RESUMO

Measurement-based care has important implications across multiple avenues in mental and behavioral health care, including clinical care, quality improvement, and accountability. Using measurement-based care to demonstrate that quality care is being provided within the context of cost-efficient care could strengthen the position of mental and behavioral health providers as critical members of the health care system. Yet when measurement-based care is used to assess performance of providers, and then that performance influences reimbursement, it must be done with great care and deliberation so as not to result in unintended consequences such as punishing providers. Given psychology's expertise in measurement, the American Psychological Association (APA) and its members are uniquely suited to be leaders in promoting measurement-based care to assess quality and value. In this policy analysis paper, we examine the importance of measurement-based behavioral and mental health care across a variety of public service populations. We describe the increased federal regulatory focus on promoting quality and cost efficient care, the importance of defining and measuring quality care, and introduce an important resource being developed by APA to promote provider engagement in measurement-based care and effective participation in payment reform efforts in health care. We conclude with specific recommendations for how the field can move forward with using measurement-based care to assess accountability. On November 19, 2011, Norine G. Johnson, the ninth woman to serve as president of the American Psychological Association (APA), lost a valiant battle with cancer. Norine's curiosity about her grandmother's strength led to much of her later work on the development of strength and resiliency in adolescent girls and in women. She received her doctorate in 1972, with a minor in child development, and she became one of the first to be considered a pediatric psychologist, a newly emerging specialty. Norine became involved in organized psychology as her children got older. When she learned there had not been a woman president of the Massachusetts Psychological Association (MPA) for almost 50 years, she was appalled. An ardent feminist, who had served on MPA's Board of Directors, she could not let the situation remain that way. She ran for president and won, serving from 1981 to 1983, and then mentored many women colleagues into the role. MPA sent her to the Council of the APA as one of their representatives, and she immediately ran for a seat on the Finance Committee, later serving as chair of the committee. During her tenure on the Finance Committee, she helped create financial policy that changed APA from an organization whose assets were threatened and shrinking to a financially solid association. Norine considered Division 35 (Psychology of Women) to be her home in APA, a place filled with warmth, collegiality, and shared values. She and Judith Worell spearheaded the extremely important and successful 1993 National Conference on Education and Training in Feminist Practice. Norine was elected to the APA Board of Directors in 1997, where she served with distinction, continuing to focus on the financial well-being of the Association. Not surprisingly, she went from that role to being elected APA president. Her focus as president was on the changes in the health care delivery system in the United States. She was a staunch champion of the biopsychosocial model of health. Under her presidential leadership, the APA Mission Statement was amended to add the word "health," confirming the importance of health to the psychological community. RESUMO

This article offers a blueprint for modernizing the delivery of high-quality behavioral health care and for improving access to care by a public sorely in need of psychological services. The blueprint brings together disparate elements of psychology practice into a more unified structure, an updated house, based upon advances in the essential building blocks: evidence-based practice, treatment guidelines, technology, classifications of function, diagnostic systems, outcomes measurement, and integrated health care. The goal is twofold: to make psychological services more accessible to the public and to position psychology for an increasingly major role in health care in order to serve the public weal in diverse communities.

RESUMO

Responds to comments by D. C. Wendt and B. D. Slife, P. H. Hunsberger, and R. B. Stuart and S. O. Lilienfeld regarding the report by the APA Presidential Task Force on Evidence-Based Practice entitled Evidence-based practice in psychology. The goal of the task force was to create a scheme that would suggest how evidence should be used to design and offer services that will benefit patients and to assure the public and the health care system that psychologists are providing evidence-based services. There were and will continue to be many scientific and philosophical issues inherent in any such enterprise, and agreement by all psychologists with every aspect of EBPP may not be possible. Nevertheless, the APA's EBPP policy and the report that accompanied it are remarkably inclusive of various perspectives while remaining unambiguous about the need to use evidence in a way that leads to effective services. What is needed at this point are clinically relevant evidence and investigations of how such evidence can be used to best benefit those served by psychological interventions.
