Lipedema, a chronic and painful disorder primarily affecting women without a definitive cure, has traditionally been managed with conservative therapy, notably complete decongestive therapy, across many countries. Recently, liposuction has been explored as a potential surgical treatment, prompting this study to evaluate its effectiveness as possibly the first-line therapy for lipedema. Through extensive literature searches in databases such as CrossRef, Web of Science, PubMed, and Google Scholar up to December 2023, and using the Newcastle-Ottawa Scale for quality assessment, the study selected seven studies for inclusion. Results showed significant post-operative improvements in spontaneous pain, edema, bruising, mobility, and quality of life among lipedema patients undergoing liposuction. However, over half of the patients still required conservative therapy after surgery. Despite these promising results, the study suggests caution due to lipedema's complexity, significant reliance on self-reported data, and limitations of the studies reviewed. Thus, while liposuction may offer symptomatic relief, it should be considered an adjunct, experimental therapy rather than a definitive cure, emphasizing the need for a comprehensive approach to care.

Objective The aim of this study is to assess the prevalence of HLA-DQ2 and HLA-DQ8 in women diagnosed with lipedema. Methods Leukocyte histocompatibility antigen (HLA) tests of 95 women diagnosed with lipedema were analyzed using non-probabilistic sampling for convenience. The prevalence of HLA-DQ2 and HLA-DQ8 was compared to the general population. Results The prevalence of HLA-DQ2+ was 47.4%, that of HLA-DQ8+ was 22.2%, the presence of any celiac disease associated HLA (HLA-DQ2+ or HLA-DQ8+) was 61.1%, both HLA (HLA-DQ2+ and HLA-DQ8+) was 7.4%, and the absence of celiac disease associated HLA was 39%. Compared to the general population, there was a significantly higher prevalence of HLA-DQ2, HLA-DQ8, any HLA, and both HLAs in lipedema patients. The mean weight of patients with HLA-DQ2+ was significantly lower than the overall study population, and their mean BMI significantly differed from the overall mean BMI. Conclusion Lipedema patients seeking medical assistance have a higher prevalence of HLA-DQ2 and HLA-DQ8. Considering the role of gluten in inflammation, further research is needed to establish if this association supports the benefit of gluten withdrawal from the diet in managing lipedema symptoms.

Introduction The current study aimed to investigate the overlap between symptoms of lipedema and attention-deficit/hyperactivity disorder (ADHD). Lipedema is a condition that causes abnormal fat accumulation and inflammation in the legs and buttocks, often accompanied by edema and pain. ADHD is a common condition characterized by difficulty paying attention and controlling behavior, affecting the social, academic, and occupational quality of life. The study's primary objective was to assess the prevalence of ADHD symptoms in a population of women with lipedema symptoms and compare the clinical characteristics. Method The study used a lipedema screening questionnaire and the Adult Self-Report Scale (ASRS-18) to assess the prevalence of ADHD in a sample of 354 female volunteers with or without a prior lipedema diagnosis. Results Of the lipedema group, 100 (77%) were ASRS positive, and 30 (23%) were ASRS negative. In the group without lipedema, 121 (54%) were ASRS positive, and 103 (46%) were ASRS negative, with a relative risk of 1.424 (p<0.0001). Conclusion Our results demonstrate a positive correlation between lipedema and ADHD and suggest that targeted strategies to improve clinic attendance for individuals with ADHD may improve lipedema treatment outcomes. Patients with lipedema symptoms are more likely to have ADHD symptoms.

Background: Lipedema is characterized as an abnormal deposition of fat in the buttocks and legs bilaterally that may be accompanied by swelling, pain, and tenderness. It is still often confused with more frequent conditions such as obesity and lymphedema. The estimated prevalence in Europe varies between 0.06% and 39%. Objectives: To evaluate the prevalence of lipedema and identify health factors related to it in the Brazilian population. Methods: Administration of a previously validated online screening questionnaire to a representative sample of the general population. The questionnaire was distributed and administered to anonymous volunteers representing the general Brazilian population using software designed for population analyses. Results: 253 women answered the questionnaire, 12.3 ± 4% (Confidence Interval [CI] 95%) of whom presented symptoms compatible with a high probability of being diagnosed with lipedema. Furthermore, anxiety, depression, hypertension, and anemia were also correlated with a high probability of the diagnosis. Conclusions: The estimated prevalence of lipedema in the population of Brazilian women is 12.3%.

