RESUMO

OBJECTIVE: The internal (structural) and external validity of a self-report measure of cognitive disengagement syndrome (CDS, formerly sluggish cognitive tempo) relative to a self-report measure of attention-deficit/hyperactivity disorder-inattention (ADHD-IN) was evaluated with adults from university outpatient psychiatric clinics in Turkey. METHODS: A total of 274 outpatients (75.9% women; ages 18-64 years; Mage = 31.06; SDage = 10.84; 50.4% anxiety disorders; 41.6% depressive disorders; 2.9% ADHD; 1.5% sleep disorders; 0.7% eating disorders; 2.9% no mental disorder) completed self-report measures of CDS, ADHD-IN, ADHD-hyperactivity/impulsivity (HI), sleep problems, depression, and stress. RESULTS: All 15 CDS symptoms measured by the Adult Concentration Inventory (ACI) showed convergent (moderate to high loadings on the CDS factor) and discriminant (loading close to zero on the ADHD-IN factor) validity. CDS also showed stronger first-order and unique associations than ADHD-IN with sleep problems, depression, anxiety, and stress, whereas ADHD-IN showed stronger first-order and unique associations than CDS with ADHD-HI. CONCLUSION: This is the first study to provide support for the scores from this 15 item self-report measure of CDS by the ACI in a clinical sample of adults, with findings consistent with previous studies examining parent and teacher rating scale measures with the same 15 CDS symptoms. These findings provide additional support for usefulness of these 15 CDS symptoms as measured by the ACI to study CDS across various cultures.

BACKGROUND: Mantle cell lymphomas are aggressive, mature B-cell neoplasms characteristically showing overexpression of cyclin D1. Although lymphadenopathy is the most common presentation, involvement of extranodal sites including bone marrow, peripheral blood, liver, gastrointestinal system, and Waldeyer ring is also seen frequently. Soft tissue localization is extremely rare. It has blastoid and pleomorphic subtypes associated with aggressive course. CASE DESCRIPTION: We describe a 74-year-old man who had been diagnosed 3 years previously with "mantle cell lymphoma-blastoid type" and presented 3 months ago with a giant mass in the right lower extremity that enlarged rapidly up to 15 cm in a few months. CONCLUSION: We present this rare presentation, which was evaluated in favor of hemangioma before biopsy, together with the data in the literature to emphasize the need for differential diagnosis, especially in cases with a clinical history.

The aim of this study was to evaluate adenomatoid tumours (AT) clinicopathologically in the female genital tract and compare the histomorphological features of ATs according to their uterine or tuba-ovarian location. Cases of AT were excised and collected from female genital tracts between the years of 2010-2017. Cases were evaluated depending on their clinical findings, localisation and pathological properties. There were 14 cases of AT. Ten cases were uterine, and 4 cases were adnexal tumours. The diagnostic ratio of uterine ATs was 64.3%, and of tuba-ovarian ATs was 21.4% (P > 0.05). The size of the largest tumour was 6 cm. Two of the uterine and one of the ovarian cases had a macrocyst; 2 uterine and one ovarian case had a microcyst; and 6 uterine had a combined microcystic/trabecular pattern. Uterine cases showed a higher number of smooth muscle component, signet-ring cells and infiltrative nature compared with other cases (P < 0.05). All uterine cases were infiltrative. Most of ATs of the female genital system were small in size and incidentally diagnosed in our cases but rarely detected as an adnexal mass forming lesion which mimics a malignancy. A comparative clinicopathologic analysis of these cases should be considered with the histomorphological and immunohistochemical features for an accurate differential diagnosis.

OBJECTIVE: The aim of the study was to conduct a scale-based evaluation of the hearing skills of unilateral, bimodal and bilateral cochlear implant (CI) users, including distinguishing, orientating and locating speech and environmental sounds in their surrounding environment that they are exposed to in different contexts of everyday life. The scale results were compared between groups. METHODS: A total of 74 cochlear implant users, 30 unilateral, 30 bimodal and 14 bilateral, were included in the study. Their ages ranged from 11 to 64 years. Participants were assessed using the Speech, Spatial and Qualities of Hearing Scale (SSQ). RESULTS: Bilateral CI users' subjective ratings of their own hearing skills were found to be significantly better than those of bimodal and unilateral CI users; bimodal users' subjective ratings were also found to be significantly better than those of unilateral CI users. Paired comparisons showed statistically significant differences between the groups in terms of total scores of Speech, Spatial, Qualities of Hearing and General SSQ (p<0.05). CONCLUSION: Our findings show that bilateral use of cochlear implants should be recommended for those presently using bimodal and unilateral devices. Moreover, subjective tests should be used regularly along with objective tests for evaluating CI patients.

RESUMO

Large-cell neuroendocrine carcinoma (LCNEC) of the uterine cervix is a rare aggressive tumor. The examination of a cervicovaginal smear from a 31-year-old patient diagnosed with LCNEC after a cervical polypectomy during the 32nd week of pregnancy was carried out. The observed atypical cells had large cytoplasm, increased nucleus: cytoplasm ratio with the nucleus containing coarse, dispersed chromatin, and were arranged in a pseudorosette formation, which all confirmed the diagnosis. In addition, adenocarcinoma in situ (AIS) was determined in the histopathological examination of the subsequent hysterectomy material. Given the rarity of this condition, we present and discuss the case herein.

Isolated breast relapse after allogeneic hematopoietic stem cell transplantation (allo-HSCT) is less often seen. Chronic graft-versus-host disease (cGVHD) is effective in preventing marrow relapse, but cGVHD seems not to be effective extramedullary relapse (EMR). We report the case of isolated breast relapse after first allo-HSCT for acute lymphoblastic leukemia (ALL). A 47-year-old female was diagnosed with ALL achieved complete remission with salvage chemotherapy and underwent allo-HSCT from an HLA-matched sibling male donor. At 17 months post-transplant, she presented with a bilateral breast masses that confirmed the diagnosis lymphoblast involvement. She had no evidence of leukemia in her marrow that determined 100 % full-donor chimerism when she was relapsed in her both breasts.
