RESUMO

Pay-for-performance (P4P) schemes have been shown to have mixed effects on health care outcomes. A challenge in interpreting this evidence is that P4P is often considered a homogenous intervention, when in practice schemes vary widely in their design. Our study contributes to this literature by providing a detailed depiction of incentive design across municipalities within a national P4P scheme in Brazil (PMAQ) and exploring the association of alternative design typologies with the performance of primary health /care providers. We carried out a nation-wide survey of municipal health managers to characterize the scheme design, based on the size of the bonus, the providers incentivized, and the frequency of payment. Using OLS regressions and controlling for municipality characteristics, we examined whether each design feature was associated with better family health team performance. To capture potential interactions between design features, we used cluster analysis to group municipalities into five design typologies and then examined associations with quality of care. A majority of the municipalities included in our study used some of the PMAQ funds to provide bonuses to family health team workers, while the remaining municipalities spent the funds in the traditional way using input-based budgets. Frequent bonus payments (monthly) and higher size bonus allocations (share of 20-80%) were strongly associated with better team performance, while who within a team was eligible to receive bonuses did not in isolation appear to influence performance. The cluster analysis showed what combinations of design features were associated with better performance. The PMAQ score in the 'large bonus/many workers/high-frequency' cluster was 8.44 points higher than the 'no bonus' cluster, equivalent to a difference of 21.7% in the mean PMAQ score. Evidence from our study shows how design features can potentially influence health provider performance, informing the design of more effective P4P schemes.

DESTAQUES ● Este mapa de evidências tem como objetivo apresentar estratégias que podem ser efetivas para o cuidado de pessoas com diabetes, hipertensão ou obesidade. ● Foram incluídas 93 revisões sistemáticas, cujos estudos primários foram realizados em sua maioria (65,1%) na América do Norte, Ásia e Europa. ● As intervenções foram classificadas em 5 categorias e 26 subcategorias. Observouse maior frequência da categoria "Teleconsulta/eHealth" e da subcategoria "cuidado assistencial". ● Os desfechos analisados foram classificados em clínicos e não clínicos. Desfechos clínicos foram relatados em 53 revisões sistemáticas, com destaque para a pressão arterial. Desfechos não clínicos foram relatados em 51 revisões sistemáticas, com destaque para a adesão ao tratamento farmacológico. ● As cinco categorias de intervenções - Teleconsulta/eHealth, Educação, Automonitoramento/autogerenciamento, Tratamento farmacológico, Serviço de saúde - apresentaram efeito positivo ou potencialmente positivo acima de 50% para os desfechos avaliados.


HIGHLIGHTS ● This evidence map aims to present strategies that may be effective for the care of people with diabetes, hypertension or obesity. ● 93 systematic reviews were included, the majority of whose primary studies were carried out (65.1%) in North America, Asia and Europe. ● Interventions were classified into 5 categories and 26 subcategories. There was a greater frequency of the "Teleconsultation/eHealth" category and the "assistance care" subcategory. ● The analyzed outcomes were classified as clinical and non-clinical. Clinical outcomes were reported in 53 systematic reviews, with emphasis on blood pressure. Non-clinical outcomes were reported in 51 systematic reviews, with emphasis on adherence to pharmacological treatment. ● The five categories of interventions - Teleconsultation/eHealth, Education, Self-monitoring/self-management, Pharmacological treatment, Health service - showed a positive or potentially positive effect above 50% for the evaluated outcomes.

Rapid review methodology aims to facilitate faster conduct of systematic reviews to meet the needs of the decision-maker, while also maintaining quality and credibility. This systematic review aimed to determine the impact of different methodological shortcuts for undertaking rapid reviews on the risk of bias (RoB) of the results of the review. Review stages for which reviews and primary studies were sought included the preparation of a protocol, question formulation, inclusion criteria, searching, selection, data extraction, RoB assessment, synthesis, and reporting. We searched 11 electronic databases in April 2022, and conducted some supplementary searching. Reviewers worked in pairs to screen, select, extract data, and assess the RoB of included reviews and studies. We included 15 systematic reviews, 7 scoping reviews, and 65 primary studies. We found that several commonly used shortcuts in rapid reviews are likely to increase the RoB in the results. These include restrictions based on publication date, use of a single electronic database as a source of studies, and use of a single reviewer for screening titles and abstracts, selecting studies based on the full-text, and for extracting data. Authors of rapid reviews should be transparent in reporting their use of these shortcuts and acknowledge the possibility of them causing bias in the results. This review also highlights shortcuts that can save time without increasing the risk of bias. Further research is needed for both systematic and rapid reviews on faster methods for accurate data extraction and RoB assessment, and on development of more precise search strategies.

O mapa de evidências foi desenvolvido tendo como referência as nove diretrizes da Política Nacional de Alimentação e Nutrição (PNAN). O objetivo foi identificar intervenções efetivas para melhorar as condições de saúde, alimentação e nutrição da população. Foram analisadas 101 revisões sistemáticas. As intervenções foram classificadas em 5 categorias e 80 subcategorias. Os desfechos foram classificados em 8 categorias e 119 subcategorias. Foram relatados efeitos positivos de 71 subcategorias de intervenção para 94 subcategorias de desfecho. Lacunas: poucas revisões sistemáticas abordaram "Atenção Nutricional" e "Força de Trabalho". Não foram identificadas revisões sistemáticas sobre "Vigilância Alimentar e Nutricional", "Gestão das Ações de Alimentação e Nutrição", "Participação e Controle Social", "Pesquisa, Inovação e Conhecimento em Alimentação e Nutrição".


