ABSTRACT

Abstract People's knowledge, attitudes, and practices related to health are built throughout life, particularly during childhood and adolescence, and they reflect the practices of their family and social circle. The aim of this study was to identify the levels of knowledge, attitudes, and practices related to oral health of first-year university students at the Paraíso Campus of the University of Costa Rica. This was done through a questionnaire of closed questions that would allow oral health educational strategies to be established in the future for this population group. For this purpose, a cross-sectional descriptive study was conducted, where 70 first-year students of the Paraíso Campus participated in the first semester of the year 2019. For data collection, a survey of 22 closed questions on knowledge, attitudes, and practices about oral health was applied, and a score on oral health knowledge was subsequently performed, based on the answers to the questions. According to the results, the level of knowledge shown about oral health is low, since the average knowledge score (7.02 points) was less than half of the maximum score (17 points). The greatest deficiency in knowledge was evidenced in topics, such as, dental biofilm, gingivitis, use of fluorides, and the role of saliva. In addition, several practices that affect the condition of the oral cavity were identified, such as harmful habits, inadequate nutrition, and poor oral hygiene habits. In this study, a general lack of knowledge was identified regarding basic concepts of oral health in young university students, who are not clear about the role of some protective or risk factors related to the main oral diseases.


Resumen Los conocimientos, actitudes y prácticas relacionados a la salud en las personas se construyen a través de la vida, particularmente durante la infancia y la adolescencia y reflejan las prácticas de su círculo familiar y social. El objetivo de este estudio fue identificar los niveles de conocimiento, las actitudes y las prácticas relacionadas a la salud bucodental en estudiantes universitarios de primer ingreso en el Recinto de Paraíso de la Universidad de Costa Rica, mediante un cuestionario de preguntas cerradas que permitirá a futuro establecer estrategias educativas en salud bucal para este grupo de población. Se trató de un estudio descriptivo transversal de tipo observacional en 70 estudiantes de primer ingreso del Recinto de Paraíso en el I semestre del año 2019. Para la recolección de datos se aplicó una encuesta de 22 preguntas cerradas sobre los conocimientos, las actitudes y las prácticas sobre la salud oral. A partir de las preguntas sobre conocimiento en salud bucal se creó un puntaje de conocimiento. Se pudo evidenciar que el nivel de conocimiento sobre la salud bucal es bajo, ya que el promedio del puntaje de conocimiento (7,02 puntos) es menos de la mitad del puntaje máximo (17 puntos). La mayor deficiencia en el conocimiento se evidenció en temas como el biofilm dental, gingivitis, uso de fluoruros y el rol de la saliva. Además, se identificaron varias prácticas que afectan la condición de la cavidad bucal, como los hábitos nocivos, una alimentación inadecuada y hábitos de higiene oral deficiente. En este estudio se identificó en general una falta de conocimiento respecto a conceptos básicos de salud bucal en personas jóvenes universitarias, quienes no tienen claro la función de algunos factores protectores o de riesgo relacionados con las principales enfermedades bucales.

Objetivo: avaliar a influência do capital social individual na qualidade de vida relacionada à saúde bucal (QVRSB) de adultos do Sistema Único de Saúde (SUS). Métodos: este estudo transversal incluiu adultos adscritos às Estratégias da Saúde da Família (ESFs) de Santa Maria, RS. A coleta de dados foi realizada de novembro de 2019 a março de 2020 em quatro ESFs por examinadores calibrados e treinados. O capital social individual, avaliado pela participação em atividades voluntárias e a confiança em vizinhos e amigos, foi considerado o preditor do estudo. Características sociodemográficas, comportamentais e de saúde periodontal também foram coletadas. A QVRSB, considerada o desfecho, foi mensurada pelo questionário OHIP-14. Modelos de regressão de Poisson avaliaram a associação entre variáveis independentes e QVRSB. Resultados: ao total, 80 adultos foram incluídos. Indivíduos do sexo feminino, da cor da pele não branca, com menor renda familiar mensal, fumantes, que visitaram o serviço odontológico a menos de um ano e que possuíam sangramento gengival apresentaram altos scores totais de OHIP-14 (P<0,05). A não participação em atividades voluntárias [razão de média (RR): 1,24; intervalo de confiança (IC) 95%: 1,03-1,50] e a ausência de confiança em vizinhos e amigos (RR: 1,24; IC 95%: 1,08-1,43) foram associadas a uma pior QVRSB dos adultos. Conclusão: a ausência de um capital social individual influenciou negativamente a QVRSB de adultos usuários do SUS.(AU)


Objective: To evaluate the influence of individual social assets on oral health-related quality of life (OHRQoL) of adult users of the Brazilian Unified Health System (UHS). Methods: This cross-sectional study included adults of the Family Health Strategy (FHS) of Santa Maria, RS. Data collection was performed from November 2019 to March 2020 in four FHSs by calibrated and trained examiners. Individual social capital, assessed by participation in volunteer activities and trust in neighbors and friends, was considered the predictor of the study. Sociodemographic, behavioral, and periodontal health characteristics were also collected. The OHRQoL, considered the outcome, was measured using the OHIP-14 questionnaire. Poisson regression models assessed the association between independent variables and OHRQoL. Results: In total, 80 adults were included. Female subjects, of non-white skin color, with lower monthly family income, smokers, who visited the dental service less than one year ago and who had gingival bleeding had high total OHIP-14 scores (P<0.05). Non-participation in voluntary activities [rate ratio (RR): 1.24; confidence interval (CI) 95%: 1.03-1.50] and the lack of trust in neighbors and friends (RR: 1.24; 95% CI: 1.08-1.43) were associated with a worse OHRQoL of adults. Conclusion: The absence of individual social assets negatively influenced the OHRQoL of adult UHS users.(AU)