Resumo Contexto O lipedema é caracterizado por deposição anormal de gordura em glúteos e pernas bilateralmente, que pode ser acompanhada por edema, dor e sensibilidade ao toque. Ainda é frequentemente confundido com condições mais frequentes, como obesidade e linfedema. A prevalência estimada na Europa varia entre 0,06% e 39%. Objetivos Avaliar a prevalência do lipedema na população brasileira e identificar fatores de saúde relacionados a essa doença. Métodos Foi aplicado um questionário de rastreamento on-line, previamente validado em amostra representativa da população geral. O questionário de rastreamento foi distribuído e aplicado em voluntárias anônimas representativas da população geral brasileira por software dedicado a análises populacionais. Resultados Um total de 253 mulheres respondeu ao questionário, e 12,3 ± 4% (intervalo de confiança de 95%) apresentaram sintomatologia compatível com alta probabilidade de diagnóstico de lipedema. Ansiedade, depressão, hipertensão e anemia foram correlacionadas com a alta probabilidade diagnóstica da doença. Conclusões A prevalência estimada do lipedema na população de mulheres brasileiras é de 12,3%.

BACKGROUND Lipedema is a chronic progressive disease characterized by the abnormal accumulation of fat in the subcutaneous region. Both medical and surgical treatments have been described in international guidelines; however, the current literature is biased toward promoting liposuction as the primary treatment of lipedema, and this can lead to the misapprehension that liposuction is the only form of definitive treatment. CASE REPORT In the present study, we report 5 cases at various stages of the evolution of lipedema, all with different therapeutic objectives. Case 1 reported having persistent bruising and pain, case 2 reported pain and fat deposition, case 3 reported night cramps and discomfort, case 4 reported leg thickening, and case 5 reported redness in the legs. All of were diagnosed with lipedema in different evolution stages. Our purpose was to demonstrate the possibility of non-surgical therapy, as well as to improve signs and symptoms of lipedema, using the QuASiL questionnaire and measuring changes in volumes and proportions. Good aesthetic outcomes improve both social and psychological status. CONCLUSIONS Currently, there are many described therapies available for lipedema. Liposuction surgery for lipedema should be considered one possible tool. Treatment objectives can be different for each patient. It is imperative to understand each patient's needs in order to offer the best therapy attainable that meets patient requirements and induces a better quality of life. Non-surgical treatment of lipedema is feasible in selected cases, and it can meet the criteria for achieving selected clinical objectives.

BACKGROUND: Lipedema is characterized by the deposition of abnormal fat in the lower and upper limbs bilaterally. It is a disease with high prevalence and genetic characteristics. Non-specific and non-quantified increases in the thickness of the subcutaneous tissue have previously been demonstrated using magnetic resonance imaging and computed tomography. OBJECTIVES: To evaluate the thickness of the dermis and subcutaneous tissue in predetermined areas as a distinguishing feature between individuals with and without lipedema using ultrasound. METHODS: Ultrasound images of 89 female patients were analyzed, including patients undergoing clinical investigation for venous insufficiency or lipedema who underwent ultrasound evaluations at our institution. Patients were divided in two groups: with lipedema clinically diagnosed and those without lipedema. They underwent a common Doppler protocol for venous mapping to assess venous insufficiency associated with the evaluation of dermis and subcutaneous thickness at pre-defined points of the lower limbs. RESULTS: There were 63 patients with lipedema. Anterior thigh, pre-tibial and lateral aspect of the leg and supra-just medial malleolar region were significantly different. Supra-just medial malleolar region was significantly different with BMI above 25. An optimal cutoff value was calculated for the ultrasound diagnosis of lipedema using thickness of the dermis and subcutaneous tissues. CONCLUSIONS: Studied criteria allow use of simple and reproducible ultrasound cutoff values to diagnose lipedema in the lower limbs. Pre-tibial region thickness measurement, followed by thigh and lateral leg thickness are recommended for the ultrasound diagnosis of lipedema.

