BACKGROUND/AIMS: The incidence of childhood insulin-dependent diabetes mellitus (IDDM) is rapidly increasing in Greece. The aim of this study was to examine nutrient patterns of children and adolescents with IDDM in Crete. METHODS: A case-control survey of eating habits was conducted among 41 children and adolescents, aged 6-17 years, with IDDM (cases) and healthy sex- and age-matched controls, recruited from Heraklion University Hospital and the village of Rouvas, Crete, respectively. Dietary data was based on 24-hour recall records. RESULTS: Dietary intake was similar between the groups. Both groups exceeded the recommendations for total and saturated fat and consumed lower than recommended amounts of dietary fibre and total carbohydrates. Children with IDDM consumed more dairy products and vegetables and less meats and cereals. Vegetables were consumed by a relatively high proportion of children and adolescents in both groups, but there was a higher proportion of fruit consumers amongst the IDDM group. For those who ate fruit and vegetables, intake approximated current recommendations of 400 g/day. CONCLUSIONS: The need to increase complex carbohydrates and reduce the consumption of total and saturated fat, in order to approach the traditional dietary pattern of Crete for this population, should be addressed via appropriate nutrition education programmes.

The study aimed to map stakeholders' evaluations of policy options to counter the rising prevalence of obesity in Greece, where the case for action on obesity is only now being made. The multi-criteria mapping method was used to capture and compare stakeholders' appraisals and to provide a policy analysis. Efficacy and practical feasibility were the issues most frequently used by stakeholders to evaluate options and were weighted more highly than cost criteria, which were often defined in terms of governmental costs. There was a broad favourable appraisal for downstream measures offering individuals the skills, information and opportunities to make healthier choices, rather than options to modify the obesogenic environment. Consistently, high rankings were given to educational options, for improving communal facilities and for some information-related options (food labelling, advertising), with particular support for policies targeting the young. There was also significant advocacy by a few for the creation of a new government body charged with intersectoral policy co-ordination. The Policy Options for Responding to the Growing Challenge of Obesity Research Project analyses thus point to support for a portfolio of measures to combat the problem of obesity in Greece as well as an appreciation that political will is an essential prerequisite.

RESUMO

The aims of this project are to formulate a strategy and action plan for developing and implementing European dietary guidelines. The project is supported by DGV/F/3 and was initiated in October 1998. The work plan has entailed the formation of Working Parties composed of prominent experts from the member states to prepare consensus documents on the state of the art in Europe and the added value of European population-based guidelines. These groups are addressing 4 inter-linked components of the project: (1) evaluation of the role of diet and lifestyles in health & disease patterns in Europe; (2) the feasibility of developing an EU-wide framework for food-based dietary guidelines; (3) public health nutrition strategies for implementing FBDGs and for enhancing physically active lifestyles in Europe and (4) an evaluation of policy, trade, economic and technological aspects to improving nutritional status and lifestyles in the EU. Stakeholders from the spectrum of interests involved are invited to assess and debate the science and policy implications of these documents, which will be presented at a European Conference in May 2000. Recommendations in the form of a practical, evidence based policy document will be presented to the European Commission, the Council of Ministers and the European Parliament.

BACKGROUND: To assess physical activity and physiological fitness parameters among six year old children and to determine whether there were any significant gender differences. DESIGN & SETTING: comparative study of a representative sample of boys and girls in school and at home. PARTICIPANTS: 569 children (305 boys and 264 girls) selected randomly from a total of 6153 registered in the 1st grade in 1992 on the island of Crete. MEASURES: assessment of physical activity was based on observational methods involving teachers and parents. Cardiorespiratory fitness was estimated by performance on the 20 meter Shuttle Run Test (20mSRT). BMI, skinfold thickness, Midarm Muscle Circumference (MMC) and hemoglobin were also measured. RESULTS: Both sexes were found to spend a very small proportion of their leisure time in Moderate to Vigorous Physical Activities (MVPA). More boys engaged in MVPA than girls at school and at home, but more girls were involved in physical activity-related lessons and classes out of school. Among the physiological fitness parameters, significant gender differences were found only for MMC. No significant gender difference was found in performance on the 20mSRT. CONCLUSIONS: The results indicate that in this culture stereotypic sex preferences in physical activity begin at a very young age, and that this differentiation cannot be attributed to gender differences in cardiorespiratory fitness. The social, environmental and possibly psychological parameters involved have implications for Health Educators and Physical Education Instructors in the appropriate targeting of physical activity promotion programs among young children.

An intervention program was undertaken to assess dietary habits and the impact of nutrition education among pregnant women in the rural county of Florina, northern Greece. Analysis of dietary habits, nutrient intake, hemoglobin, and serum vitamin concentrations in an intervention and control group indicated that the population was adequately nourished although nutrient intake was below recommended levels for pregnant women. Nutrition counseling was associated with improvements in dietary intake and significantly greater maternal weight gain (p less than 0.05). Mean birth weight was slightly higher in the intervention group but so was the incidence of low birth weight (4.5% vs 3.9%). The prematurity rate was marginally lower in the intervention group (p less than 0.04), as was the number of perinatal deaths (9 vs 11). The results indicate that nutrition counseling during pregnancy can improve dietary intake and maternal weight gain but the mediating influence on low frequency pregnancy outcomes is indeterminate in a population that is not nutritionally at risk.