The evidence map was developed with reference to the nine guidelines of the National Food and Nutrition Policy (PNAN). The objective was to identify effective interventions to improve the health, food and nutrition conditions of the population. 101 systematic reviews were analyzed. Interventions were classified into 5 categories and 80 subcategories. Outcomes were classified into 8 categories and 119 subcategories. Positive effects of 71 intervention subcategories were reported for 94 outcome subcategories. Gaps: few systematic reviews addressed "Nutritional Care" and "Workforce". No systematic reviews were identified on "Food and Nutrition Surveillance", "Management of Food and Nutrition Actions", "Participation and Social Control", "Research, Innovation and Knowledge in Food and Nutrition".

PURPOSE: We aimed to rate the importance of outcomes from a systematic review about biosimilars in oncology from patients' perspective. METHODS: This is a qualitative research with nominal group technique. Patients with cancer were selected by convenience sampling and invited for two mediated virtual meetings in 2022. Twelve outcomes from a systematic review on biosimilars for oncology developed following a protocol were explained in plain language to participants who classified them as critical, important, or not important according to the Grading of Recommendations Assessment, Development and Evaluation approach. We employed Iramuteq software for lexical categorization of the meeting transcripts, and content analysis for interpretation. RESULTS: Five women participated (three had metastatic cancer, one non-metastatic, one recurrent). Six outcomes were classified as critical: duration of response, progression-free survival, pathological complete response, overall survival, severe adverse events, and quality of life; three as important: mortality, event-free survival, and objective response; and three as non-important: neutralizing anti-drug antibody, any adverse event, and non-neutralizing anti-drug antibody. Duration of response, pathological complete response, severe adverse events, and quality of life were considered secondary in the review protocol, but critical by the patients. The main themes influencing the importance classification were related to the disease (progression and control) and treatment (recognition and healthcare setting). CONCLUSION: Patients rated most outcomes as critical or important, some of them previously regarded as secondary by the researchers, which reinforces the need to include stakeholders' perspectives in oncology research. Aspects of the disease progression and treatment effects influenced participants' judgment on outcomes' relevance.

BACKGROUND: Sexually transmitted infections (STI) can have severe consequences. In Brazil, case management is recommended by the Clinical Protocol and Therapeutical Guidelines for Comprehensive Care for People with STIs (PCDT-IST). This study assessed the quality of PCDT-IST (2021) and reviewed the main recommendations for the management of STI that cause urethral discharge compared with the World Health Organization (WHO) STI Guidelines. METHODS: The PCDT-IST (2021) quality was independently assessed by 4 appraisers using the Appraisal of Guidelines Research and Evaluation instrument, version II (AGREE II). The PCDT-IST (2021) and the WHO Guidelines for the Management of Symptomatic STI (2021) were compared considering 14 different assessment domains. RESULTS: The PCDT-IST (2021) scores in the AGREE II domains were: Rigor of Development (58%), Applicability (35%), Editorial Independence (38%), Scope and Purpose (78%), Stakeholder Involvement (74%), and Clarity and Presentation (82%). The overall score was 67%, and all appraisers recommended the Brazilian guideline. Regarding the PCDT-IST (2021) and the WHO STI Guidelines (2021) comparation, 10 domains would be relevant for further reviewing the Brazilian recommendations: Diagnostic tests; Etiological approach; Treatment for recurrent urethral discharge; Treatment for urethritis without etiological agent identification; Treatment for gonococcal urethritis; Treatment for chlamydial urethritis; Retreatment for gonococcal infections; Treatment for Mycoplasma genitalium urethritis; Treatment for Trichomonas vaginalis urethritis; 10. Flowcharts. CONCLUSIONS: The PCDT-IST (2021) has a reasonable degree of quality. However, the domains of Applicability, Rigor of Development, and Editorial Independence must be better ensured. The guidelines comparison will help to select key topics that should be addressed with priority in the following national STI guidelines updates.

BACKGROUND: Evidence-informed policymaking (EIPM) requires a set of individual and organizational knowledge, skills and attitudes that should be articulated with background factors and needs. In this regard, the development of an EIPM competency profile is important to support the diagnosis, planning and implementation of EIPM. PURPOSE: To present the process and outcomes of the development of an EIPM competency profile by an expert committee, to be applied in different contexts of the Brazilian Health System. METHODS: A committee of experts in EIPM shared different views, experiences and opinions to develop an EIPM competency profile for Brazil. In six consensus workshops mediated by facilitators, the committee defined from macro problems to key actions and performances essential for the competency profile. The development steps consisted of: (1) Constitution of the committee, including researchers, professionals with practical experience, managers, and educators; (2) Development of a rapid review on EIPM competency profiles; (3) Agreement on commitments and responsibilities in the processes; (4) Identification and definition of macro problems relating to the scope of the competency profile; and (5) Outlining of general and specific capacities, to be incorporated into the competency profile, categorized by key actions. RESULTS: The development of the EIPM competency profile was guided by the following macro problems: (1) lack of systematic and transparent decision-making processes in health policy management; (2) underdeveloped institutional capacity for knowledge management and translation; and (3) incipient use of scientific evidence in the formulation and implementation of health policies. A general framework of key actions and performances of the EIPM Competency Profile for Brazil was developed, including 42 specific and general key actions distributed by area of activity (Health Management, Scientific Research, Civil Society, Knowledge Translation, and Cross-sectional areas). CONCLUSIONS: The competency profile presented in this article can be used in different contexts as a key tool for the institutionalization of EIPM.

