During mechanical ventilation, lung function and gas exchange in structurally heterogeneous lungs may be improved when volume oscillations at the airway opening are applied at multiple frequencies simultaneously, a technique referred to as multifrequency oscillatory ventilation (MFOV). This is in contrast to conventional high-frequency oscillatory ventilation (HFOV), for which oscillatory volumes are applied at a single frequency. In the present study, as a means of fully realizing the potential of MFOV, we designed and tested a computer-controlled hybrid oscillatory ventilator capable of generating the flows, tidal volumes, and airway pressures required for MFOV, HFOV, conventional mechanical ventilation (CMV), as well as oscillometric measurements of respiratory impedance. The device employs an iterative spectral feedback controller to generate a wide range of oscillatory waveforms. The performance of the device meets that of commercial mechanical ventilators in volume-controlled mode. Oscillatory modes of ventilation also meet design specifications in a mechanical test lung, over frequencies from 4 to 20 Hz and mean airway pressure from 5 to 30 cmH2O. In proof-of-concept experiments, the oscillatory ventilator maintained adequate gas exchange in a porcine model of acute lung injury, using combinations of conventional and oscillatory ventilation modalities. In summary, our novel device is capable of generating a wide range of conventional and oscillatory ventilation waveforms with potential to enhance gas exchange, while simultaneously providing less injurious ventilation.

Srcassociated in mitosis 68 kDa protein (Sam68) is a protein encoded by the heteronuclear ribonucleoprotein particle K homology (KH) single domaincontaining, RNAbinding, signal transductionassociated protein 1 (known as KHDRBS1) gene in humans. This protein contains binding sites for critical components in a variety of cellular processes, including the regulation of gene expression, RNA processing and cell signaling. Thus, Sam68 may play a role in a variety of diseases, including cancer. Sam68 has been widely demonstrated to participate in tumor cell proliferation, progression and metastasis to be involved in the regulation of cancer stem cell selfrenewal. Based on the body of evidence available, Sam68 emerges as a promising target for this disease. The objectives of the present included summarizing the role of Sam68 in cancer murine models and cancer patients, unraveling the molecular mechanisms underlying its oncogenic potential and discussing the effectiveness of antitumor agents in reducing the malignant effects of Sam68 during tumorigenesis.

The influence of oxygen on the thermal treatment (TT) of secondary metabolite-enriched extracts (SMEEs) from Tórtola beans and procyanidin C1 (PC1) on the inhibition of advanced glycation end products (AGEs) generation in proteins was investigated. SMEE was incubated at 4 °C (control) or thermally treated at 60 °C for 2 h, at either 0 % O2 (I) or 20 % O2 (II). Treatments I and II increased the content of procyanidin dimers B2. Treatment II was more effective than the control or treatment I in preventing homocysteine oxidation and AGEs generation. TT of PC1 at 0 % or 20 % O2 generated procyanidin dimers and tetramers. PC1 TT at 20 % O2 exhibited higher oxidation potentials and lower IC50 values of fluorescent AGEs than those of controls or TT at 0 % O2. These findings indicate that SMEE from Tórtola beans after treatment II changes the degree of polymerization and oxidation procyanidins, thereby increasing their antiglycation activity.

Blue maize is used in the production of various traditional foods, and its phytochemical composition has been claimed to possess health benefits. In this study, two blue maize hybrids with pigmented germ grown in five environments were studied under the hypothesis that the germ could have a different anthocyanin profile from that of anthocyanins synthesized in the aleurone layer, and that those in the germ could increase the total anthocyanin content in the whole grain. The percentage of pigmented germ, total anthocyanin content (TA) and total soluble phenols in the germ, whole grain and tortilla were evaluated to determine how tortilla color is modified. For the first time, the anthocyanin and fatty acid profiles of pigmented germ were determined. In the anthocyanin profile, anthocyanins derived from peonidin stood out, making 50.7 %. The most abundant fatty acid was linoleic acid (40.6 %). Whole kernel TA content increased when the maize had a higher percentage of pigmented germ, with minimal changes when grain was transformed to tortilla, resulting in darker tortillas. The large variation in TA among environments highlights the importance of identifying the environments that most favor anthocyanin synthesis.

