The influence of the vehicle in topical treatment adherence remains to be elucidated. The aim of this study is to analyze the influence of the pharmaceutical dosage form on adherence to topical treatment in psoriasis patients, taking into consideration the mechanical features. The adherence was evaluated in a sample of 102 psoriasis patients, followed for approximately 45 days. Adherence was calculated with a new combined methodology using a log and medication weights. The effect of the group formulation was evaluated using logistic regression models. A complex effect of the vehicle on adherence was found, mediated by the affected area. The adherence was significantly higher for patients applying gels and creams than for those using ointments, whenever the body area affected was extensive. The opposite was found when the affected area was small. Mechanical properties can partially explain the findings since gels and creams may be easier to apply. Patient beliefs and preferences regarding vehicles and their sensory attributes might also explain the results. It is noteworthy that adherence was strikingly low, with more than 75% non-adherent patients. This real-world evidence provides an insight for pharmaceutical industries and guidance for treatment prescription by physicians aiming to address the public health emergency of treatment non-adherence.

In the Brazilian Pantanal, wildfire occurrence has increased, reaching record highs of over 40,000 km2 in 2020. Smoke from wildfires worsened the situation of isolated, as well as urban communities, already under an increasing toll of COVID-19. Here we review the impacts and the possible causes of the 2020 mega-fires and recommend improvements for public policies and fire management in this wetland. We calculated the amount of area burnt annually since 2003 and describe patterns in precipitation and water level measurements of the Paraguay River. Our analyses revealed that the 2020 wildfires were historically unprecedented, as 43% of the area (over 17,200 km2) had not been burnt previously in the last two decades. The extent of area affected in 2020 represents a 376% increase compared to the annual average of the area burnt annually in the last two decades, double than the value in 2019. Potential factors responsible for this increase are (i) severe drought decreased water levels, (ii) the fire corridor was located in the Paraguay River flood zone, (iii) constraints on firefighters, (iv) insufficient fire prevention strategy and agency budget reductions, and (v) recent landscape changes. Climate and land use change will further increase the frequency of these extreme events. To make fire management more efficient and cost-effective, we recommend the implementation of an Integrated Fire Management program in the Pantanal. Stakeholders should use existing traditional, local ecological, and scientific knowledge to form a collective strategy with clear, achievable, measurable goals, considering the socio-ecological context. Permanent fire brigades, including indigenous members, should conduct year-round fire management. Communities should cooperate to create a collaborative network for wildfire prevention, the location and characteristics (including flammability) of infrastructures should be (re)planned in fire-prone environments considering and managing fire-catalysed transitions, and depending on the severity of wildfires. The 2020 wildfires were tackled in an ad-hoc fashion and prioritisation of areas for urgent financial investment, management, protection, and restoration is necessary to prevent this catastrophe from happening again.

RESUMO

The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

INTRODUCTION: Pancreatic surgery still carries a high morbidity and mortality even in specialized centers. The aim of this study was to evaluate the influence of patients' body composition on postoperative complications and survival after pancreatic surgery. METHODS: This was a retrospective study on patients undergoing pancreatic surgery between March 2012 and December 2017. Demographics, clinical data, and postoperative complications classified according to Clavien-Dindo were recorded. Body composition was assessed using routine diagnostic or staging computed tomography (CT). Multiple Cox proportional hazards models were adjusted. RESULTS: Ninety patients were included, 55% were male, and the mean age was 68 ± 10.9 years. Of these 90, 92% had a total pancreatectomy or pancreaticoduodenectomy, 7% a distal pancreatectomy, and 1% a pancreaticoduodenectomy with multi-visceral resection; 84% had malignant disease. The incidence of major complications was 27.8% and the 90-day mortality was 8.8%. The ratio of visceral fat area/skeletal muscle area (VFA:SMA) was associated with an increased risk of complications (OR 2.24, 95% CI 1.14-4.87, p = 0.03) and 90-day survival (HR 2.13, 95% CI 1.13-4.01, p = 0.019). On simple analysis, shorter overall survival (OS) was observed in patients aged ≥70 years (p = 0.0009), with postoperative complications ≥IIIb (p = 0.01), an increased VFA:SMA (p = 0.007), and decreased muscle radiation attenuation (p = 1.6 × 10-5). In an OS model adjusted for age, disease malignancy, postoperative complications, and body composition parameters, muscle radiation attenuation remained significantly associated with survival (HR 0.94, 95% CI 0.90-0.98, p = 0.0016). A model which included only body composition variables had a discrimination ability (C-statistic 0.76) superior to a model which comprised conventional clinical variables (C-statistic 0.68). CONCLUSION: Body composition is a major determinant of postoperative complications and survival in pancreatic surgery patients.


