RESUMO

Struma ovarii is an ovarian teratoma composed mainly of thyroid tissue, which can occasionally develop a malignant thyroid tumor. We report a 61 years old female consulting for a metrorrhagia in whom an ovarian cyst was discovered. The patient was subjected to a hysterectomy and bilateral oophorectomy. The pathological study of the surgical piece revealed a focus of papillary thyroid carcinoma, follicular variety in a right struma ovarii. Three months after surgery, an abdominal CAT scan did not show any abnormality.

UNLABELLED: The study of pro-inflammatory cytokines produced in situ in heart allografts may help to understand the mechanisms of rejection and open new possibilities to control graft rejection. METHODS: A total of 23 endomyocardial biopsies obtained from 16 transplanted patients treated with triple-drug therapy (azathioprine, prednisone, and cyclosporine) were studied. mRNA expression for tumor necrosis factor (TNF)-alpha, interleukin (IL)-1 beta, IL-6, IL-10, IL-12, IL-15, transforming growth factor (TGF)-beta, and beta-actin was determined by reverse transcription polymerase chain reaction (RT-PCR) and Southern blotting. Semiquantitative analysis was done by establishing the ratio between densitometric integrated value of each cytokine with the beta-actin and correlated with the histopathologic findings. RESULTS: Three groups of biopsies were determined according to the International Society for Heart and Lung Transplantation criteria: grade 0 (control group, n=12), grade 1A (sub-clinical rejection, n=6) and 'quilty effect' (n=5). An increased expression of mRNA for TNF-alpha and IL-6 (p=0.0091 and 0.0075, respectively) was found associated with rejection grade 1A episodes, mRNA for IL-1 beta was nonspecifically expressed in all the study groups, while IL-10 mRNA was not detected in any of the biopsies studied. mRNA for IL-12 and IL-15 was not associated with rejection. Interestingly, TGF-beta was not detected in any of the biopsies with the 'quilty pattern'. CONCLUSION: The association of TNF-alpha and IL-6 mRNA in situ expression with mild histologically probed rejection episodes may be used in the monitoring of heart transplants.

RESUMO

Two renal allograft recipients who had received their organs from the same cadaver donor developed acute toxoplasmosis shortly after transplantation. Neither of the recipients had serologic evidence of previous exposure to Toxoplasma gondii at the time of surgery, but the donor had a positive indirect fluorescent antibody test. One of the recipients died during the fourth week, and multiorgan involvement with toxoplasmosis was demonstrated at autopsy. No evidence of the parasite could be found in the transplanted kidney. In the second recipient the disease was suspected, serologically demonstrated, and successfully treated. We concluded that toxoplasmosis was transmitted by the donor's kidneys, although this mode of transmission was not completely proven.

RESUMO

Sixty-five children were studied for over five years following an acute glomerulonephritis episode. Proteinuria was detected in 15 but in one, it was massive proteinuria with edema and low renal function. The other 14 children were asymptomatic and proteinuria was close to or below 0.5 gms/24hs. Only one out of 10 renal biopsies was normal. Focal mesangial proliferation was described in 9. Such renal lesion was considered as residual rather than active. From these results it is concluded that the prognosis of acute glomerulonephritis in children is good in 98% of cases.

RESUMO

The investigation on siblings of AGN patients has shown a streptococcus carrier evidence of 45%, while glomerulonephritis incidence was 15%. These data allowed us to classify our population as highly susceptible to bacterial dissemination and to the development of non-suppurative complications. Renal compromise was clinical and histologically classified. Subclinical forms showed glomerular lesions of mesangial localization, while clinical forms exhibited variable degrees of cellular proliferation. All patients recovered except one who died of a rapidly progressive form.

RESUMO

El estudio de una poblacion de hermanos de pacientes con glomerulonefritis aguda mostro una incidencia de portadores de estreptococo del 45% mientras que la frecuencia de glomerulonefritis fue del 15%. Estos datos permiten clasificar a nuestra poblacion como altamente susceptible a la diseminacion de esta bacteria y el desarrollo de sus complicaciones no supurativas. El compromiso renal fue caracterizado clinica e histologicamente en las diferentes formas de presentacion. Las formas subclinicas presentaron lesiones glomerulares de localizacion mensangial mientras que las formas clinicas mostraron grados variables de proliferacion celular. Con excepcion de un paciente que murio con una forma rapidamente progresiva, todos evolucionaron hacia una curacion aparentemente definitiva

RESUMO

El estudio de 65 pacientes con mas de 5 anos despues de un episodio de GNA mostro proteinuria en 15 de ellos. Solo en uno, la proteinuria ha sido masiva con edemas y baja funcion renal. En los 14 restantes, la proteinuria fue asintomatica, cercana o inferior a 0,5 g/24 horas. De 10 biopsias estudiadas, solo una pudo ser clasificada como normal. En las otras 9, se observo proliferacion mesangial de tipo focal. Estas lesiones fueron consideradas de caracter residual. El analisis de lo resultados permite concluir que el pronostico a largo plazo de la GNA en el nino es bueno en el 98% de los casos

RESUMO

The eighth Colombian case of lobomycosis and the one hundredth in the world literature is reported here. The patient was a 38-year-old female who showed a bilobed mass on the left forearm which had appeared at the site from where a similar lesion had been excised 19 years previously. She had always lived in a rural area, located in the humid subtropical forest. The tissue reactions to the fungus as well as the fruitless attempts to culture it are described. An analysis of the relevant literature is made and suggestions for experimental animal models are given in the text.