OBJECTIVES: To synthesize existing knowledge on the features of, and approaches to, health intelligence, including definitions, key concepts, frameworks, methods and tools, types of evidence used, and research gaps. STUDY DESIGN AND SETTING: We applied a critical interpretive synthesis methodology, combining systematic searching, purposive sampling, and inductive analysis to explore the topic. We conducted electronic and supplementary searches to identify records (papers, books, websites) based on their potential relevance to health intelligence. The key themes identified in the literature were combined under each of the compass subquestions and circulated among the research team for discussion and interpretation. RESULTS: Of the 290 records screened, 40 were included in the synthesis. There is no clear definition of health intelligence in the literature. Some records describe it in similar terms as public health surveillance. Some focus on the use of artificial intelligence, while others refer to health intelligence in a military or security sense. And some authors have suggested a broader definition of health intelligence that explicitly includes the concepts of synthesis of research evidence for informed decision making. CONCLUSION: Rather than developing a new or all-encompassing definition, we suggest incorporating the concept and scope of health intelligence within the evidence ecosystem.

BACKGROUND: Health evidence needs to be communicated and disseminated in a manner that is clearly understood by decision-makers. As an inherent component of health knowledge translation, communicating results of scientific studies, effects of interventions and health risk estimates, in addition to understanding key concepts of clinical epidemiology and interpreting evidence, represent a set of essential instruments to reduce the gap between science and practice. The advancement of digital and social media has reshaped the concept of health communication, introducing new, direct and powerful communication platforms and gateways between researchers and the public. The objective of this scoping review was to identify strategies for communicating scientific evidence in healthcare to managers and/or population. METHODS: We searched Cochrane Library, Embase®, MEDLINE® and other six electronic databases, in addition to grey literature, relevant websites from related organizations for studies, documents or reports published from 2000, addressing any strategy for communicating scientific evidence on healthcare to managers and/or population. RESULTS: Our search identified 24 598 unique records, of which 80 met the inclusion criteria and addressed 78 strategies. Most strategies focused on risk and benefit communication in health, were presented by textual format and had been implemented and somehow evaluated. Among the strategies evaluated and appearing to yield some benefit are (i) risk/benefit communication: natural frequencies instead of percentages, absolute risk instead relative risk and number needed to treat, numerical instead nominal communication, mortality instead survival; negative or loss content appear to be more effective than positive or gain content; (ii) evidence synthesis: plain languages summaries to communicate the results of Cochrane reviews to the community were perceived as more reliable, easier to find and understand, and better to support decisions than the original summaries; (iii) teaching/learning: the Informed Health Choices resources seem to be effective for improving critical thinking skills. CONCLUSION: Our findings contribute to both the knowledge translation process by identifying communication strategies with potential for immediate implementation and to future research by recognizing the need to evaluate the clinical and social impact of other strategies to support evidence-informed policies. Este mapa de evidências tem como objetivo apresentar os potenciais benefícios da atividade física para a saúde da população. ● Foram incluídas 131 revisões sistemáticas, sendo os estudos primários realizados principalmente nos Estados Unidos (46,5%). ● As intervenções foram classificadas em 4 categorias e 30 subcategorias. Observou-se maior frequência da categoria "exercícios físicos aeróbicos", e da subcategoria "práticas integrativas e complementares de saúde". ● Os desfechos foram classificados em clínicos, não clínicos e eventos adversos. Desfechos clínicos foram relatados em 100 revisões sistemáticas, com destaque para sintomas e transtornos depressivos, índice de massa corporal, e sintomas e transtornos de ansiedade. Desfechos não clínicos foram relatados em 70 revisões sistemáticas, com destaque para qualidade de vida. Eventos adversos foram relatados em 14 revisões sistemáticas. ● Três categorias de intervenções apresentaram efeito positivo acima de 50% para os desfechos avaliados: Exercícios aeróbicos; Exercícios físicos não especificados; Intervenções combinadas.


This evidence map aims to present the potential benefits of physical activity for the health of the population. ● 131 systematic reviews were included, with the primary studies conducted mainly in the United States (46.5%). ● Interventions were classified into 4 categories and 30 subcategories. There was a higher frequency of the category "aerobic physical exercises", and of the subcategory "integrative and complementary health practices". ● Outcomes were classified into clinical, non-clinical and adverse events. Clinical outcomes were reported in 100 systematic reviews, with emphasis on depressive symptoms and disorders, body mass index, and anxiety symptoms and disorders. Non-clinical outcomes were reported in 70 reviews systematic, with emphasis on quality of life. Adverse events were reported in 14 systematic reviews. ● Three categories of interventions showed a positive effect above 50% for the evaluated outcomes: Aerobic exercises; Unspecified physical exercises; Combined interventions.

