RESUMO

[Purpose] This study aimed to determine the characteristics of regional phase angles based on locomotion level among older Japanese females requiring long-term care in a health facility. [Participants and Methods] This was a cross-sectional observational study. The participants included 91 residents (mean age ± standard deviation: 90.2 ± 5.6 years) admitted to an older health facility. Based on their indoor locomotion status, the participants were divided into three groups: group I, able to walk with or without walking aids; group II, able to move in a wheelchair without assistance; and group III, able to move in a wheelchair with assistance. The regional phase angle was measured using a bioimpedance device. [Results] Significant differences were observed in the regional phase angle of the upper limbs between groups I and III and between groups II and III; in that of the lower limbs among all groups; and in that of the trunk between groups I and II and between groups I and III. [Conclusion] The level of locomotion may be explained by the regional phase angles of the lower limbs.

[Purpose] We examined the impact of the assessment items for sarcopenia on discharge destination in convalescent rehabilitation patients. [Participants and Methods] In this study, 174 patients (males; 77, mean age; 72.4 years, females; 97, mean age; 80.1 years) who were admitted and discharged from the convalescent rehabilitation wards of the International University of Health and Welfare, Shioya Hospital, were included. The participants' height, weight, grip strength, and lower calf circumference were measured. Additionally, the number of motor functional independence measure (mFIM) at discharge, length of stay in the wards, and average number of rehabilitation sessions per day were calculated as the convalescent rehabilitation wards index. [Results] Age, weight, grip strength, lower calf circumference, mFIM at discharge, length of stay in the wards, and average number of rehabilitation sessions per day had significant differences between the return-to-home and non-home groups. Binomial logistic regression analysis revealed significant differences in grip strength and mFIM at discharge. [Conclusion] This study adjusted the sarcopenia criteria for sex and age and showed that grip strength significantly affected the outcome destination. Grip strength was shown to be a predictor of home discharge in convalescent rehabilitation patients.

[Purpose] The present study aimed to investigate the association of phase angle with the continuing activities of residents in a geriatric health service facility for one year. We compared the phase angle among current residents in a geriatric health service facility with those who died within one year. [Participants and Methods] This one-year prospective observational study included 149 residents who entered a geriatric health service facility. We divided participants into two groups: an over one-year survival group and a death within one year group. The receiver operating characteristic curves were created to determine the sensitivity and specificity for predicting one-year mortality based on the phase angle. [Results] The phase angle was significantly higher in the one-year survival group than in the death within one year group and was independently and significantly associated with one-year mortality using binomial logistic regression analysis. The phase angle was shown to have predictive power (based on the area under the receiver operating characteristic curve). Based on the calculation for identifying one-year survival, the optimal cut-off value for the phase angle was 2.95° (area under the receiver operating characteristic curve=0.76, sensitivity 55.6%, specificity 83.3%). [Conclusion] The phase angle may contribute to the prognosis of one-year mortality for residents of geriatric health service facilities.

Lipid nanoparticles (LNPs) are one of the most successful technologies in messenger RNA (mRNA) delivery. While the liver is the most frequent target for LNP delivery of mRNA, technologies for delivering mRNA molecules to extrahepatic tissues are also important. Herein, it is reported on the development of an LNP that targets secondary lymphoid tissues. New types of alcohol-soluble phosphatidylserine (PS) derivatives are designed as materials that target immune cells and then incorporated into LNPs using a microfluidic technique with a high degree of scalability and reproducibility. The resulting LNP that contained the synthesized PS delivered mRNA to the spleen much more efficiently compared to a control LNP. A sub-organ analysis revealed that the PS-loaded LNP is extensively taken up by tissue-resident macrophages in the red pulp and the marginal zone of the spleen. Thus, the PS-loaded LNP reported in this study will be a promising strategy for clinical applications that involve delivering mRNA to the spleen.