ABSTRACT

Objetivo: avaliar a condição bucal de mulheres com diabetes mellitus gestacional (DMG) internadas no Hospital Escola (HE) da Universidade Federal de Pelotas (UFPel). Métodos: trata-se de um estudo transversal retrospectivo de base hospitalar, em que dois residentes treinados realizaram a coleta dos dados por meio da avaliação dos prontuários médicos e odontológicos, no período de setembro de 2019 a março de 2020. Os dados socioeconômicos e demográficos e o diagnóstico de DMG foram coletados dos prontuários médicos, enquanto hábitos e condição bucal, dos prontuários odontológicos. A análise dos dados foi realizada no programa Stata 11.0, usando os testes Exato de Fisher e Regressão de Poisson. Resultados: foram avaliados os prontuários de 83 gestantes, destas, 37 (44,6%) apresentavam DMG. A presença de DMG esteve asso-ciada com as gestantes de maior faixa etária (62,2%) e no terceiro trimestre de gestação. Em sua maioria, tinham renda de até dois salários mínimos, eram solteiras, tinham filhos e realizaram pré-natal. Em relação à avaliação bucal, apenas a presença de cálculo dental e inflamação gengival foi estatisticamente associada à presença de DMG (p= 0,030 e 0,014 respectivamente). A autopercepção do sorriso foi considerada ruim por 40,5%, e a maioria teve dentes perdidos por cárie (64,9%). Conclusões: a prevalência de DMG foi alta entre as gestantes internadas, sendo maior em mulheres de mais idade. Presença de cálculo dental e inflamação gengival foram fortemente associadas à presença de DMG, enquanto hábitos bucais e presença de cárie não apresentaram associação. Novas pesquisas, com exames periodontais completos, são necessárias para verifi-car as condições periodontais dessas mulheres.(AU)


Aim: to assess the oral condition of women with Gestacional Diabetes Mellitus (GDM) admitted in a School Hospital (HE)/UFPel. Methods: medical and dental records, from September 2019 to March 2020, were evaluated in this hospital-based retrospective cross-sectional study. Socioeconomic and demographic data were collected from medical records, while oral condition and habits were obtained from the dental records. Statistical analysis was performed with Stata 11.0 software using Fisher's exact test and Poisson regression. Results: the medical records of 83 pregnant women were evaluated, of which 37 (44.6%) had GDM. The presence of GDM was associated with pregnant women of older age (62.2%) and most in the third trimester of pregnancy. Most of them had an income of up to two minimum wages, were single, had children and underwent prenatal care. Regarding the oral evaluation, only the presence of dental calculus and gingival inflammation was statistically associated with the presence of GDM (p = 0.030 and 0.014 respectively). The self-perception of the smile was considered bad to 40.5% and many of them had lost teeth due to caries (64.9%). Conclusions: the prevalence of GDM was high in hospitalized pregnant women, being higher in older women. Presence of dental calculus and gingival inflammation were strongly associated with the presence of GDM, while oral habits and the presence of caries were not associated. Further research, with complete periodontal examinations is necessary to verify the periodontal conditions of these women.(AU)

ABSTRACT

Objetivo: avaliar a influência da equipe de saúde bucal (eSB) na qualidade de vida relacionada à saúde bu-cal (QVRSB) de adultos. Métodos: a amostra deste estudo transversal foi composta por adultos adscritos às Estratégias de Saúde da Família (ESFs) de Santa Maria, RS. Variáveis sociodemográficas, como idade, sexo, cor da pele, renda familiar e escolaridade, foram avaliadas através de questionário semiestruturado. Variáveis de serviço odontológico foram mensuradas através do motivo de procura ao serviço e da presença de eSB, avaliada a partir dos dados oficiais do município. Biofilme dental, sangramento gengival e cárie dental foram coletados através de exame clínico odontológico. O desfecho foi a QVRSB mensurada pelo questionário OHIP-14. Modelos foram construídos por meio de regressão de Poisson, para avaliar a associação entre presença de eSB e QVRSB. Resultados: ao total, 80 adultos adscritos a quatro ESF (duas com eSB) foram in-cluídos. A ausência da eSB influenciou negativamente a QVRSB de adultos (razão de média: 1,21; intervalo de confiança de 95%: 1,07-1,36). Além disso, idade, cor da pele não branca, menor renda familiar, menor escolaridade, procura ao serviço odontológico por dor dental e pior saúde bucal foram associados a uma pior QVRSB (P<0,05). Conclusão: indivíduos sem acesso à eSB apresentaram uma pior QVRSB. Estratégias de promoção de saúde devem incluir a ampliação do acesso à ESF, aumentando a equidade no acesso aos cuidados odontológicos.(AU)


Objective: to assess the influence of dental health team (DHT) on the oral health-related quality of life (OHRQoL) of adults. Methods: the sample of this cross-sectional study consisted of adults enrolled in the Family Health Strategy (FHS) in Santa Maria, RS. Sociodemographic variables such as age, sex, skin color, family income and education level were assessed using a semi-structured questionnaire. Dental service variables were measured through the reason for attending for the service and the presence of DHT, evaluated from the official data of the municipality. Dental biofilm, gingival bleeding and dental caries were collected through clinical dental examination. The outcome was OHRQoL, measured by the OHIP- 14 questionnaire. Models were built using Poisson regression to assess the association between the presence of DHT and OHRQoL. On total, 80 adults enrolled in four FHS (two with DHT) were included. The absence of DHT negatively influenced the OHRQoL of adults [mean ratio: 1.21; 95% confidence interval: 1.07-1.36]. In addition, age, non-white skin color, lower family income, less education, attending dental care due to dental pain and poorer oral health were associated with worse OHRQoL (P<0.05). Conclusion: individuals without access to DHT had a worse OHRQOL. Health promotion strategies should include expanding access to FHS, increasing equity in access to dental care.(AU)

