RESUMO

OBJECTIVE: To summarize the application experience of the pneumatic arm in transnasal sphenoidal pituitary adenoma resection under neuroendoscope. METHODS: A retrospective analysis was conducted on the clinical data of 52 patients with pituitary adenoma who underwent endoscopic transsphenoidal surgery with pneumatic arm fixation in the Neurosurgery Department of the First Affiliated Hospital of Anhui Medical University from July 2021 to March 2024. Among them, there were 5 cases of pituitary microadenoma, 35 cases of macroadenoma, and 12 cases of giant adenoma. Head CT and a full set of hormones were re-examined within 24 hours after surgery to evaluate the surgical effect. Follow-up was conducted by the outpatient department after surgery to assess the clinical symptoms, hormone level, and imaging of all patients. RESULTS: Among 52 patients, gross total resection was achieved in 48 cases (92.3%), subtotal resection in 3 cases (5.8%), and partial resection in 1 case (1.9%). Preoperatively, 43 patients had diminished vision, with 40 showing improvement postoperatively, 1 worsening, and 2 having no significant improvement. Thirty-eight patients had headaches preoperatively, and all showed varying degrees of improvement postoperatively. Routine hormone examination within 24 hours after surgery showed that all 20 prolactinoma patients had restored normal hormone levels, 10 of 12 growth hormone-secreting adenoma patients normalized, and 4 of 6 cases of adrenocorticotropic hormone-secreting adenoma immediately relieved after surgery. Postoperative complications included intracranial hematoma in 1 case, cerebrospinal fluid leakage in 2 cases, transient diabetes insipidus in 6 cases, intracranial infection in 1 case, and no death cases. The median follow-up time of 52 patients was 18.6 months (range: 1-32 mo). During the follow-up period, the initial clinical symptoms of all patients improved to varying degrees, and they were able to work and live normally. At the last follow-up, 1 patient had recurrent tumor and 1 patient had progression. CONCLUSION: Transnasal sphenoidal resection of pituitary adenoma using a pneumatic arm-fixed neuroendoscope allows the operator to perform the surgery with both hands, resulting in satisfactory overall tumor resection and fewer surgical complications. This technique has good clinical value for promotion.

RESUMO

The vascular endothelial growth factor pathway plays a key role in the pathogenesis of gastric cancer. In the multicenter, double-blind phase 3 FRUTIGA trial, 703 patients with advanced gastric or gastroesophageal junction adenocarcinoma who progressed on fluorouracil- and platinum-containing chemotherapy were randomized (1:1) to receive fruquintinib (an inhibitor of vascular endothelial growth factor receptor-1/2/3; 4 mg orally, once daily) or placebo for 3 weeks, followed by 1 week off, plus paclitaxel (80 mg/m2 intravenously on days 1/8/15 per cycle). The study results were positive as one of the dual primary endpoints, progression-free survival (PFS), was met (median PFS, 5.6 months in the fruquintinib arm versus 2.7 months in the placebo arm; hazard ratio 0.57; 95% confidence interval 0.48-0.68; P < 0.0001). The other dual primary endpoint, overall survival (OS), was not met (median OS, 9.6 months versus 8.4 months; hazard ratio 0.96, 95% confidence interval 0.81-1.13; P = 0.6064). The most common grade ≥3 adverse events were neutropenia, leukopenia and anemia. Fruquintinib plus paclitaxel as a second-line treatment significantly improved PFS, but not OS, in Chinese patients with advanced gastric or gastroesophageal junction adenocarcinoma and could potentially be another treatment option for these patients. RESUMO

