Introduction: There is a critical need to address concerns surrounding the potential impact of bleaching gels specifically on the tooth substrate. Therefore, this laboratory investigation aimed to assess the impact of a hydrogen peroxide (HP)-free bleaching (HiSmileTM) in comparison to an HP-based bleaching (Opalescence RegularTM) on the surface and mechanical characteristics of tooth substrate. Methods: Sixty sound human premolar teeth were sectioned to produce dentin fragments and divided into two primary groups based on the bleaching agent used. Each group was subdivided into three subgroups (n = 10) per distinct bleaching regimens: (T1) fragments underwent a 7-day immersion in distilled water at 37°C without any bleaching treatment, (T2) fragments underwent a 7-day immersion in distilled water at 37°C, with the application of bleaching gel occurring on the seventh day for 10 minutes, and (T3) fragments underwent a bleaching regimen for seven consecutive days, each session lasting for 10 minutes. The initial and final evaluations of surface roughness, nano-hardness, and elastic modulus were performed. Following the bleaching regimens of T3, a composite stub was fabricated on the dentin fragments for the shear bond strength (SBS) test. Statistical testing was accomplished using the analysis of variance (P < 0.05). Results: HP-based bleaching gel showed significant differences between measurement intervals in surface roughness, elastic modulus, and SBS parameters (P < 0.05). In contrast, HP-free bleaching gel showed insignificant differences within the group (P > 0.05). The SBS between dentin-composite was significantly affected with the use of HP-based bleaching gel, while HP-free bleaching gel showed insignificant difference between measurement intervals. The qualitative validation of the treatment's impact was further demonstrated using the scanning electron microscopy. Conclusion: The findings suggest that the bonding stability of composite restorations to dentin may be compromised after bleaching with an HP-based gel, whereas immediate bonding procedures can be safely conducted following the application of an HP-free bleaching gel.

OBJECTIVES: This study aimed to evaluate the effect of whitening toothpastes on the surface roughness and colour change of CAD-CAM materials. MATERIALS AND METHODS: A total of 96 samples (2 × 10 × 12 mm3) were prepared from Cerasmart (CS) and Celtra Duo blocks. Celtra Duo samples were divided into two groups. One group was fired with glaze paste (CDG) and the other was not treated (CD). All groups were then divided into 4 subgroups (n = 8). The groups were brushed with conventional (Colgate™ Max Fresh), silica (Opalescence™), charcoal (Curaprox™ Black in White) and blue covarine (Signal™ White Now) toothpastes for 30,000 brushing cycles. The initial and final surface roughness values were measured with contact profilometer and a dental spectrophotometer used for obtaining colour values. One sample from each brushed group was analyzed using a scanning electron microscope. Data was analyzed with Kruskal-Wallis and Mann-Whitney tests (p = 0.05). RESULTS: The surface roughness of CS samples brushed with Opalescence™ and Curaprox™ was significantly higher than CD and CDG. Surface roughness change values of CS samples brushed with Curaprox™ were significantly higher than the CD and CDG. Curaprox™ brushed samples showed significant difference in colour change values for all materials. CONCLUSIONS: Brushing increases the surface roughness of CAD-CAM ceramic materials. The roughness of resin-based materials is higher than zirconia-reinforced lithium silicates. Silica-contained toothpastes may cause discoloration of nanoceramic and zirconia-reinforced glass ceramic restorations. CLINICAL RELEVANCE: It should be clinically considered that whitening toothpastes may cause roughness in ceramic materials and change the desired color.