ABSTRACT

Objetivo: o modelo de uso de serviços de saúde de um determinado grupo populacional é, em grande parte, explicadopor seu perfil de necessidades em saúde. O conhecimento deste é importante para estabelecer o modocomo a atenção odontológica será ofertada. Métodos: realizou-se um estudo retrospectivo a partir de dados secundários(prontuários) (n=381) de pacientes atendidos nas clínicas odontológicas da Universidade Franciscana(UFN) de Santa Maria, RS. Os dados deste estudo foram obtidos através da análise dos prontuários de pacientesatendidos nas clínicas odontológicas da UFN entre os anos de 2005 e 2019. Apenas prontuários com a assinaturado paciente ou responsável foram inseridos neste trabalho. Para a obtenção dos dados, foi construído uminstrumento de coleta, sendo obtidas informações sociodemográficas (idade, sexo, cor, estado civil, profissão elocal de residência) e sobre o estado de saúde (doenças sistêmicas, uso de medicação, perfil de saúde bucal eCPO-D). Cada sujeito selecionado recebeu uma codificação que garantiu seu anonimato. A análise estatísticafoi do tipo descritiva e foi realizada em software estatístico. Resultados: a maioria dos pacientes atendidos erado sexo feminino (68,5%), sendo que a faixa etária mais prevalente foi entre 31 e 50 anos (44,5%). A maioriados pacientes eram estudantes (20,3%), seguidos de trabalhadores do lar (10,9%) e aposentados (7,8%).A queixa principal mais reportada foi a dor de dente (25,2%). Nos resultados deste trabalho, a presença dedoença preexistente foi reportada por 87 (22,8%) pacientes, sendo todas classificadas como doenças crônicas,de acordo com a classificação proposta por Murow e Oglesby13 (1996); a mais prevalente foi problemas alérgicos,69 (18,1%), seguidos por problemas respiratórios, 42 (11%), e problemas gástricos, 39 (10%). A utilizaçãode medicação de uso contínuo foi relatada por 150 (39,3%) pacientes. Conclusão: os pacientes das clínicasodontológicas da UFN de Santa Maria, RS, são, em sua maioria, estudantes, aposentados e trabalhadores do lar,sendo a dor e a fratura dentária as principais queixas dos usuários. Dessa maneira, acredita-se que os discentesda instituição devem estar aptos a realizarem atendimentos de caráter emergencial. O conhecimento do manejoodontológico de pacientes com doenças crônicas não transmissíveis deve ser uma preocupação para daquialguns anos, visto que o envelhecimento da população é esperado para os próximos 30 anos.(AU)


Objective: the health services use model of a given population group is largely explained by its profile of health needs. The knowledge of this is important to establish how dental care will be offered. Methods: in view of this, a retrospective study was carried out using secondary data (medical records) (n = 381) of patients seen at the dental clinics at the Franciscana University (UFN) in Santa Maria, RS. The data of this study were obtained through the analysis of the medical records of patients seen at dental clinics at Universidade Franciscana, between the years 2005 and 2019. Only medical records with the signature of the patient or guardian were included in this work. To obtain the data, an instrument for collecting and collecting sociodemographic information (age, sex, color, marital status, profession and place of residence) and health status (systemic diseases, use of medication, oral health profile and CPO-D) were obtained. Each selected subject received a code that guaranteed their anonymity. The statistical analysis was descriptive and was performed using statistical software. Results: most of the patients seen were female (68.5%), with the most prevalent age group between 31 and 50 years old (44.5%). Most patients were students (20.3%), followed by household workers (10.9%) and retired (7.8%). The main complaint most reported was toothache (25.2%). In the results of this study, the presence of pre-existing disease was reported by 87 (22.8%) patients, all of whom were classified as chronic diseases, according to the classification proposed by Murow and Oglesby13 (1996). The most prevalent were allergic problems, 69 (18.1%), followed by respiratory problems, 42 (11%), and gastric problems, 39 (10%). The use of medication for continuous use was reported by 150 (39.3%) patients. Conclusion: patients at dental clinics at UFN in Santa Maria / RS are mostly students, retirees and home workers, with dental pain and fracture being the main complaints of users. Thus, it is believed that the institution's students must be able to provide emergency care. Knowledge of the dental management of patients with chronic non-communicable diseases should be a concern for some years to come, since the aging of the population is expected for the next 30 years.(AU)

ABSTRACT

This study aimed to assess the association of demographic conditions, socioeconomic status, clinical variables, and psychosocial factors with the number of filled teeth in adolescents from public schools. This cohort study comprised 1,134 12-year-old adolescents enrolled in public schools in Santa Maria, Brazil, in 2012. They were followed-up in 2014, where 743 individuals were reassessed (follow-up rate of 65.52%) for the number of filled teeth. Data were collected via dental examinations and structured interviews. Demographic and socioeconomic characteristics were collected from parents or legal guardians. The psychosocial factor comprised students' subjective measurement of happiness (Brazilian version of the Subjective Happiness Scale - SHS). Dental examinations were performed to assess the number of filled teeth through decay, missing, and filled teeth index (DMF-T). Unadjusted and adjusted Poisson regression analyses were performed to assess the association between baseline variables and filled teeth at follow-up. The number of filled teeth in 2012 and 2014 were 193 (17.02%) and 235 (31.63%), respectively. The incidence of filled teeth in 2014 was 42 (5.65%). Adolescents with untreated dental caries, those who visited the dentist in the last 6 months, those that exhibited being happier, and those who had filled teeth at baseline were associated with a higher number of filled teeth at follow-up. We conclude that the number of filled teeth in adolescents was influenced by clinical and psychosocial factors, emphasizing the need to focus on oral health policies in individuals with higher disease burden and those who feel psychologically inferior.

INTRODUCTION: Despite the improvement in oral health conditions observed in the Brazilian population, there are still high social inequalities that must be monitored. OBJECTIVE: To evaluate income inequality in oral hygiene practices, oral health status and the use of dental services in the adult and senior Brazilian population. METHODS: Data from the National Health Survey conducted in 2013 (Pesquisa Nacional de Saúde - PNS 2013) were used for the population aged 18 years old or older. RESULTS: Inequalities were found among the income strata in most of the oral health indicators evaluated. The greatest inequalities were observed in the use of dental floss, in hygiene practices (PR = 2.85 in adults and PR = 2.45 in seniors), and in total tooth loss (PR = 6.74 in adults and PR = 2.24 in seniors) and difficulty in chewing (PR = 4.49 in adults and PR = 2.67 in seniors) among oral condition indicators. The magnitude of inequalities was high in both groups in most oral condition indicators. Income was a factor that persisted in limiting access to dental services, and even the lower income segments had high percentages that paid for dental consultations. CONCLUSION: Based on data from the first PNS, the findings of this study enabled the identification of oral health and dental care aspects more compromised by income differentials, thus, contributing to the planning of dental care in Brazil and to stimulate the monitoring of these disparities with data from future surveys.