Li6ZnO4 was chemically modified by nickel addition, in order to develop different compositions of the solid solution Li6Zn1-xNixO4. These materials were evaluated bifunctionally; analyzing their CO2 capture performances, as well as on their catalytic properties for H2 production via dry reforming of methane (DRM). The crystal structures of Li6Zn1-xNixO4 solid solution samples were determined through X-ray diffraction, which confirmed the integration of nickel ions up to a concentration around 20 mol%, meanwhile beyond this value, a secondary phase was detected. These results were supported by XPS and TEM analyses. Then, dynamic and isothermal thermogravimetric analyses of CO2 capture revealed that Li6Zn1-xNixO4 solid solution samples exhibited good CO2 chemisorption efficiencies, similarly to the pristine Li6ZnO4 chemisorption trends observed. Moreover, a kinetic analysis of CO2 isothermal chemisorptions, using the Avrami-Erofeev model, evidenced an increment of the constant rates as a function of the Ni content. Since Ni2+ ions incorporation did not reduce the CO2 capture efficiency and kinetics, the catalytic properties of these materials were evaluated in the DRM process. Results demonstrated that nickel ions favored hydrogen (H2) production over the pristine Li6ZnO4 phase, despite a second H2 production reaction was determined, methane decomposition. Thereby, Li6Zn1-xNixO4 ceramics can be employed as bifunctional materials.

ABSTRACT A young woman presented at our clinic with sudden visual loss in the right eye, recurrent vertigo, and right-sided tinnitus. We performed a complete ophthalmological evaluation. This revealed effects of the condition on the small arterioles of the peripheral retina. Susac syndrome is characterized by the clinical triad of retinal arteriolar occlusions, cochleovestibular manifestations, and encephalopathy (which can be identified by neuroimaging abnormalities). Early diagnosis and immunosuppressive therapy improved the patient's visual acuity and the remission of her other symptoms. Hemi-central retinal artery occlusion is an atypical neuro-ophthalmological finding in this disease. However, its identification as a sign of Susac syndrome may facilitate timely diagnosis and accurate treatment.

Resumo Enquadramento: A literacia em saúde (LS) é a capacidade da pessoa obter e traduzir informações a fim de manter e melhorar a saúde. Objetivo: Caracterizar o nível de LS da pessoa internada num hospital oncológico; avaliar a fiabilidade do instrumento European Health Literacy Survey in Portuguese (HLS-EU-PT). Metodologia: Estudo transversal, quantitativo, descritivo e metodológico. Amostra de 188 pessoas internados num serviço de cirurgia oncológica entre maio e setembro de 2020, os dados foram colhidos através de formulário de caracterização sociodemográfica, de saúde e instrumento de avaliação da LS. Resultados: O HLS-EU-PT apresentou elevado nível de consistência interna. Todos os domínios e níveis de processamento do instrumento se correlacionam positivamente entre si. Os participantes apresentam em média um nível problemático de LS. Conclusão: Os resultados indiciam a necessidade de um maior investimento na capacitação da LS. São necessários mais estudos nesta e noutras populações e contextos de prestação de cuidados de saúde, de forma a direcionar a prática de cuidados na resposta eficaz aos problemas de saúde.
Abstract Background: Health literacy (HL) is the ability of an individual to obtain and translate information in order to maintain and improve their health. Objective: To characterize the level of HL of patients admitted to an oncology hospital and assess the reliability of the Portuguese version of the European Health Literacy Survey (HLS-EU-PT). Methodology: Cross-sectional, quantitative, descriptive, and methodological study. Sample of 188 patients admitted to a surgical oncology unit between May and September 2020. Data were collected through a sociodemographic and health characterization form and a HL assessment tool. Results: The HLS-EU-PT had a high level of internal consistency. All domains and information-processing levels correlated positively with each other. Participants had on average a problematic level of HL. Conclusion: The results indicate the need for greater investment in HL training. Further studies are needed in this population and in other populations and healthcare settings to provide an effective response to health problems.
Resumen Marco contextual: La alfabetización en salud (AS) es la capacidad de una persona para obtener y traducir información con el fin de mantener y mejorar su salud. Objetivo: Caracterizar el nivel de AS de las personas ingresadas en un hospital oncológico; evaluar la fiabilidad del instrumento European Health Literacy Survey in Portuguese (HLS-EU-PT). Metodología: Estudio transversal, cuantitativo, descriptivo y metodológico. Muestra de 188 personas ingresadas en un servicio de cirugía oncológica entre mayo y septiembre de 2020, se recogieron datos mediante un formulario de caracterización sociodemográfica y de salud, y una herramienta de evaluación de AS. Resultados: El HLS-EU-PT mostró un alto nivel de consistencia interna. Todos los dominios y niveles de procesamiento del instrumento correlacionaron positivamente entre sí. Por término medio, los participantes tienen un nivel problemático de AS. Conclusión: Los resultados indican la necesidad de una mayor inversión en formación en AS. Son necesarios más estudios en esta y otras poblaciones, y en entornos sanitarios para orientar la práctica asistencial hacia una respuesta eficaz a los problemas de salud.