OBJECTIVE: There are many suitable strategies for addressing caries, which is an ongoing worldwide problem. Although white spot lesions (WSLs) can be either remineralized naturally or treated with non- or micro-invasive strategies, their whitish and opaque appearance may persist. To evaluate the effects of tooth bleaching as a complement to fluoride-enhanced remineralization or resin infiltration in masking WSLs, as well as in enamel surface roughness relative to that of the adjacent enamel. METHODOLOGY: Flattened rectangular bovine enamel fragments (6×3×~2.9 mm length, width and thickness) were divided into six groups (L/N, F/N, F.BL/BL, I/N, I.BL/BL, N/N; n=15). Treatments applied to the 3×3 mm left half included: L (Lesion) - WSL simulation with 50 mM acetate buffer, 96 hours, 37ºC; F (Fluoride) - WSL treatment with 2% NaF neutral gel, 1x/week, 8 weeks; I (Infiltration) - WSL treatment with H3PO4 37%/10 s; Icon®-Dry/30 s; Icon®-Infiltrant/3 min+1 min; N (Nothing) - sound enamel/control. Treatments applied to both halves after F and I included: BL (Bleaching) - Opalescence Boost 40%, 3×/20 min each; N (Nothing) - control. The differences in color (ΔE00, ΔL, Δa, Δb) and surface roughness (ΔRa) between the left and right halves were measured. Kruskal-Wallis/post-hoc tests were applied to ΔE00, ΔL, Δa and ΔRa, and 1-way ANOVA/Tukey tests to Δb (α=0.05). RESULTS: The factor under study significantly influenced ΔE00 (p=0.0001), ΔL (p=0.0024), Δb (p=0.0015), and ΔRa (p<0.001), but not Δa (p=0.1592). Both fluoride-enhanced remineralization and resin infiltration were able to mask WSL, regardless of subsequent bleaching. However, when bleaching was performed, ΔE00 median values did not exceed the acceptability threshold for color difference. Only resin infiltration reduced ΔRa between WSL and the adjacent enamel. CONCLUSIONS: Both remineralization and infiltration, particularly if complemented by bleaching, fostered satisfactory esthetic results. Only infiltration without bleaching led to really good results in surface roughness.

Tooth discoloration and the breeding of oral microorganisms pose threats to both one's aesthetic appearance and oral health. Clinical whitening agents based on H2O2 with high concentrations are effective in tooth whitening and bacterial elimination but may also cause enamel demineralization, gingival irritation, or cytotoxicity, necessitating professional supervision. Herein, leveraging sono-catalysis effects, a nondestructive and convenient tooth whitening strategy was developed, utilizing oxygen vacancies (OVs)-enriched mesoporous TiO2 nanospheres. The introduction of OVs leads to TiO2 bandgap narrowing, boosting the generation of reactive oxygen species (ROS) by TiO2 under ultrasound treatment. Additionally, through the chemocatalysis effect, the ROS yield can be further augmented by employing OVs-enriched TiO2 in conjunction with an extremely low concentration of H2O2 (1%) during ultrasound treatment. Hence, under ultrasound treatment simulating daily tooth brushing using an electronic toothbrush, the combination of OVs-enriched TiO2 and 1% H2O2 proves to be effective in whitening teeth stained by tea, coffee, and mix juice. Furthermore, the combination of OVs-enriched TiO2 and 1% H2O2 demonstrates potent bacterial-killing and biofilm-eradicating effects under ultrasound treatment within an extremely short duration (5 min). Additionally, given the mesoporous structure, curcumin, serving as an anti-inflammatory agent, can be efficiently loaded into OVs-enriched TiO2 and then controllably released through ultrasound treatment. The curcumin-loaded TiO2 facilitates the transition of macrophages to the anti-inflammatory M2 phenotype, potentially alleviating oral inflammation induced by bacterial infection without showing any biotoxicity. The OVs-enriched TiO2 based sono-catalysis tooth whitening procedure provides the convenience of whitening teeth during daily brushing without requiring professional supervision.