Contexto: As famílias participantes do Programa Bolsa Família (PBF) assumem compromissos de condicionalidade na área de saúde, com a finalidade de promover o acesso aos direitos fundamentais básicos. As ações básicas ofertadas compreendem o pré-natal das gestantes, o acompanhamento do crescimento e desenvolvimento infantil e a imunização das crianças menores de 7 anos. Essas famílias podem ter maior dificuldade de acessar e frequentar serviços de saúde, e as condicionalidades podem garantir a oferta das ações básicas, e potencializar a melhoria da qualidade de vida. Desse modo, é necessário conhecer as barreiras que impedem as famílias beneficiadas do PBF cumprirem com as condicionalidades de saúde. Pergunta: Quais são as principais barreiras encontradas pelos beneficiários do Programa Bolsa Família que impedem ou dificultam o cumprimento das condicionalidades de saúde? Métodos: As buscas de estudos foram realizadas em maio de 2023, na Biblioteca Virtual em Saúde, no Google Acadêmico e na Biblioteca Digital Brasileira de Teses e Dissertações. Utilizando atalhos de revisão rápida, apenas a seleção de estudos foi realizada em duplicata e de modo independente. Não foi realizada a avaliação da qualidade metodológica dos estudos incluídos. Resultados: De 1.438 registros identificados nas buscas das bases de dados, 9 estudos foram incluídos. As barreiras foram categorizadas como: barreiras ao acesso aos serviços de saúde (relacionadas ao próprio serviço ou a questões de vulnerabilidade social) e barreiras de comunicação. Considerações finais: Os estudos incluídos apontaram diversas barreiras ao cumprimento das condicionalidades de saúde do PBF pelas(os) beneficiárias(os). De modo geral, eles analisaram o significado do programa a partir da perspectiva de beneficiárias. Nenhum estudo objetivou diretamente avaliar barreiras ao cumprimento das condicionalidades.


Context: Families participating in the Bolsa Família Program (PBF) assume conditionality commitments in the health area, with the aim of promoting access to basic fundamental rights. The basic actions offered include prenatal care for pregnant women, monitoring of child growth and development and immunization of children under 7 years old. These families may have greater difficulty accessing and attending health services, and conditionalities can guarantee the provision of basic actions, and enhance the improvement of quality of life. Thus, it is necessary to know the barriers that prevent families benefiting from the PBF from complying with health conditionalities. Question: What are the main barriers encountered by beneficiaries of the Bolsa Família Program that prevent or hinder compliance with health conditionalities? Methods: Searches for studies were carried out in May 2023, in the Virtual Health Library, in Google Scholar and in the Brazilian Digital Library of Theses and Dissertations. Using rapid review shortcuts, only the selection of studies was performed in duplicate and independently. The evaluation of the methodological quality of the included studies was not carried out. Results: Of 1,438 records identified in database searches, 9 studies were included. Barriers were categorized as: barriers to accessing health services (related to the service itself or issues of social vulnerability) and communication barriers. Final considerations: The included studies pointed out several barriers to the fulfillment of PBF health conditionalities by beneficiaries. Overall, they analyzed the meaning of the program from the perspective of female beneficiaries. No study directly aimed to assess barriers to compliance with conditionalities.

Objective: To map the policies related to the prevention and control of antimicrobial resistance from a human health perspective in Brazil and systematize the historical course of these policies. Method: A scoping review was performed following Joana Briggs Institute and PRISMA guidelines. A literature search was performed in December 2020 in the LILACS, PubMed and EMBASE databases. The terms "antimicrobial resistance" AND "Brazil" as well as their synonyms were used. Using the same keywords, Brazilian government websites were searched for documents published until December 2021. Studies of all designs were included, with no language or date restrictions. Clinical documents, reviews and epidemiological studies that did not focus on antimicrobial resistance management policies in Brazil were excluded. Categories based on World Health Organization documents were used for data systematization and analysis. Results: In Brazil, policies related to antimicrobial resistance such as the National Immunization Program and hospital infection control programs can be traced back to before the creation of the Unified Health System. In the late 1990s and 2000s, the first specific policies on antimicrobial resistance (surveillance networks and programs) and education strategies were established; especially noteworthy is The National Action Plan for the Prevention and Control of Antimicrobial Resistance in the Single Health Scope (PAN-BR) of 2018. Conclusions: Despite the long history of policies related to antimicrobial resistance in Brazil, gaps were identified, particularly in monitoring the use of antimicrobials and surveillance of antimicrobial resistance. The PAN-BR, the first government document prepared from a One Health perspective, represents an important milestone.