INTRODUÇÃO: A cirurgia pancreática continua associada a uma elevada morbimortalidade mesmo em centros de referência. O objetivo do presente estudo foi avaliar a influência da composição corporal nas complicações e sobrevivência após cirurgia pancreática. MÉTODOS: Estudo retrospetivo em doentes submetidos a cirurgia pancreática entre Março 2012 e Dezembro 2017. Foram registadas variáveis demográficas, clínicas, complicações pós-cirúrgicas classificadas de acordo com a classificação de a classificação de Clavien Dindo. A composição corporal foi avaliada utilizando imagens de Tomografia Axial Computorizada (TAC) realizada no diagnóstico. Modelos de riscos porporcionais de Cox foram ajustados. RESULTADOS: Incluídos 90 doentes, 55% homens e média de idade de 68 ± 10.9 anos; 92% submetidos a pancreatectomia/pancreatoduodenectomia, 7% pancreatectomia distai e 1% pancreatoduodenectomia com ressecção multivisceral; 84% tinham doença maligna. A incidência de complicações major foi de 27.8% e a mortalidade aos 90 dias de 8.8%. O rácio da Área de Gordura Visceral (AGV) / Área de Tecido Muscular Esquelético (ATME) associou-se a um risco acrescido de complicações (OR 2.24, 95% Cl 1.14­4.87, p = 0.03) e de morte aos 90 dias (HR 2.13, 95% Cl 1.13­4.01, p = 0.019). Observámos uma sobrevivência global mais baixa em doentes com idade ≥70 anos (p = 0.0009), com complicações ≥IIIb (p = 0.01), com AGV/ ATME aumentada e atenuação muscular diminuída (p = 1.6 × 10−5). A atenuação muscular ajustada para a idade, malignidade, complicações, manteve-se associada à sobrevivência global (HR 0.94, IC 95% 0.90­0.98, p = 0.0016). Um modelo que inclua apenas variáveis de composição corporal mostrou uma capacidade discriminatória (C-statistic 0.76) superior a um modelo convencional com variáveis clínicas (C-statistic 0.68). CONCLUSÃO: A composição corporal é um determinante major de complicações pós-cirúrgicas e de sobrevivência de doentes submetidos a cirurgia pancreática.

The present study proposes the use of endophytic fungi for mercury bioremediation in in vitro and host-associated systems. We examined mercury resistance in 32 strains of endophytic fungi grown in culture medium supplemented with toxic metal concentrations. The residual mercury concentrations were quantified after mycelial growth. Aspergillus sp. A31, Curvularia geniculata P1, Lindgomycetaceae P87, and Westerdykella sp. P71 were selected and further tested for mercury bioremediation and bioaccumulation in vitro, as well as for growth promotion of Aeschynomene fluminensis and Zea mays in the presence or absence of the metal. Aspergillus sp. A31, C. geniculata P1, Lindgomycetaceae P87 and Westerdykella sp. P71 removed up to 100% of mercury from the culture medium in a species-dependent manner and they promoted A. fluminensis and Z. mays growth in substrates containing mercury or not (Dunnett's test, p < 0.05). Lindgomycetaceae P87 and C. geniculata P1 are dark septate endophytic fungi that endophytically colonize root cells of their host plants. The increase of host biomass correlated with the reduction of soil mercury concentration due to the metal bioaccumulation in host tissues and its possible volatilization. The soil mercury concentration was decreased by 7.69% and 57.14% in A. fluminensis plants inoculated with Lindgomycetaceae P87 + Aspergillus sp. A31 and Lindgomycetaceae P87, respectively (Dunnet's test, p < 0.05). The resistance mechanisms of mercury volatilization and bioaccumulation in plant tissues mediated by these endophytic fungi can contribute to bioremediation programs. The biochemical and genetic mechanisms involved in bioaccumulation and volatilization need to be elucidated in the future.