OBJECTIVE: To evaluate the potential support of schools for oral health promotion and associated factors in Brazilian capitals. METHODS: Data from 1,339 public and private schools of the 27 Brazilian capitals were obtained from the National Survey of School Health (PeNSE) 2015. Data from the capitals were obtained from the United Nations Development Program and the Department of Informatics of the Brazilian Unified Health System (Datasus). The indicator " ambiente escolar promotor de saúde bucal " (AEPSB - oral health promoting school environment) was designed from 21 variables of the school environment with possible influence on students' oral health employing the categorical principal components analysis (CATPCA). Associations between the AEPSB and characteristics of schools, capitals and regions were tested (bivariate analysis). RESULTS: Ten variables comprised CAPTCA, after excluding those with low correlation or high multicollinearity. The analysis resulted in a model with three dimensions: D1. Within-school aspects (sales of food with added sugar in the canteen and health promotion actions), D2. Aspects of the area around the school (sales of food with added sugar in alternative points) and D3. prohibitive policies at school (prohibition of alcohol and tobacco consumption). The sum of the scores of the dimensions generated the AEPSB indicator, dichotomized by the median. From the total of schools studied, 51.2% (95%CI 48.5-53.8) presented a more favorable environment for oral health (higher AEPSB). In the capitals, this percentage ranged from 36.6% (95%CI 23.4-52.2) in Rio Branco to 80.4% (95%CI 67.2-89.1) in Florianópolis. Among the Brazilian regions, it ranged from 45.5% (95%CI 40.0-51.2) in the North to 67.6% (95%CI 59.4-74.9) in the South. Higher percentages of schools with higher AEPSB were found in public schools [58.1% (95%CI 54.9-61.2)] and in capitals and regions with higher Human Development Index [61.0% (95%IC 55.8-66.0) and 57.4% (95%CI 53.2-61.4), respectively] and lower Gini index [55.7% (95%CI 51.2-60.0) and 52.8 (95%CI 49.8-55.8), respectively]. CONCLUSIONS: The potential to support oral health promotion in schools in Brazilian capitals, assessed by the AEPSB indicator, was associated with contextual factors of schools, capitals and Brazilian regions.

BACKGROUND: The Syrian crisis has started eight years ago and has, directly and indirectly, affected all the aspects of the Syrians lives. A lot of new war-related factors contributed to change the socio-economic status, the demographical distribution and the ability to access the public health services. Moreover, the crisis created the biggest displacement crisis both inside and outside Syria. Therefore, it is important to study the prevalence of dental caries and oral health in these specific circumstances in order to build a database to assess and compare future results of preventive programs and to assess health and social needs of the communities affected by war or crisis. The aim of this research is to Study the level of oral health among children during the Syrian crisis, as well as the relation between oral health and socioeconomic status (SES). METHODS: A cross-sectional study to assess the oral health of children in Damascus city by using DMFT index and other dental indices. The data were collected from ten randomly selected schools covering all the areas of Damascus city, and the final sample size was 811 children. RESULTS: DMFT index was used to assess the oral health of the children. The average number was (3.36) among all children; 14% of the sample size had a good oral health, while 86% had at least one decayed, missed, or filled tooth. There was also a strong association between SES of the child and the oral health represented as DMFT Index (P = 0.03), Pearson's correlation test displayed an inverse association between the SES and oral health (P = - 0.074). CONCLUSION: This study highlights the impact of the Syrian crisis on the SES of the Syrian children and their oral health. Bad oral health has been recorded and it has a significant relation with the SES of the children.

ABSTRACT

The oral hygiene of individuals with Parkinson disease (PD) is markedly impaired by difficulties in toothbrushing due to motor impairment and tremors. Additionally, it appears that other features associated with PD have an impact on the quality of oral health. Objective: this cross-sectional observational study characterized the oral health condition of individuals with PD. Methods: fifty individuals with PD, aged 53 to 94 years, users of medication for such condition were examined. The research participants had their oral cavities examined to assess prosthetic use and need; index of Decayed, Missing and Filled Teeth (DMFT); Community Periodontal Index (CPI); Periodontal Attachment Loss (PAL); and oral mucosa. In addition, a questionnaire was applied to obtain personal data, general health, and oral health. The data were analyzed using descriptive statistics. Results: a high prosthetic use was observed in the upper arch (92%), while the lower arch revealed high prosthetic need (66%). The DMFT index showed a high number of missing teeth, CPI showed a prevalence of 43% of dental calculus, and PAL revealed 28% of attachment loss of 4-5 mm. The most frequent findings in the assessment of oral mucosa were denture stomatitis and inflammatory fibrous hyperplasia. Conclusion: PD patients refer xerostomia and present high number of missing teeth, a minority of healthy teeth, and oral lesions, representing a target population for specialized oral health care. A higiene bucal de indivíduos com doença de Parkinson (DP) é claramente prejudicada por dificuldades na escovação devido ao comprometimento motor e aos tremores. Além disso, outros aspectos relacionados à doença parecem impactar a qualidade da saúde bucal. Objetivo: este estudo observacional seccional caracterizou a condição de saúde bucal de indivíduos com DP. Métodos: foram examinados cinquenta indivíduos com DP, com idades variando entre 53 e 94 anos, que utilizavam medicamentos para DP. Os participantes da pesquisa foram submetidos a exame bucal para avaliar: uso e necessidade de prótese, índice de dentes cariados, perdidos e obturados (CPOD), Índice Periodontal Comunitário (IPC), Índice de Perda de Inserção Periodontal (PIP) e mucosa bucal. Além disso, foi aplicado um questionário para obtenção de dados pessoais e sobre saúde geral e saúde bucal. Os dados foram analisados usando estatística descritiva. Resultados: observou-se alto percentual de uso de prótese superior (92%), enquanto o arco inferior apresentou elevada necessidade de prótese (66%). O CPOD revelou um alto número de dentes perdidos; o IPC apresentou prevalência de 43% de cálculo dentário; e o PIP revelou 28% de perda de inserção de 4-5 mm. Os achados mais frequentes na avaliação da mucosa bucal foram estomatite por dentadura e hiperplasia fibrosa inflamatória. Conclusão: os indivíduos com DP mencionam xerostomia, apresentam alto número de dentes faltantes, um número reduzido de dentes saudáveis e lesões bucais, representando uma população-alvo para o cuidado de saúde bucal especializado. ABSTRACT