RESUMEN El objetivo esencial de este documento es mostrar los aspectos más significativos del libro Antropología: por qué importa de Tim Ingold. Para tal fin, se explora, describe e interpreta los cinco capítulos de manera analítica. Se concluye que el texto es un aporte novedoso, radical, holístico, original y crítico del panorama antropológico. En tal sentido, resulta de utilidad para intuir las condiciones de posibilidad de lo humano, esto es, la búsqueda de la diversidad cultural, sociopolítica, ambiental y humanista. Por otro lado, lo prolijo y sugestivo del marco proyectivo teórico del autor constituye una atrayente visión establecida en un arquetipo panorámico de la antropología y su quehacer pragmático. Por ende, este libro es de gran utilidad para estudiantes de pregrado y postgrado.
ABSTRACT The essential objective of this document is to show the most significant aspects of the book entitled Anthropology: Why It Matters by Tim Ingold. To this end, the five chapters are explored, described and interpreted analytically. It is concluded that the text is a novel, radical, holistic, original and critical contribution to the anthropological panorama. In this sense, it is useful to intuit the conditions of possibility of the human, that is, the search for cultural, sociopolitical, environmental and humanistic diversity. On the other hand, the prolix and suggestive nature of the author's theoretical projective framework constitutes an attractive vision established in a panoramic archetype of anthropology and its pragmatic work. Therefore, this book is very usefulfor undergraduate andgraduate students.

Abstract Introduction: The Short Health Anxiety Inventory is a commonly used tool for assessing health anxiety, but its psychometric properties and internal structure have not been examined in a Latin American Spanish-speaking population. This study aimed to establish the psychometric properties among Colombian university students. Method: The goodness of fit of four latent structure models of the Short Health Anxiety Inventory was tested using confirmatory factor analysis in a sample of 1004 Colombian university students. Results: The results show that the original model's structure does not fit well (CFI = .808; RMSEA = .074), and the reliability was .796 and .703 for the original two variables. Conclusions: The findings do not support the utilization of the Colombian version of the Short Health Anxiety Inventory by researchers and clinicians among Colombian university students.
Resumen Introducción: El Short Health Anxiety Inventory es una herramienta común para evaluar la ansiedad relacionada con la salud, pero sus propiedades psicométricas y estructura interna no han sido evaluadas en una población latinoamericana de habla hispana. Este estudio tuvo como objetivo establecer las propiedades psicométricas en estudiantes universitarios colombianos. Método: Se evaluó la bondad de ajuste de cuatro modelos de estructura latente del Short Health Anxiety Inventory, mediante análisis factorial confirmatorio, en una muestra de 1004 estudiantes universitarios colombianos. Resultados: La estructura no se ajusta al modelo original (CFI = .808; RMSEA = .074) y la confiabilidad fue de .796 y .703 para las dos variables originales. Conclusiones: Los resultados no respaldan el uso de la versión colombiana del Short Health Anxiety Inventory entre estudiantes universitarios colombianos, tanto para investigadores como para clínicos.