Objetivo: Determinar qué políticas de prevención y control de la resistencia a los antimicrobianos desde la perspectiva de la salud humana se han adoptado en Brasil y sistematizar su evolución histórica. Método: Se hizo una revisión exploratoria según las directrices del Instituto Joana Briggs y de PRISMA. La búsqueda bibliográfica se realizó en diciembre del 2020 en las bases de datos LILACS, PubMed y EMBASE. Se utilizaron los términos "antimicrobial resistance" AND "Brazil" y sinónimos. Se efectuó una investigación documental con los mismos términos en los sitios web del gobierno brasileño hasta diciembre del 2021. Se incluyeron estudios de todos los diseños, sin restricciones de idioma ni de fecha. Se excluyeron los documentos clínicos, revisiones y estudios epidemiológicos que no hicieran referencia a las políticas de gestión de la resistencia a los antimicrobianos en Brasil. Para la recolección y el análisis de datos se establecieron categorías basadas en documentos de la Organización Mundial de la Salud. Resultados: Desde antes de la creación del Sistema Único de Salud, Brasil tenía políticas de resistencia a los antimicrobianos, como el Programa Nacional de Inmunización y los programas de control de infecciones hospitalarias. A finales de las décadas de 1990 y 2000 se establecieron las primeras políticas específicas de resistencia a los antimicrobianos (redes y programas de vigilancia) y estrategias de educación. Entre ellas se destaca el Plan de Acción Nacional de Prevención y Control de la Resistencia a los Antimicrobianos en el marco del enfoque de "Una salud" (PAN-BR) del 2018. Conclusiones: A pesar de la larga historia de las políticas de resistencia a los antimicrobianos en Brasil, se encontraron lagunas, particularmente en el seguimiento del uso de antimicrobianos y la vigilancia de la resistencia a los mismos. El PAN-BR, primer documento gubernamental elaborado desde la perspectiva de "Una salud", marca un hito en las políticas formuladas en Brasil.

Objective: To synthesize the evidence about the effects of telehealth interventions provided through smart-phone apps and text messages on the behavior of adults regarding healthy food consumption. Method: A rapid systematic review of the literature was performed through searches in nine electronic databases to identify systematic reviews published in English, Portuguese, and Spanish that evaluated telehealth strategies compared to face-to-face interventions to improve dietary intake in the adult population (18 to 59 years old). Searches were performed in November 2020 and updated in April 2022. The included systematic reviews were assessed for methodological quality using the AMSTAR 2 tool. Results: Five systematic reviews were included. Methodological quality was moderate in one review and critically low in four. There was a dearth of studies comparing the use of telehealth strategies with face-to-face interventions for the promotion of healthy eating in adults. The most consistent results refer to an increase in the consumption of fruits and vegetables with the use of an app or text messages, in addition to improvement in the dietary habits of people with diabetes or glucose intolerance with the use of text messages. Conclusion: Positive effects were observed on healthy eating outcomes for most interventions using mobile apps or text messages; however, the findings refer to a few clinical trials with small samples that were analyzed in the systematic reviews covered in the present rapid review, most of which had low methodological quality. Thus, the current knowledge gap warrants the performance of further methodologically robust studies.


Objetivo: Sintetizar la evidencia sobre los efectos de las intervenciones de telesalud ofrecidas por medio de aplicaciones de telefonía móvil y mensajes de texto en el comportamiento de la población adulta relacionado con una alimentación saludable. Métodos: Se realizó un examen rápido mediante búsquedas en nueve bases bibliográficas electrónicas para localizar revisiones sistemáticas publicadas en español, inglés y portugués en las que se evaluaran estrategias de telesalud en comparación con atención presencial para mejorar la alimentación de la población adulta (de 18 a 59 años). Las búsquedas se realizaron en noviembre del 2020 y se actualizaron en abril del 2022. La calidad metodológica de las revisiones sistemáticas incluidas se evaluó con la herramienta AMSTAR 2. Resultados: Se incluyeron cinco revisiones sistemáticas, una con un grado de confianza moderado y las otras con un grado de confianza sumamente bajo. Se comprobó una falta de estudios en los que se compararan el uso de estrategias de telesalud con la atención presencial para promover la alimentación saludable de la población adulta. Los resultados más coherentes se refieren al aumento del consumo de frutas y verduras con el uso de aplicaciones móviles o de mensajes de texto, así como a la mejora del patrón alimentario de las personas con diabetes o con intolerancia a la glucosa con el uso de mensajes de texto. Conclusión: La mayoría de los análisis de las intervenciones en las que se emplearon aplicaciones de telefonía móvil o mensajes de texto mostraron efectos positivos en los resultados relativos a una alimentación saludable. Sin embargo, estos hallazgos se refieren a unos pocos ensayos clínicos con pequeñas muestras de participantes incluidos en las revisiones sistemáticas del presente examen rápido, en su mayoría de baja calidad metodológica. En conclusión, existe una laguna en los conocimientos y es importante realizar estudios con una metodología más sólida.