Based on the unique fivefold equatorial coordination of UO22+, water-compatible pentadentate planar ligands, H2saldian and its derivatives, were designed for the strong and selective capture of UO22+ in seawater. In the simulated seawater condition (0.5 M NaCl + 2.3 mM HCO3-/CO32-, pH 8), saldian2- shows the strongest complexation with UO22+ to form UO2(saldian) (log ß11 = 28.05 ± 0.07), which is more than 10 order of magnitude greater than amidoxime-based or -inspired ligand systems most commonly employed for U capture from seawater. Good selectivity for UO22+ from other metal ions coexisting in seawater was also demonstrated.

[Purpose] To characterize depression related to nutritional status in older adults requiring long-term care. [Participants and Methods] One hundred and six individuals (66 males and 40 females) over the age of 65 who required support level 1/2 or care level 1 and were receiving day care through the long-term care insurance (LTCI) system, were enrolled. The survey items included basic attributes, comorbidities, previous medical history, requiring support/care under Japan's LTCI system, the Mini Nutritional Assessment-Short Form, the Geriatric Depression Scale 15 (GDS-15), and body mass index (BMI). The factors associated with malnutrition/risk of malnutrition were evaluated. In addition, the relationship between nutritional status and the GDS-15 items was evaluated. [Results] Depression, LTCI, BMI, and gender were identified as related factors for malnutrition/risk of malnutrition. Compared with the favorable nutritional status group, the malnutrition/risk of malnutrition group reported GDS-15 items such as "Dropped activities and interests", "Feel that life is empty", "In good spirits most of the time (reversed)", "Afraid of something bad", "Prefer to stay at home", and "Feel full of energy (reversed)". [Conclusion] The results show certain parameters characteristic of depression in Japanese older adults with malnutrition requiring long-term care/support. These data will help guide future research and interventions.

[Purpose] To clarify the effects of longitudinal changes in older adults by evaluating the relationship between changes in spinal kyphosis and respiratory function over time in patients with certified need of care in the long-term care insurance system. [Participants and Methods] We included 57 older adults (28 males and 29 females) aged ≥65 years who were identified as requiring long-term care or support. The participants were community-dwelling individuals undergoing ambulatory rehabilitation. We assessed the longitudinal changes in spinal kyphosis index, respiratory function and muscle strength, and body composition over 1 year. [Results] The spinal kyphosis index was 10.5 at the first measurement and 14.6 at 1 year after the first measurement, showing a significant increase. We did not detect any significant differences in respiratory function and muscle strength, or body composition. [Conclusion] This 1 year longitudinal comparison suggests that the spinal kyphosis indexes were high, and the respiratory function and trunk muscle mass remained unchanged. Therefore, the relationships were negligible.

Marine forests have declined in many urbanized regions in recent years. One cause is the inflow of fine particles into coastal rocky shores. We examined the influence of sub-micrometre (sub-micro) particles on the early growth stages of the large brown macrophyte Ecklonia bicyclis. The percentage of substrate attachment of zoospores decreased with an increase in sub-micro sediments. As the size of the particles decreased, the negative effect became greater. There was an increase in poor levels of gametophyte survival and growth as more and smaller sediment was deposited. We consider that the causes of these phenomena owing to increasing amounts of sediment is a decrease in availability of substrate for zoospore attachment and that of area for substance exchange on the gametophytes. We also evaluated the effects in sea areas, based on the amount and size distribution of seabed sediment in the algal communities deforested by particles, and found that the inhibition of zoospore attachment and gametophyte growth by sub-micro particles was remarkably large. The sub-micro sediment on the substrate has seriously negative effects on the early stages of macrophytes. Inflow of very fine particles to natural marine forests may result in severe degradation of rocky reef ecosystems.