Mulberry leaf has been recognized as a traditional Chinese medicinal plant, which was distributed throughout the Asia. The aqueous extract of mulberry leaf extract (MLE) has various biologically active components such as polyphenols and flavonoids. However, the inhibitory effect of MLE in hepatocarcinogenesis is poorly understood. In this study, we determined the role of MLE supplementation in preventing hepatocarcinogenesis in a carcinogen-initiated high-fat diet (HFD)-promoted Sprague-Dawley (SD) rat model. The rats were fed an HFD to induce obesity and spontaneous hepatomas by administering 0.01% diethylnitrosamine (DEN) in their drinking water for 12 weeks (HD group), and also to fed MLE through oral ingestion at daily doses of 0.5%, 1%, or 2%. At the end of the 12-week experimental period, the liver tumors were analyzed to identify markers of oxidative stress and antioxidant enzyme activities, and their serum was analyzed to determine their nutritional status and liver function. Histopathological analysis revealed that MLE supplementation significantly suppressed the severity and incidence of hepatic tumors. Furthermore, compared with the HFD + DEN groups, the expression of protein kinase C (PKC)-α and Rac family small GTPase 1 (Rac1) was lower in the MLE groups. These findings suggest that MLE prevents obesity-enhanced, carcinogen-induced hepatocellular carcinoma development, potentially through the protein kinase C (PKC)α/Rac1 signaling pathway. MLE might be an effective chemoprevention modality for nonalcoholic fatty liver disease (NAFLD)/nonalcoholic steatohepatitis (NASH)-related hepatocarcinogenesis.

RESUMO

BACKGROUND: Most published reports on SAMD9L-related ataxia-pancytopenia syndrome (ATXPC) have emphasized the hematologic findings. Fewer details are known about the progression of neurologic manifestations and methods for monitoring them. CASES: We present six individuals from two families transmitting a heterozygous variant in SAMD9L, exhibiting clinical variations in their hematologic and neurologic findings. Serial motor function testing was used to monitor motor proficiency over a 2 to 3 year period in the proband and his father from Family 1. CONCLUSIONS: Our case series focuses on the neurologic progression in patients with heterozygous variants in SAMD9L. Patients with ATXPC should be followed to evaluate a wide range of neurologic manifestations. Serial motor function testing using a standardized method is helpful to track changes in balance and coordination in children and adults with ATXPC and could aid in a future extended natural history study.

RESUMO

Thin films of crystalline solids with substantial free volume built from organic chromophores and metal secondary building units (SBUs) are promising for engineering new optoelectronic properties through control of interchromophore coupling. Zn-based SBUs are especially relevant in this case because they avoid quenching the chromophore's luminescence. We find that layer-by-layer spin-coating using Zn acetate dihydrate and benzene-1,4-dicarboxylic acid (H2BDC) and biphenyl-4,4'-dicarboxylic acid (H2BPDC) linkers readily produces crystalline thin films. However, analysis of the grazing-incidence wide-angle X-ray scattering (GIWAXS) data reveals the structures of these films vary significantly with the linker, and with the metal-to-linker molar ratio used for fabrication. Under equimolar conditions, H2BPDC creates a type of structure like that proposed for SURMOF-2, whereas H2BDC generates a different metal-hydroxide-organic framework. Large excess of Zn2+ ions causes the growth of layered zinc hydroxides, irrespective of the linker used. Density functional theory (DFT) calculations provide structural models with minimum total energy that are consistent with the experimentally observed diffractograms. In the broader sense, this work illustrates the importance in this field of careful structure determination, e. g., by utilizing GIWAXS and DFT simulations to determine the structure of the obtained crystalline metal-organic thin films, such that properties can be rationally engineered and explained.

RESUMO

Early diagnosis of peri-implantitis (PI) is crucial to understand its pathological progression and prevention. This study is committed to investigating the signature genes, relevant signaling pathways and their associations with immune cells in PI. We analyzed differentially expressed genes (DEGs) from a PI dataset in the gene expression omnibus database. Functional enrichment analysis was conducted for these DEGs. Weighted Gene Co-expression Network Analysis was used to identify specific modules. Least absolute shrinkage and selection operator and support vector machine recursive feature elimination were ultimately applied to identify the signature genes. These genes were subsequently validated in an external dataset. And the immune cells infiltration was classified using CIBERSORT. A total of 180 DEGs were screened from GSE33774. Weighted Gene Co-expression Network Analysis revealed a significant association between the MEturquoise module and PI (cor = 0.6, P < .0001). Least absolute shrinkage and selection operator and support vector machine recursive feature elimination algorithms were applied to select the signature genes, containing myeloid-epithelial-reproductive tyrosine kinase, microfibrillar-associated protein 5, membrane-spanning 4A 4A, tribbles homolog 1. In the validation on the external dataset GSE106090, all these genes achieved area under curve values exceeding 0.95. GSEA analysis showed that these genes were correlated with the NOD-like receptor signaling pathway, metabolism of xenobiotics by cytochrome P450, and arachidonic acid metabolism. CIBERSORT revealed elevated levels of macrophage M2 and activated mast cells in PI. This study provides novel insights into understanding the molecular mechanisms of PI and contributes to advancements in its early diagnosis and prevention.

