First-line treatment with R-CHOP or rituximab-bendamustine in patients with follicular lymphoma grade 3A-results of a retrospective analysis.

Based on centroblast frequency, follicular lymphoma (FL) is subdivided into grades 1-2, 3A, and 3B. Grade FL3A frequently coexists with FL1-2 (FL1-2-3A). Based on clinical trials, FL1-2 is treated with rituximab (R) or obinutuzumab plus bendamustine (B) or CHOP, while FL3B is treated with R-CHOP. In contrast, there are little data guiding therapy in FL3A. We present a retrospective, multicenter analysis of 95 FL3A or FL1-2-3A and 203 FL1-2 patients treated with R-CHOP or R-B first-line. R-CHOP facilitated a higher response rate (95% versus 76%) and longer overall survival (OS) (3-year OS 89% versus 73%, P = 0.008) in FL3A or FL1-2-3A, whereas the difference in progression-free survival (PFS) did not reach statistical significance. While transformation rates into aggressive lymphoma were similar between both groups, there were more additional malignancies after R-B compared with R-CHOP (6 versus 2 cases). In FL1-2, R-B achieved a higher 3-year PFS (79% versus 47%, P < 0.01), while there was no significant difference regarding OS or transformation. With the limitations of a retrospective analysis, these results suggest a benefit for R-CHOP over R-B in FL3A or FL1-2-3A. Confirmatory data from prospective clinical trials are needed.

New technique to enlarge the aortic annulus.

A new technique is described to enlarge the aortic annulus and the supraaortic area by using the anterior mitral leaflet. This technique is used when it is necessary to replace both the aortic and mitral valves.

[Coronary surgery in patients with poor ventricular function]. / La chirurgie coronarienne chez les patients avec mauvaise fonction ventriculaire.

The surgical indication of coronary by-pass surgery in patients with bad ventricular function is difficult. We analysed retrospectively 87 patients operated on between February 1982 and November 1983. We show the benefit of this surgery even in patients without angina pectoris.

The Hewlett-Packard 1000--Vitalmetrics VM220 connection: a description of the automated ultrasonic urine output measurement system in the CICU of Genolier Clinic.

After over eighteen years experience using an automated weighing technique to monitor the urine output, we have changed our system in favor of the Vitalmetrics VM220 ultrasonic urine/bladder temperature measurement devices. Last year we began with one VM220 interfaced directly to the Hewlett-Packard 1000 computer's standard ASCII circuit card (H-P 12966A ASCII BACI). We found that we could read and interpret the ASCII string transmitted by the VM220; then started a search for a suitable multiplexing device through which we could manage all six of our VM220's with only one interface card. Perhaps there are many more, but we found two which we shall discuss in this report: 1. The Baytech Associates model 528-B 8 channel switching device and 2. the SEA Corporation signal distribution controller (also known as the cluster controller) which has 16 channels. In our post-operative cardiovascular intensive care unit (CICU), we have used the Baytech 528-B multiplexer to monitor the urine output for over 250 open heart surgery patients. Later this year, we acquired a cluster controller from SEA Corporation in Birmingham, Alabama (USA). We have written software for testing and incorporating this application into a Hewlett-Packard Patient Data Management System (PDMS).

Double inlet right ventricle. Report of a case and review of the literature.

A case of double inlet right ventricle in which both atrioventricular valves opened completely into the right ventricle is described. The left atrium communicated with the right ventricle by way of an opening low in the atrial septum and there was a large ventricular septal defect. The pulmonary trunk and aorta took their origin from the right and hypoplastic left ventricles respectively. The literature on this rare cardiac anomaly is reviewed and the morphological findings in this case are compared with those of the recorded cases. A short discussion of the possible embryogenesis is given.
