in university settings. RESUMEN

Introduction Traditionally, gender-affirming hormonal therapy (GAHT) is initiated in secondary care, but a primary care based approach has been developed to reduce access barriers. Aim We aim to describe the demographics, hormone choices, and additional referrals made for young people initiating GAHT in a primary care setting in Aotearoa New Zealand. Methods Clinical notes were reviewed for all patients who commenced GAHT between 1 July 2020 to the end of 2022 at a tertiary education health service. Data were collected on age, ethnicity, gender, type of hormones prescribed, and any additional referrals. Results Eighty five patients commenced GAHT during the review period (64% assigned male at birth and starting oestrogen-based GAHT, 36% assigned female at birth and commencing testosterone-based GAHT). Fourty seven percent of patients identified as transgender female, 38% as non-binary, and 15% as transgender male. Spironolactone was the most common choice of testosterone blocker (81%). The choice of oestrogen formulation was fairly equal between patches (54%) and tablets (46%). Eighty percent of those assigned male at birth chose to preserve fertility, 54% requested voice therapy, and 87% of those assigned female at birth requested top surgery. Discussion There is a need for improved understanding of non-binary gender-affirmation needs, in particular those of Maori and Pasifika youth. An informed consent approach in primary care can reduce barriers and distress for transgender youth seeking GAHT. The high unmet need for top surgery for transgender people assigned female at birth requires attention.

This study aimed to present the results of a psychoeducational psychotherapy group developed with Nursing students. Six meetings were held with five students from a public university in the state of São Paulo. The meetings were audio-recorded, transcribed and submitted to a reflexive-thematic analysis. The results found allowed the construction of five thematic categories: (1) Challenges of adapting to higher education; (2) Conceptions about mental health at the university; (3) Self-care and balance between university and personal life; (4) Interpersonal relationships and university life; (5) Perspectives and expectations about training. The process of integration into the university requires cognitive, affective and social mobilization, and relationships with peers, teachers and family members were highlighted as important for this adaptation. The participants highlighted knowing the importance of mental health care, but also admitted difficulties in promoting self-care. The group space was used as a listening environment, bonding and self-care. Psychoeducational groups proved to be important for the construction of coping strategies and a space for the reception of emerging demands.

RESUMEN

University students' health and well-being is critical, especially in the aftermath of the coronavirus disease 2019 pandemic; however, a comprehensive and integrated approach in academic institutions remains neglected. In this context, the local experience from a pilot university-based Student Health Center at an urban campus in Greece is presented. Select health promotion and disease prevention screening and monitoring initiatives are summarized from the viewpoint of a Strengths, Weaknesses, Opportunities, and Threats analysis, with emerging health needs and policy implications. Long-term sustainability is feasible, only if synergies and close collaboration with other university units and local health authorities are developed. A post-pandemic call to action for intervention programs that integrate physical and mental health care, as well as raise awareness among university stakeholders and health policy makers, is issued. RESUMEN

OBJECTIVE: Community college students engage in more risky sexual behavior compared to their four-year counterparts, yet have access to fewer reproductive health services. The study's purpose was to examine whether California Community College student health centers' participation in Family PACT, a state publicly-funded family planning program, increased reproductive health services to community colleges students. PARTICIPANTS: California community college student health centers. METHODS: Bivariate analyses of student health centers with and without Family PACT participation and qualitative analysis of five participating campuses. RESULTS: Among the 60 colleges in the study, 25 student health centers participated in the Family PACT program. Family PACT campuses reported greater provision of sexual and reproductive health services and higher levels of staffing and revenue. CONCLUSIONS: Key benefits of Family PACT participation among community colleges include expansion of sexual and reproductive health services to an underserved population and increased student health centers' financial sustainability.

RESUMEN

Objective: To determine whether university students are aware of the sexual health services offered by the student health center. Participants: 522 undergraduate students at a southeast public university. Methods: Students were given a list of 19 sexual health services and tests and were instructed that for each one to check "offered," "not offered," or "I'm not sure." Results: Students were generally unaware that the health center offered testing for a variety of sexually transmitted infections (13-27.4% aware), the Gardasil vaccine for human papillomavirus (HPV) (15.5% aware), the IUD (8.8% aware), emergency contraception (18.6% aware), and breast (24.9% aware) and pelvic examinations (16.5% aware). The only exceptions were for free male condoms (63% aware) and women's awareness of birth control pills (55.3% aware) and pregnancy testing (50.3% aware). Nearly half the students were not aware that parents/guardians could not obtain health center medical records without the student's permission. Conclusions: Student health centers cannot be effective in reducing unwanted pregnancies and the spread of STIs if students are unaware of the services provided. Health centers must do a better job of educating students.

