RESUMO

Successfully treating bone infections is a major orthopedic challenge. Clinically, oral, intravenous, or intramuscular injections of drugs are usually used for direct or complementary treatment. However, once the drug enters the system, it circulates throughout the body, leading to an insufficient local dose and limiting the therapeutic effect because of the lack of targeting in the drug system. In this study, ß-cyclodextrin, modified with poly (ethylene glycol) [PEG] and aspartic acid hexapeptide (Asp6-ß-CD), was used to specifically target the hydroxyapatite (HA) component of the bone. It was then loaded with norfloxacin (NFX) to treat bone infections. The antibacterial ability of NFX was enhanced by loading it into Asp6-ß-CD, because the solubility of Asp6-ß-CD@NFX increased significantly. Moreover, Asp6-ß-CD could target bone tissue in nude mice and showed significantly enhanced accumulation (10 times) than the unmodified ß-CD. In addition, in a rat model of osteomyelitis, Asp6-ß-CD@NFX targeted HA well and exerted its antibacterial activity, which reduced inflammation and promoted bone tissue repair. This study indicates that the Asp6-ß-CD based drug delivery system can efficiently target bone tissue to enable potential applications for treating bone-related diseases.

RESUMO

Medical equipment maintenance in modern hospital management is an emerging marginal discipline which is one of the important branches of hospital management with the dual function of management and technology. The good or bad management of medical equipment directly affects the quality of hospital work. With the development of science and technology, the scale of hospitals continues to expand and develop; hospitals are equipped with more and more medical equipment and its technical content is getting higher and higher, which makes the medical equipment management and maintenance practitioners more demanding. However, the situation of existing medical equipment management and maintenance personnel is not optimistic. For this reason, this paper designs a good database structure, which can effectively reduce data redundancy and better avoid the problems that may occur during the database operation. As a reference guideline for database design, the three database paradigms can be studied and understood in depth to help software designers design more "robust" databases, enabling effective maintenance and management of medical imaging equipment.

A glucose-sensitive multilayer film was fabricated by the layer-by-layer (LbL) assembly method with positively charged 21-arm poly[2-(dimethylamino)ethyl methacrylate] (star PDMAEMA) and negatively charged insulin and glucose oxidase (GOD) in the form of {(Star PDMAEMA/Insulin)(4) + (Star PDMAEMA/GOD)(4) + Star PDMAEMA}. The multilayer film shows an on-off regulation of insulin release in response to stepwise glucose challenge in vitro. It is found that the unique structure of star PDMAEMA and interdiffusion of charged insulin are the main factors to control the on-off status of the film. Reversible surface morphology transitions of the multilayer film were also observed, revealing a phase separation and large-scale reorganization process. Furthermore, the multilayer film could continuously release enough insulin in vivo after being subcutaneously implanted in streptocozotin-induced diabetic rats and reduce the blood glucose level for at least two weeks. It is indicated that such system may have substantial potential as a glucose-sensitive carrier for insulin due to its distinct mechanism.

A series of branched cationic beta-cyclodextrin polymers (CPbetaCDs) with designed chemical structures were synthesized from beta-cyclodextrin (beta-CD), epichlorohydrin (EP) and choline chloride (CC). Indomethacin (IDM), an anionic drug, was chosen as a model drug to evaluate the drug loading capacities of CPbetaCDs. The formation of IDM-CPbetaCD complex was confirmed by (1)H NMR and DSC. Phase solubility studies and Job plots indicated that CPbetaCDs can solubilize IDM up to 100 times of its intrinsic solubility in a 1:1 complexation form. Mechanism studies with the help of adamantane revealed that the effective complexation is a combination of inclusion complexation, charge interaction and hydrophobic interaction. In addition, IDM-CPbetaCDs loaded alginate hydrogels were prepared and obtained controllable release profile in dissolution tests. The tunable structures of CPbetaCDs make them promising drug carriers with superior drug loading capacities and controllable drug release abilities.
