BACKGROUND: Acne is a common chronic inflammatory disease, which can result in permanent scarring. Different types of treatments have been used in order to treat acne scars. However, esthetic results have proved variable. Furthermore, none of these treatments has had an impact on the underlying inflammatory process. OBJECTIVE: The main purpose of this case-report is to suggest a new potential therapy for acne scar management combining esthetic filling with an anti-inflammatory and a regenerative action. METHODS: A Platelet rich plasma (PRP) and Nanofat mixture was injected into the pathological dermis in order to treat and fill severe acne scars. RESULTS: After a one- year follow-up, skin elasticity had improved, scar reduction and a reversal of the inflammation process had been observed. CONCLUSIONS: PRP and Nanofat could represent a new and promising therapeutic approach in the treatment of the inflammatory scarring process in severe acne.

Ingrown nails come down to a conflict between the nail plate and the periungual tissue, causing pain, morbidity and impairment. The feet are mainly affected. The treatments are numerous: medical, podiatric, or surgical. Surgical management has two approaches: the techniques focused on the periungual skin and those on the nail plate. Among the latter, partial nail avulsion followed by matrix phenolization, known as "Boll's technique", is still highly regarded.

BACKGROUND: Microdermal implants are a new uprising form of body modification very close to surface body piercing. They can be placed almost everywhere on the skin, giving the appearance that the jewellery is simply stuck on the skin's surface. CASE REPORT: We report on two young patients, aged 21 and 23, who underwent surgical excision of peri-umbilical microdermal implants, while previous ones were spontaneously rejected during their 8th and 5th months of pregnancy, respectively. CONCLUSION: Young women should be careful when choosing the setting of their microdermal implants. They should be discouraged to have it performed on the abdominal area in case of wanting pregnancy.

Cutaneous reactions to tattoos are not uncommon and various histologic patterns have been reported, including lichenoid, granulomatous, eczematous, and pseudolymphomatous reactions. Such patterns may develop with highly variable delay after the tattooing procedure. We report three strikingly similar cases of a fast-occurring, tattoo-induced, cutaneous reaction strictly restricted to the red parts of the tattoos in two cases and displaying an unusual histologic pattern, i.e. pseudoepitheliomatous hyperplasia. Clinical differential diagnosis of this rare condition includes viral warts, keratoacanthoma, and verrucous carcinoma. It may be difficult to rule out the last two diagnoses and making the diagnosis usually requires full excision of the lesion, comprehensive histologic analysis, and careful follow-up.

Phrynoderma is a rare form of follicular hyperkeratosis associated with deficiencies in vitamins A or C or essential fatty acids. We report a 6-year-old boy with an unusual presentation of phrynoderma, characterized by multiple minute digitate hyperkeratoses associated with hair casts and related to a severe deficiency in vitamins A and C complicating a chronic intestinal giardiasis. The lesions responded well to oral vitamins A and C combined with albendazole treatment. Vitamin A deficiency-related phrynoderma is rare in western countries and is usually caused by digestive malabsorption resulting from large intestine resection or pancreatic failure. To our knowledge, this is the first reported instance of phrynoderma related to a chronic intestinal parasitic infection by Giardia intestinalis with intestinal malabsorption as a likely consequence.

Cutaneous metastasis of malignant melanoma usually presents as pigmented or non-pigmented, sometimes angiomatous nodules of various sizes. However, we recently observed a further example of haemorrhagic-like cutaneous metastasis, a quite infrequent clinical pattern, remarkable for the multiplicity of lesions.

Pegylated interferon alfa is a pegylated formulation of recombinant human interferon (IFN) conjugated with polyethylene-glycol (PEG). The major advantages of this formulation, compared to standard IFN, is a prolonged half-life which allows for once-weekly injection. Its antiviral efficacy in association with ribavirin as a new standard treatment of chronic hepatitis C has been recently documented. Efficacy of PEG-IFN in the therapy of HIV infection is currently being evaluated in prospective pilot studies. We describe herein the first observation of cutaneous necrosis at the sites of PEG-IFN injection in an HIV-infected patient. A 50-year-old man, HIV infected, was treated with antiretroviral bitherapy combining zidovudine and didanosine for 30 months. Weekly subcutaneous injections of PEG-IFN-alpha-2b were started at a dose of 1.5 microg/kg. Nine months later, two successive necrotizing cutaneous lesions developed at the site of injection. The cutaneous ulcerations slowly healed under local therapy without interruption or dose modification of the PEG-IFN. We review the literature on previously reported cases of cutaneous necrosis following standard or pegylated IFN-alpha injection and discuss the different pathophysiological mechanisms that might be involved.