RESUMEN

Undergraduate and graduate students have a higher risk of mental illness than the general population. Multiple barriers may contribute to decreased help-seeking, including mental health stigma, lack of awareness of available resources, lack of prioritization of self-care, busy class schedules, and different preferences in communication styles. Over the past six years, we have increased the utilization of our Student and Resident Mental Health Clinic on the University of Colorado Anschutz Medial Campus by more than 100%. This viewpoint article highlights eight initiatives that address some of the barriers that mental health clinics on undergraduate and graduate campuses may face. These initiatives include involving students to reduce stigma and increase engagement, matching capacity of clinic services to demand, using technology, being as visible as possible, modeling vulnerability and self-care, adapting services to your audience needs, providing food (other than pizza!), and building a team and promoting collegiality.

INTRODUCTION: Anxiety, stress and depression are currently the main symptoms of mental health problems presented by university students, it is estimated that about one third of this population presents some of these symptoms. OBJECTIVE: To analyse the clinical evidence on the benefits of meditation on the mental health of university students through a systematic review. METHODOLOGY: We identified randomised controlled trials on the effects of meditation on mental health in university students, written in Portuguese, English and Spanish, and published between 2018 and 2022. We excluded quasi-randomised trials, non-randomised trials, observational studies, clinical trial protocols, preliminary studies, studies that do not have meditation as the main intervention, reports and articles without access to the full text. The searches were conducted in November 2022, in the databases: Embase; LILACS; MedLine/PubMed; SciELO and; The Cochrane Library. The RoB 2 tool was used to assess the risk of bias and the articles were subjected to a qualitative synthesis analysis. RESULTS: Nineteen articles were selected on mindfulness practices with 2054 participants. Most studies analysed outcomes on stress, depression, anxiety and mindfulness, and the others on quality of life, life satisfaction and self-compassion. The studies were divided into three thematic categories: asynchronous interventions (8); synchronous online interventions (4); face-to-face interventions (7). DISCUSSION: In general, the results express improvements in clinical symptoms of stress, anxiety and depression, as well as promoting mindfulness in university students. Meditation is a health practice that can be promoted by video calls, podcasts or videos and can reach distance learning students, since online initiatives seem to be very effective compared to face-to-face interventions. FINAL CONSIDERATIONS: The results of this review contribute to the evidence that meditation, especially mindfulness, can benefit the mental health of university students by reducing symptoms of stress, anxiety and depression, as well as improving mindfulness. These results appear to be forthcoming whether through face-to-face or online encounters, synchronously or asynchronously. This allows for a wide range of application possibilities in the health of university students. Further studies are needed to evaluate the medium- and long-term effects, as well as to analyse and compare the characteristics, effectiveness and applicability of meditation applied in person, online synchronously or asynchronously.

With the COVID-19 pandemic, the university context, which had already been the stage for discussions regarding mental health, has experienced more severe crises by students. In view of this scenario, the Solidarity Listening project was developed, aimed at the mental health of undergraduate and graduate students. In this article, we aim to discuss online psychological care with psychology students during the period of strict social isolation (May to June 2020). We carried out, with volunteer psychologists, a focus group aimed at the experience of short-termonline psychological care in the pandemic context. This is a qualitative study, carried out with 11 clinical psychologists participating in the aforementioned project. From a critical phenomenological analysis, the results were divided into five categories: a) limitations and contributions of the project; b) the importance of training and clinical supervision for the quality of the project; c) online psychological care; d) being a clinical psychologist during the COVID-19 crisis; and e) emerging demands in psychological care in quarantine. Finally, we discuss the importance of demystifying psychological care in crisis situations, especially in the online modality, promoting questions regarding the training and performance of professionals, to be aware of the psychological demands that the context of crisis entails in society.(AU)

Enrollment in the university is marked by intense personal transformations and the need to adapt to a more autonomous teaching model. This scenario can contribute to the emergence of feelings of loneliness and impact the quality of life of college students. This study aimed to investigate the relationship between loneliness and quality of life in college students at a public university in the interior of the state of Minas Gerais. A total of 268 undergraduate students randomly select took part in the study. The instruments used were a sociodemographic and lifestyle questionnaire, the Medical Outcomes Study 36 - item Short Form (SF-36), and the Brazilian Loneliness Scale (UCLA-BR). Descriptive and inferential analyzes were conducted (correlation, Kruskal-Wallis, and logistic regression). The highest score in the quality of life's domain was Functional Capacity and the lowest was Vitality. Regarding loneliness, 60.82% of the sample had a minimum level. There was a significant association between loneliness and six quality of life domains. Loneliness doubled the chance of worse quality of life for the domains: social aspects (β = 2.66), emotional limitation (β = 2.18), and mental health (β = 2.61). In conclusion, loneliness negatively impacts the quality of life of university students and, therefore, interventions are required for this population.(AU)
