RESUMO

Link prediction methods use patterns in known network data to infer which connections may be missing. Previous work has shown that continuous-time quantum walks can be used to represent path-based link prediction, which we further study here to develop a more optimized quantum algorithm. Using a sampling framework for link prediction, we analyze the query access to the input network required to produce a certain number of prediction samples. Considering both well-known classical path-based algorithms using powers of the adjacency matrix as well as our proposed quantum algorithm for path-based link prediction, we argue that there is a polynomial quantum advantage on the dependence on N, the number of nodes in the network. We further argue that the complexity of our algorithm, although sub-linear in N, is limited by the complexity of performing a quantum simulation of the network's adjacency matrix, which may prove to be an important problem in the development of quantum algorithms for network science in general.

RESUMO

Despite Brazil's tradition of successful mass immunization programs, the country has been experiencing alarming declines in vaccination coverage, especially among children. That is aggravated by the growth of anti-vaccine movements and the spread of health misinformation in social media in the last decade, which have worsened during the COVID-19 outbreak. Several reports link populism and far-right politicians to anti-vaccination support worldwide, which was also the case in Brazil during president Jair Bolsonaro's administration. This project aimed to identify the circulating pro and anti-vaccine narratives in Portuguese on Twitter, during a crucial decision-making period regarding childhood vaccination in Brazil, from December 9, 2021, until February 9, 2022. From the over one million tweets and four million retweets collected, we identified two well-defined groups, one in favor and another against vaccination. Within the sample, we selected 1500 influencer tweets with the highest impact (>500 retweets) and conducted content analysis. Although the pro-vaccine influencers were more retweeted than anti-vaxxer ones, we observed that anti-vaccine movements were more succesful in framing discussions on Twitter. The subject of COVID-19 was the target of political polarization embedded in populist, anti-science and anti-traditional media discourses promoted by anti-vaxxers. As a counterpart, the pro-vaccine influencers reacted inarticulately, focusing on criticizing the anti-vaccination actors, attitudes, and policies instead of promoting vaccines. Based on reults, we claim that a well-coordinated network of health communicators from science centers and health institutions, in partnership with properly briefed social media influencers and fact-checking sources, would more efectively pre-tempt the public about vaccine misinformation.

RESUMO

INTRODUCTION: open tibial fractures are challenging due to the frequent severe bone injury associated with poor soft tissue conditions. This is relevant in low- and middle-income countries, mainly related to delayed definitive fixation and lack of adequate training in soft tissue coverage procedures. Due to these factors, open tibial fracture is an important source of disability for Latin American countries. Herein we sought to provide an epidemiological overview of isolated open tibial shaft fracture across seven hospitals in southern cone of Latin America. The secondary goal was to assess the impact on quality of life based on return-to-work rate (RWR). METHODS: patients with an isolated open tibial shaft fracture treated in seven different hospitals from Brazil and Argentina from November 2017 to March 2020 were included in the study. Clinical and radiographic results were evaluated throughout the 120-day follow-up period. Final evaluation compared RWR with the SF-12 questionnaire, bone healing, and gait status. RESULTS: Seventy-two patients were treated, 57 followed for 120 days and 48 completed the SF-12 questionnaire. After 120 days, 70.6% had returned to work, 61.4% had experienced bone healing. Age, antibiotic therapy, type of definitive treatment, and infection significantly influenced the RWR. Gait status exhibited strong correlations with RWR and SF-12 physical component score. CONCLUSIONS: Isolated open tibial shaft fractures are potentially harmful to the patient's quality of life after 120 days of the initial management. RWR is significantly higher for younger patients, no history of infection, and those who could run in the gait status assessment.

RESUMO

Water crises in Latin America are more a consequence of poor management than resource scarcity. Addressing water management issues through better coordination, identification of problems and solutions, and agreement on common objectives to operationalize integrated water resources management (IWRM) could greatly improve water governance in the region. Composite indices have great potential to help overcome capacity and information challenges while supporting better IWRM. We applied one such index, the Freshwater Health Index (FHI) in three river basins in Latin America (Alto Mayo, Perú; Bogotá, Colombia; and Guandu, Brazil) to assess freshwater ecosystem vitality, ecosystem services, and the water governance system in place. The approach included convening management agencies, water utilities, planning authorities, local NGOs and industries, community groups and researchers to co-implement the FHI. The results provide detailed information on the ecological integrity of each basin and the sustainability of the ecosystem services being provided. All three basins show very low scores for governance and stakeholder engagement, thus improving both in the region should be a priority. The results also shed light on how the FHI framework can help inform decision-making to improve IWRM implementation by facilitating stakeholder engagement while contributing to coordination, identification of problems and solutions as well as agreement on common objectives. Because implementation of IWRM is part of the solution for the United Nations Sustainable Development Goal (SDG) 6.5 ("By 2030, implement IWRM at all levels, including through transboundary cooperation as appropriate"), our case studies can serve as examples to other Latin American countries to achieve SDG 6.5.