BACKGROUND: Lipedema is characterized as abnormal bilateral deposition of fat in the buttocks and legs that may be accompanied by orthostatic edema in women. A questionnaire for assessment of lipedema symptoms has previously been published in German and English to assess treatment progress. OBJECTIVES: To translate, culturally adapt, and validate the lipedema symptoms assessment questionnaire for Brazilian Portuguese. METHODS: The process involved three translations and two back-translations performed by independent translators, followed by construction of a consensus version and adaptation according patients' comprehension. The questionnaire was converted into a digital version and administered to 56 volunteers and then administered to 154 patients from a vascular clinic and correlated with volume determined by segmental bioimpedance. RESULTS: The 20 pre-test patients were female and at least 90% of the interviewees were able to understand the questions in the final phase. Volunteers had a 96.4% rate of comprehension of the digital online version and a mean completion time of 4 minutes. In 154 patients, limb volume was positively correlated with intensity of symptoms, as assessed by the translated questionnaire, and 3 out of 15 questions were weakly correlated with educational level. CONCLUSIONS: The translated and culturally adapted Brazilian Portuguese version of the lipedema symptoms assessment questionnaire (QuASiL) is a practical tool that is easy and quick to administer and can be used in our population. Additional studies are still needed to assess the instrument's sensitivity as an aid for diagnosis of lipedema.

We created a lipedema screening questionnaire based on a questionnaire for assessing symptoms. OBJECTIVES: The study objectives were to identify relevant clinical questions, develop a screening questionnaire, and construct a model for predicting lipedema. METHOD: A simplified questionnaire was constructed and administered to a sample of patients with and without lipedema and then the probability of correct diagnosis was analyzed. RESULTS: All 109 patients who answered the questionnaire were female and all of them understood the questions. A predictive model using individual question scores achieved an excellent probability of correct diagnosis, at 91.2%, and a predictive model based on total score also achieved a good probability of correct diagnosis, at 86.15%. CONCLUSIONS: The lipedema screening questionnaire is a practical instrument that is quick and easy to administer and can be used with our population for identification of possible lipedema patients, raising the level of suspicion when taking a patient's history and conducting a physical examination.

Resumo Contexto O lipedema é muito subdiagnosticado e faltam ferramentas auxiliares diagnósticas de baixo custo. Baseado em um questionário de avaliação sintomática, criamos e validamos um questionário de rastreamento do lipedema. Objetivos Os objetivos do trabalho foram a identificação de perguntas clínicas relevantes, a elaboração de questionário de rastreamento e a criação de modelo de predição do lipedema. Métodos Um questionário simplificado foi criado e aplicado em um grupo de pacientes com e sem lipedema, sendo avaliada a probabilidade de acerto no diagnóstico. Resultados Os 109 pacientes que responderam ao questionário eram do sexo feminino e as questões foram compreendidas. O modelo preditivo com perguntas individuais mostrou excelente probabilidade de acerto, de 91,2%, e o modelo preditivo com somatória de pontos também teve boa probabilidade de acerto, de 86,15%. Conclusões O questionário de rastreamento do lipedema é um instrumento prático, de fácil e rápida aplicação, que pode ser utilizado em nossa população para a identificação de possíveis pacientes com lipedema, aumentando o nível de suspeição no momento da anamnese e exame físico.

Resumo Contexto O lipedema é caracterizado como uma deposição anormal de gordura em glúteos e pernas bilateralmente, que pode ser acompanhada por edema ortostático em mulheres. Foi publicado questionário de avaliação sintomática em lipedema em alemão e inglês para avaliação de evolução de tratamento. Objetivos Tradução, adaptação cultural e validação do questionário de avaliação sintomática de lipedema para português brasileiro. Métodos O processo consistiu em três traduções e duas retrotraduções realizadas por tradutores independentes, seguida da elaboração de versão consensual e adaptação conforme compreensão das pacientes. O questionário foi convertido em versão digital, aplicado em 56 voluntárias e em 154 pacientes de consultório vascular e correlacionado com volumetria por bioimpedância segmentar. Resultados As 20 pacientes do pré-teste eram do sexo feminino e as questões foram compreendidas por pelo menos 90% das entrevistadas na fase final. Voluntárias mostraram compreensão de 96,4%, com tempo médio de resposta de 4 minutos. Em 154 pacientes, o volume dos membros mostrou correlação positiva com a intensidade dos sintomas avaliada pelo questionário traduzido e correlação fraca e moderada em apenas 3 perguntas, num total de 15 com nível de escolaridade. Conclusões A versão traduzida e adaptada culturalmente para o português do Brasil do questionário de avaliação sintomática do lipedema (QuASiL) é um instrumento prático, de fácil e rápida aplicação, que pode ser utilizado em nossa população. Ainda são necessários estudos adicionais para avaliar a sensibilidade do instrumento para o auxílio no diagnóstico do lipedema.

