RESUMO

Two studies were carried out to evaluate heat-killed Mycobacterium vaccae SRL172 as an immunotherapeutic agent for patients with metastatic, post-nephrectomy, renal cell carcinoma. In the first study, 60 patients in France and the UK received injections of SRL172, and their survival was compared with that of historical controls who had been treated either with biological response modifiers (IL-2, IFN-alpha) or chemotherapy. In the second study, 36 patients were randomised to receive treatment with IL-2 alone or IL-2 plus SRL172. Survival and adverse events related to the treatments were assessed and compared between treatment groups. The first study showed that those treated with SRL172 alone survived equally as long as those receiving IL-2 or IFN-alpha and both treatment groups survived longer than those on chemotherapy (p<0.001), a result supported by Cox's proportional hazards regression analysis. The second study, stopped early due to drug supply issues, showed that the addition of SRL172 to IL-2 made no difference to survival compared to IL-2 alone, in the limited numbers treated. Adverse events occurring in those receiving SRL172 in the first study were mild and in the second study those receiving IL-2 alone had significantly more adverse events than those receiving SRL172 plus IL-2 (p<0.001). It is concluded that SRL172 may have activity in metastatic renal cancer and has very low toxicity, making it worthy of further study.

AIMS: The contribution of the glutamate decarboxylase (GAD) acid resistance system to survival and growth of Listeria monocytogenes LO28 in modified atmosphere-packaged foods was examined. METHODS AND RESULTS: The survival and growth of the wild-type LO28 and four GAD deletion mutants (DeltagadA, DeltagadB, DeltagadC, DeltagadAB) in packaged foods (minced beef, lettuce, dry coleslaw mix) during storage at 4, 8 and 15 degrees C were studied. Survival and growth patterns varied with strain, product type, gas atmosphere and storage temperature. In minced beef, the wild-type LO28 survived better (P < 0.05) than the GAD mutant strains at 8 and 15 degrees C. In both packaged vegetables at all storage temperatures, the wild-type strain survived better (P < 0.05) than the double mutant DeltagadAB. The requirement for the individual gad genes varied depending on the packaged food. In the case of lettuce, gadA played the most important role, while the gadB and gadC genes played the greatest role in packaged coleslaw (at 15 degrees C). CONCLUSIONS: This work demonstrates that elements of the GAD system play significant roles in survival of L. monocytogenes LO28 during storage in modified atmosphere-packaged foods. SIGNIFICANCE AND IMPACT OF THE STUDY: A better understanding of how L. monocytogenes behaves in modified atmosphere-packaged foods, and how it responds to elevated carbon dioxide atmospheres.

Temozolomide is an alkylating agent with activity in the treatment of melanoma metastatic to the brain. Lomustine is a nitrosurea that crosses the blood brain barrier and there is evidence to suggest that temozolomide may reverse resistance to lomustine. A multicentre phase I/II study was conducted to assess the maximum-tolerated dose (MTD), safety and efficacy of the combination of temozolomide and lomustine in melanoma metastatic to the brain. Increasing doses of temozolomide and lomustine were administered in phase I of the study to determine the MTD. Patients were treated at the MTD in phase II of the study to six cycles, disease progression or unacceptable toxicity. Twenty-six patients were enrolled in the study. In phase I of the study, the MTD was defined as temozolomide 150 mg m(-2) days 1-5 every 28 days and lomustine 60 mg m(-2) on day 5 every 56 days. Dose-limiting neutropaenia and thrombocytopaenia were observed at higher doses. Twenty patients were treated at this dose in phase II of the study. No responses to therapy were observed. Median survival from starting chemotherapy was 2 months. The combination of temozolomide and lomustine in patients with brain metastases from melanoma does not demonstrate activity. The further evaluation of this combination therefore is not warranted.

AIMS: To establish the relative importance of the osmo- and cryoprotective compounds glycine betaine and carnitine, and their transporters, for listerial growth and survival, in foods and during infection. METHODS AND RESULTS: A set of Listeria monocytogenes mutants with single, double and triple mutations in the genes encoding the principal betaine and carnitine uptake systems (gbu, betL and opuC, respectively) was used to determine the specific contribution of each transporter to listerial growth and survival. Food models were chosen to represent high-risk foods of plant and animal origin i.e. coleslaw and frankfurters, which have previously been linked to major human outbreaks of listeriosis. BALB/c mice were used as an in vivo model of infection. Interestingly, while betaine appeared to confer most protection in foods, the hierarchy of transporter importance differs depending on the food type: Gbu>BetL>OpuC for coleslaw, as opposed to Gbu>OpuC>BetL in frankfurters. By contrast in the animal model, OpuC and thus carnitine, appears to play the dominant role, with the remaining systems contributing little to the infection process. CONCLUSIONS: This study demonstrates that the individual contribution of each system appears dependent on the immediate environment. In foods Gbu appears to play the dominant role, while during infection OpuC is most important. SIGNIFICANCE AND IMPACT OF THE STUDY: It is envisaged that this information may ultimately facilitate the design of effective control measures specifically targeting this pathogen in foods and during infection.