BACKGROUND: Seasonal Affective Disorder is a recurrent depressive disorder which usually begins in the fall/winter and enters into remission in the spring/summer, although in some cases may occur in the summer with remission in the autumn-winter. In this study the authors evaluated the association between seasonal changes in mood and behavior with psychiatric disturbance. METHOD: Descriptive, cross-sectional study. Participants, students attending higher education and vocational courses (N = 324), were evaluated with the Seasonal Pattern Assessment Questionnaire (SPAQ) and the Screening Scale for Mental Health (ER80). RESULTS: Among the respondents, 12.7% showed seasonal affective disorder (SAD), 29.0% showed subsyndromal seasonal affective disorder (s-SAD) and 58.3% did not show significant seasonal affective symptomatology. As for psychiatric morbidity, 36.6% of subjects with SAD and 13.8% of those with s-SAD were considered "psychiatric cases" whereas for subjects without SAD this value was only 3.2%. CONCLUSIONS: There is a statistically significant association between psychiatric morbidity and seasonal affective disorder. This association corroborates the importance of the Seasonal Pattern Assessment Questionnaire in screening for seasonal fluctuations in mood and behavior related disorders, and the clinical need for recognition of these conditions, particularly associated suffering and disabilities.

RESUMO

Motor neuron disease (MND) is a systemic disease with a broad clinical spectrum. It is characterized by primary involvement of the lower or upper motor neuron (UMN), or both, simultaneously, represented by the most common form, amyotrophic lateral sclerosis (ALS). ALS is rapidly progressive and fatal disease that evolve to death due to respiratory failure, on average, in three to five years since the onset of symptoms. This fact attends to the early and correct diagnosis of the disease. OBJECTIVE: To evaluate clinical, epidemiological and electrophysiological variables for the early diagnosis of ALS. METHODS: This is an observational, descriptive and retrospective study, conducted from the collect of the database, in which the variables were submitted to statistical analysis: Mann-Whitney test and Fisher's exact test. RESULTS: When correlating clinical, epidemiological and electrophysiological findings of patients with ALS and other forms of MND, the variables: age of onset of symptoms (P=0,02) hyperreflexia (P=0,001), presence of bulbar symptoms/signs (P<0,001), pathological reflexes (P=0.001), and presence of fasciculation in electromyography (P=0,001) presented statistical significance for the diagnosis of ALS. CONCLUSION: Despite the small sample size, the findings reinforce the importance of well- done neurological examination, to search for signs of involvement of the UMN, in the first evaluation of patients with suspected MND. And that more research is needed to better understand the different phenotypes of the disease in order to obtain an increasingly early diagnosis to offer improvements in the quality of life of these patients

We introduce two generalizations of core percolation in graphs to hypergraphs, related to the minimum hyperedge cover problem and the minimum vertex cover problem on hypergraphs, respectively. We offer analytical solutions of these two core percolations for uncorrelated random hypergraphs whose vertex degree and hyperedge cardinality distributions are arbitrary but have nondiverging moments. We find that for several real-world hypergraphs their two cores tend to be much smaller than those of their null models, suggesting that covering problems in those real-world hypergraphs can actually be solved in polynomial time.

RESUMO

Protected areas are intended to safeguard biodiversity in perpetuity, yet evidence suggests that widespread legal changes undermine protected area durability and efficacy. We documented these legal changes-protected area downgrading, downsizing, and degazettement (PADDD) events-in the United States and Amazonian countries and compiled available data globally. Governments of the United States and Amazonian countries enacted 269 and 440 PADDD events, respectively. Between 1892 and 2018, 73 countries enacted 3749 PADDD events, removing 519,857 square kilometers from protection and tempering regulations in an additional 1,659,972 square kilometers; 78% of events were enacted since 2000. Most PADDD events (62%) are associated with industrial-scale resource extraction and development, suggesting that PADDD may compromise biodiversity conservation objectives. Strategic policy responses are needed to address PADDD and sustain effective protected areas.

