RESUMO

Background: Determining lung cancer (LC) risk using personalized risk stratification may improve screening effectiveness. While the Prostate, Lung, Colorectal, and Ovarian Cancer Screening Trial (PLCO) is a well-established stratification model for LC screening, it was derived from a predominantly Caucasian population and its effectiveness in a safety net hospital (SNH) population is unknown. We have developed a model more tailored to the SNH population and compared its performance to the PLCO model in a SNH setting. Methods: Retrospective dataset was compiled from patients screened for LC at SNH from 2015 to 2019. Descriptive statistics were calculated using the following variables: age, sex, race, education, body mass index (BMI), smoking history, personal cancer history, family LC history, chronic obstructive pulmonary disease (COPD), and emphysema. Variables distribution was compared using t- and chi-square tests. LC risk scores were calculated using SNH and PLCO models and categorized as low (scores <0.65%), moderate (0.65-1.49%), and high (>1.5%). Linear regression was applied to evaluate the relationship between models and covariates. Results: Of 896 individuals, 38 were diagnosed with LC. Data reflected the SNH patient demographics, which predominantly were African American (53.5%), current smokers (69.9%), and with emphysema (70.1%). Among the non-LC cohort, SNH model most frequently categorized patients as low risk, while PLCO model most frequently classified patients as moderate risk. Among the LC cohort, there was no significant difference between mean scores or risk stratification. SNH model showed 92.1% sensitivity and 96.8% specificity while PLCO model showed 89.4% sensitivity and 26.1% specificity. Emphysema demonstrated a strong association in SNH model (P<0.001) while race showed no relation. Conclusions: SNH model demonstrated greater specificity for characterizing LC risk in a SNH population. The results demonstrated the importance of study sample representation when identifying risk factors in a stratification model.

BACKGROUND: Sexual and gender minority (SGM) persons experience stark health disparities. Efforts to mitigate disparities through medical education have met some success. However, evaluations have largely focused on subjective perspectives rather than objective measures. This study aimed to quantify Boston University School of Medicine's sexual and gender minority (SGM) education through surveys of course directors (CDs) and medical students regarding where SGM topics were taught in the preclerkship medical curriculum. Responses were compared to identify concordance between faculty intention and student perceptions regarding SGM education. METHODS: A cross-sectional survey was distributed to preclerkship CDs and current medical students in Spring 2019 and 2021, respectively, regarding where in the mandatory preclerkship curriculum CDs deliberately taught and where first- and second-year students recalled having learned 10 SGM topic domains. RESULTS: 64.3% of CDs (n = 18), 47.0% of the first-year class (n = 71), and 67.3% of the second-year class (n = 101) responded to the surveys. Results indicate that, as anticipated, deliberate SGM teaching correlates with greater student recall as students recalled topics that were reported by CDs as intentionally taught at a significantly higher rate compared to those not intentionally taught (32.0% vs. 15.3%; p < 0.01). Students most commonly recalled learning SGM-related language and terminology, which is likely partly but not entirely attributed to curricular modifications and faculty development made between distribution of the faculty and student surveys, indicating the importance of all faculty being trained in appropriate SGM terminology and concepts. Discordance between faculty intention and student recall of when topics were taught reveals opportunities to enhance the intentionality and impact of SGM teaching. CONCLUSIONS: Students perceive and recall SGM content that is not listed as learning objectives, and all faculty who utilize this material in their teachings should receive foundational training and be thoughtful about how information is framed. Faculty who intentionally teach SGM topics should be explicit and direct about the conclusions they intend students to draw from their curricular content.

Radiofrequency ablation (RFA) is commonly used to treat atrial fibrillation (AF). However, the outcome is often compromised due to the lack of direct real-time feedback to assess lesion transmurality. In this work, we evaluated the ability of polarization-sensitive optical coherence tomography (PSOCT) to measure cardiac wall thickness and assess RF lesion transmurality during left atrium (LA) RFA procedures. Quantitative transmural lesion criteria using PSOCT images were determined ex vivo using an integrated PSOCT-RFA catheter and fresh swine hearts. LA wall thickness of living swine was measured with PSOCT and validated with a micrometer after harvesting the heart. A total of 38 point lesions were created in the LA of 5 living swine with the integrated PSOCT-RFA catheter using standard clinical RFA procedures. For all lesions with analyzable PSOCT images, lesion transmurality was assessed with a sensitivity of 89% (17 of 19 tested positive) and a specificity of 100% (5 of 5 tested negative) using the quantitative transmural criteria. This is the first report of using PSOCT to assess LA RFA lesion transmurality in vivo. The results indicate that PSOCT may potentially provide direct real-time feedback for LA wall thickness and lesion transmurality.