AA amyloidosis is a rare complication of chronic inflammatory disorders and has been associated with rheumatoid arthritis and ankylosing spondylitis. We present a case of AA amyloidosis secondary to Sjogren's syndrome (SS). A 79-year-old woman presented with rapidly progressive renal failure and complaints of asthenia, anorexia and generalized oedema. She had severe renal failure (creatinine 6.0 mg/dl), with microscopic haematuria, nephrotic proteinuria and low serum albumin levels, and an increased erythrocyte sedimentation rate. Serum protein electrophoresis revealed a peak in the gamma globulin zone. The patient was started on haemodialysis and corticosteroids. Clinical results showed the patient met the diagnostic criteria for primary SS, and neoplastic haematological disease was excluded. Renal biopsy revealed a diagnosis of AA amyloidosis. Renal AA amyloidosis is a rare condition in patients with primary SS. However, in patients with proteinuria and/or renal failure, it should be included in the differential diagnosis and a renal biopsy should be performed. LEARNING POINTS: Sjogren's syndrome should be regarded as a predisposing condition for the development of renal AA amyloidosis.Sjogren's syndrome and renal AA amyloidosis have been diagnosed simultaneously in some patients.A renal biopsy should be performed in patients with Sjogren's syndrome and proteinuria and/or renal failure.

Peritoneal loose bodies are usually diagnosed incidentally. Only a few cases are documented in the literature. The pathophysiology of this condition is not fully known and its origin may possibly be related to the twisting and separation of epiploic appendages. The authors describe the case of a patient with a spherical solid lesion 6 cm in diameter, identified incidentally in the pelvic cavity of a 64 year old man. The patient underwent laparotomy and a free ovoid shaped white body was identified. Histological examination described a calcified and encapsulated mass with a steato-necrosis core. A review of all cases of giant peritoneal loose bodies described in the literature is included. Surgical excision is recommended when the giant peritoneal loose body diagnosis is uncertain or when they are symptomatic. Doctors should be aware of its existence in order to establish a proper diagnosis and treatment plan.


Os corpos peritoneais livres são habitualmente diagnosticados de forma incidental. Apenas alguns casos estão documentados na literatura. A sua fisiopatologia não é totalmente conhecida, estando eventualmente relacionada com a torção e separação dos apêndices epiplóicos. É descrito o caso de um homem com 64 anos de idade, com uma lesão sólida esférica de 6 cm de diâmetro na cavidade pélvica, identificada incidentalmente por tomografia computorizada. Foi realizada laparotomia exploradora, identificando-se uma lesão livre, de morfologia ovoide e esbranquiçada. O exame histológico mostrou uma massa de esteatonecrose calcificada e capsulada. Foi feita uma revisão dos casos de corpos peritoneais livres gigantes descritos na literatura. A excisão cirúrgica é recomendada quando o diagnóstico é duvidoso ou quando as lesões são sintomáticas. Os médicos devem ter conhecimento desta entidade, para poderem estabelecer um diagnóstico e plano de tratamento corretos.