The purpose of this work was to determine a radiographic standard of normality for lateral and medial ungulae in 3-year-old bovine females of the Nellore breed. Twenty-six cows underwent radiographic exposure of the medial and lateral ungulae of the hindlimbs and forelimbs; abaxioaxial projections were used. The images were analyzed; distances were measured between the dorsal surface of the distal phalanx and the surface of the hoof capsule. The two angles formed, i.e., an angle between the dorsal surface of the distal phalanx and a line parallel to the sole (inner), and an angle between the dorsal surface of the hoof and a line parallel to the sole (outer), were also recorded. The results suggested a standard of normality for the proximal, middle and distal distances of the ungulae of the forelimbs: 0.96 ± 0.12 cm. For the measurement of hindlimbs, except for the distal distances of the side ungulae, the suggested value was 1.02 ± 0.14 cm; for the distal measurement of the lateral ungulae of the hindlimbs, the value was 1.09 ± 0.12 cm. For inner and outer angles, the values were 36.45° ± 4.4° for the medial ungulae of the forelimbs and 42.50° ± 3.8° for other ungulae of the forelimbs and hindlimbs (standard deviations of the values suggested for the angles should be used as variations between different animals, limbs and ungulae; they should not be used between angles of the same ungula).

PURPOSE: To evaluate the effect of pulsed ultrasound toothbrush on the removal of biofilm formed by Streptococcus mutans (S. mutans). METHODS: S. mutans biofilm grown on apatite pellet was destructed with four different sonic action toothbrushes: 1) pulsed ultrasound with sonic vibration (PUV); 2) continuous ultrasound with sonic vibration (CUV); 3) sonic vibration only (SV); and 4) no ultrasound nor sonic vibration (control). After 3 minutes of noncontact brushing, the amount of water-insoluble glucan was measured, and the residual biofilm was observed by scanning electron microscopy. RESULTS: PUV group revealed the smallest amount of the residual water-insoluble glucans (32 ± 19%), followed by the CUV group (54 ± 12%) and the SV group (64 ± 13%). The PUV group showed a significantly lower amount of the residual water-insoluble glucan than the SV group, while no significant difference was found between SV and CUV. The bacterial adherence and aggregation notably decreased in the PUV group, compared to the remaining three groups. CLINICAL SIGNIFICANCE: The sonic vibration with pulsed ultrasound showed more reduction of the biofilm compared to the control and the sonic vibration with and without continuous ultrasound. Thus, pulsed ultrasound action may be beneficial for biofilm removal of interproximal regions.

We successfully treated a Class II Division 2 patient with maxillary group distalization using interradicular miniscrews. A woman, aged 28 years 11 months, had a convex profile and an excessive overjet caused by a skeletal Class II jaw-base relationship. After leveling and alignment, titanium miniscrews were obliquely implanted between the maxillary second premolar and first molar. To distalize the maxillary dentition, nickel-titanium closing coil springs with a 2-N load were placed between the screws and the hooks on the archwire. After 28 months of active orthodontic treatment, a proper facial profile and an acceptable occlusion were achieved with a 4-mm distalization of the maxillary dentition. The resultant occlusion was stable throughout a 5-year retention period. Interradicular miniscrews were useful to distalize the maxillary dentition for correcting a Class II malocclusion. This new strategy, group distalization with miniscrews, can make the treatment simpler with greater predictability.

We successfully treated a 32-year-old woman who had facial asymmetry and unilateral mandibular condylar osteochondroma using ipsilateral mandibular condylectomy and contralateral ramus osteotomy. Mirror image analysis with a noncontact 3-dimensional image scanner showed that the soft tissue on the deviated side was protruded more than 5.50 mm compared with the nondeviated side. The patient was diagnosed as having facial asymmetry with a skeletal Class III jaw-base relationship caused by unilateral mandibular condylar osteochondroma. After 18 months of preoperative orthodontic treatment, an ipsilateral condylectomy and a contralateral sagittal split ramus osteotomy were performed. As the result of postoperative orthodontic treatment for 20 months, an ideal occlusion with a Class I molar relationship and an adequate interincisal relationship was achieved. Facial asymmetry and mandibular protrusion were dramatically improved, and the total differences between the deviated and nondeviated sides were decreased to less than 1.11 mm. The acceptable occlusion and the symmetric face were maintained throughout the 1-year retention period. Our results indicated stability after condylectomy without condylar reconstruction in a patient with unilateral condylar osteochondroma.