La fiebre reumática aguda es una enfermedad con baja incidencia en nuestra región. Sus complicaciones conllevan una elevada morbi-mortalidad. El diagnóstico es un desafío en la actualidad, teniendo la clínica un rol preponderante; permitiéndonos mantener una alta sospecha a pesar de su baja incidencia. Se presenta el caso clínico de un paciente sexo masculino cuyo diagnóstico fue un reto para el equipo de salud.
Acute rheumatic fever is a disease with low incidence in our region. Its complications involve high morbidity and mortality. Its diagnosis is currently a challenge, with clinical presentation playing a predominant role, allowing us to maintain a high diagnostic suspicion despite its low incidence. The clinical case of a male patient is presented, whose diagnosis posed a challenge for the healthcare team.
A Febre reumática aguda é uma doença com baixa incidência em nossa região. Suas complicações envolvem alta morbidade e mortalidade. Seu diagnóstico é atualmente um desafio, com a apresentação clínica desempenhando um papel predominante, permitindo-nos manter uma alta suspeita diagnóstica apesar de sua baixa incidência. É apresentado o caso clínico de um paciente do sexo masculino, cujo diagnóstico representou um desafio para a equipe de saúde.

Abstract Introduction/Objective: In romantic relationships, jealousy can influence how individuals feel emotionally and interact with their partners. This study analysed the metric properties of the Brief Jealousy Scale (BJS) in young adults from Peru using current methods. Method: A total of 297 individuals were selected to participate in the study. They averaged 26.52 years of age (with a variation of 7.75 years), and 74.10% were women. The study used a technique called Item Response Theory (IRT) with a two-parameter model to confirm the structure and reliability of the BJS. Result: The BJS demonstrated high reliability and fit through goodness-of-fit indices. DIF analysis indicated slight gender biases in some items, reflecting differences in the perception of jealousy between men and women. Conclusions: The study highlights the importance and usefulness of the Jealousy Scale in relationships, as it helps to understand relationship dynamics and guides interventions. Future studies should address issues such as participant selection and study design and examine whether the scale can be used in different cultures.
Resumen Introducción/Objetivo: En las relaciones de pareja, los celos pueden influir en cómo se sienten emocionalmente y en cómo interactúan las personas con su pareja. Este estudio analizó las propiedades métricas de la Escala Breve de Celos en jóvenes y adultos de Perú utilizando métodos actuales. Método: Se seleccionaron 297 personas para participar en el estudio. Tenían en promedio 26.52 años (con una variación de 7.75 años) y el 74.10 % eran mujeres. El estudio utilizó una técnica llamada teoría de respuesta al ítem (IRT) con un modelo de dos parámetros para confirmar la estructura y fiabilidad del BJS. Resultados: El BJS demostró una gran confiabilidad y ajuste a través de índices de bondad de ajuste. El análisis DIF indicó ligeros sesgos de género en algunos ítems, lo que refleja diferencias en la percepción de los celos entre hombres y mujeres. Conclusiones: El estudio resalta la importancia y utilidad de la Escala de Celos en las relaciones, ya que ayuda a entender cómo funcionan y a saber cómo intervenir. En el futuro, los estudios deben ocuparse de problemas como la selección de participantes y el tipo de estudio realizado, y examinar si la escala se puede usar en diferentes culturas.