Background: Hydroxychloroquine (HCQ) is a recommended drug in systemic lupus erythematosus (SLE). It has a long terminal half-life, making it an attractive target for therapeutic drug monitoring. The aim of this study was to establish a relationship between blood HCQ concentration and lupus nephritis activity. Methods: We conducted a retrospective observational study with data collected from clinical and laboratory records. Inclusion criteria were patients followed in the lupus clinic with biopsy-proven International Society of Nephrology/Renal Pathology Society Classes III, IV or V lupus nephritis on HCQ for at least 3 months (200-400 mg daily) and with HCQ levels measured during treatment. Exclusion criteria were patients on renal replacement therapy at baseline or patients lost to follow-up. Results: In 171 patients, the HCQ level was measured in 1282 samples. The mean HCQ blood level was 0.75±0.54mg/L and it was bimodally distributed. An HCQ level <0.20 mg/L [232 samples (18.1%)] appeared to define a distinct group of abnormally low HCQ levels. For patients in complete or partial remission at baseline compared with those remaining in remission, patients with renal flare during follow-up had a significantly lower average HCQ level (0.59 versus 0.81 mg/L; P= 0.005). Our data suggest an HCQ target level to reduce the likelihood of renal flares >0.6 mg/L (600 ng/mL) in those patients with lupus nephritis. Conclusion: HCQ level monitoring may offer a new approach to identify non-adherent patients and support them appropriately. We propose an HCQ minimum target level of at least 0.6 mg/L to reduce the renal flare rate, but this will require a prospective study for validation.

The endophytic fungal communities of Polygonum acuminatum and Aeschynomene fluminensis were examined with respect to soil mercury (Hg) contamination. Plants were collected in places with and without Hg+2 for isolation and identification of their endophytic root fungi. We evaluated frequency of colonization, number of isolates and richness, indices of diversity and similarity, functional traits (hydrolytic enzymes, siderophores, indoleacetic acid, antibiosis and metal tolerance) and growth promotion of Aeschynomene fluminensis inoculated with endophytic fungi on soil with mercury. The frequency of colonization, structure and community function, as well as the abundant distribution of taxa of endophytic fungi were influenced by mercury contamination, with higher endophytic fungi in hosts in soil with mercury. The presence or absence of mercury in the soil changes the profile of the functional characteristics of the endophytic fungal community. On the other hand, tolerance of lineages to multiple metals is not associated with contamination. A. fluminensis depends on its endophytic fungi, since plants free of endophytic fungi grew less than expected due to mercury toxicity. In contrast plants containing certain endophytic fungi showed good growth in soil containing mercury, even exceeding growth of plants cultivated in soil without mercury. The data obtained confirm the hypothesis that soil contamination by mercury alters community structure of root endophytic fungi in terms of composition, abundance and species richness. The inoculation of A. fluminensis with certain strains of stress tolerant endophytic fungi contribute to colonization and establishment of the host and may be used in processes that aim to improve phytoremediation of soils with toxic concentrations of mercury.

When species within guilds perform similar ecological roles, functional redundancy can buffer ecosystems against species loss. Using data on the frequency of interactions between fish and fruit, we assessed whether co-occurring frugivores provide redundant seed dispersal services in three species-rich Neotropical wetlands. Our study revealed that frugivorous fishes have generalized diets; however, large-bodied fishes had greater seed dispersal breadth than small species, in some cases, providing seed dispersal services not achieved by smaller fish species. As overfishing disproportionately affects big fishes, the extirpation of these species could cause larger secondary extinctions of plant species than the loss of small specialist frugivores. To evaluate the consequences of frugivore specialization for network stability, we extracted data from 39 published seed dispersal networks of frugivorous birds, mammals and fish (our networks) across ecosystems. Our analysis of interaction frequencies revealed low frugivore specialization and lower nestedness than analyses based on binary data (presence-absence of interactions). In that case, ecosystems may be resilient to loss of any given frugivore. However, robustness to frugivore extinction declines with specialization, such that networks composed primarily of specialist frugivores are highly susceptible to the loss of generalists. In contrast with analyses of binary data, recently developed algorithms capable of modelling interaction strengths provide opportunities to enhance our understanding of complex ecological networks by accounting for heterogeneity of frugivore-fruit interactions.