There has been an increased demand for dental bleaching globally irrespective of age and gender. Main drawbacks associated with conventional tooth bleaching agents have been compromised strength and mineral-content of tooth enamel which results in sensitivity, discomfort, roughness, and structure loss of human teeth. Currently, nanoparticles synthesized by green synthesis have gained popularity especially in medical and dental applications because of their versatile and beneficial nano-scaled features. Titanium dioxide nanoparticles (TiO2Nps) in this study were prepared from green ecofriendly source using the aloe vera plant extract and were then characterized via dynamic light scattering (DLS), scanning electron microscope (SEM), X-ray diffraction spectroscopy (XRD), and energy dispersive X-ray (EDX), for size, shape, composition and true-phase. These TiO2 Nps were incorporated in commercial bleaching gel containing hydrogen peroxide to form a novel TiO2-bleaching gel which was used to bleach extracted anterior teeth belonging to four different age groups: 20-29 years, 30-39 years, 40-49 years and ≥50 years. These teeth were investigated for micro-hardness (Vickers microhardness tester) and mineral-content (EDX spectroscopy) including sodium, magnesium, phosphorus, calcium in an in-vitro environment both before and after bleaching. Results revealed that TiO2 Nps prepared by aloe vera plant were nanos-sized of about 37.91-49 nm, spherical shape, true anatase phase with pure titanium and oxygen in their composition. The values of Vickers micro-hardness and mineral-content (Na, Mg, P, Ca) of enamel specimens belonging to different age groups enhanced in a linear pattern before bleaching with the increase in age (p value < 0.05). There was negligible reduction observed in Vickers micro-hardness and mineral-content elements (Na, Mg, P, Ca) of all enamel specimens belonging to different ages after the bleaching (p value > 0.05). The novel TiO2-bleaching gel prepared was effective enough in preventing the declination in Vickers micro-hardness strength and mineral-content of all the enamel specimens belonging to different age groups even after the bleaching procedure which makes it a promising biomaterial.

PURPOSE: To compare the color dimensions, color discrepancies (ΔE00), and surface roughness of milled materials before and after application of a bleaching agent. MATERIALS AND METHODS: A total of 10 extracted molars were obtained. Each tooth was cut in transverse sections to create disks (3-mm thick, 10-mm diameter; control group). Disk specimens of eight materials (n = 10 per group) were fabricated: polymethyl methacrylate (PMMA) interim material (PMMA-Telio group), two resin nanoceramics (RNC-Ultimate group and RNC-Cerasmart group), two hybrid ceramics (HC-Shofu group and HC-Enamic group), lithium disilicate (LD-Emax group), zirconia-reinforced glass ceramic (ZGC-Suprinity group), and zirconia (Zr-InCeram group). Color measurements were obtained using a spectrophotometer before and after applying 35% hydrogen peroxide-based bleaching agent. Pre- and postbleaching surface roughness (Sa) analyses were completed using a profilometer. RESULTS: Significant L*, a*, b*, and ΔE00 value differences were found (P < .05). Color discrepancies (ΔE00) ranged from 0.30 ± 0.14 to 4.82 ± 0.10. The highest color discrepancies were measured on the PMMA-Telio group, while the lowest color discrepancies were computed for ZGC-Suprinity, RNCUltimate, and RNC-Cerasmart. Significant surface roughness differences were found (P < .05). The largest increase of surface roughness values between the pre- and postbleaching measurements was obtained in the PMMA-Telio group with a mean ΔSa value of 4.73 ± 3.02, while the largest decrease of surface roughness values between the pre- and postbleaching measurements was obtained in the Zr-InCeram group with a mean ΔSa value of -1.58 ± 0.10. CONCLUSIONS: The milled materials showed significant pre- and postbleaching color and surface roughness discrepancies.

BACKGROUND: To predict the long-term performance of restorative materials in the oral environment, it is important to evaluate their resistance to chemical and mechanical degradation and to know the toxic potential of the type and amount of ions eluted from the filling material. In this study, home bleaching was applied to dental materials with different contents and it was aimed to determine the type and amount of ions released from these materials. METHODS: In this study, amalgam, posterior composite resin, anterior composite resin, bulk fill composite resin, indirect composite resin, hybrid ceramic and all-ceramic were used as restorative materials. 10 specimens of each material were prepared according to the manufacturer's instructions. Each material group was divided into two subgroups as the bleached group and the control group. After bleaching, all specimens were stored in 1 ml of 75% ethanol/water solution. Solutions were renewed after 1, 14 and 28 days. The type and amount of ions released from the materials were determined using Inductively Coupled Plasma-Mass Spectrometer (ICP-MS). Data were analyzed using the Friedman, Wilcoxon Signed Ranks, and Mann-Whitney U tests (α = 0.05). RESULTS: It was determined that the amount of ions release from the restorative materials decreased over time (p < 0.05). According to the results of the Mann-Whitney U test, there was no difference between the bleaching and control groups in most of the restorative materials (p > 0.05). CONCLUSION: Within the limits of this study, home bleaching system does not have a significant effect on ion release from restorative materials.