Objetivo: conhecer as representações sociais sobre o planejamento reprodutivo entre mulheres em gravidez não planejada na Estratégia Saúde da Família. Método: estudo qualitativo, orientado pela Teoria das Representações Sociais, realizado com 15 gestantes, entre abril e maio de 2019. Utilizou-se a entrevista semiestruturada. Os dados foram organizados por meio do Discurso do Sujeito Coletivo, com auxílio do software DSCsoft©. Protocolo de pesquisa aprovado pelo Comitê de Ética em Pesquisa. Resultados: as representações sociais das mulheres em gravidez não planejada evidenciadas pelo Discurso do Sujeito Coletivo foram representadas por oito ideias centrais, a saber: "eu não me preveni, nem ele", "nós nos prevenimos", "eu comprava", "pegava no posto", "construir uma família", "ter esse acesso", "estou por fora" e "eu sei que é disponível". Conclusão: as representações sociais nos discursos das mulheres em gravidez não planejada estavam pautadas no desconhecimento acerca do planejamento reprodutivo, dos anticoncepcionais disponíveis e seu uso correto.
Objective: to understand the social representations of reproductive planning among women with unplanned pregnancies in the Family Health Strategy. Method: qualitative study, guided by the Theory of Social Representations, carried out with 15 pregnant women between April and May 2019. Semi-structured interviews were used. The data was organized using the Discourse of the Collective Subject, with the aid of DSCsoft© software. Research protocol approved by the Research Ethics Committee. Results: the social representations of women with unplanned pregnancies as evidenced by the Collective Subject Discourse were represented by eight central ideas, namely: "I didn't prevent myself, nor did he", "we prevented ourselves", "I would buy it", "I would get it at the health center", "build a family", "have this access", "I am not aware" and "I know it is available". Conclusion: the social representations in the women's speeches about unplanned pregnancies were based on a lack of knowledge about reproductive planning, the contraceptives available and their correct use.

Método: estudio cualitativo, basado en la Teoría de las Representaciones Sociales, realizado con 15 mujeres embarazadas, entre abril y mayo de 2019. Se utilizaron entrevistas semiestructuradas. Los datos fueron organizados mediante el Discurso del Sujeto Colectivo, con ayuda del software DSCsoft©. El protocolo de investigación fue aprobado por el Comité de Ética en Investigación. Resultados: las representaciones sociales de las mujeres con embarazo no planificado reveladas por el Discurso del Sujeto Colectivo fueron representadas por ocho ideas centrales, a saber: "yo no me cuidé y él tampoco", "nos cuidamos", "yo los compraba", "los buscaba en el centro de salud", "construir una familia", "tener acceso", "no participo" y "sé que está disponible". Conclusión: las representaciones sociales en los discursos de las mujeres con embarazo no planificado se basaron en la falta de conocimiento sobre la planificación reproductiva, en los anticonceptivos disponibles y su uso correcto.