Osteoporotic fracture has become a major public health problem, and until today, the treatments available are not satisfactory. While there is growing evidence to support the individual treatment of parathyroid hormone (PTH) administration and low-intensity pulsed ultrasound (LIPUS) exposure as respectively systemic and local therapies during osteoporotic fracture healing, their effects have not yet been investigated when introduced concurrently. This study aimed to evaluate the effects of combined treatment with PTH (1-34) and LIPUS on fracture healing in ovariectomized (OVX) rats. Thirty-two, 12-week-old female Sprague-Dawley rats were OVX to induce osteoporosis. After 12 weeks, the rats underwent surgery to create bilateral mid-diaphyseal fractures of proximal tibiae. All animals were randomly divided into 4 groups (n = 8 for each): control group as placebo, PTH group, LIPUS group, and combined group. PTH group had PTH administration at a dose of 30 µg/kg/day for 3 days/week for 6 weeks. LIPUS group received ultrasound 5 days/week for 20 min/day for 6 weeks and combined group had both PTH administration and LIPUS exposure for 6 weeks. Fracture healing was observed weekly by anteroposterior radiography and micro-CT. Five weeks after the fracture, the tibia were harvested to permit histological assessments and at week 6, for mechanical property of the fracture callus. Micro-CT showed that the PTH and combined groups exhibited significantly higher BMD and trabecular bone integrity than control group at weeks 4-6. Radiography, fracture healing score and mean callus area indicated that the combined group revealed better healing processes than the individual groups. Mechanically, bending moment to failure was significantly higher in LIPUS, PTH and combined groups than in control group. These data suggest that the combined treatment of PTH and LIPUS have been shown to accelerate fracture bone healing and enhance bone properties rather than single agent therapy, and may be considered as a treatment remedy for fracture healing in postmenopausal osteoporosis.

INTRODUCTION: Low-intensity pulsed ultrasound (LIPUS) has been known to promote bone healing by nonthermal effects. In recent studies, LIPUS has been shown to reduce inflammation in injured soft tissues. Xerostomia is one of the most common symptoms in Sjögren syndrome (SS). It is caused by a decrease in the quantity or quality of saliva. The successful treatment of xerostomia is still difficult to achieve and often unsatisfactory. The aim of this study is to clarify the therapeutic effects of LIPUS on xerostomia in SS. METHODS: Human salivary gland acinar (NS-SV-AC) and ductal (NS-SV-DC) cells were cultured with or without tumor necrosis factor-α (TNF-α; 10 ng/ml) before LIPUS or sham exposure. The pulsed ultrasound signal was transmitted at a frequency of 1.5 MHz or 3 MHz with a spatial average intensity of 30 mW/cm(2) and a pulse rate of 20 %. Cell number, net fluid secretion rate, and expression of aquaporin 5 (AQP5) and TNF-α were subsequently analyzed. Inhibitory effects of LIPUS on the nuclear factor κB (NF-κB) pathway were determined by Western blot analysis. The effectiveness of LIPUS in recovering salivary secretion was also examined in a MRL/MpJ/lpr/lpr (MRL/lpr) mouse model of SS with autoimmune sialadenitis. RESULTS: TNF-α stimulation of NS-SV-AC and NS-SV-DC cells resulted in a significant decrease in cell number and net fluid secretion rate (p < 0.01), whereas LIPUS treatment abolished them (p < 0.05). The expression changes of AQP5 and TNF-α were also inhibited in LIPUS treatment by blocking the NF-κB pathway. Furthermore, we found that mRNA expression of A20, a negative feedback regulator, was significantly increased by LIPUS treatment after TNF-α or interleukin 1ß stimulation (NS-SV-AC, p < 0.01; NS-SV-DC, p < 0.05). In vivo LIPUS exposure to MRL/lpr mice exhibited a significant increase in both salivary flow and AQP5 expression by reducing inflammation in salivary glands (p < 0.01). CONCLUSIONS: These results suggest that LIPUS upregulates expression of AQP5 and inhibits TNF-α production. Thus, LIPUS may restore secretion by inflamed salivary glands. It may synergistically activate negative feedback of NF-κB signaling in response to inflammatory stimulation. Collectively, LIPUS might be a new strategic therapy for xerostomia in autoimmune sialadenitis with SS.