OBJECTIVE: To examine if socioeconomic inequalities exist in periodontal disease among adult with optimal oral health behaviours. MATERIALS AND METHODS: Data were from the Adult Dental Health Survey 2009, a national survey of England, Wales and Northern Ireland. Overall, 4738 participants aged 35 years and older were included in the analysis. Periodontal disease indicated by pocket depth or loss of attachment ≥4 mm, and gingival bleeding were used as periodontal outcomes. Education and deprivation indicated socioeconomic position. Behavioural factors were dental visits, toothbrushing and smoking. The subset of adults with and without optimal health related behaviours included 2916 and 1822 participants, respectively. The associations between periodontal disease and socioeconomic position were tested adjusting for demographic and behavioural factors. Additional models stratifying the sample to those with and without optimal behaviour subgroup were constructed. RESULTS: Education and deprivation were significantly associated with periodontal disease in the partially adjusted models. In the analysis of those with optimal behaviours, only deprivation and highest level of education showed significant association with periodontitis (PD), but not with gingival bleeding. Among those without optimal behaviours, all socioeconomic factors were associated with all outcomes except deprivation and PD. CONCLUSIONS: Oral health behaviours marginally contributed to inequalities in gingival bleeding and periodontal disease. Socioeconomic inequalities were attenuated among those with optimal behaviours and persisted among those without optimal behaviours. Behaviours appeared to be an effect modifier for the relationship between periodontal outcomes and socioeconomic factors.

Introdução: A ansiedade é uma condição emocional relacionada a situações desconhecidas comumente associada ao atendimento odontológico. Objetivo: Avaliar os níveis de ansiedade dos pacientes frente ao atendimento odontológico. Métodos: O presente estudo, do tipo transversal analítico, foi realizado com uma amostra de conveniência composta por 300 pacientes oriundos de um centro de referencia em Odontologia da região sul do estado do Ceará - Brasil. Foram coletadas informações sobre idade, sexo, percepção da saúde bucal, dor de dente, experiência pregressa à tratamentos odontológicos e sobre aspectos psicológicos. O status de ansiedade dos pacientes foi avaliado através do instrumento Modified Dental Anxiety Scale. O presente estudo foi aprovado pelo Comitê de Ética em Pesquisa local sob o número de protocolo 1.759.075. Resultados: Dos 300 pacientes selecionados para o estudo, 222 (74,0 porcento) eram do sexo feminino e 78 (26,0 porcento) do masculino, com idade média de 32,7 anos. Em relação à ansiedade, 22,7 porcento (n= 68) dos indivíduos foram considerados ansiosos. Através da análise bivariada verificou-se associação estatisticamente significativa entre o status de ansiedade e os indivíduos do sexo feminino (p= 0,004) e pacientes com histórico de experiências desagradáveis no dentista (p= 0,015). Conclusão: A prevalência de ansiedade é maior nos indivíduos do sexo feminino e experiências odontológicas pregressas desagradáveis ou de sofrimento podem se configurar como um importante fator associado à ansiedade odontológica(AU)

El estado de ansiedad se evaluó usando el instrumento Modified Dental Anxiety Scale. El estudio fue aprobado por el Comité de Ética en Investigación local con el número de protocolo 1.759.075. Resultados: De los 300 pacientes seleccionados para el estudio, 222 (74.0 por ciento) eran mujeres y 78 (26,0 por ciento) eran hombres, con una media de 32,7 años. Con respecto a la ansiedad, el 22,7 por ciento (n= 68) de los pacientes del estudio presentaron ansiedad. El asesoramiento de análisis bivariado mostró una asociación estadísticamente significativa entre el estado de ansiedad y las mujeres (p= 0,004) y pacientes con historial de experiencias desagradables en el dentista (p= 0,015). Conclusiones: La prevalencia de la ansiedad es mayor en las mujeres, y las experiencias odontológicas desagradables previas o el sufrimiento pueden ser un factor importante asociado con la ansiedad dental(AU)


Introduction: Anxiety is an emotional state related to the unfamiliar situations commonly associated with dental care. Objective: Evaluate the anxiety status of patients during dental care. Methods: An analytical cross-sectional study was conducted of a convenience sample composed of 300 patients attending a dental care reference center in the southern region of the state of Ceará in Brazil. Information was collected about age, sex, perception of oral health, toothache, previous dental treatment and psychological aspects. Anxiety status was evaluated with the Modified Dental Anxiety Scale. The study was approved by the local Research Ethics Committee with protocol number 1.759.075. Results: Of the 300 patients selected for the study, 222 were women (74.0 percent) and 78 were men (26.0 percent); mean age was 32.7 years. Regarding anxiety, 22.7 percent (n= 68) of the patients studied had anxiety symptoms. Bivariate analysis revealed a statistically significant relationship between anxiety and women (p= 0.004) and patients with a history of unpleasant experiences at the dentist (p= 0.015). Conclusions: Prevalence of anxiety is higher among women. Previous unpleasant experiences or suffering at the dentist may be important factors associated to dental anxiety(AU)

Introducción: La caries dental infantil constituye un problema de salud pública en el Ecuador. Los niños de 3 a 5 años que padecen caries dental decidua, sufren complicaciones como dolor al comer y dormir, alteración del estado de ánimo, inasistencia escolar; en general, un impacto negativo en su calidad de vida. Los padres cumplen un rol clave en el cuidado de la salud bucal de sus hijos, el cual es determinado muchas veces por su nivel educativo. Objetivo: Determinar el impacto que la salud bucal tiene en la calidad de vida de preescolares, evaluado por sus padres o responsables y relacionado con su nivel de educación. Métodos: Se realizó un estudio descriptivo y transversal. Se evaluaron a 382 padres y/o representantes de preescolares niños y niñas entre 3 a 5 años pertenecientes a cuatro provincias del Ecuador seleccionadas a conveniencia del investigador; quienes previa aceptación de participación y firma de consentimiento informado respondieron la encuesta ECOHIS (Early Childhood Oral Health Impact Scale) versión en español, que valora la relación entre salud bucal y calidad de vida en preescolares. Los datos recolectados fueron tabulados en tablas de Excel y analizados mediante software libre R.V3.4.1 empleando intervalos de verosimilitud al 14,7 por ciento equivalente al 95 por ciento de confianza para proporciones, y prueba estadística de chi cuadrado. Resultados: Los valores de significancia fueron: al relacionar con el dolor referido por los niños (p=0,152), las dificultades al beber (p= 0,003), consumir alimentos (p= 0,03), al sonreír (p= 0,002), al hablar (p= 0,09) y la repercusión económica asociada a problemas de salud bucal (p= 0,003). Conclusiones: Existe asociación entre el nivel de educación de los padres y su percepción sobre la influencia que tiene la salud bucal en la calidad de vida de sus hijos(AU)