Abstract Introduction: Leptodactylus latinasus and Physalaemus cuqui are sympatric anuran species with similar environmental requirements and contrasting reproductive modes. Climatic configuration determines distribution patterns and promotes sympatry of environmental niches, but specificity/selectivity determines the success of reproductive modes. Species distribution models (SDM) are a valuable tool to predict spatio-temporal distributions based on the extrapolation of environmental predictors. Objectives: To determine the spatio-temporal distribution of environmental niches and assess whether the protected areas of the World Database of Protected Areas (WDPA) allow the conservation of these species in the current scenario and future. Methods: We applied different algorithms to predict the distribution and spatio-temporal overlap of environmental niches of L. latinasus and P. cuqui within South America in the last glacial maximum (LGM), middle-Holocene, current and future scenarios. We assess the conservation status of both species with the WDPA conservation units. Results: All applied algorithms showed high performance for both species (TSS = 0.87, AUC = 0.95). The L. latinasus predictions showed wide environmental niches from LGM to the current scenario (49 % stable niches, 37 % gained niches, and 13 % lost niches), suggesting historical fidelity to stable climatic-environmental regions. In the current-future transition, L. latinasus would increase the number of stable (70 %) and lost (20 %) niches, suggesting fidelity to lowland regions and a possible trend toward microendemism. P. cuqui loses environmental niches from the LGM to the current scenario (25 %) and in the current-future transition (63 %), increasing the environmental sympathy between both species; 31 % spatial overlap in the current scenario and 70 % in the future. Conclusion: Extreme drought events and rainfall variations, derived from climate change, suggest the loss of environmental niches for these species that are not currently threatened but are not adequately protected by conservation units. The loss of environmental niches increases spatial sympatry which represents a new challenge for anurans and the conservation of their populations.
Resumen Introducción: Leptodactylus latinasus y Physalaemus cuqui son especies de anuros simpátricos con requerimientos ambientales similares y modos reproductivos contrastantes. La configuración climática determina los patrones de distribución y promueve la simpatría de los nichos ambientales, pero la especificidad/selectividad determina el éxito de los modos reproductivos. Los modelos de distribución de especies (MDE) son una herramienta valiosa para predecir distribuciones espacio-temporales basadas en la extrapolación de predictores ambientales. Objetivos: Determinar la distribución espacio-temporal de los nichos ambientales y evaluar si las áreas protegidas de la base de Datos Mundial de Áreas Protegidas (DMAP) permiten la conservación de estas especies en el escenario actual y futuro. Métodos: Aplicamos diferentes algoritmos para predecir la distribución y superposición espacio-temporal de nichos ambientales de L. latinasus y P. cuqui dentro de América del Sur en el último máximo glacial (UGM), Holoceno medio, actual y futuro. Evaluamos el estado de conservación de ambas especies con las unidades de conservación de la DMAP. Resultados: Todos los algoritmos aplicados mostraron un alto rendimiento para ambas especies (TSS = 0.87, AUC = 0.95). Las predicciones de L. latinasus mostraron amplios nichos ambientales desde LGM hasta el escenario actual (49 % de nichos estables, 37 % de nichos ganados y 13 % de nichos perdidos), sugiriendo fidelidad histórica por regiones climático-ambientales estables. En la transición actual-futura L. latinasus incrementaría la cantidad de nichos estables (70 %) y perdidos (20 %), sugiriendo fidelidad por regiones de tierras bajas y la posible tendencia hacia el microendemismo. P. cuqui pierde nichos ambientales desde el LGM al escenario actual (25 %) y en la transición actual-futura (63 %), incrementando la simpatría ambiental entre ambas especies; 31 % de superposición espacial en el escenario actual y 70 % en el futuro. Conclusión: Los eventos de sequía extrema y las variaciones de precipitaciones, derivados del cambio climático, sugieren la pérdida de nichos ambientales para estas especies, actualmente no se encuentran amenazadas, pero no están adecuadamente protegidas por las unidades de conservación. La pérdida de nichos ambientales aumenta la simpatría espacial que representa un nuevo desafío para estos anuros y la conservación de sus poblaciones.

Ganoderma species have been studied for their pharmacological approaches, such as anticancer, antitumor, antiproliferative, and antioxidant activity. Elicitors are used to increase Ganoderma bioactive composite production. This study aims to evaluate the antiproliferative activity of ethanolic extracts from mycelium of Ganoderma tuberculosum (G. tuberculosum) grown in a liquid medium with vineyard pruning waste (VPW) extracts as elicitors. Ethanolic and aqueous VPW extracts contain resveratrol dimer 4, resveratrol tetramer 1, and naringenin, while toluene and chloroform extracts contain tetradecanoic acid, hexadecanoic acid, and octadecanoic acid. Polar and nonpolar extracts could be promising elicitors for increasing bioactive molecules. Catechin gallate showed the highest correlation (r = 0.66) with biomass. Mycelial ethanolic extracts of G. tuberculosum (native strain from the Sonoran Desert) and Ganoderma lucidum (G. lucidum) (control) were analyzed by ESI-IT-MS, and 27 molecules were identified for the two species. They showed antiproliferative activity against the A549 and C-33 A cell lines but not for ARPE-19. G. tuberculosum culture with VPW had quinic acid, ganodermenonol, ganoderic acid I (GA-I), C2 (GA-C2), and 20-hydroxylucidenic acid P, among others. Molecular docking of ganodermenonol, GA-I, and GA-C2 demonstrates significant interaction with tumor necrotic factor (TNF-α). These ethanolic extracts of Ganoderma are promising sources of bioactive triterpenoids. Their antiproliferative activity did not change between species or treatment. Likewise, the G. tuberculosum and G. lucidum extracts only affected cancer cell lines. This property seems promising for pharmacological applications of these fungal extracts.