The study aimed to investigate the influence of H2O2-based and H2O2-free in-office bleaching on the surface and mechanical attributes of CAD/CAM composite blocks. CAD/CAM composite blocks from five different composite materials (CC1, CC2, CC3, CC4, and CC5) were randomly divided into two groups according to bleaching application (H2O2-based and H2O2-free). The surface topography, morphology, nanohardness, elastic modulus, flexural strength, and fracture toughness were measured. A paired and unpaired sample t-tests gauged the effect of pre- and post-bleaching on the substrates. The estimated mean differences (before-after bleaching) suggested an increase in surface roughness for two materials CC2 and CC4, and a significant decrease in nanohardness for material CC4 and in elastic modulus for materials CC2 and CC4 with H2O2-based bleaching, whereas H2O2-free bleaching resulted in changes compatible with no change in these properties. Flexural strength and fracture toughness showed no evidence of changes, irrespective of the bleaching gel used. Scanning electron microscopic analysis revealed erosive effects and micropore formation due to H2O2-based bleaching. H2O2-based bleaching deteriorates the surface of CAD/CAM composite materials while H2O2-free bleaching gel had an insignificant effect on both surface and bulk properties. The clinician should carefully evaluate the potential effects of H2O2-based bleaching on the surface properties of CAD/CAM composites.

OBJECTIVES: To evaluate in vitro the effects of nano-sized sodium trimetaphosphate (TMPnano) and sodium fluoride (F) added to a 17.5 % hydrogen peroxide (H2O2) bleaching gel on the color change, enamel mechanical and morphological properties, and H2O2 transamelodentinal diffusion. MATERIALS AND METHODS: Bovine enamel/dentin discs (n = 180) were divided according to the bleaching gel: 17.5 % H2O2 (17.5 % HP); 17.5 % H2O2 + 0.1 % F (HP/F); 17.5 % H2O2 + 1 % TMPnano (HP/TMPnano); 17.5 % H2O2 + 0.1 % F + 1 % TMPnano (HP/F/TMPnano) and 35 % H2O2 (35 % HP). The gels were applied for 40 min on three sessions, each session spaced 7 days apart. The total color change (ΔE*ab) according to the Commission Internationale de l'Eclairage (CIE) L*a*b* color change measured by CIEDE2000 (ΔE00), whitening index (ΔWID), surface hardness (SH), surface roughness (Ra), cross-sectional hardness (ΔKHN), and transamelodentinal diffusion were assessed. Enamel surfaces were examined using Scanning Electron Microscopy (SEM) and Energy Dispersive X-ray (EDS) analysis. The data were analyzed using ANOVA, followed by the Student-Newman-Keuls test (p < 0.05). RESULTS: ΔE*ab, ΔE00, and ΔWID values were comparable among the gels that produced a bleaching effect post-treatment (p < 0.001). The HP/F/TMPnano group exhibited lower mineral loss (SH and ΔKHN), Ra, and H2O2 diffusion compared to the 17.5 % HP and 35 % HP groups, which had the highest values (p < 0.001). SEM/EDS analysis revealed surface changes in all bleached groups, though these changes were less pronounced with F/TMPnano. CONCLUSIONS: The 17.5 % HP gel containing F/TMPnano maintains the bleaching effect while reducing enamel demineralization, roughness, H2O2 diffusion, and enamel morphological changes. CLINICAL RELEVANCE: Low-Concentration H2O2 bleaching gel containing F/TMPnano can be used as a novel approach to enhance safety and maintain the performance of aesthetic effects.