RESUMO

Timely detection of Groundwater pollution is essential to protect human health, especially for pesticide pollution. To solve this issue, we proposed a novel solution to realize the prediction of pesticide in groundwater by using the electronic nose (e-nose). The main work of this paper was divided into three steps: 1) checking whether sample was polluted by pesticides, 2) further predicting the pesticide type, brand and pollution degree when the sample was polluted by pesticides, and 3) optimizing the sensor array. Random forest was used to complete the first step, which had the best accuracy and sensitivity of 100 %. Support vector machine was applied to complete the second step, and the accuracy reaching 98.08 %. As for the third step, recursive feature elimination was used to optimize the sensor array. After optimization, the number of sensors was reduced from 26 to 8. In addition, the e-nose developed in this paper was compared with a commercial e-nose. The results showed that the cost of the developed e-nose was much lower than that of the commercial e-nose despite its slightly weaker prediction performance. Thus, this e-nose can be employed to recognize the pesticides in groundwater, and even can be integrated into the while drilling technology to realize the in-situ detection of groundwater.

RESUMO

Although sodium alginate (SA) is frequently utilized because of its good gelling properties, the substance's dearth of adsorption active sites prevents it from effectively removing heavy metals. Herein, SA was used as the base material to form a cross-linked structure with Fe3+ and Mg2+, and gel beads with a diameter of 2.0 ± 0.1 mm with specific adsorption on As(V) were synthesized as adsorbent (Fe/Mg-SA). Fe/Mg-SA was systematically characterized, and its adsorption properties were investigated by varying several conditions. Fe/Mg-SA had a wide pH application range. The adsorption kinetics revealed that a quasi-secondary kinetic model was followed. The adsorption process is linked to the complexation of hydroxyl and AsO43-, chemisorption predominated the adsorption process. The maximal adsorption capacity of Fe/Mg-SA is determined by fitting the Langmuir model to be 37.4 mg/g. Compared to other adsorbents, it is simpler to synthesis, more effective and cheaper. Each treatment of 1 m3 wastewater of Fe/Mg-SA only costs ¥ 38.612. The novel gel beads synthesized provides a better option for purifying groundwater contaminated with As(V).

RESUMO

Electron-phonon interactions, crucial in condensed matter, are rarely seen in Metal-Organic Frameworks (MOFs). Detecting these interactions typically involves analyzing luminescence in lanthanide- or actinide-based compounds. Prior studies on Ln- and Ac-based MOFs at high temperatures revealed additional peaks, but these were too faint for thorough analysis. In our research, we fabricated a high-quality, crystalline uranium-based MOF (KIT-U-1) thin film using a layer-by-layer method. Under UV light, this film showed two distinct "hot bands," indicating a strong electron-phonon interaction. At 77 K, these bands were absent, but at 300 K, a new emission band appeared with half the intensity of the main luminescence. Surprisingly, a second hot band emerged above 320 K, deviating from previous findings in rare-earth compounds. We conducted a detailed ab-initio analysis employing time-dependent density functional theory to understand this unusual behaviour and to identify the lattice vibration responsible for the strong electron-phonon coupling. The KIT-U-1 film's hot-band emission was then utilized to create a highly sensitive, single-compound optical thermometer. This underscores the potential of high-quality MOF thin films in exploiting the unique luminescence of lanthanides and actinides for advanced applications.

RESUMO

Due to the different physical and chemical properties such as surface charge and ion morphology between As(V) and Cd(II), it is challenging to remove As(V) and Cd(II), especially at low concentrations. This study constructed a novel three-dimension nanocomposite adsorbent Mg/Al/Fe-CLDH (CFMA) by "hydrothermal + calcination method". And different initial concentration ratios (Cd: As=1: 2, 1: 1, 2: 1) were used to investigate the removal performance of CFMA for Cd(II) and As(V). When the concentration ratio Cd: As=1: 2, the residual concentrations of As(V) and Cd(II) were 8.7 µg/L and 4.2 µg/L, respectively, which met the drinking water standard; In the co-adsorption system, As(V) and Cd(II) influence each other's adsorption behavior due to the anionic bridge and shielding effect of As(V) on Cd(II), As(V) gradually changed from monolayer adsorption to multi-layer adsorption dominant, while Cd(II) gradually changed from multi-layer adsorption to monolayer adsorption dominant. In this paper, the structure-activity relationship between material structure and synchronous removal of arsenic and cadmium was clarified, and the mechanism of synchronous removal was revealed, which provided technical guidance for synchronous removal of As(V) and Cd(II) from non-ferrous metal smelting wastewater.

