BACKGROUND: Scapho-lunate advanced collapse (SLAC) and scaphoid nonunion advanced collapse (SNAC) of the wrist are the most common types of wrist arthritis. We compared the union rate and complication profile of patients with SLAC and SNAC wrist undergoing 4 corner arthrodesis with partially threaded or fully threaded headless compression screws. METHODS: A single-center retrospective review was conducted to identify all patients treated for SLAC and SNAC with 4 corner fusion using headless compression screws from 2016 to 2021. A total of 33 patients undergoing surgery on 35 wrists were identified and included in the study. Demographics, comorbidities, complication profile, and radiographs were collected and compared between groups. RESULTS: One hundred percent (16/16) of partially threaded and 84.2% (16/19) of fully threaded screws demonstrated union by minimum 10-week follow-up. The total complication rate (avascular necrosis of lunate, screw loosening, etc.) was 31.4%; 52.6% of wrists implanted with fully threaded screws experienced complications compared with a 6.3% complication rate with partially threaded screws. The difference was statistically significant between the 2 groups (P = .004). CONCLUSIONS: Four corner arthrodesis using antegrade compression screws is an effective, reproducible method to achieve fusion in the wrist. The use of fully threaded screws was associated with more complications than with partially threaded screws, although union rate was not significantly different. Future studies with larger sample sizes would be useful to fully elucidate differences between these 2 constructs.

RESUMO

Rotating-bearing total knee arthroplasty has been theorized to have some advantages in the kinematics and wear characteristics of total knee arthroplasty. A rare complication of rotating-bearing total knee arthroplasty is rotary dislocation, spinout, of the polyethylene component. When these dislocations occur, they typically result in a 90-degree dislocation in respect to the axial axis of the knee. This case is unique in that it presents a complete 180-degree polyethylene dislocation without trauma.

OBJECTIVE: The aim of this study was to determine the impact of a demanding work schedule involving long, cumulative work shifts on response time and balance-related performance outcomes and to evaluate the prevalence of musculoskeletal disorders between day and night shift working nurses. METHODS: A questionnaire was used to identify the prevalence of past (12-month) and current (7-day) musculoskeletal disorders. Nurses worked three 12-hour work shifts in a 4-day period. Reaction time and balance tests were conducted before and after the work period. RESULTS: The work period induced impairments for reaction time, errors on reaction time tasks, and balance performance, independent of shift type. Musculoskeletal symptom prevalence was high in workers of both work shifts. CONCLUSIONS: Compressed work shifts caused performance-based fatigue in nurses. Reaction time and balance tests may be sensitive fatigue identification markers in nurses.

We investigated the time course of neuromuscular and hypertrophic adaptations associated with only four weeks of barbell squat and deadlift training. Forty-seven previously untrained women (mean±SD, age=21±3years) were randomly assigned to low volume training (n=15), moderate volume training (n=16), and control (n=16) groups. The low and moderate volume training groups performed two and four sets, respectively, of five repetitions per exercise, twice a week. Testing was performed weekly, and included dual X-ray absorptiometry and vastus lateralis and rectus femoris B-mode ultrasonography. Bipolar surface electromyographic (EMG) signals were detected from the vastus lateralis and biceps femoris during isometric maximal voluntary contractions of the leg extensors. Significant increases in lean mass for the combined gynoid and leg regions for the low (+0.68kg) and moderate volume (+0.47kg) groups were demonstrated within three weeks. Small-to-moderate effect sizes were shown for leg lean mass, vastus lateralis thickness and pennation angle, and peak torque, but EMG amplitude was unaffected. These findings demonstrated rapid muscular adaptations in response to only eight sessions of back squat and deadlift training in women despite the absence of changes in agonist-antagonist EMG amplitude.
