INTRODUCTION: Suicidal attempts are frequent during adolescence and concern the whole family, particularly parents whose role is crucial in provision of therapeutic support. Yet very few studies have been performed bearing on their lived experience. In this study, we will analyze the experience of parents after the suicidal attempt of their adolescent. PARTICIPANTS & METHODS: Qualitative study based on semi-structured interviews of 13 parents of teenagers followed in child and adolescent psychiatry for suicidal behavior. Interview included 5 to 6 open and conversational questions. We analyzed interviews, after transcription, using the Interpretative Phenomenological Analysis approach. RESULTS: The parents' wellness and behavior are directly affected by their child's mental status. When they are informed of the suicidal attempt, they feel very deep sadness, in a form of initial distress which is part of the grieving process regarding their image of idealized parents. They feel anger against the teenager, their anger is also directed against family members and close friends as well as against caregivers. Given the uncertainty, they doubt their own educative and empathetic competences, and lack confidence in their capacity for providing help. They express their need for help and support with regard to understanding their child's suicidal attempt, with regard to the emotional turmoil they experience and in reinforcing their competencies as carers. CONCLUSION: It appears that parents are extremely touched by their teenager suicidal attempt. They need a personal follow up, including familial medical care but also a personal space of support. Parents group, as well as psycho educative intervention, are also welcomed to give efficient method to lift their child.

RESUMO

At the Symposium entitled "Health, Culture, and Quality of Life" held in Hanoi in 1993, the authors of this report met the physicians in charge of the Traditional Medicine Institute of Hanoi which works with the World Health Organization for traditional medicine. After an historical overview of the foundation of this center providing research, care, and training under the auspices of the Hanoi School of Medicine, Professor Hoang Bao Chau described a practice based on revision of ancestral traditions in accordance with scientific methods, the resulting synthesis being up-to-date yet in keeping with heritage. This novel and pragmatic approach, which takes into account the complexity of man, offers an opportunity to reflect on the current status of medical practices in the world and on their adaptation to each population at a time when the experience of the Other seems almost impossible to communicate.

We have used as depression criteria those of Pichot and Hassan, described during the Sint-Moritz Symposium in 1973 on masked depression. According to their clinical experience in Zaïre and Senegal, the authors consider the possibility that the "bouffée délirante" (acute psychotic reaction), frequent in black Africa, is in fact a manifestation of depression. They remind the five criterias of Pichot and Hassan, i.e.: 1 degree evidence of depression symptoms; 2 degrees a background and a particular underlying personality; 3 degrees evolutional characteristics; 4 degrees familial antecedents and hereditary factors; 5 degrees the response to antidepressive treatment. They present ten cases of acute psychosis: five from Zaïre and five from Senegal. These ten patients have been successfully treated with antidepressive drugs imipramine type, without any neuroleptic drugs; the outcome has always been a rapid remission. Some socio-cultural references are described; it permits to better comprehend the psychological frame of African people. An attempt to psychodynamically interpret the results ends the article.

RESUMO

The Psychopharmacological Studies Group of Fann, Dakar, considered the cultural expression of mental illness as it would help for a new definition of the nosographic frames, and inside of them, the appreciation of symptoms. This new definition would be applied "universally" if it may be considered that some symptoms are "culture-free" and others deeply marked by the discourse held by the person or by his social group. The use of valuation scales for symptomatic change during a treatment (here anti-depressive one) become harder by the undifferenciation of these two types of symptoms. Further, it may exist nosographic masks which can hide more curable substructures ; as often happens here, in masked depressions when the presented symptoms are those of acute or chronic psychotic manifestations. The efficacy of depressive treatment is then remarkable.
