HYPOTHESIS: The purpose was to evaluate long-term clinical outcomes and tendon structural integrity after repair of isolated subscapularis (SSC) tendon tears. METHODS: Sixty-one patients who underwent repair of isolated SSC tears were evaluated. The mean interval from symptom onset to surgery was 5.3 months. Shoulder function was investigated using the Constant score and Subjective Shoulder Value (SSV). Structural integrity of the repair and quality of the repaired tendon were assessed using magnetic resonance imaging. Independent preoperative factors affecting clinical outcomes, including patient sex, age, smoking habits, injections, dominant shoulder, profession, and tear onset (chronic vs. traumatic), were evaluated. RESULTS: A total of 35 patients (9 women and 26 men; mean age, 54 years) were included at the final 10-year follow-up because 21 patients were lost to follow-up and 5 underwent reoperations. The mean Constant score improved from 55.1 points preoperatively to 75.4 points postoperatively (P = .001). The postoperative SSV was 80.9, and the retear rate was 12.9%. Postoperative fatty infiltration increased in 26% of the patients with grades 3-4, but it was not related to lower clinical outcomes. Multivariable regression analysis revealed no correlation among the preoperative factors, including sex, age, smoking habits, injections, dominant shoulder, profession, and tear onset, and the postoperative Constant score, SSV, and tendon healing. CONCLUSION: At a mean of 10 years after repair of isolated SSC tears, clinical results were satisfactory and functional improvement was maintained in the long term. Severe fatty infiltration increased with time, but it was not related to clinical outcomes and the retear rate.

PURPOSE: To evaluate the arthroscopic double-layer lasso loop repair technique for delaminated posterosuperior rotator cuff tears. METHODS: Forty-one patients underwent arthroscopic rotator cuff repair of a delaminated posterosuperior rotator cuff tear by the double-layer lasso loop technique. Their preoperative and postoperative clinical and functional scores were compared to evaluate surgical outcomes. The prerequisite for inclusion was a minimum follow-up period of 2 years. We excluded patients with a history of shoulder surgery before the double-layer lasso loop repair. RESULTS: Statistically significant improvements (P < .001) were found in the Constant score (54 vs 83) and University of California, Los Angeles functional score (6.4 vs 9.5). Pain and strength improved according to the Jobe test, bear-hug test, belly-press test, Gerber lift-off test, and external rotation test (P < .001). No significant difference in strength was noted between the operated and nonoperated sides. There was only 1 complete rerupture (3.1%), whereas 5 patients (15.6%) had partial ruptures. CONCLUSIONS: The arthroscopic double-layer lasso loop repair technique for delaminated posterosuperior rotator cuff tears is an effective procedure. Our series showed a low rerupture rate. At a mean follow-up of 44 months, postoperative recovery with resultant functional, pain, and patient satisfaction scores was good to excellent and was comparable with the nonoperated side. BACKGROUND AND HYPOTHESIS: The Latarjet procedure has been shown to be a reliable method to prevent recurrent anterior shoulder instability. Coracoid bone graft osteolysis is a potential catastrophic complication and can lead to recurrent instability. The purpose of our study is to present a novel quantitative method to measure the amount of coracoid bone osteolysis using 3-dimensional (3D) computed tomography (CT) scan imaging. MATERIALS AND METHODS: This is a prospective study with 15 patients (16 shoulders) who underwent an arthroscopic Latarjet procedure. Three-dimensional CT scans were obtained at 6 weeks and 6 months. Using volumetric analysis, we quantified the amount of bone loss using our described method. Interobserver reliability and intraobserver reliability were calculated. RESULTS: On the basis of our new volumetric analysis of the arthroscopic Latarjet procedure using 3D CT scans, we found that the superior half of the coracoid bone graft undergoes a significant amount of osteolysis at 6 months postoperatively. The interobserver reliability and intraobserver reliability were excellent. DISCUSSION: This study presents a reproducible method to quantify and compare coracoid bone graft osteolysis after an arthroscopic Latarjet procedure. We also developed a description system that may be used for comparison studies. To our knowledge, this is the first method that quantifies the amount of coracoid bone graft osteolysis using more accurate 3D CT scanning. CONCLUSION: The 3D analysis we propose is a valid method to measure the amount of coracoid bone graft osteolysis after an arthroscopic Latarjet procedure. Our description system may guide the surgeon regarding possible revision surgery when faced with significant osteolysis of the coracoid bone graft.

The purpose of this study was to validate the 'Forgotten Joint' score (FJS-12), a 12-item questionnaire designed to analyze the patient's ability to forget the joint in everyday life, in French and to compare the results of this Patient Reported Outcome (PRO) score in patients who had other than total joint arthroplasties. The score was compared in 122 patients that had either medial unicompartmental (N=51), patellofemoral (N=21) or total knee arthroplasty (N=50). After having validated the FJS-12 in French, a similar PRO was observed in unicompartmental and postero-stabilized total knee arthroplasty. Patellofemoral resurfacing had a significantly lower score than the two other types of arthroplasty, which can be explained by a significantly younger and smaller patient group.

BACKGROUND: Hallux valgus is a functional deformity of the first ray. When incorrectly aligned, the first ray is less effective as a support structure, and the stresses then theoretically transfer to the second metatarsal which could lead to increased cortical density. The aim of this study was to evaluate if surgical correction of hallux valgus lead to changes in the cortical density of the second metatarsal. MATERIALS AND METHODS: This was a retrospective study involving 13 patients who had surgery for isolated hallux valgus. The average postoperative followup was 16 (range, 11 to 24) months. We analyzed pre- and postoperative radiographs of the feet. We assessed the variations in the medial and lateral cortical thickness, the medullary thickness and the shaft thickness (ST) of the second and fourth metatarsals. RESULTS: A significant reduction of the medial cortical bone thickness of the second metatarsal (MT2) (p < 0.001) and a significant medullary increase of MT2 (p < 0.001) were observed postoperatively. However, no significant difference in the lateral cortical thickness or shaft thickness of the metatarsal was found. A significant reduction of the MCT/ST ratio (p < 0.001) was also observed. Regarding the fourth metatarsal, which was used as a control, no significant change was observed. CONCLUSION: The isolated cortical variations of the second metatarsal suggests that isolated hallux valgus correction led to a redistribution of the stresses on to the first ray.