RESUMO

Artificial photosynthesis from selective methane oxidation or nitrogen reduction to value-added chemicals provides a promising pathway for the sustainable chemical industry, while still remaining a great challenge due to the extreme difficulty in C-H and N≡N bond cleavage under ambient conditions. Catalysts that can cocatalyze these two reactions simultaneously are rarely reported. Here, Fe-ZSM-5 with highly dispersed extra-framework Fe-oxo species enables efficient and selective photocatalytic conversion of methane and nitrogen to coproduce methanol and ammonia using H2O as the redox reagent under ambient conditions. The optimized Fe-ZSM-5 photocatalyst achieves up to 0.88 mol/molFe·h of methanol products with 97% selectivity. Meanwhile, the productivity of ammonia is 0.61 mol/molFe·h. In situ EPR and DRIFT studies disclose that water serves as a redox reagent to provide hydroxyl radicals for methane oxidation and protons for nitrogen hydrogenation. Quantum chemical calculations revealed that Fe-oxo species play a significant role in the coactivation of methane and nitrogen molecules, which lowers the energy barriers of rate-determining steps for methanol and ammonia generation.

RESUMO

Lysosome dysfunction has been shown to play an important role in cancer progression. However, few research studies have reported the role of lysosomes in head and neck squamous cell carcinoma (HNSCC) progression. Lysosome-related genes (LRGs) were collected from the Molecular Signatures Database. Differentially expressed lysosome-related genes (DELRGs) were identified from the TCGA-HNSCC dataset. The least absolute shrinkage and selection operator and multivariate Cox regression analysis were used to identify the prognostic genes. The prognostic values and expression of hub DELRGs were further validated by GEO datasets. Estimation of STromal and Immune cells in MAlignant Tumors using Expression data and the single-sample gene set enrichment analysis were applied to evaluate the correlation between cathepsin G (CTSG) and immune infiltrates. Twenty-two DELRGs were identified. Among them, CTSG was an independent prognostic biomarker for HNSCC patients. Gene set enrichment analysis indicated that the potential mechanism of CTSG in regulating HNSCC was associated with the immune- and inflammation-related pathways. CTSG expression was highly correlated with immune cell infiltration. Finally, two potential compounds (CH and MAN) targeting CTSG protein were identified, and their reliability was validated through molecular docking analysis. CTSG was associated with immune infiltration and had prognostic value in HNSCC patients, which may be a potential biomarker for predicting the outcome of immunotherapy.

RESUMO

PURPOSE: In a phase IIb trial of nimotuzumab plus gemcitabine, substantial clinical benefits were observed in patients with locally advanced or metastatic pancreatic cancer (PC). Therefore, we conducted a phase III clinical study to verify the efficacy and safety of this combination regimen in patients with K-Ras wild-type tumors (ClinicalTrials.gov identifier: NCT02395016). PATIENTS AND METHODS: Eligible patients were randomly assigned to receive nimotuzumab (400 mg once per week) or placebo followed by gemcitabine (1,000 mg/m2 on days 1, 8, and 15, once every 4 weeks) until disease progression or unacceptable toxicity. The primary end point was overall survival (OS) and the secondary end points were progression-free survival (PFS), response rates, and safety. RESULTS: A total of 480 patients were screened; 92 patients were enrolled and 82 patients with K-Ras wild-type tumors were eligible. In the full analysis set, the median OS was 10.9 versus 8.5 months, while the restricted mean survival time (RMST) was 18.05 versus 11.14 months for the investigational versus control arm (ratio of control v investigation = 0.62 [0.40-0.97]; P = .036). Median PFS was 4.2 versus 3.6 months in the investigational versus control arm (log-rank P = .04; hazard ratio, 0.60 [0.37-0.99]) and the restricted mean PFS time was 8.08 versus 4.76 months (RMST ratio, 0.58 [0.38-0.90]; P = .036). Both OS and PFS were longer in the nimotuzumab group than in the placebo group. The objective response rates and disease control rates were 7% versus 10% and 68% versus 63% for the investigational and control groups, respectively. The incidence of adverse events were comparable between the two groups. CONCLUSION: In patients with locally advanced or metastatic K-Ras wild-type PC, nimotuzumab plus gemcitabine significantly improved OS and PFS with a good safety profile.