RESUMO

We present the case of a patient who suffered a comminuted supracondylar periprosthetic femur fracture. The patient was an 86-year-old lady who suffered a minor fall at home and presented at our hospital with a right comminuted distal femur fracture around a total knee arthroplasty. The patient was submitted to a cruciate-sacrificing total knee replacement 6 years before at the same institution. Despite severe metaphyseal fragmentation and short distal fragment, the prosthesis was stable; thus, open fracture reduction and stabilization with internal fixation were performed. The surgical technique included the use of a nonvascularized autologous fibular strut graft as an augmentation technique in conjunction with double plating fixation. Clinically, patient presented a painless aligned knee 12 months after femur fixation, although she was not able to return to an independent level of activity. No pain involving the donor graft site was reported at the time of the most recent follow-up examination. This case study demonstrates the use of free nonvascularized autogenous fibular strut bone graft as an option to bridge major bone defects. This proved to be a relatively simple, not expensive procedure that can be done percutaneously and does not need high-quality training.

RESUMO

Ulnar neuropathy at the wrist (UNW) is rare, and always challenging to localize. To increase the sensitivity and specificity of the diagnosis of UNW many authors advocate the stimulation of the ulnar nerve (UN) in the segment of the wrist and palm. The focus of this paper is to present a modified and simplified technique of sensory nerve conduction (SNC) of the UN in the wrist and palm segments and demonstrate the validity of this technique in the study of five cases of type III UNW. The SNC of UN was performed antidromically with fifth finger ring recording electrodes. The UN was stimulated 14 cm proximal to the active electrode (the standard way) and 7 cm proximal to the active electrode. The normal data from amplitude and conduction velocity (CV) ratios between the palm to finger and wrist to finger segments were obtained. Normal amplitude ratio was 1.4 to 0.76. Normal CV ratio was 0.8 to 1.23.We found evidences of abnormal SNAP amplitude ratio or substantial slowing of UN sensory fibers across the wrist in 5 of the 5 patients with electrophysiological-definite type III UNW.

INTRODUCTION: Injuries in the distal third of the leg may expose noble areas that should be covered. The lateral supramalleolar flap is one of a series of flaps described in the last decade; it is an elevated fasciocutaneous flap in the lateral aspect of the lower leg, and is employed as a distally based pedicled flap. The purpose of this case report is to present the surgical results and the effectiveness of the lateral supramalleolar flap in extensive lower limb injuries with significant loss of substance in a 4-year-old child. CASE REPORT: Patient A. K. E. S., a girl, was admitted 13 years previously. She had been in an accident, and had extensive injuries on the dorsum of the left foot, with exposure of noble areas approximately 6 cm in diameter. After debridement, lesion coverage was performed on the fifth day by using a fasciocutaneous lateral supramalleolar flap. The patient was discharged for outpatient monitoring and progressed without complication. She only returned to the hospital 13 years later, overweight and complaining of swelling on the dorsal aspect of the foot. Lipectomy of the distal third of the flap was performed, with uncomplicated progress. A second procedure for the proximal third was scheduled six months later. DISCUSSION: Injuries in the distal third of the leg may expose noble areas. Lateral supramalleolar, sural, and pedicled perforating flaps have similar applications, but show individual technical peculiarities. The lateral supramalleolar flap has no need for microsurgery, is rapidly performed, and simulates the receiving area while considering several local characteristics. CONCLUSION: This flap is easily implemented and reproducible, and can be used in children younger than 5 years of age. It is an alternative for injuries in the lower third of the leg, the ankle, and the foot.