Colchicine has an expanding role in cardiovascular disease treatment. Colchicine overdose is a toxicologic emergency. Direct cellular toxicity interferes with myocardial contractility, leading to cardiovascular collapse. We present a case of a patient with a colchicine overdose supported with venoarterial extracorporeal membrane oxygenation, highlighting the challenges and limitations.

INTRODUCTION: The use of pneumoperitoneum is an essential step for performing laparoscopic and robotic surgery. Pneumoperitoneum insufflation can cause complications such as pneumothorax, subcutaneous emphysema, and pneumomediastinum. The purpose of this meta-analysis is to compare the safety of using the conventional insufflation system versus a Valveless insufflation system as devices for manufacturing pneumoperitoneum in robotic-assisted nephrectomy. METHODS: A comprehensive literature search was conducted on PUBMED, EMBASE, SCOPUS, and Cochrane, from inception until January 2024. Randomized and nonrandomized prospective studies were included in the meta-analysis, performed by the R+ Rstudio. RESULTS: Three publications encompassing 478 patients were included. We observed no difference in SCE (OR 0.60, CI 95% 0.27;1.34, p = 0.134, I2 = 0), PNM (OR 0.82, CI 95% 0.24;2.78, p = 0.558, I2 = 0), When comparing groups conventional insufflation system (CIS) versus AirSealTM insufflation system (AIS) with pneumoperitoneum pressure of 15 mmHg or 12 mmHg. CONCLUSION: The presented data showed no difference between the AIS when compared with CIS in Pneumoperitoneum insufflation-related complications.

BACKGROUND: While acute compartment syndrome (ACS) is a well-reported complication after pediatric tibial shaft fractures, prior literature has suggested that pediatric patients with tibial tubercle fractures may be at increased risk of ACS due to the proximity of the tibial tubercle to the anterior tibial recurrent artery. However, this theory was largely based on a series of early case reports without substantiation in larger-scale studies. HYPOTHESIS/PURPOSE: The purpose of this study is to conduct a population-level analysis of the incidence and risk factors of acute compartment syndrome following pediatric tibial tubercle and tibial shaft fractures. We hypothesize that the rate of ACS would be lower in patients with tibial tubercle fractures when compared with those with tibial shaft fractures. METHODS: A retrospective cohort analysis of the PearlDiver Mariner database was performed by querying all patients diagnosed with tibial tubercle and tibial shaft fractures between January 2010 and October 2022. Matched cohorts (n=25,483) of patients with pediatric tibial tubercle and tibial shaft fractures were captured using International Classification of Diseases, Ninth Revision (ICD-9), Tenth Revision (ICD-10) billing codes, and age as inclusion/exclusion criteria. Rates of subsequent compartment syndrome were calculated by querying for insurance claims with associated CPT codes for fasciotomy and/or ICD-9/ICD-10 billing codes for compartment syndrome. Student t test and χ2 analyses were used to compare demographics between the tibial tubercle and tibial shaft cohorts. RESULTS: The rate of ACS was significantly lower in the tibial tubercle cohort (0.46%) than in the tibial shaft cohort (0.70%, P<0.001). Male sex and increased age were associated with an increased risk of developing ACS in both cohorts (P<0.001). In addition, polytrauma was found to be a risk factor for ACS among patients with tibial shaft fractures (P<0.001). CONCLUSION: We report a low rate of ACS following both pediatric tibial tubercle fractures and pediatric tibial shaft fractures. Among patients with tibial tubercle fractures, male sex, and increased age were noted to be risk factors for ACS, whereas male sex, older age, and polytrauma were risk factors for ACS among patients with tibial shaft fractures.