INTRODUCTION: Health Data Research UK designated seven UK-based Hubs to facilitate health data use for research. PIONEER is the Hub in Acute Care. PIONEER delivered workshops where patients/public citizens agreed key principles to guide access to unconsented, anonymised, routinely collected health data. These were used to inform the protocol. METHODS: This paper describes the PIONEER infrastructure and data access processes. PIONEER is a research database and analytical environment that links routinely collected health data across community, ambulance and hospital healthcare providers. PIONEER aims ultimately to improve patient health and care, by making health data discoverable and accessible for research by National Health Service, academic and commercial organisations. The PIONEER protocol incorporates principles identified in the public/patient workshops. This includes all data access requests being reviewed by the Data Trust Committee, a group of public citizens who advise on whether requests should be supported prior to licensed access. OBJECTIVES: This review aims to summarize the outcomes used to describe effectiveness of treatments for paediatric wrist fractures within existing literature. METHOD: We searched the Cochrane Library, Scopus, and Ovid Medline for studies pertaining to paediatric wrist fractures. Three authors independently identified and reviewed eligible studies. This resulted in a list of outcome domains and outcomes measures used within clinical research. Outcomes were mapped onto domains defined by the COMET collaborative. RESULTS: Our search terms identified 4,262 different papers. Screening of titles excluded 2,975, leaving 1,287 papers to be assessed for eligibility. Of this 1,287, 30 studies were included for full analysis. Overall, five outcome domains, 16 outcome measures, and 28 measurement instruments were identified as outcomes within these studies. 24 studies used at least one measurement pertaining to the physiological/clinical outcome domain. The technical, life impact, and adverse effect domains were recorded in 23, 20, and 11 of the studies respectively. Within each domain it was common for different measurement instruments to be used to assess each outcome measure. The most commonly reported outcome measures were range of movement, a broad array of "radiological measures" and pain intensity, which were used in 24, 23, and 12 of the 30 studies. CONCLUSION: This study highlights the heterogeneity in outcomes reported within clinical effectiveness studies of paediatric wrist fractures. We provided an overview of the types of outcomes reported in paediatric wrist fracture studies and identified a list of potentially relevant outcomes required for the development of a core outcome set.

Soil thickness is a fundamental variable in many earth science disciplines due to its critical role in many hydrological and ecological processes, but it is difficult to predict. Here we show a strong linear relationship (r2 = 0.87, RMSE = 0.19 m) between soil thickness and hillslope curvature across both convergent and divergent parts of the landscape at a field site in Idaho. We find similar linear relationships across diverse landscapes (n = 6) with the slopes of these relationships varying as a function of the standard deviation in catchment curvatures. This soil thickness-curvature approach is significantly more efficient and just as accurate as kriging-based methods, but requires only high-resolution elevation data and as few as one soil profile. Efficiently attained, spatially continuous soil thickness datasets enable improved models for soil carbon, hydrology, weathering, and landscape evolution.

Continental ice sheets typically sculpt landscapes via erosion; under certain conditions, ancient landscapes can be preserved beneath ice and can survive extensive and repeated glaciation. We used concentrations of atmospherically produced cosmogenic beryllium-10, carbon, and nitrogen to show that ancient soil has been preserved in basal ice for millions of years at the center of the ice sheet at Summit, Greenland. This finding suggests ice sheet stability through the Pleistocene (i.e., the past 2.7 million years). The preservation of this soil implies that the ice has been nonerosive and frozen to the bed for much of that time, that there was no substantial exposure of central Greenland once the ice sheet became fully established, and that preglacial landscapes can remain preserved for long periods under continental ice sheets.
