BACKGROUND/AIMS: Transient apical breakdown (TAB) is a phenomenon that indicates temporary apical periodontal destruction and root resorption after tooth luxation injuries, followed by the healing process of the dental pulp. Andreasen in 1986, reported that TAB was seen in approximately 4.2% of all luxation injuries. However, there have been no reports thereafter on the types and frequency of the luxation traumatic injuries in which TAB occurs. Therefore, this retrospective study was aimed to assess the incidence and pathogenesis of dental trauma-induced TAB and to suggest a possible mechanism of subsequent healing based on a series of cases. METHODS: Data analysis included mature teeth (n = 56) of 49 patients aged 9-30 years who presented in a private dental office over a period of 10 years (2012-2022) to investigate the incidence and healing sequala of TAB. RESULTS: TAB was observed in 43.8% of subluxation, 62.5% of extrusive luxation, and 75% of lateral luxation injuries. The average age of patients who developed TAB was 14.5 years, ranging from 9 to 28 years old. CONCLUSIONS: TAB can be expected in many cases of luxation injuries with minimal dislocation. Therefore, mild injuries (subluxation, extrusion, and lateral luxation), may exhibit spontaneous healing, recovery of dark discoloration of the crown, disappearance of a periapical radiolucent lesion and return to normal response to EPT as long as 12 months after the traumatic injury. Thus, a decision to perform endodontic treatment in these cases might be postponed until clear evidence for an infection exists.

Tooth loss is an adverse consequence of oral diseases and traumatic dental injuries. Although several treatment options exist to treat a missing or hopeless tooth, especially in young individuals, most of the existing alternatives (such as orthodontic treatment, removable or fixed partial dentures, and dental implants) impose a challenge in children due to the nature of the developing jaw bones. Tooth autotransplantation is the replacement of a tooth with another functional tooth within the patient's dentition. Autotransplantation can serve as a promising treatment alternative in cases of tooth loss not only in children and adolescents but also in adult patients. Autotransplantation is a technique-sensitive procedure, that requires proper and thorough planning as well as careful and knowledgeable execution in order to improve the chances for long-term success and survival of the transplanted tooth. Thus, the aim of this article was to provide a step-by-step clinical guide, emphasizing key points and highlights for planning and performing a successful autotransplantation procedure. Autotransplantation is a very predictable treatment modality that can serve as a viable option to replace a missing tooth, especially in young patients. Proper planning and careful execution of the procedure are important to achieve optimal long-term results.

The aim of autotransplantation of teeth (ATT) is to replace a lost tooth with a functional tooth within the same patient. Although it has recently become more of a recognized and viable treatment approach in dentistry, the long-term outcomes are still not well-documented. The principal author (M.T.) has performed more than 1000 ATTs for reasons such as treating missing teeth, deep caries, poor endodontic results, and periodontitis over the past 30 years in private practice. During the course of private practice, 2 separate analyses were performed on a total of 319 cases with follow-up ranging from 2-26 years. The results showed a tendency toward higher success rates in younger patients; the success rate was highest in ATTs performed on immature teeth (about 95%), about 90% in patients younger than 30 years of age and approximately 80% in patients older than 30. The failures were most often caused by replacement resorption (ie, ankylosis-related resorption). The purpose of this case series was to show successful long-term outcomes of ATT as well as to provide clinical insights and describe tendencies noted over the course of 30 years of performing ATTs.

This is a case series of 3 immature permanent teeth with dens evaginatus, partial pulp necrosis, and apical periodontitis that were treated by calcium hydroxide pulpotomy. In all cases, follow-up examinations revealed that the radicular pulp vitality was preserved, periapical lesions were healed, and the root canal walls were narrowed by hard tissue deposition. Repair of the apical root differed from case to case; further apical root development occurred in 2 cases, but apical closure was apparent in all of them. This case series report opens a perspective on the use of pulpotomy as an alternative option to treat immature teeth with partial vital pulp tissue and apical periodontitis.

The purpose of this report was to describe the morphological changes in the alveolar bone after delayed replantation of avulsed teeth using three dimensional cone-beam computed tomography in 11 during the time period 2003-2012. The radiographic observations revealed the following: Delayed replantation results in ankylosis-related replacement root resorption; the resorption is delayed or arrested around the cervical area superior to the alveolar crest. The buccal bone is reduced in thickness but not the palatal bone. The buccal bone resorption of the alveolar crest progresses approximately to the root canal space of the ankylosed root. Delayed replantation does not completely maintain the bone volume. The buccal profile of alveolar bone in the maxillary anterior region is depending on teeth with viable periodontal ligament.

Avulsion of permanent teeth is the most serious of all dental injuries. The prognosis depends on the measures taken at the place of accident or the time immediately after the avulsion. Replantation is the treatment of choice, but cannot always be carried out immediately. An appropriate emergency management and treatment plan is important for a good prognosis.