Introduction: Childhood dental caries is a public health problem in Ecuador. Children aged 3-5 years with deciduous dental caries experience unpleasant symptoms such as pain while eating or sleeping, mood changes and school absenteeism, all of which has a negative impact on their quality of life. The key role played by parents in the oral care of their children is often determined by their educational level. Objective: Determine the impact of oral health on the quality of life of preschoolers aged under 5 years as evaluated by their parents or guardians and in relation to their educational level. Methods: A descriptive cross-sectional study was conducted of 382 parents and/or guardians of preschool children aged 3-5 years from four Ecuadorian provinces selected by convenience sampling, who gave their informed consent and answered the ECOHIS (Early Childhood Oral Health Impact Scale) survey, Spanish version, which assesses the relationship between oral health and quality of life among preschoolers. The surveys were identified with codes. The data collected were transferred onto Excel tables and analyzed with the free software R.V3.4.1 using likelihood ratios at 14.7 percent equivalent to 95 percent confidence intervals for proportions, as well as the chi-square statistical test. Results: The following significance levels were obtained: for pain as reported by children (p=0.152), for difficulty when drinking (p= 0.003), eating (p= 0.03), smiling (p= 0.002), speaking (p= 0.09), and for economic impact associated to oral health problems (p= 0.003). Conclusions: An association was found between the educational level of parents and their perception of the influence of oral health upon the quality of life of their children(AU)

ABSTRACT

PURPOSE: The aim of this study was to determine whether there are differences in the distribution of various indicators of oral health among elderly people with and without multimorbidity (ie, two or more chronic diseases). SUBJECTS AND METHODS: A cross-sectional, comparative study was conducted using a sample of Mexican elderly individuals aged ≥60 years. The average age of the cohort was 79.06±9.78 years, and 69.1% were women. The variables indicating oral health were as follows: functional dentition, edentulism, hyposalivation, xerostomia, root caries and periodontitis. The multimorbidity variable was operationally categorized as follows: 0= subjects with no chronic disease or one chronic disease and 1= subjects with two or more chronic diseases. Questionnaires were used to collect information on various variables regarding general health. Likewise, the participants underwent a clinical oral examination. The analysis was performed using Stata 11.0. RESULTS: The overall prevalence of multimorbidity was 27.3%. The prevalences of various oral health indicators were as follows: without functional dentition 89.9%; hyposalivation 59.7%; edentulism 38.9% and self-reported xerostomia 25.2%. Dental caries were observed in 95.3% of the subjects, and the prevalence of severe periodontitis was 80%. We found a significant difference only in edentulism; its prevalence was higher among subjects with multimorbidity (55.3% vs 32.7%, P=0.015) than among those without multimorbidity. CONCLUSION: The presence of edentulism in this sample of Mexican older adults was higher in subjects with multimorbidity. Multimorbidity and oral diseases constitute a true challenge in elderly people, because they affect quality of life and are associated with high health care costs.

ABSTRACT OBJECTIVE To evaluate the potential support of schools for oral health promotion and associated factors in Brazilian capitals. METHODS Data from 1,339 public and private schools of the 27 Brazilian capitals were obtained from the National Survey of School Health (PeNSE) 2015. Data from the capitals were obtained from the United Nations Development Program and the Department of Informatics of the Brazilian Unified Health System (Datasus). The indicator " ambiente escolar promotor de saúde bucal " (AEPSB - oral health promoting school environment) was designed from 21 variables of the school environment with possible influence on students' oral health employing the categorical principal components analysis (CATPCA). Associations between the AEPSB and characteristics of schools, capitals and regions were tested (bivariate analysis). RESULTS Ten variables comprised CAPTCA, after excluding those with low correlation or high multicollinearity. The analysis resulted in a model with three dimensions: D1. Within-school aspects (sales of food with added sugar in the canteen and health promotion actions), D2. Aspects of the area around the school (sales of food with added sugar in alternative points) and D3. prohibitive policies at school (prohibition of alcohol and tobacco consumption). The sum of the scores of the dimensions generated the AEPSB indicator, dichotomized by the median. From the total of schools studied, 51.2% (95%CI 48.5-53.8) presented a more favorable environment for oral health (higher AEPSB). In the capitals, this percentage ranged from 36.6% (95%CI 23.4-52.2) in Rio Branco to 80.4% (95%CI 67.2-89.1) in Florianópolis. Among the Brazilian regions, it ranged from 45.5% (95%CI 40.0-51.2) in the North to 67.6% (95%CI 59.4-74.9) in the South. Higher percentages of schools with higher AEPSB were found in public schools [58.1% (95%CI 54.9-61.2)] and in capitals and regions with higher Human Development Index [61.0% (95%IC 55.8-66.0) and 57.4% (95%CI 53.2-61.4), respectively] and lower Gini index [55.7% (95%CI 51.2-60.0) and 52.8 (95%CI 49.8-55.8), respectively]. CONCLUSIONS The potential to support oral health promotion in schools in Brazilian capitals, assessed by the AEPSB indicator, was associated with contextual factors of schools, capitals and Brazilian regions.


