RESUMO

We present a case of a patient with pulmonary sarcoidosis who developed ulcerative sarcoidosis with granulomatous vasculitis on the lower legs. Ulceration is a rare presentation for sarcoidosis and this case gives further evidence that an underlying vasculitis may be an aetiological factor. Granulomatous vasculitis, classically associated with Wegener's granulomatosis, lymphomatoid granulomatosis, or Churg-Strauss syndrome, is also commonly described in pulmonary sarcoidosis. It has, however, only rarely been described in cutaneous sarcoidosis. Our patient's leg ulcers have responded well to immunosuppressive therapy with tapered corticosteroid and low-dose azathioprine.

RESUMO

Collagenomas are considered to be connective tissue nevi composed predominantly of collagen. Collagenomas have been classified into 4 different entities: familial cutaneous collagenoma, the Shagreen patch of tuberous sclerosis, eruptive collagenoma, and other isolated collagenomas. We describe a patient with multiple collagenomas that appeared and multiplied during gestation and discuss the reported cases of collagenomas that have been linked to pregnancy.

Transplant recipients receive a number of immunosuppressive medications that result in an increased risk of infection, including infections with microbes that are normally not pathogenic. We describe a patient with end-stage renal disease who underwent kidney transplantation. Six months postoperatively, he presented with a lesion on his ankle, multiple thigh nodules, and right testicular pain. Biopsy of the ankle lesion demonstrated Pseudallescheria boydii (Scedosporium apiospermum), a common environmental fungus. Following orchiectomy, multiple fungal elements were found that were initially described as Aspergillus species, but later identified as P. boydii. In addition, multiple brain abscesses were found on magnetic resonance imaging. Despite treatment with multiple antifungal medications, the patient died of cardiac dysrhythmia. Current diagnostic and therapeutic alternatives for P. boydii are reviewed.

Fas ligand (FasL), a cell surface molecule belonging to the tumour necrosis factor family, binds to its receptor Fas and thus induces apoptosis of Fas-bearing cells such as activated lymphocytes. In this paper, we report the expression of FasL on melanoma cell lines and patient tumour specimens, and compare it with the expression of interleukin-10 (IL-10), a putative immunosuppressive factor. Apoptosis of Fas-bearing Jurkat cells was increased after interferon-alpha treatment of the FasL-positive melanoma cell line A375, suggesting a regulation of FasL function. We also tested whether FasL and IL-10 were ever co-expressed. Immunohistochemistry studies showed that IL-10 expression was highly positive in the same tumour samples which expressed FasL. In the melanoma patients with thin primaries, 10 of the 12 primaries and six of the seven metastatic lesions were positive for IL-10. In the melanoma patients with thick primaries (> 0.75 mm), four of the five primary lesions and nine of the 10 metastatic lesions were positive for IL-10. In contrast, FasL was generally negative in primary tumours and positive in metastatic tumours. In the thin primary melanoma patients, two of the 12 primaries and five of the seven metastatic tumours were positive for FasL. From the thick melanomas, one of the five primaries and five of the 10 metastatic lesions were positive for FasL. The function of melanoma-derived FasL was confirmed by four different cytotoxicity assays.

BACKGROUND: It was the purpose of this study to assess the histologic skin changes of free tissue transfers used to reconstruct intraoral defects. METHODS: Patients who were at least 12 months after intraoral free tissue reconstruction were selected for study. A 3-mm punch biopsy of the intraoral portion of the flap was performed and submitted for routine histologic analysis and periodic acid-Schiff (PAS) staining. The adjacent native oral mucosa was also biopsied and served as control. RESULTS: Eight patients (42 to 71 years old) were studied. A variety of skin flaps were employed and included free rectus abdominis (4), radial forearm (2), scapula (1), and osteocutaneous fibula flaps (1). Histologic examination demonstrated that all specimens maintained normal skin architecture and maturation. Adnexal structures were demonstrated in all but one. Acute inflammation in conjunction with diffuse parakeratosis and spongiosis were seen with 4 cases. Fungal forms consistent with Candida species were identified in the stratum corneum by PAS stain in these 4 cases. CONCLUSION: The histologic integrity of skin appears to be maintained in cutaneous free flaps placed intraorally. Previous studies have noted an inflammatory infiltrate in the epithelial tissues of intraorally placed flaps, yet have not commented on the significance of this finding even when it was noted in a chronic setting. Our study is the first to correlate these changes with the presence of fungi. The effect of the acute inflammation and associated fungal forms may suggest the need for chronic therapy and merits further study.

Two cases of metastasizing cellular dermatofibroma (cutaneous fibrous histiocytoma) are presented. The first patient, an 18-year-old man, had a nodule excised from his right upper thigh. He developed three local recurrences at 1.5, 2, and 2.5 years and metastasis to inguinal lymph nodes. He underwent lung segmentectomies for metastases 1.5 and 4 years later and was alive with no evidence of tumor at latest follow-up, which was 15 months after the last surgery. The second patient, a 33-year-old man, had a nodule removed from his right posterior neck. The tumor recurred 3 months later and was reexcised. Right cervical lymph node metastases were excised at 7 and 8 years. A year later, a right cervical lymph node dissection yielded one positive node of 35, and multiple metastases were excised from the right lung. The patient was alive with lung metastases 6 years later, which was the latest follow-up. Grossly, both tumors were single 2 cm nonulcerated dermal-subcutaneous nodules. Histologically, they were characteristic of cellular dermatofibroma; they were composed of plump to spindled "fibrohistiocytic" cells arranged in a storiform pattern and had areas of hemorrhage, hemosiderin, and infiltration between dermal collagen bundles peripherally. Recurrences and metastases were histologically similar except that lung metastases were cystic. The alternative diagnosis of angiomatoid malignant fibrous histiocytoma was considered for these two cases but was excluded because the tumors were partly dermal, had a well-defined storiform pattern, and lacked large blood lakes, multinodularity, a fibrous pseudocapsule, and surrounding chronic inflammation. We conclude that dermatofibromas can rarely metastasize. Risk factors for metastasis may include relatively large size, high cellularity, and local recurrence. Judging from these two cases, metastasizing dermatofibromas behave in an indolent manner.