RESUMO

Accurate and rapid prediction of pesticides in groundwater is important to protect human health. Thus, an electronic nose was used to recognize pesticides in groundwater. However, the e-nose response signals for pesticides are different in groundwater samples from various regions, so a prediction model built on one region's samples might be ineffective when tested in another. Moreover, the establishment of a new prediction model requires a large number of sample data, which will cost too much resources and time. To resolve this issue, this study introduced the TrAdaBoost transfer learning method to recognize the pesticide in groundwater using the e-nose. The main work was divided into two steps: (1) qualitatively checking the pesticide type and (2) semi-quantitatively predicting the pesticide concentration. The support vector machine integrated with the TrAdaBoost was adopted to complete these two steps, and the recognition rate can be 19.3% and 22.2% higher than that of methods without transfer learning. These results demonstrated the potential of the TrAdaBoost based on support vector machine approaches in recognizing the pesticide in groundwater when there were few samples in the target domain.

RESUMO

For public health consideration, it is important to ensure the wastewater discharged from wastewater treatment plant is within the regulatory limits. This problem can be effectively solved by improving the accuracy and rapid characterization of water quality parameters and odor concentration of wastewater. In this paper, we proposed a novel solution to realize the precisive analysis of water quality parameters and odor concentration of wastewater by the electronic nose device. The main work of this paper was divided into three steps: 1) recognizing wastewater samples qualitatively from different sampling points, 2) analyzing the correlation between electronic nose response signals and water quality parameters and odor concentration, and 3) predicting the odor concentration and water quality parameters quantitatively. Combined with different feature extraction methods, support vector machine and linear discriminant analysis were applied as classifiers to recognize samples at different sampling points, which reported the best recognition rate of 98.83%. Partial least squares regression was applied to complete the second step, and R2 was reaching 0.992. As for the third step, ridge regression was used to predict water quality parameters and odor concentration with the RMSE less than 0.9476. Thus, electronic noses can be applied to determine water quality parameters and odor concentrations in the effluent discharged from wastewater plants.

RESUMO

Easy-to-integrate, remote read-out thermometers with fast response are of huge interest in numerous application fields. In the context of optical read-out devices, sensors based on the emission of lanthanides (Eu(III), Tb(III)) are particularly promising. Here, by using a layer-by-layer (LbL) approach in the liquid-phase epitaxy process, a series of continuous, low-thickness lanthanide-MIL-103 SURMOFs were fabricated to yield highly sensitive thermometers with optical readout. These Ln-SURMOFs exhibit remarkable temperature-sensing photoluminescence behavior, which can be read out using the naked eye. High transmittance is realized as well by precisely controlling the film thickness and the quality of these Ln-SURMOF thermometers. Moreover, we demonstrate that the thermal sensitivity can be improved in the temperature regime above 120 K, by controlling the energy transfer between Tb(III) and Eu(III). This performance is achieved by employing a sophisticated supramolecular architecture, namely MOF-on-MOF heteroepitaxy.

RESUMO

Wettability is a central surface property of functional thin films. Here, we present a nanoporous film made of an azobenzene-containing metal-organic framework material where the wettability is controlled by photoswitching of the fluorinated azobenzene moieties and by reversible incorporation of guest molecules with different polarities in the pores. Using both, the optical and the chemical stimuli, the water contact angle was modified over a wide range, from 23° to 97°.