RESUMO

The maned sloth Bradypus torquatus (Pilosa, Bradypodidae) is endemic to a small area in the Atlantic Forest of coastal Brazil. It has been listed as a threatened species because of its restricted geographic range, habitat loss and fragmentation, and declining populations. The major objectives of this study were to estimate its potential geographic distribution, the climatic conditions across its distributional range, and to identify suitable areas and potential species strongholds. We developed a model of habitat suitability for the maned sloth using two methods, Maxent and Mahalanobis Distance, based on 42 occurrence points. We evaluated environmental variable importance and the predictive ability of the generated distribution models. Our results suggest that the species distribution could be strongly influenced by environmental factors, mainly temperature seasonality. The modeled distribution of the maned sloth included known areas of occurrence in the Atlantic Forest (Sergipe, Bahia, Espírito Santo, and Rio de Janeiro), but did not match the observed distributional gaps in northern Rio de Janeiro, northern Espírito Santo or southern Bahia. Rather, the model showed that these areas are climatically suitable for the maned sloth, and thus suggests that factors other than climate might be responsible for the absence of species. Suitable areas for maned sloth were located mainly in the mountainous region of central Rio de Janeiro throughout Espírito Santo and to the coastal region of southern Bahia. We indicate 17 stronghold areas and recommended survey areas for the maned sloth. In addition, we highlight specific areas for conservation, including the current network protected areas. Our results can be applied for novel surveys and discovery of unknown populations, and help the selection of priority areas for management and conservation planning, especially of rare and relatively cryptic species directed associated with forested habitats.

Palpebral ptosis or blepharoptosis is characterized by the dysfunction or inability of the patient to normally open the palpebral fissure. Usually, it is due to the involvement of the eyelid levator muscle. The congenital form occurs in 60%-70% of cases. In this study, we describe a case of moderate congenital palpebral ptosis in a 9-year-old patient who presented with a good eyelid levator muscle function in the left eye. The patient underwent surgical treatment with the Lester Jones technique, as indicated for cases of moderate ptosis, and achieved satisfactory aesthetic and functional results.

ABSTRACT Introduction: The indications for lower limb replantation in children are still controversial in the literature. Furthermore, they are more selective than those for upper limb replantation. Replantations of lower limbs are less frequent for various reasons, including that the traumas leading to amputations at the lower limb level are usually high-kinetic-energy injuries and involve severe tissue damage, in addition to their frequent association with other organ damage that discourage the use of the procedure. Because of the association with frequent serious postoperative complications and unsatisfactory outcomes in some cases, many surgeons feel discouraged to replant these segments. There are few literature reports on lower limb replantations with functional success. Case Report: Patient E.S.S. is a 4-year-old girl who was run over by a train in July 1997. The accident resulted in the crushing of the lower third of her left leg and the amputation of her left ankle. She was admitted to the emergency room of Santa Casa de Campo Grande-MS, in good overall condition. Replantation of the amputated limb was recommended. She was then transferred to the operating room, and her left foot was replanted after 8 h of ischemia. Conclusion: Clearly, in selected cases, such as the patient reported here, microsurgical replantation is a valid and extremely valuable option, both from a functional and an aesthetic point of view.

RESUMO

There are few studies reporting the association between hepatitis C virus (HCV) infection and disautonomia. We have evaluated the autonomic cardiovascular function in 12 patients with sensory small-fiber polyneuropathy infected by HCV. The mean age was 49 ± 13 years old. The mean infection time was 9.6 years in six (50%) patients. Thermal and pinprick hypoesthesia was observed in distal legs in all patients. Autonomic symptoms were referred by eight (66.7%) patients. Among patients with abnormal autonomic cardiovascular test, five (41.7%) showed abnormal results in two or more tests. Valsalva maneuver was abnormal in seven (58.3%) patients. We can consider that there is an association of both parasympathetic and sympathetic efferent cardiovascular dysfunction in this group of patients.

RESUMO

There are few studies reporting the association between hepatitis C virus (HCV) infection and disautonomia. We have evaluated the autonomic cardiovascular function in 12 patients with sensory small-fiber polyneuropathy infected by HCV. The mean age was 49±13 years old.

Patients with severe burns complicated by joint involvement and exposure of noble structures require immediate local cover, which can be achieved using a variety of surgical procedures. Local flaps are the first choice due to the simplicity of their preparation and the resulting acceptable cover. However, the tissue adjacent to the burned area is often of low quality as a consequence of local changes that mainly affect blood circulation. When local flaps cannot be used, distant and/or microsurgical flaps can be applied. However, distant flaps generally require reconstructions performed in separate surgical procedures, which can prolong bed rest and immobilization of the patient. For more than 3 decades, microsurgery has been used to repair significant tissue losses in a single surgical procedure. This technique enabled the use of tissue transplantation for the repair of burn sequelae. Often in association with other established surgeries, such as skin graft or tissue expansion, these procedures provide better functional and aesthetic results. In the present study, aspects of this therapeutic strategy, as well as the indications, contraindications, and technical details of tissue transplantation are discussed.
