RESUMO

The role of microflora was studied in patients with diseases of the buccal mucosa (aphthous stomatitis, lichen planus, leukoplasia, desquamative glossitis, etc.). Quantitative and qualitative composition of the microflora and its sensitivity to antibiotics, antifungal agents, and bacteriophages were studied. The following changes in the microflora were distinguished: dysbiotic shift, dysbacteriosis of the I-II, III, and IV degree. Microbiological status of patients with diseases of the buccal mucosa is essential for the course, outcome, and prognosis of the underlying diseases.

RESUMO

Oral mucosa was examined in 23 patients with acute myeloblastic leukemia before and after cytostatic polychemotherapy. Lesions of the oral mucosa were observed in all patients after polychemotherapy: its color was changed, atrophic processes developed in it, and salivary secretion was impaired. Destructive ulcerative changes were revealed in 35% cases; terms of their appearance, regression, and epithelialization were established and a clear-cut correlation between their development and severity of leukocytopenia was detected. Ranging of stomatitis severity is proposed, according to which medium-severe stomatitis predominates in this patient population. The study will help develop more effective approaches to prevention and treatment of oral complications and improve the results of therapy of this most grave category of hematological patients.

RESUMO

Legislative basis of medical service is developing in Russia and implantation methods of treatment are introduced in practical dentistry; documents regulating legal relations between the patient and doctor (institution) in cases with treatment involving implantation are offered. The proposed clinical examinations can lay the basis for creation of a universal information network which will help develop the references for evaluating the efficacy of types and systems of implants and objectively study the causes of failures in dental implantology.

RESUMO

Impressions from various sites of the buccal mucosa of 20 normal subjects aged 16 to 33 years were examined. Mature epitheliocytes of the V differentiation stage predominate in the cytological impressions of the lingual and buccal mucosa. Cells of the V stage of differentiation predominate in the lower lip mucosa, and there are just few cells of the IV (transitional) stage. In impressions of the palatal mucosa mainly cells without nuclei (stage IV) were found. These data will help more objectively assess the cytological findings in different diseases involving the buccal mucosa.

Four patients with acute leukemia suffering from a chronic secondary illness (CSI) which developed 1.5 to 34 months after allogenous transplantation of bone marrow are described. One patient (female) was followed up over time: 14, 15, and 20 months after CSI was diagnosed, the rest patients were examined once, 15.5, 23, and 29 months, respectively, after the disease was diagnosed. Besides visual characterization of the buccal mucosa, results of cytologic examination of smear impressions are presented. Specific features of mucosa involvement at the peak of clinical manifestation of CSI and later, when the rest symptoms of secondary disease to a certain measure regressed, are described. The authors emphasize a similarity between the detected changes in the buccal mucosa and those observed in diseases which might be caused by immunity disorders.

RESUMO

The results of a clinical observation of 30 patients with oral mucosa disease are presented in the article. All patients got "Tantum Verde" as a local treatment in the dose of 15.0 ml 4 times a day for 6 days. The efficiency of the medicine as a mean of symptomatic therapy of acute and chronic oral mucosa diseases is proved as well as prospects for its use both separately and for complex therapy of these diseases.

Clinical changes in oral mucosa minor salivary glands were examined in 30 patients with grave diabetes mellitus treated by transplantation of pancreatic islet cells without immunosuppressant therapy and in 35 patients suffering from chronic renal failure 18 of these operated on for transplantation of an allogenic kidney with immunosuppressant therapy. Clinical condition and function of the minor salivary glands were assessed according to a scheme developed by the authors with consideration for clinical signs: edemas, hyperemia, hyperplasia, retention, and destruction. Morphologic examinations were carried out in various periods after transplantation.

RESUMO

.1 percent sodium hypochlorite solution was used to induce full-value gingival regeneration. The drug was applied to the surgical wound for 10 min during radical gingivo-osteoplasty according to V. A. Kiselev. Morphologic examination of gingival biopsy specimens carried out 2 months after surgery has shown not only epithelial structure recovery, but recovery of the connective-tissue gingival base as well, no cellular infiltrates, whereas the controls developed only epithelial recovery and connective-tissue base fibrous structures were represented by coarse collagen fibre bundles, cellular element accumulations (infiltrates) were seen. Due to a favorable course of the postoperative period patients' invalidity periods were cut down 2.5-fold.

RESUMO

Immunoregulatory cell activities were examined in 40 pregnant women suffering from various periodontitis forms. The results evidence that excessive activation of T-lymphocytes and T-helpers may contribute to the pathogenesis of this condition. The data permit using T-lymphocyte functional activity assessment in the diagnosis and prediction of the course of periodontitis.

RESUMO

The features of clinical course of chronic generalized periodontitis were studied from the viewpoint of analysis of cellular cooperative connective tissue basis. This allowed to single out 2 groups of patients. In the 1st group, a potent lymphoid (seldom lymphoplasmocytic) infiltration was detected within the gingival connective tissue basis, and sIgA preserved or even increased in the mixed saliva. In the 2nd group, the oral cavity physiological neutralization of antigenic substances was impaired and neutrophilic leucocytes were dominating in the gingival connective tissue framework with considerably increased levels of serum IgG. The gingival tissue basophils population was investigated in these groups of patients and their variants given the histochemical characterization. Prognostic criteria of treatment effectiveness are derived from studies of biopsied gingival tissues.