OBJECTIVE: This study aims to synthesize, characterize, and assess the penetration of hydrogen peroxide (HP), color change (CC), and surface morphology changes after the application of two distinct nano-hydroxyapatite (nano-HAp). METHODS: Two nano-HAp were previously synthesized by co-precipitation: one with rod-shaped particles (RS) and the other with spherical-shaped particles (SS). The surface charge of the nano-HAp particles was determined at varying pH levels and characterized by X-ray diffraction patterns and Fourier transform infrared spectroscopy. The morphology of the samples was assessed using scanning electron microscopy (SEM). The nano-HAp particles were applied before the dental bleaching procedure. Forty teeth were divided into four groups (n = 10) according to the bleaching treatment: no treatment, bleaching with 35 % HP only, RS application and bleaching with 35 % HP, and SS application and bleaching with 35 % HP. HP concentration (µg mL-1) was measured using UV-Vis, while CC was evaluated with a digital spectrophotometer (ΔEab, ΔE00 and WID). Additionally, four teeth from each group were selected for SEM analysis. Statistical analysis encompassed one-way ANOVA, Tukey's, and Dunnet's tests. RESULTS: RS and SS were successfully synthesized by coprecipitation, primarily differing in pH during synthesis. Both variations of nano-HAp morphology significantly reduced HP diffusion into the pulp chamber (p < 0.001). Regarding enamel morphology, groups analyzed post dental bleaching exhibited greater HAp deposition on the enamel surface. Notably, this deposition did not impede CC. SIGNIFICANCE: The utilization of different nano-HAp morphologies prior to dental bleaching appears to be a promising strategy for mitigating adverse effects associated with dental bleaching procedures.

This study evaluated the changes in surface properties of three resin-based restorative materials after two laser-assisted, in-office tooth bleaching protocols using erbium, chromium: yttrium-scandium-gallium-garnet (Er,Cr:YSGG) or diode (980 nm) lasers. A nanohybrid composite (Enamel Plus HRi), a Bis-GMA-free composite (Enamel Plus HRi Bio Function), and a resin-matrix CAD-CAM ceramic (Shofu Block HC) were tested. Forty specimens for each material were prepared and divided into four groups (n = 10/group). The control specimens did not undergo any bleaching treatment, whereas group 2 received bleaching with 40% hydrogen peroxide (H2O2), while groups 3 and 4 underwent the same bleaching procedure with the use of diode (980 nm) or Er,Cr:YSGG lasers, respectively. Surface microhardness and roughness measurements were conducted using a Vickers tester and an optical profilometer. Microhardness was lower in bleached specimens, with the nanohybrid composite exhibiting the largest difference from the no bleaching group. For the Bis-GMA-free composite the microhardness difference between no bleaching and laser-assisted bleaching were smaller than seen for the conventional bleaching technique. Surface roughness was higher in bleached specimens, with nanohybrid composite showing the largest differences from the control specimens. The examined laser-assisted tooth bleaching protocols were found not to impact surface microhardness and roughness of the tested resin-based specimens and they are deemed suitable for clinical use.

The increasing desire for aesthetically pleasing teeth has resulted in the widespread use of tooth whitening treatments. Clinical tooth whitening products currently rely on hydrogen peroxide formulations to degrade dental pigments through oxidative processes. However, they usually cause side effects such as tooth sensitivity and gingival irritation due to the use of high concentrations of hydrogen peroxide or long-time contact. In recent years, various novel materials and reaction patterns have been developed to tackle the issues related to H2O2-based tooth whitening. These can be broadly classified as advanced oxidation processes (AOPs). AOPs generate free radicals that have potent oxidizing properties, which can thereby increase the oxidation power and/or reduce the exposure time and can probably minimize the side effects of tooth bleaching. While there have been several reviews on clinical tooth whitening and the application of novel nanomaterials, a review based on the concept of AOPs in tooth bleaching application has not yet been conducted. This review describes the common types and mechanisms of AOPs, summarizes the latest research progress of new tooth bleaching materials based on AOPs, and proposes a model for tooth bleaching and a rate control step at the molecular level. The paper also reviews the shortcomings and suggests future development directions.