UNLABELLED: Miniscrew implants (MSIs) are currently used to provide absolute anchorage in orthodontics; however, their initial stability is an issue of concern. Application of low-intensity pulsed ultrasound (LIPUS) can promote bone healing. Therefore, LIPUS application may stimulate bone formation around MSIs and enhance their initial stability. AIM: To investigate the effect of LIPUS exposure on bone formation after implantation of titanium (Ti) and stainless steel (SS) MSIs. METHODS: MSIs made of Ti-6Al-4V and 316L SS were placed on rat tibiae and treated with LIPUS. The bone morphology around MSIs was evaluated by scanning electron microscopy and three-dimensional micro-computed tomography. MC3T3-E1 cells cultured on Ti and SS discs were treated with LIPUS, and the temporary expression of alkaline phosphatase (ALP) was examined. RESULTS: Bone-implant contact increased gradually from day 3 to day 14 after MSI insertion. LIPUS application increased the cortical bone density, cortical bone thickness, and cortical bone rate after implantation of Ti and SS MSIs (P<0.05). LIPUS exposure induced ALP upregulation in MC3T3-E1 cells at day 3 (P<0.05). CONCLUSION: LIPUS enhanced bone formation around Ti and SS MSIs, enhancing the initial stability of MSIs.

Low-intensity pulsed ultrasound (LIPUS) suppresses synovial hyperplasia and synovial cell proliferation characterized for rheumatoid arthritis, but the molecular mechanisms remain unknown. The purpose of this study was to examine the mechanotransduction pathway via the integrin/mitogen-activated protein kinase (MAPK) pathway in LIPUS exposure on the synovial membrane cells. Rabbit knee synovial membrane cell line, HIG-82, was cultured with or without FAK phosphorylation inhibitor, PF-573228. One hour after stimulation with PF-573228, the cells exposed to LIPUS for 20 min or sham exposure. A possible integrin/MAPK pathway was examined by immunofluorescence and Western blotting analysis with antibodies targeting specific phosphorylation sites on intracellular signaling proteins. LIPUS exposure increased phosphorylation of FAK, JNK, ERK, and p38, but the phosphorylation was inhibited by PF-573228. In conclusion, LIPUS exposure might be involved in cell apoptosis and survival of synovial membrane cells via integrin/FAK/MAPK pathway.

Tropomyosins are common heat-stable crustacean allergens. However, their heat stability and their effects on antigenicity have not been clarified. We purified tropomyosin in this study from raw kuruma prawns (Marsupenaeus japonicus) without heat processing. SDS-PAGE of the purified protein showed a band at approximately 35 kDa that cross-reacted with IgE from the serum of a shrimp-allergic patient, identifying it as Pen j 1. The circular dichroism spectrum of native Pen j 1 revealed the common α-helical structure of tropomyosins which easily collapsed upon heating to 80 °C. However, there were no insoluble aggregates after heating, and the protein regained its native CD spectral pattern after cooling to 25 °C. There was no significant difference in total IgG production between mice sensitized with native and heated Pen j 1. These results suggest that heat-denatured Pen j 1 refolded upon cooling and maintained its antigenicity following the heat treatment.

RESUMO

Channelrhodopsin-2 (ChR2), one of the algal light-gated cation channel rhodopsins, contains five peculiar glutamic acid residues in the N-terminal region corresponding to the second to third transmembrane helices. Here we made systematic mutations of these polar amino acid residues of ChR2 into nonpolar alanine, and evaluated their photocurrent properties. Amongst them, the photocurrent generated by the E97A mutation, ChR2(E97A), was much smaller than expected from its expression. The ChR2(E97A) photocurrent was similar to wild-type ChR2 in the kinetic profiles, the reversal potential and the dependency to the light power density. Our results suggest that the residue E97 is one of the molecular determinants involved in the ion flux regulation.