Objective: To identify strategies to increase adherence to systemic arterial hypertension (SAH) treatment and describe the barriers and facilitators for implementing these strategies in primary health care (PHC). Method: A rapid evidence review was performed. We included systematic reviews with or without meta-analyses, published in English, Spanish or Portuguese, covering adults (age 18 to ≤ 60 years) with SAH followed in the PHC. Searches were performed in nine databases in December 2020 and updated in April 2022. The systematic reviews were assessed for methodological quality using the AMSTAR 2 tool. Results: Fourteen systematic reviews on treatment adherence strategies and three on barriers and facilitators for implementation were included. Regarding methodological quality, one systematic review was classified as moderate, four as low, and the others as critically low. Four strategies were identified as options for health policies: actions performed by pharmacists; actions performed by non-pharmaceutical health professionals; self-monitoring, use of mobile apps, and text messages; and subsidies for the purchase of medicines. Low digital literacy, limited access to the internet, work process and incipient training were barriers for professionals. The users' educational and health literacy levels, accessibility to health services and good relationships with professionals were facilitators. Conclusions: Positive effects of strategies related to pharmaceutical care, self-monitoring, and the use of cell phone applications and text messages were identified to increase adherence to the treatment of SAH in the context of PHC. However, for implementation purposes, it is necessary to consider barriers and facilitators, in addition to the methodological limitations of the analyzed systematic reviews.


Objetivo: Buscar estrategias para aumentar la adhesión al tratamiento de la hipertensión arterial sistémica y describir las barreras y los elementos facilitadores para la implementación de esas estrategias en el ámbito de la atención primaria de salud. Métodos: Se realizó una revisión rápida de la evidencia. Se incluyeron revisiones sistemáticas, con o sin metaanálisis, publicadas en español, inglés o portugués, en las cuales se evaluaban a personas adultas (de 18 a ≤ 60 años) con hipertensión arterial sistémica observadas en el ámbito de la atención primaria de salud. Las búsquedas se realizaron en nueve bases de datos en diciembre del 2020 y se actualizaron en abril del 2022. Se evaluó la calidad metodológica de las revisiones sistemáticas incluidas con la herramienta AMSTAR 2. Resultados: Se incluyeron 14 revisiones sistemáticas sobre estrategias de adhesión al tratamiento y tres sobre obstáculos y elementos facilitadores para la implementación. La calidad metodológica fue moderada en una revisión sistemática, baja en cuatro y críticamente baja en las demás. Se identificaron cuatro estrategias como opciones para las políticas de salud: medidas tomadas por farmacéuticos; medidas tomadas por profesionales no farmacéuticos; autocontrol, uso de aplicaciones para teléfonos celulares y mensajes de texto; y oferta de subvenciones para la compra de medicamentos. La escasa alfabetización digital, el acceso limitado a internet y los procesos de trabajo y formación incipiente fueron obstáculos para los profesionales. Los elementos facilitadores fueron el nivel de educación y de conocimientos de los usuarios en materia de salud, la accesibilidad a los servicios de salud y las buenas relaciones con los profesionales. Conclusiones: Se observaron efectos positivos de las estrategias relacionadas con la atención farmacéutica, el autocontrol y el uso de aplicaciones de telefonía celular y mensajería de texto para aumentar la adhesión al tratamiento de la hipertensión arterial sistémica en el ámbito de la atención primaria de salud. Sin embargo, para fines de implementación, es necesario considerar los obstáculos y los elementos facilitadores, además de las limitaciones metodológicas de las revisiones sistemáticas analizadas.

In Brazil, there have been some initiatives to improve the development of Ministry of Health clinical protocols and therapeutic guidelines (PCDTs in Portuguese, and clinical practice guidelines-CPGs, in English) and their implementation so that best practices can be disseminated and adopted at multiple levels of health systems. One of the initiatives was to conduct a pilot project to improve the format of these CPGs. The objective of this article is to present the processes and results of the pilot project, including the development of a new standardized format for CPGs to promote national dissemination and uptake. The pilot project was designed in three phases: identification and selection of strategies to effectively implement clinical practice guidelines, definition of the ideal characteristics for the format of CPGs, and development and implementation of the new format. Initially, an overview of systematic reviews was conducted to map the global evidence on the effectiveness of dissemination and implementation strategies of CPGs. Among the most effective interventions, a low-cost strategy was selected to improve the format of CPGs, namely a full format and a short format. The two formats were evaluated for usefulness and acceptability by professionals who use or develop CPGs, and after several reiterations, the formats were finalized, considering the progression of care (from diagnosis of the disease to treatment, including specific technologies indicated in each stage of the disease). Related to the technical aspects, the visual presentation of the CPGs was improved, ensuring that key information was easily identified for decision-making by end users. The initial phase of implementation involved 33 clinical conditions, equating to approximately 20% of published CPGs. It is anticipated that disseminating the CPGs in the new formats will promote the accessibility of information and implementation of standardized CPGs by health professionals in the public health sector (servicing more than 210 million Brazilians). Further research should be considered to determine the impact of the use of the new CPGs formats, contributing to the knowledge base related to the implementation of guidelines in Brazil and internationally.