AIM: To evaluate and compare the effect of various antioxidants on the shear bond strength of composite resin to bleached enamel. MATERIALS AND METHODS: Labial enamel surfaces of 90 extracted human maxillary central incisors were randomly divided into five groups based on the antioxidant used as follows: Group I ( n = 20): Bleaching with 40% hydrogen peroxide gel for 10 min, without the use of an antioxidant; Group II ( n = 20): Bleaching followed by the use of 10% sodium ascorbate solution; Group III ( n = 20): Bleaching followed by the use of 5% grape seed extract solution; Group IV ( n =20): Bleaching followed by the use of 5% pine bark extract solution; and Group V ( n =20): Control group in which no bleaching was done. Groups I-IV were further subdivided into two subgroups of 10 teeth each, whether the antioxidant application and composite build-up was done immediately (subgroup A) or after 2 weeks of postbleaching (subgroup B). Shear bond strength of the specimens was tested under a universal testing machine. RESULTS: One-way analysis of variance followed by Mann-Tukey posthoc multiple comparison test was used to analyse the data. The level of significance was established at P < 0.05. CONCLUSION: Within the limitations of this study, it was observed that the use of antioxidants effectively reversed the compromised bond strength of bleached enamel. Among the antioxidants, 5% pine bark extract application after bleaching showed better bond strength.

OBJECTIVES: This study assessed the effects of 15% and 20% carbamide peroxide (CP) on color, surface roughness, and hardness of computer-aided design/computer-aided manufacturing (CAD/CAM) dental ceramics. MATERIALS AND METHODS: This in vitro study was conducted on 120 Vita Mark II, Celtra Duo, and Suprinity CAD/CAM ceramic specimens. The ceramic specimens in each group (n = 40) were randomly assigned to two subgroups (n = 20) for polishing and glazing, and their baseline color, surface roughness (Ra), and hardness were assessed. In each subgroup, half of the specimens were exposed to 15% CP, while the other half were exposed to 20% CP. Their color change (ΔE), surface roughness, and hardness were then measured again. Surface roughness, hardness, and color were analyzed sequentially by profilometer, Vickers hardness tester, and spectrophotometer, respectively. Data were analyzed by repeated measures ANOVA, one-way ANOVA, and post hoc Bonferroni test (α = 0.05). RESULTS: The surface roughness of all groups significantly increased after bleaching treatment (p < 0.05). Surface hardness of all groups decreased after bleaching treatment, but this reduction was only significant in Vita Mark II subgroups (glazed, polished, 15%, and 20% CP). The ΔE was not clinically and visually perceivable in any group. CONCLUSION: The present results revealed that concentration of CP and type of surface treatment affected the surface properties of CAD/CAM ceramics. Type of surface treatment only affected the surface hardness of Vita Mark II ceramics (p < 0.05). Concentration of CP had a significant effect only on polished Vita Mark II.

BACKGROUND: This study aimed to investigate the effect of whitening toothpastes on the color stability and surface roughness of resin composites stained with coffee and cigarette smoke. METHODS: Seventy-two disk-shaped specimens (6 × 2 mm) of suprananohybrid resin composite were randomly divided into two groups and exposed to coffee and cigarette smoke (n = 36). After staining, the samples randomly divided into four groups according to whitening toothpastes and were brushed for 4 min: Opalescence Whitening (OW); Colgate Optic White (COW); Curaprox Black is White (CPX) and, distilled water (control) (n = 9). Color was measured with spectrophotometer at the initial, after staining, and after brushing, and surface roughness was measured with profilometer at the initial and after brushing. A surface morphology analysis was examined using scanning electron microscopy and atomic force microscopy. The obtained data were statistically analyzed. (p < 0.05). RESULTS: Cigarette smoke caused a significantly higher color change than coffee in the resin composite (p < 0.05). Brushing with hydrogen peroxide and silica-containing whitening toothpaste showed significant differences in color change (p < 0.05). The lowest whitening effect was found in activated charcoal-containing toothpaste. While all toothpastes increased the degree of surface roughness of resin composites, the highest roughness was caused by whitening toothpastes containing activated charcoal. (p < 0.05). CONCLUSIONS: The color stability and surface properties of resin composites can be affected by brushing them with whitening toothpaste. The utilization of whitening toothpaste containing hydrogen peroxide can be considered a safe method for increasing the whiteness of discolored resin composites.