This study aimed to recognized the preferential location of species of the tree sinusiae in response to a moisture gradient in Pantanal Matogrossense, Brazil. We established sampling plots of arboreal sinusiae along a soil moisture and flood gradient. Piezometers were installed, allowing monthly measurements of water table depth and flood height during one year. Detrended Correspondence Analysis, Gradient Direct Analysis, Multi-response Permutation Procedures and Indicator Species Analysis were performed to evaluate the effect of moisture gradient on tree distribution. The annual variation of water table is shallower and similar in Seasonally Flooded Forest and Termite Savanna, with increasing depths in Open Savanna, Savanna Forest and Dry Forest. Circa 64% of the species were characterized as having a preferential location in "terrestrial habitats normally not subjected to inundation", while 8% preferentially occur in "wet habitats". Lowest tree richness in flood-affected vegetation types is related to both present-day high climatic seasonality and Late Pleistocene dry paleoclimates in the Pantanal wetland. The tree distribution across different formations in the Pantanal shows a direct relationship with soil moisture gradient.

Atypical haemolytic uraemic syndrome (aHUS) is a rare, life-threatening, chronic, genetic disease due to uncontrolled alternative pathway complement activation. In this report, we discuss the case of a heterozygous carrier of a mutation on both factor H and membrane cofactor protein, who persistently presents haemolytic anaemia without need for blood transfusions, normal platelet count, normal renal function and no signs or symptoms of organ injury due to thrombotic microangiopathy 4 years after the diagnosis of aHUS.

In wetlands as the Pantanal, the floristic composition and community structure respond primarily to the flood pulse and, in order to understand the species that enter this biome, these plant communities need to be floristically and structurally described. Therefore, the objectives of this paper are characterize floristic composition and vegetation structure on a cordilheira (paleolevees), in the Pirizal region, Nossa Senhora do Livramento County, MT; define the type of vegetation and determine whether there is a greater floristic similarity degree between the object of this study and other cerrado s.s. or cerradão areas in Central Brazil. 102 contiguous plots of 20 x 5 m were distributed systematically arranged in six transects towards edge-center-edge cordilheira, thus having a total area of 1,02 ha. For sampling purposes, only individuals with entire stem DBH > 3 cm including dead trees and palms. 82 different species of trees and shrubs were identified, which in turn were spread in 63 genera, 32 families and one unidentified species. Being 37 species characteristics of areas with cerradão, 19 of cerrado s.s., nine distributed both in areas of cerradão and in cerrado s.s., and 10 percent belonging to the Pantanal wetland environments. Comparing the studied area with other communities across the DCA, the results indicated the existence of two floristic groups relatively distinct, the first formed by areas composed of cerrado s.s. and the second by cerradões areas. Among the areas formed by cerradões, there was a separation of the sampled areas in the region of the Pantanal (wetlands) those occurring elsewhere in the Centro-Oeste (dry land). The vegetation on cordilheira showed greater floristic and structural relationship with cerradões than with cerrado s.s. of west center region of Brazil, and may thus be characterized as a vegetation type of cerradão. The savannas of the Pantanal had a lower floristic relationship with other regions of the Centro-Oeste, this floristic heterogeneity may be related to the flood pulse which acts as a filter for most species in other areas of the Planalto Central.