RESUMO

BACKGROUND: Elevated triglycerides (TG) and reduced high-density lipoprotein cholesterol (HDL-C) are recognized as essential and independent hazard factors for total death and major adverse cardiovascular events (MACE) in patients with coronary heart disease (CHD). However, whether the increased TG/HDL-C forecasted the prognosis of CHD is still unknown. Therefore, we performed a meta-analysis to investigate the relationship between the elevated TG/HDL-C ratio and poor prognosis of CHD. METHODS: A systematic literature search was conducted in PubMed, Web of Science, EMBASE, and The Cochrane Library, until August 30, 2021. Prospective observational studies regarding the association between TG/HDL-C and long-term mortality/MACEs in CHD patients were included. RESULTS: In total, 6 independent prospective studies of 10,222 participants with CHD were enrolled in the systematic and meta-analysis. Our outcomes of the meta-analysis indicated that the elevated TG/HDL-C group had a significantly increased risk of long-term all-cause mortality (hazard ratio [HR] = 2.92, 95% confidence interval [CI]: 1.75-4.86, P < .05) and long-term MACEs (HR = 1.56, 95%CI 1.11-2.18, P < .05). CONCLUSION: In patients with CHD, the present study showed that the high TG/HDL-C was associated with increased risk of long-term all-cause mortality and MACE.

RESUMO

For the first time, a procedure has been established for the growth of surface-anchored metal-organic framework (SURMOF) copper(II) benzene-1,4-dicarboxylate (Cu-BDC) thin films of thickness control with single molecule accuracy. For this, we exploit the novel method solution atomic layer deposition (sALD). The sALD growth rate has been determined at 4.5 Å per cycle. The compact and dense SURMOF films grown at room temperature by sALD possess a vastly superior film thickness uniformity than those deposited by conventional solution-based techniques, such as dipping and spraying while featuring clear crystallinity from 100 nm thickness. The highly controlled layer-by-layer growth mechanism of sALD proves crucial to prevent unwanted side reactions such as Ostwald ripening or detrimental island growth, ensuring continuous Cu-BDC film coverage. This successful demonstration of sALD-grown compact continuous Cu-BDC SURMOF films is a paradigm change and provides a key advancement enabling a multitude of applications that require continuous and ultrathin coatings while maintaining tight film thickness specifications, which were previously unattainable with conventional solution-based growth methods.

RESUMO

Objective: Peri-implantitis (PI) is one of the main reasons for dental implant failure. Until now, the etiology and pathogenesis of PI remain unclear. Methods: In this study, we used differentially expressed genes (DEGs) analysis and gene function enrichment analysis to assess the expression profile of peri-implant bone tissue and gingiva in PI public data from the Gene Expression Omnibus (GEO) database. Then, we used gingival tissues from patients with PI and healthy individual to construct gene coexpression networks to reveal the biological functions of the genes in PI using RNA sequencing data. Afterward, key gene modules were selected to reveal the critical biological process or signaling pathway using Hallmark's gene enrichment and expression analysis of the related pathway members in PI. Results: DEGs were enriched in the formation of cellular responses to external stimuli in bone tissue. Cytokine production, lymphocyte activation, immune response-regulating signaling pathway, and blood vessel development were the top GO biology process or pathways of the DEGs in gingival tissue. Weighted gene coexpression network analysis (WGCNA) of RNA-seq data was used to assess the results of correlation analysis between modules and traits and correlation analysis between modules and functions. kMEpurple, kMEgreen, and kMEred modules were selected as the key gene modules. Signaling pathways and gene expression analysis were performed on selected modules, such as IL2/STAT5 signaling pathway, TNFα signaling pathway via NFκB, and angiogenesis were enriched in kMEpurple module. Hedgehog signaling pathway, Wnt ß-catenin signaling pathway, and IL2/STAT5 signaling pathway were enriched in kMEgreen module. Peroxisome, IL2/STAT5 signaling pathway, and epithelial-mesenchymal transformation process were enriched in kMEred module. All the enrichment results of key modules contained IL2/STAT5 signaling pathway. Conclusion. Differential gene and enrichment analysis based on public data showed differences in gene expression patterns and biological process between bone and gingival tissues in PI. This spatial-temporal heterogeneity is reflected in the formation of cellular responses to external stimuli, which was enriched in bone tissue, but cytokine production, lymphocyte activation, immune response regulating signaling pathway, and blood vessel development were enriched in gingival tissue. WGCNA and Hallmark gene sets enrichment analysis of the gingival tissue expression profile and showed that IL2-mediated activation of immune cells could be a critical mechanism in PI. As a new clinical treatment alternative, we suggest that IL2/STAT5 pathway blockers could be helpful in the treatment of PI.
