BACKGROUND: Ageing and its consequences on quality of life is one of the main issues to be addressed by public organizations; therefore the development of tools for its evaluation is a priority issue to orientate lines of action. Therefore, the objective of this research is to carry out cross-cultural adaptation and present the psychometric properties of the Spanish version of the Older People's Quality of Life Questionnaire-Brief (OPQOL-Brief). METHODS: a cross-cultural adaptation was carried out together with a process of translation and back-translation of the scale. The OPQOL-Brief and a sociodemographic questionnaire were administered to 120 older adults aged over 65 from a healthcare setting in the region of Extremadura. RESULTS: the exploratory analysis revealed a factor structure through a matrix of polychoric correlations divided in two intercorrelated factors, consisting of 12 items and with excellent sample adequacy indices (KMO = 0.846'; Bartlett test = 1268.1; p = 0.000). Then, the confirmatory factor analysis established the definitive model with exceptional goodness-of-fit indices (NNFI = 0.99; CFI = 0.99; CMIN/DF = 0.96; Ρ (χ2) = 0.22; RMSEA = 0.037 and RMSR = 0.040). Finally, Cronbach's alpha and McDonald's omega for internal consistency reported good values of 0.830 and 0.851, respectively. CONCLUSION: our findings show that the OPQOL-Brief exhibits a solution with 12 elements and related constructs, providing stable goodness-of-fit indicators as well as good and remarkable consistency ratings.

OBJECTIVE: The objective of this study was to determine whether history of kidney transplant is a risk factor for increased complications in patients who undergo abdominal aortic aneurysm (AAA) repair. BACKGROUND: The incidence of renal failure and subsequent kidney transplant is steadily rising. Many risk factors leading to AAA overlap with those of renal disease. Due to these similarities, a rising incidence of kidney transplant patients undergoing AAA repair is expected. We surmised a notable difference in AAA surgical repair outcomes in renal transplant recipients compared to the general population. METHODS: A retrospective analysis was performed on 59,836 adult patients with history of AAA repair and kidney transplant from 2008 to 2015. Data were obtained from the Nationwide Inpatient Sample database developed for the Healthcare Cost and Utilization Project. RESULTS: Significant differences in age, race, hospital characteristics, and complications were identified. The results suggest that patients with prior transplant generally have AAA repair at a significantly younger age (P < .001). A difference in race (P = .017), with 75% vs 87.4% non-Hispanic whites and 5% vs 1.5% Asian/Pacific Islander in the transplant and nontransplant groups, respectively, was shown. Procedures at transplant centers had significantly longer lengths of stay (P < .001) and higher total charges (P < .001). In addition, transplant recipients exhibited a higher in-hospital mortality index (P < .001) than the nontransplanted population. CONCLUSION: A history of kidney transplant significantly influences multiple aspects of care and complications regarding future AAA repair and is associated with increased in-hospital mortality index. Significant findings include increased total charges, longer lengths of stay, postoperative complications, and differences in age and race.

The Americas have an elevated number of leishmaniasis cases (accounting for two-thirds of the worldwide disease burden) and circulating Leishmania species, and are therefore of interest in terms of epidemiological surveillance. Here, we present a systematic review of Leishmania parasite species circulating in the countries of the American continent, together with complementary information on epidemiology and geospatial distribution. A database was built from data published between 1980 and 2018 on Leishmania species identified in most of the American countries. A total of 1499 georeferenced points were extracted from published articles and subsequently located to 14 countries in the Americas. This database could be used as a reference when surveilling the occurrence of Leishmania species in the continent.

RESUMEN El síndrome de Lyme crónico representa un término controversial que agrupa un conjunto de síntomas persistentes e inespecíficos, en su mayoría reumatológicos o neurológicos, dentro de los cuales se encuentra la enfermedad postLyme, cuya incidencia epidemiológica es baja, el conocimiento fisiopatológico es controversial y el abordaje diagnóstico se basa especialmente en la interpretación clínica, dirigiendo su enfoque terapéutico hacia la resolución sintomática. Se presenta un caso de una mujer de 17 años, con enfermedad de Lyme de reciente diagnóstico, de aparente evolución de 3 años, con diagnóstico previo de enfermedad desmielinizante, que consultó por deterioro neurológico. Se revisan consideraciones científicas sobre su aproximación clínica y principios terapéuticos, los cuales se contrastan con los aplicados.


SUMMARY Chronic Lyme syndrome represents a controversial term that groups together a set of persistent and nonspecific symptoms, mostly rheumatological and / or neurological, among which is the Post Lyme Disease, whose epidemiological incidence is low, pathophysiology is controversial and the diagnostic approach is based especially on clinical interpretation, directing its therapeutic approach towards symptomatic resolution. We present a case of a female patient of 17 years of age with Lyme disease of recent diagnosis of apparent evolution of 3 years, with previous diagnosis of demyelinating disease, who consulted for neurological deterioration. Scientific considerations on its clinical approach and therapeutic principles are reviewed, contrasting them with those applied.