Contexto O Ministério da Saúde, por meio da Portaria GM/MS nº 28, de 20 de janeiro de 2023, declarou a Emergência em Saúde Pública de Importância Nacional (ESPIN) em decorrência de desassistência à população Yanomami. Desde então, entre 19 crianças indígenas de seis meses e cinco anos de idade com desnutrição grave que foram atendidas pela Casa de Apoio à Saúde Indígena (Casai) de Boa Vista/RR, 15 (78%) ganharam peso e estão evoluindo de quadros graves para moderados de desnutrição, a partir dos protocolos e diretrizes do Ministério da Saúde. No Brasil, a recomendação do Ministério da Saúde para o tratamento da desnutrição grave inclui esquemas para alimentação utilizando preparado alimentar inicial ­ F-75 (75 kcal e 0,9g de proteína/100ml) e o preparado alimentar para crescimento rápido - F-100 (100 kcal e 2,9g de proteína/100ml). O guia de prática clínica (GPC) da Organização Mundial da Saúde (OMS) refere que a maioria das crianças de seis meses ou mais com desnutrição aguda grave, sem complicações médicas, pode ser tratada com segurança por meio de alimentos terapêuticos prontos para uso (Ready-to-use-therapeutic-food - RUTF), sem necessidade de internação em unidade de saúde. Pergunta Qual é a segurança e eficácia de RUTF, no curto e longo prazo, para o tratamento de crianças menores de cinco anos com desnutrição grave? Métodos Após realização de protocolo de pesquisa, cinco bases da literatura eletrônica foram acessadas em março de 2023 para identificar estudos que pudessem oferecer resposta à pergunta de investigação. Utilizando atalhos de revisão rápida, foram realizados os processos de seleção de estudos, extração de dados e avaliação da qualidade metodológica das RS incluídas com a ferramenta AMSTAR 2. Resultados Duas revisões sistemáticas (RS) foram incluídas, sendo avaliadas como de confiança baixa e moderada. Uma RS realizou metanálises, indicando que não houve diferença entre os grupos RUTF e F-100 para ganho de peso e mortalidade. A maioria dos estudos primários, 3 de 5 ensaios clínicos randomizados (ECR) citados nas RS mostram que não houve diferença entre os grupos RUTF e F-100 quanto à altura, circunferência do braço e desnutrição aguda. Dois estudos primários incluídos nas RS indicam que o tratamento com RUFT apresenta os seguintes resultados positivos: maior probabilidade de atingir o escore Z de peso para altura; menor probabilidade de recaída e de recidiva; melhora no ganho de peso e redução do tempo de recuperação. Ressalta-se que um destes ECR apresenta conflito de interesses. Nenhuma RS analisou desfechos acerca da segurança do uso de RUTF. Dois GPC da Organização Mundial da Saúde foram selecionados mediante busca manual. Um GPC recomenda que o RUTF pode ser utilizado para crianças com desnutrição aguda grave que apresentam diarreia aguda ou persistente da mesma forma que crianças sem diarreia, quer sejam tratadas como pacientes internados ou ambulatoriais. O outro GPC recomenda que para tratamento ambulatorial de crianças com desnutrição aguda grave seja utilizado o RUTF padrão (com pelo menos 50% de proteína proveniente de laticínios). Considerações finais Esta revisão rápida aponta que há poucos estudos sobre o tema. As evidências são insuficientes para afirmar quais intervenções (RUTF ou F-100) são mais eficazes. As recomendações dos GPC também são baseadas em evidências escassas. Portanto, não há evidências que sustentem o uso do RUTF em detrimento da F-100, pois não há comprovação de maior eficácia e nenhum estudo abordou a segurança a longo prazo do uso de RUTF.


Context: The Ministry of Health, through Ordinance GM/MS No. 28, of January 20, 2023, declared a Public Health Emergency of National Importance (ESPIN) due to lack of assistance to the Yanomami population. Since then, among 19 indigenous children aged between six months and five years old with severe malnutrition who were assisted by the Casa de Apoio à Saúde Indígena (Casai) in Boa Vista/RR, 15 (78%) have gained weight and are evolving from serious conditions. for moderate malnutrition, based on the protocols and guidelines of the Ministry of Health. In Brazil, the recommendation of the Ministry of Health for the treatment of severe malnutrition includes feeding schemes using initial food preparation - F-75 (75 kcal and 0.9g of protein/100ml) and food preparation for rapid growth - F-100 (100 kcal and 2.9g of protein/100ml). The clinical practice guide (CPG) of the World Health Organization (WHO) states that most children aged six months or more with severe acute malnutrition, without medical complications, can be safely treated with ready-to-use therapeutic foods ( Ready-to-use-therapeutic-food - RUTF), without the need for hospitalization in a health unit. Question: What is the safety and efficacy of RUTF, in the short and long term, for the treatment of children under five with severe malnutrition? Methods: After carrying out a research protocol, five electronic literature databases were accessed in March 2023 to identify studies that could answer the research question. Using rapid review shortcuts, the processes of study selection, data extraction and methodological quality assessment of the SR included with the AMSTAR 2 tool were carried out. Results: Two systematic reviews (SR) were included, being assessed as having low and moderate confidence. An SR performed meta-analyses, indicating that there was no difference between the RUTF and F-100 groups for weight gain and mortality. Most of the primary studies, 3 out of 5 randomized clinical trials (RCTs) cited in the SR show that there was no difference between the RUTF and F-100 groups in terms of height, arm circumference and acute malnutrition. Two primary studies included in the SR indicate that treatment with RUFT has the following positive outcomes: increased likelihood of achieving weight-for-height Z-score; lower probability of relapse and recurrence; improvement in weight gain and reduced recovery time. It should be noted that one of these RCTs has a conflict of interest. No SR analyzed outcomes regarding the safety of using RUTF. Two World Health Organization CPGs were selected by manual search. A CPG recommends that RUTF can be used for children with severe acute malnutrition who have acute or persistent diarrhea in the same way as children without diarrhea, whether they are treated as inpatients or outpatients. The other CPG recommends that standard RUTF be used for outpatient treatment of children with severe acute malnutrition (with at least 50% protein from dairy products). Final considerations: This quick review points out that there are few studies on the subject. There is insufficient evidence to state which interventions (RUTF or F-100) are most effective. The CPGs recommendations are also based on sparse evidence. Therefore, there is no evidence to support the use of RUTF over F-100, as there is no evidence of greater efficacy and no study has addressed the long-term safety of using RUTF.

