INTRODUCTION: This study aims to assess the accuracy of glenohumeral joint injections through an anterosuperior approach using anatomical landmarks as a guide and arthroscopic visualization as the control method and to evaluate whether there is any association between accuracy, the physicians training, and the patient's pathology. MATERIALS AND METHODS: A sample size of 124 patients was calculated. A prospective cohort study including 164 consecutive patients was conducted. All patients over 18 years of age who underwent shoulder arthroscopy during the study period were included. A needle was placed using an anterosuperior approach before the beginning of the surgery alternating between a shoulder surgeon and a resident. Direct visualization through a posterior arthroscopic view was used to verify correct needle placement. Each case was classified as success or failure based on the arthroscopic intra-articular visualization of the needle. Univariate and multivariate analyses were performed to evaluate the relationship between injection accuracy, operator experience, and patient pathology. A p-value less than 0.05 was considered statistically significant. RESULTS: Of the 164 needles placed, 131 were intra-articular, giving a total correct placement rate of 80% (95% CI, 73-86%). Experts had an accuracy of 88%, compared to a precision rate of 72% for residents (p<0.001). A logistic regression was performed to evaluate which factors are independently associated with injection accuracy failure. Patients diagnosed with adhesive capsulitis had an OR of 6.15 for injection failure. CONCLUSIONS: This study shows that an anterior-superior approach shoulder injection technique performed by a shoulder specialist without image guidance has a high precision rate. However, in physicians with no experience in shoulder surgery, as well as in some pathologies such as adhesive capsulitis, the accuracy of the procedure decreases significantly and thus, in these cases, the use of some type of image guidance during the procedure may be recommended to achieve greater precision.

INTRODUCTION: Knee osteochondral lesions represent a frequent pathology within young active patients. One possible indication for severe lesions or in case of impossibility of harvesting an autograft is the use of fresh frozen allograft. The objective of this study was to retrospectively analyze functional results and failure rate after osteochondral transplants using fresh frozen allografts. METHODS: We analyzed data from patients who underwent knee osteochondral transplant using mosaicplasty technique with fresh frozen allografts at our institution between 2014 and 2019. We included those patients with at least two-year follow-up. Demographic characteristics such as age at the moment of intervention and size of the defect were included. Functional results were assessed using pre and postoperative Lysholm and IKDC scores. Patients who underwent a knee replacement were considered failures. RESULTS: Twenty-five patients were included. The median age was 43.5 years (IQR 29-50), 45% were female and the mean follow-up was 83 months (SD 54.6). Mean osteochondral defect size was 4 cm2. Mean pre and postoperative Lysholm scores were 39 (SD 19.3) and 82 (SD 15.4) respectively (p < 0.01). Mean pre and postoperative IKDC scores were 42 (SD 13.8) and 60 (SD 13.5) respectively (p < 0.01). Five patients (20%) underwent a knee replacement afterwards and were considered failures. DISCUSSION: Our results after a mean seven-year follow-up evidenced an overall improvement in functional scores and a failure rate of 20%. Osteochondral transplant using fresh frozen allografts is a reliable and feasible treatment for patients with large osteochondral defects.


Introducción: Las lesiones osteocondrales de rodilla son una afección frecuente en jóvenes. Los trasplantes alogénicos usando injerto congelado se presentan como una opción de tratamiento en pacientes con lesiones grandes o sin zona dadora. Este trabajo buscó analizar retrospectivamente los resultados funcionales y la tasa de falla de los trasplantes osteocondrales con injerto cadavérico congelado. Métodos: Se incluyeron pacientes sometidos a trasplantes osteocondrales de rodilla con injerto cadavérico congelado en nuestra institución, entre 2014 y 2019, con dos años de seguimiento mínimo. Variables evaluadas: edad al momento de la intervención, escalas funcionales International Knee Documentation Committee (IKDC) y Lysholm pre y post operatorios, complicaciones y tasa de falla. Resultados: Incluimos 25 pacientes. La edad media fue de 43.5 años (RIQ 29-50), 45% fueron mujeres y el seguimiento promedio fue de 83 meses (DS 54.6). El tamaño promedio del defecto osteocondral fue de 4 cm2. La escala de Lysholm promedio pre y postoperatorio fue de 39 (DS 19.3) y 82 (DS 15.4) respectivamente (p < 0.01). El IKDC promedio pre y postoperatorio fue de 42 (DS 13.8) y 60 (DS 13.5) respectivamente (p < 0.01). La tasa de falla fue del 20% (n=5). Discusión: Los pacientes presentaron una mejoría postoperatoria evidenciada en los resultados funcionales, y una tasa de falla del 20%. El uso de trasplante osteocondral congelado se presenta como un recurso útil para el tratamiento de lesiones condrales graves.

Resumen El condrosarcoma ocupa el segundo lugar en frecuencia, entre los tumores malignos primarios óseos, en pacientes adultos. Típicamente afecta a adultos de entre 40 y 70 años, siendo la mayoría de las series publicadas de este grupo etario. El objetivo de este estudio fue comunicar una serie de pacientes adultos jóvenes (16 a 40 años) con diagnóstico de condrosarcoma óseo, que buscó describir las particularida des en la forma de presentación en esta población. De un total de 37 pacientes de 16 a 40 años operados por condrosarcoma en nuestro centro, entre los años 2008 y 2019, 18 presentaron tumores de bajo grado (G1/tumor cartilaginoso atípico), 16 de grado intermedio (G2), quedando una minoría con tumores de alto grado (2 con condrosarcoma G3 y 1 con un condrosarcoma desdiferenciado). La tasa de supervivencia global fue del 97% (IC 95% 82%-99%) a 2 años y 90.5% (IC 95% 73%-96%) a 5 años y la tasa libre de recurrencia local fue de 92% (IC 95% 77%-97.5%) a los 12 meses y de 89% a los 2 y 5 años (IC 95% 73%-96%). El grado histológico fue el factor pronóstico más relevante. La concordancia del grado histológico entre el diagnóstico preoperatorio de la biopsia y el material de resección quirúrgica fue del 81% (30/37).