Aims - to compare the color changes, the surface roughness and morphology of the enamel bleached with two different bleaching solutions (chemical and laser activated), preceded or not with acid etching. Thirty teeth of bovine prepared and haphazardly assigned to 2 groups (n=15) depending on bleaching technique. Each group subdivided to 3 subgroup (n=5) consistent with acid etching by 37% phosphoric acid. Atomic force microscopy and VITA easy shade spectrophotometer were performed twice for all the specimens before and after bleaching. ANOVA, the Paired sample t-test, and the independent sample t-test used for statistical analysis. As for the color changes, the groups that were bleached by the chemical method, the difference among the three subgroups was statistically significant. This also applies to the groups bleached with the laser method. When comparing the results of the chemical bleaching subgroups with the laser bleaching ones, the difference was not significant. Roughness results showed significant differences between certain subgroups and non-significant differences among others. However, the difference was statistically significant between the chemical and laser groups, laser technique resulted in less surface roughness than the chemical one. Acid etching before bleaching produced better colour change in both the chemical and laser assisted bleaching. In chemical bleaching, surface roughness was higher when acid etching was used. This was also true for laser bleaching technique. In general, laser assisted bleaching produced less surface roughness than chemical bleaching.

BACKGROUND: To evaluate the decomposition rate of active hydrogen peroxide (HP) and bleaching efficacy during in-office bleaching using high-concentration HP gels with different pHs. METHOD: A randomized, parallel, double-blind controlled trial was conducted with 40 volunteers randomized into four groups (pH 5.4; pH 7.0; pH 7.7, and pH 8.0). During the first session in-office bleaching, approximately 0.01 g of the gel was collected and titrated with potassium permanganate to obtain the concentration of active HP and pH values were measured using an electrode. Bleaching efficacy was assessed using a spectrophotometer [∆Eab, ∆E00, and WID], Vita Classical and Vita Bleachedguide scales [∆SGU]. The decomposition rate of HP concentration and pH values change were calculated using ANOVA one-way. The bleaching efficacy was assessed using two-way repeated measures ANOVA. Tukey's test was applied as a post-hoc test (p < 0.05). RESULTS: All gels experienced decreasing HP concentration over time. pH 5.4 gel showed greatest reduction after 50 min (p < 0.001). pH 8.0 and 7.7 gels remained stable; pH 5.4 remained acidic, while pH 7.0 turned acidic (p < 0.001). No significant difference in bleaching degree was observed among gels. They all showed a similar and clinically important color change after two clinical sessions, remained stable 1-month post-treatment (p > 0.05). CONCLUSIONS: All bleaching gels kept at least 70% of their HP content after 50 min, suggesting that there is a surplus of HP. They provided similar whitening efficacy 1-month after bleaching. PRACTICAL IMPLICATIONS: It is possible that lower HP concentrations may be equally effective in achieving desired results while reducing the potential for side effects. OBJECTIVE: Evaluate the penetration of hydrogen peroxide (HP) into the pulp chamber, bleaching efficacy (BE) and amount of gel expended during in-office bleaching using an applicator brush tip and conventional tip from different commercial brands. MATERIALS AND METHODS: 104 human premolars were randomly distributed into thirteen groups (n = 8) according to the commercial brand: DSP White Clinic 35 % Calcium (DW), Nano White 35 % (NW), Total Blanc One-Step 35 % (TS), Whiteness HP Blue 35 % (WB), Potenza Bianco Pro SS 38 % (PB), Opalescence XTra Boost 40 % (OB), no bleaching (negative control), and application method: applicator brush tip and conventional tip for all groups. Initial HP concentration (%) was determined via titration and pH was measured with digital pH meter. Concentration (µg/mL) of HP into the pulp chamber was measured using UV-Vis spectrophotometry, the BE (ΔE*ab, ΔE00 and ΔWID) was evaluated with a digital spectrophotometer, and the amount of gel expended was evaluated using a precision analytical digital balance. Statistical analysis included two-way ANOVA, Tukey's, and Dunnett's test. Comparison between HP into the pulp chamber vs BE was performed with Person's correlation (α = 0.05). RESULTS: Brush tip demonstrated a low amount of HP in the pulp chamber compared to the conventional method for all bleaching gels (p < 0.0003), as well as lower amount of gel expended (p < 0.002). The brush tip did not result in a significant difference in BE compared to the conventional (p > 0.05). No correlations were found between both factors (p > 0.05). CONCLUSION: Brush tip showed lower penetration of HP in the pulp chamber and a reduced volume of spent gel when compared to the conventional tip, for all commercial brands. CLINICAL RELEVANCE: Brush tip is recommended for bleaching gels in an attachable syringe due to its ability to reduce the penetration of HP into the pulp chamber and minimize the amount of bleaching gel used.