Introduction: Manipulation of images using three-dimensional multiplanar reconstruction algorithms (3D MPR) and maximum intensity projection (MIP) is dependent on prior understanding of the method's true validity and its superiority over traditional semi-digital or analog methods of measurement. Objective: To assess the understanding of doctors who attended the course of the methodology they routinely employed for planning endovascular surgery before taking the course and determine their choice of method after completion of the course. Methods: A survey was conducted with the students who took the course using an intranet questionnaire. Results: One hundred and sixty-one participants responded to the questionnaire. In relation to their prior knowledge, 38.8% reported no knowledge, 45.6% reported little knowledge, 15% reported basic knowledge and just 0.6% considered their prior knowledge was advanced. With respect to the measurement method used, 12.5% relied on the measurements in the radiologist's report, 14% used printed plates and manual compasses, 36.8% used axial images to take measurements, 11.8% used axial images in OsiriX, 14% used the 3D MPR method and 11% used the 3D MPR method and 3D MPR combined with MIP. Additionally, 81.5% of the participants stated that they repeated measurements intraoperatively using a centimeter pigtail catheter, despite having taken measurements in advance using one of the methods listed above. Conclusion: The study showed that continuing education in specialized course was effective for imparting understanding of the importance of the three-dimensional multiplanar reconstruction image analysis method and of image optimization algorithms. .


Introdução: A manipulação de imagens utilizando os algoritmos de reconstrução tridimensional multiplanares (3D MPR) e a intensidade de projeção máxima (MIP) demanda, inicialmente, a constatação da real validade do método e da sua superioridade sobre os métodos tradicionais analógicos ou semidigitais de mensuração. Objetivo: Avaliar a compreensão dos médicos que realizaram o curso da metodologia por eles aplicada anteriormente, para programação cirúrgica endovascular, e sua opção de método após realização do curso. Método: A pesquisa foi realizada entre os alunos que realizaram o curso, que responderam um questionário em intranet. Resultados: Um número de 161 participantes respondeu ao questionário proposto. Com relação a seu conhecimento prévio, 38,8% reportaram nenhum conhecimento, 45,6% reportaram pouco conhecimento, 15% responderam conhecimento básico e apenas 0,6% considerou seu conhecimento como avançado. Com relação ao método de mensuração utilizado, 12,5% confiavam nas medidas do laudo do Radiologista; 14% utilizavam as chapas impressas e usavam compasso; 36,8% utilizavam as imagens axiais para fazer as medidas; 11,8% utilizavam as imagens axiais no próprio OsiriX; 14% utilizavam o método 3D MPR, e 11% utilizavam o 3D MPR e o 3D MPR associado ao MIP. Dos participantes, 81,5% afirmaram refazer as medidas no intraoperatório com o uso do cateter centimetrado, apesar de ter feito as medidas anteriormente por um dos métodos supracitados. Conclusão: A pesquisa revelou que a educação continuada, por meio de curso especializado, mostrou-se eficaz na compreensão da importância do método de análise de imagens por reconstrução tridimensional multiplanar e algoritmos de otimização de imagens. .

BACKGROUND: Fully implantable catheters are important for oncology treatments. They can be functionally categorized as valved or nonvalved. Theoretically, a valve prevents spontaneous blood reflux into the catheter, reducing the incidence of complications. We sought to compare the results from the implantation of valved and nonvalved fully implantable 8-French catheters inserted via ultrasound-guided puncture in cancer patients undergoing chemotherapy treatment. METHODS: A retrospective analysis of 100 patients who underwent long-term catheter implantation guided by ultrasound was performed looking for early (≤30 days) or late (≥90 days) complications. They were evaluated regarding the use of valved or nonvalved catheter and assessed on univariate model. RESULTS: The only early complication (hematoma) was identified in the valved group. Twenty-two late complications were identified (72.72% in the valved group and 27.27% in the nonvalved group; P = 0.009). Blood reflux dysfunction, which occurred in 12 patients in the valved and none in the nonvalved devices group, was the only complication with an incidence that was significantly different between the groups. CONCLUSION: Fully implantable valved catheters with Groshong (Bard Medical Division, Covington, GA) valves have a higher rate of blood reflux dysfunction compared with nonvalved catheters, but this did not interfere with the efficacy of the treatment.