RESUMO OBJETIVO Avaliar o potencial de suporte do ambiente escolar para a promoção da saúde bucal e fatores associados nas capitais brasileiras. MÉTODOS Os dados de 1.339 escolas públicas e privadas das 27 capitais brasileiras foram obtidos da Pesquisa Nacional de Saúde do Escolar (PeNSE) 2015. Os dados das capitais foram obtidos do Programa das Nações Unidas para o Desenvolvimento e do Datasus. Foi elaborado o indicador "ambiente escolar promotor de saúde bucal" (AEPSB), a partir de 21 variáveis do ambiente escolar com possível influência na saúde bucal dos escolares empregando a análise de componentes principais para dados categóricos (CATPCA). Associações entre o AEPSB e características das escolas, das capitais e das regiões foram testadas (análises bivariadas). RESULTADOS Dez variáveis compuseram a CAPTCA, após exclusão daquelas com baixa correlação ou alta multicolinearidade. A análise resultou em modelo com três dimensões: D1. aspectos intraescolares (venda de alimentos com açúcar adicionado na cantina e ações de promoção de saúde), D2. aspectos do entorno escolar (venda de alimentos com açúcar adicionado em pontos alternativos) e D3. políticas proibitivas na escola (proibição do consumo de álcool e tabaco). A soma dos escores das dimensões gerou o indicador AEPSB, dicotomizado pela mediana. Do total de escolas estudadas, 51,2% (IC95% 48,5-53,8) apresentaram ambiente mais favorável à saúde bucal (maior AEPSB). Nas capitais, esse percentual variou de 36,6% (IC95% 23,4-52,2) no Rio Branco a 80,4% (IC95% 67,2-89,1) em Florianópolis. Entre as regiões brasileiras, variou de 45,5% (IC95% 40,0-51,2), no Norte a 67,6% (IC95% 59,4-74,9) no Sul. Percentuais maiores de escolas com maior AEPSB foram encontrados na rede pública [58,1% (IC95% 54,9-61,2)] e em capitais e regiões com maior índice de desenvolvimento humano [61,0% (IC95% 55,8-66,0) e 57,4% (IC95% 53,2-61,4), respectivamente] e menor índice de Gini [55,7% (IC95% 51,2-60,0) e 52,8 (IC95% 49,8-55,8), respectivamente]. CONCLUSÕES O potencial de suporte à promoção da saúde bucal de escolas das capitais brasileiras, avaliado pelo indicador AEPSB, foi associado a fatores contextuais das escolas, das capitais e das regiões brasileiras.

AIM: Most children in war zone countries often experience post-traumatic stress disorder (PTSD). This turns the war into a health issue of great importance. The aim was to evaluate dental and gingival status of children suffering from PTSD. METHODS: A case control study, which included 60 children (30 diagnosed with PTSD and 30 healthy controls) between 9 and 14 years, was undertaken. Dental status was assessed using decayed, missing, and filled teeth (DMFT) for permanent and decayed, missing and filled teeth (dmft) for primary. Gingival status was examined using plaque (PI) and gingival (GI) indices. Child PTSD reaction index (CPTSD-RI) was used to evaluate the severity of PTSD. RESULTS: There was no significant difference in DMFT and dmft indices between the PTSD children and the controls. However, PTSD children had more decay in permanent (DT) and primary teeth (dt), more primary missing teeth mt and less filled teeth in their permanent (FT) and primary (ft) dentition. Significant difference was found between PTSD children and controls regarding PI (P < 0.00001) and GI (P < 0.0001). CPTSD-RI was positively correlated with DMFT (P = 0.002), PI (P = 0.038), and GI (P = 0.007). CONCLUSION: PTSD children had poor dental and gingival status than matched controls and they were affected by PTSD severity.

Objective: Little is known about exploring the oral health related quality of life (OHRQoL) of school children in Saudi Arabia. The importance of examining this topic has centred on its ability to measure associations between the oral symptoms, impacts and clinical measures. This study aims to explore associations between the (OHRQoL) based on the children's Oral Impacts on Daily Performance (Child-OIDP) index and oral diseases of Saudi school children living in Albaha region. Materials and methods: A cross-sectional study was conducted between November 2017 to January 2018 using two-stage randomised sampling technique. The oral diseases and impacts were examined using the World Health Organization's (1997) guidelines and the Child-OIDP index, respectively. Data analysis was conducted using the SPSS software version 20.0. Chi-squared, bivariate and multivariate logistic regression tests were used, as appropriate. Results: In total 349 Saudi male school children, 12 to 15 years old, were recruited. The Child-OIDPrate was significantly high (75.1%). The participants reported minor, moderate and major (severe) oral impacts (29.4%, 50%, and 20.6% respectively). The overall mean of the Child-OIDP score was 2.15±1.40. Conclusions: The high overall Child-OIDP score was in accordance with the participants' dental status. Further research should focus on improving knowledge and implementing education programmes to advance the community's oral diseases and practices.

ABSTRACT: Introduction: Despite the improvement in oral health conditions observed in the Brazilian population, there are still high social inequalities that must be monitored. Objective: To evaluate income inequality in oral hygiene practices, oral health status and the use of dental services in the adult and senior Brazilian population. Methods: Data from the National Health Survey conducted in 2013 (Pesquisa Nacional de Saúde - PNS 2013) were used for the population aged 18 years old or older. Results: Inequalities were found among the income strata in most of the oral health indicators evaluated. The greatest inequalities were observed in the use of dental floss, in hygiene practices (PR = 2.85 in adults and PR = 2.45 in seniors), and in total tooth loss (PR = 6.74 in adults and PR = 2.24 in seniors) and difficulty in chewing (PR = 4.49 in adults and PR = 2.67 in seniors) among oral condition indicators. The magnitude of inequalities was high in both groups in most oral condition indicators. Income was a factor that persisted in limiting access to dental services, and even the lower income segments had high percentages that paid for dental consultations. Conclusion: Based on data from the first PNS, the findings of this study enabled the identification of oral health and dental care aspects more compromised by income differentials, thus, contributing to the planning of dental care in Brazil and to stimulate the monitoring of these disparities with data from future surveys.


RESUMO: Introdução: Apesar da melhora das condições de saúde bucal constatada na população brasileira, persistem elevadas desigualdades sociais que precisam ser monitoradas. Objetivo: Avaliar a desigualdade de renda nas práticas de higiene bucal, nas condições bucais e no uso de serviços odontológicos na população brasileira de adultos e idosos. Métodos: Foram utilizados dados da Pesquisa Nacional de Saúde realizada em 2013 (PNS 2013) referentes à população de 18 anos ou mais. Resultados: Detectaram-se desigualdades entre os estratos de renda na maioria dos indicadores de saúde bucal avaliados. As desigualdades de maior magnitude foram verificadas no uso de fio dental, nas práticas de higiene (RP = 2,85 nos adultos e RP = 2,45 nos idosos), e na perda de todos os dentes (RP = 6,74 nos adultos e RP = 2,24 nos idosos) e dificuldade de mastigar (RP = 4,49 nos adultos e RP = 2,67 nos idosos) entre os indicadores de condições bucais. Na maioria dos indicadores de condições bucais a magnitude das desigualdades foi elevada em ambos os grupos. A renda mostrou-se um fator que persiste limitando o acesso aos serviços odontológicos e, mesmo os segmentos de menor renda apresentaram elevados percentuais que pagam por consulta odontológica. Conclusão: Por meio dos dados da primeira PNS, os achados do estudo permitiram identificar aspectos de saúde e de atenção bucais mais comprometidos pelos diferenciais de renda, podendo, nesse sentido, contribuir para o planejamento da assistência odontológica no país e para estimular o monitoramento destas disparidades com dados das próximas pesquisas.