Em áreas úmidas, como o Pantanal, a composição florística e estrutura comunitária respondem principalmente ao pulso de inundação. E para o entendimento das espécies que adentram esse bioma, essas comunidades de plantas precisam ser descritas florística e estruturalmente. O presente estudo teve como objetivos: caracterizar a composição florística e a estrutura da vegetação sobre uma cordilheira na região do Pirizal, Município de Nossa Senhora do Livramento ,MT; definir o tipo de vegetação e determinar se há maior grau de semelhança florística entre a vegetação estudada com outras áreas de cerrado s s. ou de cerradão do Centro-Oeste brasileiro. Foram alocadas 102 parcelas contíguas 20 x 5 m distribuídas sistematicamente em seis transecções dispostas no sentido borda-centro-borda da cordilheira, totalizando 1,02 ha de área amostrada. Mediram-se os indivíduos com DAP >3 cm, incluindo mortos e palmeiras. Foram identificadas 82 espécies arbustivo-arbóreas, distribuídas em 63 gêneros, 32 famílias e uma espécie indeterminada. Sendo 3 espécies características de áreas com cerradão; 19 do cerrado s.s., nove distribuídas tanto em áreas de cerradão quanto em cerrado s.s., e 10 por cento que pertencem a ambientes inundáveis no Pantanal. Comparando a área de estudada com outras comunidades através da DCA, os resultados indicaram a existência de dois grupos florísticos relativamente distintos, o primeiro formado por áreas compostas por cerrado s.s. e o segundo por áreas com cerradão. Entre as áreas formadas por cerradões, ocorreu uma separação das áreas amostradas na região do no Pantanal (áreas inundáveis) daquelas que ocorrem em outras localidades do Centro-Oeste (áreas secas). A vegetação estudada sobre cordilheira apresentou maior relação florística e estrutural com os cerradões do que com os cerrados s.s. do Centro-Oeste, podendo, dessa forma, ser caracterizada como uma fitofisionomia de cerradão. Os cerrados do Pantanal apresentaram menor relação florística com os de outras regiões do Centro-Oeste, essa heterogeneidade florística pode estar relacionada ao pulso de inundação que atua como filtro para a maioria das espécies das outras áreas do Planalto Central.

Este trabalho teve por objetivo caracterizar e quantificar as unidades fitofisionômicas em mesoescala e estabelecer suas relações com a intensidade e duração da inundação e com a topografia. O estudo foi realizado no Sítio de Amostragem de Longa Duração (SALD), implantado em uma área de 25 km² localizado na planície de inundação do rio Cuiabá, Pantanal de Mato Grosso. As unidades fitofisionômicas foram determinadas a partir da classificação supervisionada de imagens CBERS 2B sensor CCD. Os mapas de intensidade e duração da inundação e topografia foram gerados a partir da interpolação espacial de levantamentos planialtimétricos de campos. Foi evidenciada a existência de quatro unidades, sendo o Campo (pastagem inundável) a unidade com maior área de ocorrência, seguido pelas Cordilheiras (savana arbórea densa), Landizal (floresta inundável sempre verde) e Cambarazal (floresta inundável monodominante de Vochysia divergens Pohl.) respectivamente. O Campo apresentou sua maior distribuição em áreas de alta intensidade e duração de inundação e topografia baixa. O Cambarazal predomina em áreas de intensidade média e duração e topografia alta, o Landizal em áreas com intensidade e duração alta e topografia baixa, enquanto as cordilheiras são caracterizadas por intensidade e duração baixa e topografia alta. Testes estatísticos indicaram que a inundação foi o principal fator responsável pela distribuição das unidades fitofisionômicas em mesoescala no Pantanal norte.


The purpose of this work was to characterize and quantify phytophysiognomic units at mesoscale and evaluate their relationships with flood intensity, flood duration and topography. The study was conducted at a 25 km² Long-Term Sampling Site (LTSS), located within the floodplain of the Cuiabá River in the Pantanal of Mato Grosso. The phytophysiognomic units were determined from the supervised classification of CBERS 2B sensor CCD satellite images. The maps of flood intensity, flood duration and of topography were generated from the spatial interpolation of planialtimetric field surveys. Four phytophysiognomic units were identified; Campo inundável (flooded grassland) was the unit with the largest area of occurrence, followed by Cordilheiras (dense arboreal savanna), Landizal (seasonally flooded evergreen forest), and Cambarazal (monodominant Vochysia divergens Pohl. forest), respectively. Campo inundável is distributed mainly in areas of high flood intensity and duration and low topographic positions. Cambarazal predominates in areas of medium flood intensity and duration and intermediate elevation, Landizal in areas of high flood intensity and duration and low topographic position, whereas Cordilheiras are characterized by low flood intensity and duration and location in higher elevations. Statistical tests indicated that flood intensity is the main factor responsible for the mesoscale distribution of phytophysiognomic units in the northern Pantanal.