OBJECTIVES: To evaluate the effect of whitening toothpastes with different hydrogen peroxide (HP) concentrations on HP permeability, color change, and physicochemical properties, compared to at-home bleaching treatment. MATERIALS AND METHODS: Forty-nine premolars were randomized into seven groups (n = 7): untreated (control); at-home bleaching with 10% carbamide peroxide gel (AH; 10% CP) with 14 and 28 applications of 180 min each (AH [14 × 180 min] and AH [28 × 180 min]); three whitening toothpastes (3% HP; 4% HP and 5% HP) and 10% CP brushed 28 times for 90 s each (TB [28 × 90 s]). HP permeability was measured using a UV-VIS spectrophotometer and color change by a digital spectrophotometer (ΔEab, ΔE00, and ΔWID). Initial concentration, pH, and viscosity were measured through titration, digital pH meter, and rheometer, respectively. Statistical analysis included one-way ANOVA, Tukey's test, and Dunnett's test (α = 0.05). RESULTS: 4% HP group showed acidic pH, the lowest viscosity and the highest HP concentration into the pulp chamber (p < 0.05). The 10% CP groups had lower HP in the pulp chamber and greater color change than other groups (p < 0.05), except the 5% HP group in ΔEab and ΔE00. For ΔWID, the 10% CP AH groups showed greater whitening than other groups (p < 0.05). CONCLUSIONS: Whitening toothpaste with up to 5% HP resulted in higher HP permeability and less color change compared to 10% CP. Higher HP commercial concentrations in toothpaste increased whitening effect; however, acidic pH toothpastes exhibited greater HP permeability. CLINICAL RELEVANCE: Whitening toothpastes with high hydrogen peroxide concentrations were less effective than at-home bleaching, resulting in less color change and greater permeability of hydrogen peroxide, potentially increasing the risk of tooth sensitivity.

OBJECTIVE: To evaluate the effects of different polishing techniques and coffee staining on the color stability of four commercially available bleach-shade composite resins, namely microhybrid, nanohybrid, nanofilled, and injectable nanohybrids. MATERIAL AND METHODS: A total of 224 discs (8 mm diameter and 2 mm thickness) were fabricated from four different bleach-shade composite resins, namely microhybrid (Gradia Direct Anterior), nanohybrid (Palfique LX5), nanofilled (Filtek Universal), and injectable nanohybrid (flowable G-aenial universal injectable). The composite resin groups were polished via four techniques: no polishing, 4-step polishing using aluminum oxide discs, 3-step polishing using silicon rubber diamond discs, and one-step polishing. Half of each group was immersed in water, while the other half was immersed in coffee for 12 days (n = 7). Colors were measured using a clinical spectrophotometer, and color differences were calculated (ΔE). The results were analyzed statistically. RESULTS: The alterations in color were significantly influenced by the techniques employed for finishing and polishing techniques, composite resin type, and degree of coffee staining. Regardless of the polishing technique and storage medium, different material types showed a significant color change (ΔE) at P < 0.001. Filtek exhibited the most significant color change, followed by Gradia and Palfique, with no significant differences between them. In addition, Different polishing techniques resulted in significant color changes (P < 0.001). The highest degree of color change was seen in the no-polishing group, followed by the 4-step and 1-step polishing groups, with negligible differences between each other. Also, Storage media had a significant effect on ΔE values. CONCLUSION: Appropriate finishing and polishing procedures can improve the color stability of bleach-shaded composite resins. Coffee has a deleterious effect on color; however, injectable flowable nanohybrid composites are more resistant to staining.