The survival and growth of Escherichia coli O157:H7 (ATCC 43888 and NCTC 12900) and Listeria monocytogenes (ATCC 19114 and NCTC 11994) during storage (4 and 8 degrees C) on ready-to-use (RTU) packaged vegetables (lettuce, swedes (rutabaga), dry coleslaw mix, soybean sprouts) were studied. The vegetables were sealed within oriented polypropylene packaging film, and modified atmospheres developed in packs during storage due to produce respiration. Survival and growth patterns were dependent on vegetable type, package atmosphere, storage temperature and bacterial strain. Populations of L. monocytogenes and E. coli O157:H7 increased (P<0.05, by 1.5 to 2.5 log cycles, depending on strain) during a 12-day storage period on shredded lettuce (8 degrees C). L. monocytogenes populations also increased (by approximately 1 log cycle) on packaged swedes, did not change significantly (P>0.05) in packages of soybean sprouts and decreased by approximately 1.5 log cycles (P<0.05) on coleslaw mix (8 degrees C). E. coli O157:H7 populations on packaged coleslaw and soybean sprouts increased (by 1.5 to 2.5 log cycles) up to day 5, but declined during subsequent storage (8 degrees C). On packaged swedes (8 degrees C), populations of E. coli O157:H7 strain ATCC 43888 increased (by approximately 1 log cycle) during storage, whereas populations of strain 12900 increased between days 2 and 5, and declined during subsequent storage. Reducing the storage temperature from 8 to 4 degrees C reduced the growth of L. monocytogenes and E. coli O157:H7 on packaged RTU vegetables. However, viable populations remained at the end of the storage period at 4 degrees C.

Escherichia coli fed-batch cultivations at 22 m3 scale were compared to corresponding laboratory scale processes and cultivations using a scale-down reactor furnished with a high-glucose concentration zone to mimic the conditions in a feed zone of the large bioreactor. Formate accumulated in the large reactor, indicating the existence of oxygen limitation zones. It is suggested that the reduced biomass yield at large scale partly is due to repeated production/re-assimilation of acetate from overflow metabolism and mixed acid fermentation products due to local moving zones with oxygen limitation. The conditions that generated mixed-acid fermentation in the scale-down reactor also induced a number of stress responses, monitored by analysis of mRNA of selected stress induced genes. The stress responses were relaxed when the cells returned to the substrate limited and oxygen sufficient compartment of the reactor. Corresponding analysis in the large reactor showed that the concentration of mRNA of four stress induced genes was lowest at the sampling port most distant from the feed zone. It is assumed that repeated induction/relaxation of stress responses in a large bioreactor may contribute to altered physiological properties of the cells grown in large-scale bioreactor. Flow cytometric analysis revealed reduced damage with respect to cytoplasmic membrane potential and integrity in cells grown in the dynamic environments of the large scale reactor and the scale-down reactor.

The growth dynamics of indigenous aerobic mesophilic populations (AMP), lactic acid bacteria (LAB) and inoculated (Listeria spp.) microbial populations on cooked and fresh vegetable products, packaged as separate entities and in combination, subjected to temperature fluctuation, were assessed. Microbial proliferation was temperature and product dependent, being most pronounced at 12 degrees C in all products with maximum growth rates of 0.140, 0.175 and 0.126 log10 CFU/g per h being identified for Listeria, aerobic mesophilic and LAB populations, respectively. Listeria spp. and AMP generally demonstrated higher rates of growth within products containing cooked vegetables. Prolonged storage at 3 degrees C resulted in a reduced ability by AMP and Listeria spp. to proliferate upon exposure to growth temperatures; this was not the case with LAB populations. Comparison of Listeria population estimates made using selective (Oxford) and non-selective (nutrient agar) identified reduced recovery on the former. The magnitude of the deviation increased with the duration of exposure of Listeria populations to 3 degrees C with recoveries on selective systems being reduced by 6.3% immediately after inoculation and 82.3% after 168 h at 3 degrees C, respectively. Growth of populations associated with exposure to abuse temperatures was not accompanied by significant changes in product colour (P < 0.05).