BACKGROUND: Evidence-informed policy-making (EIPM) requires a set of individual and organizational capacities, linked with background factors and needs. The identification of essential knowledge, skills and attitudes for EIPM can support the development of competency profiles and their application in different contexts. PURPOSE: To identify elements of competency (knowledge, skills and attitudes) for EIPM, according to different professional profiles (researcher, health professional, decision-maker and citizen). METHODS: Rapid umbrella review. A structured search was conducted and later updated in two comprehensive repositories (BVSalud and PubMed). Review studies with distinctive designs were included, published from 2010 onwards, without language restrictions. Assessment of the methodological quality of the studies was not performed. A meta-aggregative narrative synthesis was used to report the findings. RESULTS: Ten reviews were included. A total of 37 elements of competency were identified, eight were categorized as knowledge, 19 as skills and 10 as attitudes. These elements were aggregated into four competency profiles: researcher, health professional, decision-maker and citizen. The competency profiles included different sets of EIPM-related knowledge, skills and attitudes. STRENGTHS AND LIMITATIONS: This study is innovative because it aggregates different profiles of competency from a practical perspective, favouring the application of its results in different contexts to support EIPM. Methodological limitations are related to the shortcuts adopted in this review: complementary searches of the grey literature were not performed, and the study selection and data extraction were not conducted in duplicate. FINAL CONSIDERATIONS: CONCLUSIONS AND IMPLICATIONS OF THE FINDINGS: EIPM requires the development of individual and organizational capacities. This rapid review contributes to the discussion on the institutionalization of EIPM in health systems. The competency profiles presented here can support discussions about the availability of capacity and the need for its development in different contexts.

O problema: A Política Nacional de Promoção à Saúde indica entre seus objetivos específicos o estabelecimento de estratégias de comunicação social e de mídia para fortalecer os princípios e as ações de promoção da saúde. A comunicação eficaz, integrada e coordenada possui papel fundamental na construção de um futuro mais saudável para as pessoas. Esta síntese rápida de evidências tem como objetivo apresentar resultados de estudos que analisaram os efeitos de campanhas em mídia de massa ou em comunidades. Opções para enfrentar o problema: Buscas realizadas em 25/11/2023, em quatro bases de dados, identificaram 749 registros. Após processo de seleção e elegibilidade, quarenta e seis revisões sistemáticas (RS) foram incluídas nesta síntese narrativa. Os resultados foram organizados em onze opções para políticas na promoção de comportamentos e estilo de vida saudáveis ou prevenção de doenças. Efeitos positivos foram relatados nas RS para uma variedade de estratégias, no entanto, muitas incertezas também foram apontadas. Quanto à qualidade metodológica, a grande maioria das RS foi classificada como criticamente baixa.


The problem: The National Health Promotion Policy indicates among its specific objectives the establishment of social communication and media strategies to strengthen the principles and actions of health promotion. Effective, integrated and coordinated communication plays a fundamental role in building a healthier future for people. This rapid synthesis of evidence aims to present results from studies that have analyzed the effects of mass media or community campaigns. Options to face the problem: Searches carried out on 11/25/2023, in four databases, identified 749 records. After the selection and eligibility process, forty-six systematic reviews (SR) were included in this narrative synthesis. The results were organized into eleven options for policies in promoting healthy behaviors and lifestyles or disease prevention. Positive effects were reported on SR for a variety of strategies, however, many uncertainties were also pointed out. As for methodological quality, the vast majority of SRs were classified as critically low.

The Supporting Policy Relevant Reviews and Trials Tools were used to define the problem and the search strategy, which was carried out in the following databases: PubMed, Health Systems Evidence, Campbell, Cochrane Collaboration, Rx for Change Database, and PDQ-Evidence. Selection and assessment of methodological quality was performed by two independent reviewers. The results were presented in a narrative synthesis. This study selected 19 systematic reviews that pointed out four strategies to address the judicialization of health in Brazil: 1) Rapid response service, 2) Continuous education program, 3) Mediation service between the parties involved, and 4) Adoption of a computer-based, online decision-making support tool and patient-mediated interventions. This study therefore presented and characterized four options that can be considered to address the judicialization of health. The implementation of these options must ensure the participation of different actors, reflecting on different contexts and the impact on the health system. The availability of human and financial resources and the training of teams are critical points for the successful implementation of the options.