ABSTRACT

Abstract This study aimed to assess the association of demographic conditions, socioeconomic status, clinical variables, and psychosocial factors with the number of filled teeth in adolescents from public schools. This cohort study comprised 1,134 12-year-old adolescents enrolled in public schools in Santa Maria, Brazil, in 2012. They were followed-up in 2014, where 743 individuals were reassessed (follow-up rate of 65.52%) for the number of filled teeth. Data were collected via dental examinations and structured interviews. Demographic and socioeconomic characteristics were collected from parents or legal guardians. The psychosocial factor comprised students' subjective measurement of happiness (Brazilian version of the Subjective Happiness Scale - SHS). Dental examinations were performed to assess the number of filled teeth through decay, missing, and filled teeth index (DMF-T). Unadjusted and adjusted Poisson regression analyses were performed to assess the association between baseline variables and filled teeth at follow-up. The number of filled teeth in 2012 and 2014 were 193 (17.02%) and 235 (31.63%), respectively. The incidence of filled teeth in 2014 was 42 (5.65%). Adolescents with untreated dental caries, those who visited the dentist in the last 6 months, those that exhibited being happier, and those who had filled teeth at baseline were associated with a higher number of filled teeth at follow-up. We conclude that the number of filled teeth in adolescents was influenced by clinical and psychosocial factors, emphasizing the need to focus on oral health policies in individuals with higher disease burden and those who feel psychologically inferior.

ABSTRACT

The study aimed to: (1) describe the work process in Brazil's oral health teams, based on the essential attributes of primary health care, according to geographic region, type of team, and the municipality's socioeconomic characteristics and (2) verify whether the data in the work process of the oral health teams in the Brazilian National Program to Improve Access and Quality in primary health (PMAQ-AB) were capable of measuring such attributes. This was a nationwide ecological study with data from cycle I of PMAQ-AB. The study included descriptive, exploratory factor, and confirmatory factor analyses (α = 5%). Constructs were analyzed in light of the essential attributes of primary health care (first contact, coordination of care, comprehensiveness, and continuity). The first three constructs and a fourth factor were formed, called dental prosthesis actions. However, the continuity attribute was not formed. The models' goodness-of-fit measures were satisfactory. Factor loads were greater than 0.5, except for the two variables in factor 3. The actions most frequently performed by the oral health teams (> 60%) were in first contact, and the least frequent were those in comprehensiveness, highlighting referrals to specialties (7.6%). There were differences in the work process in oral health teams between regions of the country, type of team, and certification strata (p < 0.05). In conclusion, data on the work process in oral health teams from cycle primary health care in the services' work routine. Further research is recommended on continuity of care. In addition, the oral health teams participating in cycle I of PMAQ-AB should make further progress in actions related to comprehensiveness and coordination of care.


Os objetivos do estudo foram: (1) descrever o processo de trabalho das equipes de saúde bucal (ESB) do Brasil, com base nos atributos essenciais da atenção primária à saúde, segundo regiões, tipo de equipe e características socioeconômicas dos municípios; e (2) verificar se os dados do processo de trabalho das ESB do Programa Nacional de Melhoria do Acesso e da Qualidade da Atenção Básica (PMAQ-AB) foram capazes de aferir tais atributos. Estudo ecológico, de abrangência nacional, com dados do ciclo I do PMAQ-AB. Foram feitas análises descritivas, fatoriais exploratória e confirmatória (α = 5%). Os construtos formados foram analisados à luz dos atributos essenciais da atenção primária à saúde (primeiro contato, coordenação do cuidado, integralidade e longitudinalidade). Formaram-se os três primeiros construtos e um quarto fator, denominado ações em prótese dentária. Porém, o atributo longitudinalidade não foi conformado. As medidas de ajuste dos modelos foram satisfatórias. As cargas fatoriais foram maiores que 0,5, exceto para duas variáveis do fator 3. As ações mais realizadas pelas ESB (> 60%) foram as do primeiro contato e as menos comuns foram as da integralidade, destacando-se ter referência para especialidades (7,6%). Houve diferenças no processo de trabalho das ESB entre as regiões, tipo de equipe e estrato de certificação (p < 0,05). Conclui-se que os dados de processo de trabalho das ESB do ciclo I do PMAQ-AB foram capazes de discriminar três dos quatro atributos essenciais da atenção primária à saúde na rotina dos serviços. Sugere-se aprofundar a avaliação da longitudinalidade. Ademais, as ESB participantes do ciclo I do PMAQ-AB precisam avançar nas ações relacionadas à integralidade e coordenação do cuidado.


Los objetivos de este trabajo fueron: (1) describir el proceso de trabajo de los equipos de salud bucal (ESB) en Brasil, conforme los atributos esenciales de la atención primaria a la salud, según regiones, tipo de equipo y características socioeconómicas de los municipios; además de (2) verificar si los datos del proceso de trabajo de las ESB en el Programa Nacional de Mejora del Acceso y Calidad de la Atención Básica (PMAQ-AB) fueron capaces de evaluar tales atributos. Es un estudio ecológico, de cobertura nacional, con datos del ciclo I del PMAQ-AB. Se realizaron análisis descriptivos, factoriales exploratorios y confirmatorios (α = 5%). Los constructos creados se analizaron a la luz de los atributos esenciales de la atención primaria a la salud (primer contacto, coordinación del cuidado, integralidad y longitudinalidad). Se generaron los tres primeros constructos, y un cuarto factor, denominado acciones en prótesis dental. No obstante, el atributo longitudinalidad no se configuró. Las medidas de ajuste de los modelos fueron satisfactorias. Las cargas factoriales fueron mayores que 0,5, excepto en dos variables del factor 3. Las acciones más realizadas por las ESB (> 60%) fueron las de primer contacto, y las menos comunes fueron las de integralidad, destacándose contar con referencias para especialidades (7,6%). Además, las ESB participantes del ciclo I del PMAQ-AB necesitan avanzar en acciones relacionadas con la integralidad y coordinación del cuidado.