Objective: Pre-slaughter transportation adversely impacts the welfare, meat yield, and quality of broilers, yet the effects of different crate types on broiler chickens during winter remain underexplored. The goal of this study was to investigate the effect of plastic and iron crates in transit on meat quality, carcass, and physiological traits of broiler chickens during winter. Methods: A total of 175, 35-day-old Ross 308 male broilers with an average body weight of about 1,708 ± 33.3 g (mean ± standard error of the mean) were picked after 4 hours of feed withdrawal before transport. The control group comprises birds in the farm (n = 15) without transportation at 173 cm2/kg density. The birds were transported into fixed iron (25 birds per crate) and plastic crates (15 birds per crate) with four replicates per crate type at the same 173 cm2/kg densities. The transportation distance was 20 km for 40 min at an average speed of 30-50 km/h early morning at 8:00 am under - 1 â and 47% relative humidity. Results: There was no difference (p>0.05) in carcass traits among the treatments. Concerning meat quality, broilers transported in both crate types exhibited lower (p<0.01) a* values compared to the control group. Additionally, the iron crate group demonstrated higher (p<0.05) b* values for the breast meat compared to the other groups. In terms of blood metabolites, the iron crate group had higher (p<0.05) cortisol, glucose, and lactate levels compared to the control group that did not transport. Conclusion: Broilers transported in the iron crates increase stress levels in terms of higher cortisol, glucose, and lactate contents in the blood plasma compared to untransported broilers during the winter. Therefore, employing plastic crates, which induce significantly reduced cortisol and numerically lower glucose levels compared to iron crates, appears more favorable for animal welfare by mitigating stress.

Importance: Since 2018, a movement has emerged to define Alzheimer disease (AD) as a purely biological entity based on biomarker findings. The recent revision of the Alzheimer Association (AA) criteria for AD furthers this direction. However, concerns about a purely biological definition of AD being applied clinically, the understanding of AD by society at large, and the translation of blood-based biomarkers into clinical practice prompt these International Working Group (IWG) updated recommendations. Objective: To consider the revised AA criteria and to offer an alternative definitional view of AD as a clinical-biological construct for clinical use. The recommendations of the 2021 IWG diagnostic criteria are updated for further elaborating at-risk and presymptomatic states. Evidence Review: PubMed was searched for articles published between July 1, 2020, and March 1, 2024, using the terms "biomarker" OR "amyloid" OR "tau" OR "neurodegeneration" OR "preclinical" OR "CSF" OR "PET" OR "plasma" AND "Alzheimer's disease." The references of relevant articles were also searched. Findings: In the new AA diagnostic criteria, AD can be defined clinically as encompassing cognitively normal people having a core 1 AD biomarker. However, recent literature shows that the majority of biomarker-positive cognitively normal individuals will not become symptomatic along a proximate timeline. In the clinical setting, disclosing a diagnosis of AD to cognitively normal people with only core 1 AD biomarkers represents the most problematic implication of a purely biological definition of the disease. Conclusions and Relevance: The ultimate aim of the field was to foster effective AD treatments, including preventing symptoms and dementia. The approach of diagnosing AD without a clinical and biological construct would be unwarranted and potentially concerning without a clear knowledge of when or whether symptoms will ever develop. It is recommended that those who are amyloid-positive only and, more generally, most biomarker-positive cognitively normal individuals, should not be labeled as having AD. Rather, they should be considered as being at risk for AD. The expansion of presymptomatic AD is viewed as a better diagnostic construct for those with a specific pattern of biomarkers, indicating that they are proximate to the expression of symptoms in the near future.
