RESUMO

Cardiac syndrome X (CSX) is coronary heart disease (CHD) in the absence of or with minimal atherosclerotic changes in coronary arteries. This condition has a more favourable prognosis than classical angina but is associated with relatively poor quality of life. None of the many hypothesis of angina pathogenesis in CSX patients explains it exhaustively. We undertook to consider minor atherosclerotic changes in patients with CSX based on modem concepts of atherogenesis implying its relation to the level and metabolism of lipid fatty acids. Comparative analysis of their composition in blood and saliva provided material for the characteristic of CHD patients with intact and affected coronary arteries.

RESUMO

In persons with impaired social regulation, the balance of value-need regulation is shifted towards the predominance of needs. Directions in their life are absent or their set is very limited. Their behavior is frequently situation-dependent. Social and psychological dysadaptation results from that the individual inadequately appraises the life situation and hence inadequately responds to the latter. The socially unacceptable behavior becomes normal up to criminogenic (theft, violence). In the homeless with tuberculosis, a change occurs in the structural hierarchy of vital activity motives by the motive-for-objective type. In this connection, there is a paradoxical attitude towards the disease for the purpose of acquiring a social benefit. In the homeless, the factors that are frustrating and inducing stress in ordinarily persons cause no emotional responses. This was a result of the mental defense worked out under constant stress.

RESUMO

The levels of calcium, magnesium and iron were examined on an automated biochemical analyzer in saliva and blood serum of 102 patients with chronic obstructive disease of lungs (CODL) and of 20 healthy subjects. Reliably reduced levels of magnesium and iron in saliva as well as low permeability of the hematosalivary barrier (HSB) for magnesium and its high permeability for iron were detected in group 2 and 3 of patients. The dynamic level of iron in saliva reached the normal value, whereas, a stable salivary deficiency was typical of magnesium. The above tests of levels of magnesium and iron in saliva and the determination of the distribution factor for the above cations can be used in evaluating a severity of bronchial obstruction and therapy efficiency in patients with CODL.

AIM: To study content of interleukin-1 beta (IL-1 beta) and free radical oxidation in neutrophils of blood, saliva and bronchoalveolar lavage (BAL) in chronic bronchitis for prediction of obstructive defects. MATERIAL AND METHODS: Enzyme immunoassay was used to measure IL-1 beta in the blood, saliva, BAL of 49 males with chronic bronchitis aged 39-72 years. Functional ability of the neutrophils to generate free oxygen radicals was assessed by luminol-dependent chemiluminescence. RESULTS: Patients with chronic obstructive bronchitis exhibited excessive accumulation of IL-1 beta in the blood and BAL, enhanced activity of hematosalivary barrier for IL-1 beta and production of active oxygen radicals by systemic and tissue neutrophils. CONCLUSION: Elevated concentration of systemic and tissue IL-1 beta, activation of free radical oxidation by blood neutrophils and BAL, enhancement of hematosalivary barrier for IL-1 beta can be considered as prognostically unfavourable factors of bronchial obstruction in patients with chronic bronchitis.

As many as 21 children suffering from infectious endocarditis (IE) were examined for the clinical and morphological picture of the disease. The patients' age ranged from 2 to 15 years. In 15 children, IE developed in association with different congenital diseases; in one patient, in the presence of rheumatic heart disease, and in 5 patients, in unaffected heart valves. The sectional material was examined in 19 cases whereas the operational material in 2 cases. Analysis of the data obtained has shown that early diagnosis of IE (primary and secondary) may not infrequently be fairly difficult, which is likely to result in the late administration of antibacterial therapy. In addition to the involvement of the valvular apparatus of the heart, the majority of the patients demonstrated the signs of focal myocarditis and postmyocarditic cardiosclerosis, which is one of the causes of the development of refractory heart insufficiency. IE children mostly die from heart insufficiency and thromboembolism.

RESUMO

Ultrasound research methods were applied in 32 children with the clinical signs of infectious endocarditis (IE). The patients' age varied from 3 to 16 years. In 5 patients, IE turned out primary in the intact valves whereas in 27 patients, it superadded on different congenital heart disease. In 10 patients, IE had developed before surgical correction of the diseases and in 17, within different times after operation. The direct and indirect echo-signs of IE were delineated. The direct echo-signs included vegetations which were discovered in 22 out of the 32 patients. The indirect echo-signs included the signs of regurgitation in the externally unchanged or minimally changed cusps of the valves and blood drop at the level of the ventricles in connection with the defect reshunting in the interventricular septum. The data obtained are in good agreement with the clinical picture, surgical and morbid anatomy data.
